My husband was released from ER and sent to a Skilled Nursing Facility for further care and Physical Therapy. He was placed at Trabuco Hills in Lake Forest, CA. In Orange County, CA. We were very pleased with his care. The Staff were very helpful and caring. The food was very good and they could prepare it for those needed different levels of assistance. The facility is very clean. They manage the medicines carefully for each patient. And the Physical Therapy was daily. Our Case Manager was Gil Gapay and he was excellent. He oversaw Peter's care and when getting prepared for discharge to a Board and Care Home, he arranged: special Transport using a wheelchair, arranging Home Nurse Care, Physical and ordering a new Wheelchair (coordinating with our insurance). I would recommend them. It is not an easy situation for any family to go through. Navigating all the details can be quite overwhelming. Having great help makes all the difference. read more

My father was admitted to Trabuco Hills Post-Acute after being discharged from Hoag Irvine. Against our better judgment, and under pressure from insurance, we agreed to the transfer. It is the biggest regret of my life, worst two and a half days of my dad's life. On Friday, January 30, 2026, during a routine visit, I found my father visibly distressed. He was agitated, complaining of pain, had a severely red and swollen left eye, and his blood pressure and blood sugar were both dangerously high. Given his recent stroke history, this was extremely alarming. What was even more alarming was the staff's response -- or lack of one. I asked if a doctor was on call. There wasn't. I asked if he had been seen by a doctor. The head nurse did not know. She then called the physician because there were no notes or treatment plan documented. We were told the doctor "thought it was just an infection." They downplayed a potentially serious medical situation while my father was clearly suffering. Because I did not feel he was safe in their care, I made the decision to have him evaluated in the ER to rule out something more serious. Instead of support, I was met with pushback and attitude from the nursing staff. During this urgent situation, a member of administration, Mike Perez, came into my father's room -- not to assist with medical care -- but to pressure me into signing admission paperwork. Meanwhile, I could not get clear medical answers or a doctor to properly assess my father. When paramedics arrived, I requested he be transferred to his usual hospital in Orange. The head nurse declined and attempted to go around me during the transfer process. After the hospital evaluated and treated my father, Trabuco Hills refused to accept him back. Even ER staff at UCLA Medical Center were extremely apologetic about the situation and expressed disbelief that the facility would decline to readmit him after only four hours in the ER. They had difficulty reaching the facility, and when I finally got through, Mila, a night nurse informed me that because I advocated for my father and he had been in the ER briefly, they had already given his bed away. Given. His. Bed. Away. As a result, my father spent days in the ER in limbo waiting for placement at another facility -- all because I insisted he receive proper medical evaluation. I will never regret advocating for my father. He is now in a better facility where he is treated with dignity and appropriate care. But no family should have to go through what we experienced. If you love your family member, think very carefully before choosing Trabuco Hills Post-Acute. And if you have no choice, know your rights and contact the Ombudsman's office. My mom was transferred here for rehab. The staff has been so helpful. Anytime I needed something the admission manager, Lu, was so attentive and quick on responses. All the CNA's, PT, Treatment staff , have been amazing. They have all been so helpful and all have a positive attitude. The facility of very clean and you are always seeing someone clean. Would not recommend. The facility needs a lot of attention to it's cleanliness. Only very, very few staff were helpful. Most staff lacked patient care, and some were just downright rude. They would ignore you or their famous answer is, "I'll let someone know." When my dad was checked in, there was feces on the wall. The shower rooms were disgusting and had mold. The doctor couldn't manage his medical needs. He was very under medicated, or over, and we had to get him out of there. I even questioned the doctor's medical expertise. It was a horrible experience, and later I found out, this facility had a reputation for poor care. In the end, I reported them to Medicare and the Better Business Bureau. And I strongly suggest that you not bring your loved ones here. read more

Trabuco Hills Post-Acute's care was a nightmare. The room was dirty, the shared bathroom floor had holes in it. The food was unappetizing and usually lukewarm. Worse of all, the nursing assistants are not training or monitored. My mother got an E. Coli UTI that took her down for a couple of weeks and then C. Diff for another week after that. E. Coli in the urinary track is caused by stool being transferred to the urinary area during care. C. Diff is another infection caused by stool. My mother also had bruises that no one was aware of or knew how they happened. Lastly, I hid $40 in a glasses case in her drawer so she could get her haircut, which never happened in 5 weeks. I found that was missing when she was discharged. All this happened while I was visiting her for almost 8 hours a day. Imagine if I wasn't there. I filed a grievance and had a meeting with staff, but the damage is done. My mother will never be the same. read more

I was so impressed with how Aileen answered all of Stella's questions and took the time to assure her that everything would be taken care of in her release. We have now had a medication issue, they forgot to give my moms medication because she was at dialysis. When I called at 2:30, they first told me she had her meds at 9. I reminded them she was not there. She wasn't back until 11. Then they tried to tell me she got back at 2 and have not had time to give meds yet. But I had spoken to her at 11:10 and she was there. They finally gave medication late, and her blood pressure was high. They do not care here. They don't even pretend to care. Met and spoke to a few admins/ managers. They do not care. This is honestly the best place to study as a student. The environment is welcoming, calm, and perfect for focusing on school work. Everyone here is so kind and supportive, which makes a big difference when you are trying to learn. The nurses are amazing because they take time to teach me and answer all of my questions without making me feel rushed. I always feel comfortable asking for help, and I learn something new every time I am here. I really appreciate how encouraging and patient the staff are with students like me. Had an accident and was eventually brought to Trabuco Hills for care. After a couple days, I was brought to Activities and was absolutely shocked and most happy to see and feel and be involved in all the excellent and joy creating activities that Sharon Allman and Carmen Huerta provided for me. Given my age, I've been to a number of facilities already, but the way these two gracious and wonderful ladies treated all of us, will never leave my mind and will always be a reminder to focus on joy and happiness to reduce pain and unhappiness. These ladies provided tremendous joy and love which easily resulted in comfort and less pain. I hope that the rest of the people in this organization and other organizations will utilize their loving , passionate and highly professional behavior and practice and role model their behavior. I just completed 44 days at this facility and it was mostly very good. My main need was therapy and Lynn and Sarah really diligently guided me back to full walking strength with excellent therapy! The activities center here is full of kindness and thoughtful involvement with each patient! Carmen, Sharon, and Kathia work tirelessly daily! I so appreciated their making my time pass quickly because of involvement. Nurses Ray and Georgina were so attentive to my medical needs! The facility is old but under 2 year old new management, which is undertaking many projects to improve appearance and functionality. If you are fairly young and healthy and just here for very short term recovery, you might be ok. Otherwise, run the other direction. I have a friend who stayed here and this place was a nightmare. There are a few nurses who actually seemed caring, but the gaslighting here is on another level. As a family member or friend, you know what your loved one's baseline behavior is. You tell them something is off and not right and they just say "we'll let someone know" just to get you off their back. You ask for blood work and they say no because "the patient is always this way" only to eventually have them get sepsis (more than once). Fairly certain my friend was not turned nearly as often as should have been done. Cleaning of the patient while changing their diaper was not great. I saw where they would just let feces sit under pressure injury bandages. Call lights are not answered very quickly. It's like they are just waiting for the patients to die or something. I hear that all SNFs are this way, but if that's the case, the system is failing us. read more

Update, to my post, my mom died last week, as a diabetic, the bed sores she developed while under the care of Trabuco Hills ended up killing her. If you have loved ones, keep them away. Care like this is criminal. Shame on the administration. Also, the reply from the facility below is a joke, because they do not return phone calls or emails. They have no desire to listen or make changes. They only have a desire not to look quite as bad, but don't be fooled, this place is dangerous for loved ones. read more

I was there for 10 long days!! I had trouble with PT & OT! They both came in @ the same time just as breakfast was being served! One wanted me to do this & one wanted me to do that! Really just pull me in 2 directions! When I got upset they both got another & left! Later each one came back to scold me that they had their jobs to do & I didn't comply with them! The PT told me that the minute he steps into my room my time with him started....REALLY DOING NOTHING BUT WALKING???? Plus one of them told me that when a meal is served they can not continue.... Yet they did not leave when breakfast was served. Plus I was told I would get 6 days a week PT & OT, yet the first week I saw them not even 2 hours & they billed my insurance for 6 days @ 1 hour each!!!! Admin is always a big joke.... I been in 3 different rehabs & they are all the same!!! read more

NOW this place is trying to bill my dad $26,000! They said he had to leave because his insurance was no longer covering but while he was there for three weeks, his insurance had already ran out and we were never notified. This place is a scam and they've been reported for occupational therapy billing that they never provided to him. He left here not being able to walk anymore. read more

I typically never write reviews, especially bad ones but I can't go on without saying something to those of you thinking to board a family member/ friend here. I made a list throughout her stay of some red flags that stood out to me regarding her health and safety. -no water to drink in room -no bed rails -left dinner from night before all over -doesn't answer red call button -no diapers the first two days -call button was not at chair when she was at the chair -complained TO ME about cleaning her diarrhea up- I was shocked -patient checks only every 2 hours -patient getting up on her own for bathroom since no one will answer the red call bottom -food so bland and gross -website hasn't been updated since before covid -takes away walker and puts it across the room so patient doesn't get up on their own (my grandma got up anyways and took a terrible fall) -does not have a sitter/cameras -door always closed, patient ignored -not taking BP three times a day like requested by doctor -no nurse to be found, no receptionist anywhere -no one picks up phone (nurses and receptionist) -night staff isn't responsive (doesn't respond to red button- made my grandma get up and go find help at front desk at 4am due to pain) -no one took her food preferences for first four days -was supposed to get therapy on a few different dates- didn't happen -didn't check wrist wound, same bandage on for two and a half weeks (there was a date written in sharpie from we previous hospital stay) -telling us her diarrhea is gone when it's not at all -released her with mrsa, uti and C. Diff. (after telling us she was clear, all came back positive the same day she left) -took her to a new nursing home. read more

You know that quality of service is poor when there are more scores rated at one than any other score available. I have seen several skilled nursing facilities, this is absolutely the worst. If you have any other option than this, take it The level of care is terrible. I watched a family member get a bedsore that went from a rash to a stage 3+ in 2 weeks. They don't change diapers often enough, don't move patients off their back so they don't develop bedsores, the rooms are dirty, they don't change the patients clothes, and the list goes on and on. The nursing care he received was exceptional and we appreciate the open communication and willing Throughout his stay, we witnessed the dedication and compassion of the staff, and it made a significant impact on my dad's healing journey. Dr. Gosh's medical interventions and referrals were invaluable, ensuring Dad received the necessary care he needed to regain his health. We cannot thank Dr. Gosh and the staff enough for their support during this difficult time. We have posted a picture of some of the staff Val, Nancy, and Kate. One thing that meant the world to us was the regular updates Theresa, the Nurse Practicioner, and Tanya, the physical therapist provided. Theresa would update me on his medical condition. Tanya would call and inform us about Dad's progress, sharing the milestones he achieved and offering valuable insights into his ongoing therapy. This level of communication not only reassured us but also made us feel involved in his recovery process, despite not being able to be physically present all the time. The only area where we believe there is room for improvement is the food provided at the facility. Unfortunately, the meals were not up to par and lacked the necessary nutrients for someone in Dad's condition. Too much sugar in juices and deserts. While we understand that dietary preferences can vary, we were hoping for a more balanced and nutritious menu, especially considering the importance of proper nutrition during the recovery process. The dedication of the staff and the medical care outweighed this concern. While we appreciated the open attitude towards suggestions, we hope that Trabuco Hills Post Acute can address the issue of food quality and nutrition to enhance the overall experience for patients and their families. In conclusion, we are immensely grateful to Trabuco Hills Post Acute for their exceptional care and support during Dad's recovery. Dr. Gosh and the staff truly made a difference in his healing journey, and for that, we highly recommend this facility to others in need of skilled nursing care. I was there for 2 1/4 weeks, from the beginning I was by myself in room for 3days, than older man came in on a Tuesday, he had a machine to pump oxygen, it was a very old an extremely loud knocking machine it kept me up all night, after one week the nursing home finally replaced the machine with a newer and somewhat quieter unit. But the best part of the story coming up exactly at 2:00AM., this man in his late Eighties started to called out for his Mother, I found out she died 50 years ago. Plus every 10 minutes or so he yells out for no reason, he stops about 6:00AM. While his craziness goes on at night, he asks me who am I, so I told him I'm a Cop, he quite down for a hour. The next night starts yelling again at 2:00am. the hold retrain starts up again. This want on for 10 Hell Nights , then one time again he asked who am I, I said, I'm Almighty God. When you get your meals I could not eat everything has sugar in it. I'm have Diabetes. About 2 days before I left a they broke down and give me proper Food. I left there to go back to Little home, I appreciate being home. Away from these idiots. My Brother , who is Dr., Drove me home I only lived 2 1/2 miles from Hell Nursing Home. As I forgot to mention my Mother, Died there in 2004. It had a different name. The End. read more

we recently placed our mother in this facility after a lengthy stay in the hospital hoping to get her rehabbed so she could come home. this is a decent facility with with all of the equipment and activities needed to get someone up and moving. but what makes this place so good is the staff. the doctors, nurses, and all staff are very kind, loving and supportive. our mother was treated with dignity and respect. all that being said, there are an awful lot of sad, lonely elderly patients in this facility that somehow seem to have been forgotten - perhaps this is a failing of their respective families and not so much the facility, I'm not sure. while I personally hope to never have to go to any rehab facility, if I ever do, I hope the staff would be as kind as this one. read more

NEVER LET ANYONE GO TO THIS FACILITY. They are beyond neglectful. My husband had been there only 4 days. The second night his bed broke and started to tilt. He has no movement in either leg or in his left arm so he could not try to adjust himself. He slid and hit his head on the night stand. He pushed the nurses call button at 10:30 PM. No one ever responded. He is unable to yell due to his stroke. Finally at 4:45 AM he called 911. The sheriff got the nurse to come into his room. She verified that the call button had been pressed at 10:30, but they hadn't noticed it. I asked my husband if they were checking his blood sugar, and he said, "No." I waited 30 minutes at the nurse's station because no one was there (around 6:00 pm). FInally, a nurse came and said they didn't know he was diabetic even though it was clearly marked on his intake documents. She assured me they would check his blood sugar and make sure he was given insulin and the appropriate diet. Last night, I went and for dinner he was served a large glass of orange juice (not sugar free), a portion on noodles, and a slice of apple pie (not sugar free) along with other foods. I spoke to the nurse again and she said he was on a regular diet. I reminded her that he is diabetic and needs a diabetic diet. She said she would make sure he would be given a diabetic meal from now on. This morning he was given a glass of juice and a large slice of raisin streusel bread! Full of sugar! This place is beyond my worst nightmare. Please don't let anyone you care about go here. read more

My mother was discharged from the hospital Tuesday, January 17th and was doing quite well, she was transported to Lake Forest Nursing Center. The next morning, Wednesday, January 18th, my sister went to visit my mom, when she entered her room my mom was covered head to toe in a sheet, my sister kept yelling our "Mom, Mom" thinking she had covered herself. Finally, a nurse took her aside to tell her that my Mom had passed away, no phone call to let us know, nothing. She passed only 12 hours of being there. We received no apology for not informing us of our mother's passing. If you want your loved ones to survive DO NOT BRING THEM TO THIS PLACE, WHO KNOWS WHAT THEY DID TO HER TO CAUSE HER TO DIE . read more
