PT and OT are great. They have worked hard to improve my strength, help me stand and I know I will walk soon. The CNA's, RN's work very hard to take care of everyone, even though they seem understaffed at times. They do have fun activities daily for those who like those things. read more

My sister was sent here from the hospital because she had an external icd and they were one of the few who knew how to handle that. Out of the list of places she could go, this was rated the best. If you care about the person who comes here, know you need to be MORE on top of things than other places. My sister was diabetic. When i questioned her increased shakes (like parkinsons) i asked them to take her blood sugar especially since dinner was served. The nurse was like surprised and said, "so you want me to take her blood sugar?" I said yes since she is diabetic and you should be taking it multiple times a day before each meal. She replied that there is no where in her chart from the hosp or from the other skilled nursing facility. I said thats impossible since they take her blood sugar daily. She was a relatively new nurse though. So please make sure you go through all treatment plans with the nurses. My sister went without ANY insulin for 2 weeks!!! My sister particularly did not like the nurse Mirta. She was going to give my sister hydrolazine for high bp, but it should only be given if her bp is elevated. Before she gave it to her, i asked the nurse what was her blood pressure. She didn't recall but said she would look it up. She said she took her bp before i arrived.. i arrived 4 hours ago. She should have it taken it again before distributing meds. !! I was also told Mirta told others that my sister passed from another stroke.. reality was she aspirated from breakfast, and the external icd alarm alerted staff. What happened there? No idea. Did they sit her up before eating? Again no idea. so if you need to go here or anywhere for that matter, be involved in your loved one's care. Ask questions! read more

My mom was in there for 2 weeks . They did not take care of our mom . They were supposed to to take her Friday on sept 4 to get her staples out but no one took her. This place is not a good rehab home and if you ask questions they just say ok I'll get in touch. My mom got an infection in her back . read more

It looks good from the outside but they do not have enough stffl to take care of patient needs. The "ain't my job" attitude permeates the entire experience. I am a patient along with my wife with dementia.. I spend much more of my time getting her taken care of than my own rehab. read more

Let's see if you want to know about this place you really need to hear what we feel about this place .Type of Care is it is suppose to be a rehab rehabilitation place because she had back surgery and it's supposed to have skilled nursing and stuff well I like the nurses but everybody else sucks like you're lucky that word I put instead of something else because I'm sorry I will never ever ever ever let somebody go there again. We trusted this place. The hospital made it seem like there's such a great place that they're gonna do physical therapy with her all the time to get her moving in and they're gonna take care of her her wounds and everything and they're gonna be watching her. They're gonna be on her to help her get her therapy going well that was all so not true .now let me tell you about the type of care quality they have the care quality they don't get you up and going then don't take care of you. The nurses will come, but they're so understaffed that it's so hard for the nurses to keep up, but the nurses are the sweetest in the world and they do care but they can't get to everybody all in time. It's not their fault. It's the people who run a place don't know how to help them out the staff there not the nurses now everybody else the office people I'm sorry they should not be there because there's no quality of care you tell them from day one that your mom has appointment to get staples out and the whole time she's there for two weeks. They miss her doctor appointment. They didn't take her to her doctor appointment to get a staples taken out and then they were supposed to be doing five days a week therapy. She only had therapy 3×3 to 4 times tops out of two weeks day and then to top it all off when she had to go to the bathroom they would tell her go ahead and use the bathroom and in your bed and will and we'll come and clean it up after and leave her there for how long in her diaper and and now her back surgery is completely infected. I took her out of there because they couldn't even get her to her original doctor that they said over and over that they would take her too. They couldn't even get her there. It was just excuse after excuse why they didn't take her there when they promised they would take her there. It is ridiculous. You know you have to yell at people in order for people to even acknowledge shit there you know it's like I'm sorry but if I had to read it, it would be like 0.9990 Like way past zero you know it's horrible. Whoever owns this place you need to check your work. You need to get a whole new management team there because I'm sorry this is not a nice place my mom right now is at home with her back super infected because you guys didn't take care of it. You guys are supposed to have wound care what happened to that she wait. We took her to the freaking another doctor after we left there her back is so infected. And her staples are protruding into her incision and every day they check this nobody told us that she had a bad infection. Nobody gave her nothing for it. You guys are so horrible to leave them in their diapers and dodo and piss knowing they got back surgery, you know incision all the way down her back . So because they made her sit in it for so long so many times I guess that's why her back got infected. please everybody don't ever send your family members there the kitchen they come to your room and ask you what you don't want and what you want and everything that my mom said she didn't want they gave it to her.Im sorry they don't care about the people there. They just want the money plus they lie saying they did theropy on days they did not I hope somebody checks this out because it's so wrong that they be doing that Nobody show up to help her do all of his stuff that they were supposed to it's horrible .Wait if you really hate somebody then you should go ahead and send them there because that's the perfect place to send somebody you don't like cause they'll treat them like crap. It'll be great so yeah if you want to send somebody somewhere that make them feel horrible and don't care about their well-being and let them get all messed up and infected and everything. If you really hate somebody that send them there. That's the best thing you get a five stars for the worst place in the world but if you care about somebody, don't send him here I'm sorry to do this company to whoever you is but your people running this place is 0 zero negative negative negative percent screwed in the head because they don't know how to run that place. If I never took my mom out of there she would have gotten way worse with his bad her back got infected and it could have been way worse how can you treat people like that trusting you with our elders and you do this to them .you Straight Are the Worst ever I will make sure to tell everybody not to let family's members go there . read more

My mom was sent there from the hospital, it's not just a rehab facility it's also a nursing home. My mom ended up getting C-diff while she was there. My mom wasn't happy with the place. Not enough CNA'S. On the outside of her room was a 3 drawer plastic bin, on top were the masks and bleach wipes. C-diff is very contagious didn't provide yellow covering, the gloves were inside the door. My mom requested to go somewhere else but the doctor wouldn't do anything about it. I spoke to case management and just took her out AMA. My dad was home Hospice, my mom wanted to be at home. I was not at all impressed with the place. The room was small. When you get to main area the residents stare at you. I wouldn't put a family member in this facility. There are much nicer ones off Saint Rose Parkway. read more

If I could give zero stars I would. A relative of mine spent time there, suffering from the lack of any real care, being lied to with incorrect and sometimes intentionally false information given, such as to resolve financial problems that required leaving the facility to do so, (the bank wouldn't work with them unless they came in person.) that due to policies that are nonsense, they were not allowed to deal with this. The actual care itself was neglectful at best with only one or two who were actually good at their job who bothered to help, the rest would simply never bother to come for over an hour in many cases, resulting in ..... accidents... Of the biological waste variety. And having to stew in it for hours unassisted, and if they went to go to the bathroom thekself they would get yelled at for going against orders. read more

I went there for physical therapy it took 6 days to start therapy they never gave us water even though we asked 2nd day I need to go to bathroom 2 hours later still no answer crawled to bathroom pulled myself on to the toilet when done pulled emergency bell waited for a hour no answer got of toilet crawled in to bed 2nd day same problem again got up used bathroom called pull red cord waited 1 1/2 hour no response took matters in hands got up got to go to bed used wall for support it wasn't a wall but a barn door that slides fell cracked my head fell and landed on butt nurse at my door called for help it took about 10 minutes to respond when I fell my feet landed under bed instead of raising bed they lowered but squashed my feet took 2 people to get me back in bed no pain bills payed there all night in pain told the day shift about my fall they were not informed about fall they sent me to the hospital c scan showed lacerations on back side of head lacerations on tailbone they won't allow me to have incident report I believe they didn't make one it was so bad I finally made the decision to check out they blocked me at front door I had to threaten them that I was going to call the police they called my husband to come get me they kept blah blah blah me asked for discharge paper from hospital on Aug 9th also from the fall discharge on the 11th no paper work also from the 18th no paper work if you our a family that needs medal service or physical therapy please do not look at the pretty atmosphere you will not the you require then please don't ues this place its not worth the money. Liars liars from all employees but there are a few nurses and a couple of cnas but all the rest are useless please over look this place for your safety read more

I am a Las Vegas entertainer who performs 4 shows on average per week at different senior facilities throughout the Las Vegas area and in Southern Utah. I have a client list of several dozen locations that range from senior apartment and housing communities to assisted living and memory care facilities. This afternoon I performed a show at Trellis Paradise and, after reading the reviews, expected to find a house of horrors. I discovered a surprisingly large facility that appeared to be amazingly and efficiently run by a staff of friendly, smiling people who were doing their jobs with energy and a strong sense of purpose. I saw no clusters of gossiping employees. Everyone was at their duties in a most professional manner. The rooms, hallways and common areas were spotless. No dust, spills, or broken equipment or fixtures. No unpleasant smells. The woman I connected with was La Shonda, Activities Director. Efficient, friendly, knowledgeable, and incredibly helpful. She promoted my event with flyers and announcements that filled the performance area with happy residents. After the show, I visited with at least ten of my audience and they had everything good and nothing bad to say about Trellis. This facility is unique in that residents are short term, often only a few days. This is different from the usual places where I perform, where residents are there for a much longer time. I'm looking forward to a return engagement and, for what it's worth, would not hesitate to recommend a friend or family member to the service of Trellis Paradise. read more

This place is awful! The way the patients are neglected, left sitting in urine. My dad was in there, and he fell in the bathroom and was left on the floor for hours until someone found him. My mom was just recently at this facility. unfortunately they were horrible. They kept messing up on her meds that she takes for congestive heart failure and COPD. Her pick line bandage she had was only ch once in the week she was there. It started to fall off, she asked for it to be change, they put scotch tape on it. The lust goes on. To much to list. My brother got pneumonia and no one knew the only it came to the point he couldn't breathe and was rushed to emergenciy . they lack in help it's really under staff he has mobility problems he had such a hard time getting help to the bathroom we made it a mission to be there as much assist possible. I did like the people but something has to change he is not going back I'm making sure of that! I don't recommend this facility at all. My brother was in Trellis Paradise and I have to say that the staff are the most difficult to reach. I had to leave numerous messages to get vital medical information about his medical treatment. They are never in the office and they don't answer their voice mail messages. The staff have a don't-care, don't bother-me attitude about everything. The worst one is Mae, the case worker. She is never in the office and doesn't return voice mail messages. read more

I had to come back and repeat omce again if you have any love for your family please i repeat do not take them to this facility. They ate understaffed and the ones that are there have no sense of urgency. How i know is because i myself, husband, brothers and sister-n-law went there every single day to check on my mother.I went to the director of nusing ms. Giovionna was informed what was going on with the complaints with my mother and she absolutely did nothing along with other lies of what she was going to do.my mom is still in the hospital behind the uncaring of this facility. As far as rehabing goes she seemed to be getting weaker cause they absolutely doesnt have the right trained staffed to do the jobs they are getting paid to do. I could go on and on to explain all the details but it would be irrevelant now cause i got her out of this horrible place. Oh yeah for the 5 star ratings is probably from the ones that work their trying to make it look good just for ratings which 0 is not included cause thats the only rating they would get from me. Do not let any hospital talk you into going to this facility. False advertising. We found a catipillar and spider in her room crawling around i promise you no lie. This is by far the best facility my mother has been in. Even being short staffed, 95% of the employees are smiling and laughing. Activities kept her involved: games, crafts, live entertainment... PT & OT were awesome. The only issue was the food dept. My mom has a food allergy. Had to get angry the 4th time. read more

Please don't send anyone you care about to this place. They never answer the phone. The staff nurse and cna's treat the clients terrible. I was there visiting and watch an 86 year old woman bawling because of the treatment she received from a CNA. She felt so helpless and powerless. My own friend had issues. When I reached out to the charge nurse no one could find them. The next day I called all day and no answer. Absolutely unacceptable. If I don't get some help I will file a complaint with the state. read more

Two people in this facility stand out as exceptional individuals. Vickie (receptionist) and Dana (Therapist), both outstanding in professionalism, attention to detail and customer, knowledge and proficiency..... A pleasure to have met both of you. If they were the only two individuals we had dealt with this facility would have 5 star rating, unfortunately they stand alone.... That leaves the remainder of this facility as a mass incompetence, with a total lack how to treat patients as people. Schooling you may all have, but common sense and respect for your customers you have none of. The majority of the staff speaks a foreign language, that is their heritage I assume. Not a problem, I have my heritage as well. But you are in an American institution caring for Americans and they deserve your attention. Using your foreign language may be easier for you but doing it repeatedly in front of patients who do not speak or understand it, who the majority are elderly, possible hard of hearing and having difficulty determining who you are speaking to is extremely disrespectful. Thus confusion and irritation occur for the patients. I repeatedly asked the case worker (Lorna) to slow down, annunciate the words, speak slowly. But to no avail, each time I visited there had been another confrontation visit by her to the patient. Numerous times for signatures on documents that my friend could not read and did not understand and yet persistence on her behalf for signatures and always in a hurry. Discharge day was to say the least a "circus with no ring leader". All ready and eager to leave was my friend, oh yes. Put did he have his meds, "Oh all is well the Rx's have all been called in to the pharmacy" per the staff. So off we go to the pharmacy, allowing an hour or so as staff had only called them in, so they said, that morning, yet the printed list provided was dated the day before (4/10/24). Hummmm. OOPS, No meds at pharmacy, no record on file of any call in or order for any meds... Back to Trellis..... "So Happy". After some serious discussions it is learned "OH NO" lead nurse had told the staff to not call in any of the meds, rather to give the patient the remainder of the blister paks of meds they had been using and his GP could write Rx as needed next week. Really, so does anyone know what the hell is going on....... Eventually we were given the blister paks of meds, NO they were not in the patients bag..... Only God knows where they actually were.... Shall we leave again, not sure there is an ambulance, fire truck and police car out front lights flashing.... Oh it was not for us. Did we hear an "I'm so sorry".... No, not even a Have a Nice Day or Kiss my a.... Did I forget to mention the hall clock that is consistently at the wrong time, the board in patients room is worthless as no one has marker to put the date or etc on it, that there is barely enough parking for staff let alone visitors, that policy on checking in clothing-personal items for the patients is a joke-asked for the form-staff took list on back of a piece of paper-never seen the list again and certainly not all the clothes came back with patient. IOh, did I mention that absolutely no one in the Case Worker Department ever returns a phone call or message. Probably enough to say I'm not reserving a place for my rehab stays........ I wish I could give this place 0 stars! My sister was admitted a couple days before Thanksgiving. I have been able to speak to her two times briefly since because you can't get anyone to answer your calls. It rings 20+x then disconnects. When someone does answer they say they're transferring you to the nurses station but it goes to voicemail which never gets returned. I've sent a message through the facilities website asking for help to talk to my sister. I even sent an E-card from their website which they claim to print and read to the patient. In my e-card I explain to my sister that we've been trying to reach her and the difficulties we're having so she doesn't think we abandoned her yet still no call back. I left a message for nursing director more than a week ago with no call back. I called 6 hours ago and the nurse told me my sister was asleep but as soon as she wakes she'd have her call me. No call ! Shocker ! So Tonight , 6 hours later, after 30+ min of non stop calling a nurse answered and hung up on me twice ! The third time I told her I was gonna call the police to do a welfare check so she put me on hold and got my sisters nurse who told me that there are no phones in patients rooms and would not put my sister in a wheelchair and push her to the nurses station to speak. She just said I'll give you an update, she's awake watching tv ... so I asked for the directors name and phone number. The nurse gave it to me and asked me to call in the morning and maybe I could talk to her then .... POORLY RUN FACILITY ! If you don't have phones in patients rooms then why don't you have a cordless phone ????? read more

Sorry I do not know how well they perform, but if it is like trying to get a hold of a loved one then it could be pretty bad. She was admitted December 12, 2022 and trying to speak with her has not been successful in 9 days. Dec.21 2022 today. Always one reason after another of why she can't talk according to the staff-- nothing medical. Maybe short of staff since they do not have phones in her room and they need to use their own personal cell phone. Spot on what Robin C review said. Very poorly organized and run. We don't live in the same state so visiting her is almost out of the question. Checking on elder abuse issues. read more

One star too high a review! My husband ordered by local hospital Dr. to rehab for two weeks of OT & PT. I picked Trellis after going first to check it out. The Marketing director gave me a tour. It looked very nice and she said she was glad I went. That out of 100 + beds only two were left & my husband would get one. That was the first lie. I got lost the second day my husband was there and two long halls all had empty beds. I could write a book about the two week nightmare. Two weeks were up & head nurse said they could keep him 20 days because ins. paid up to that time. He was never bathed, wouldn't eat & because he has memory issues he couldn't remember if he'd been showered. I addressed these things to no avail. I did not agree to the 2O days. POAs mean nothing, but the decision was mine to make. I had him discharged Oct. 13//22. Helped him undress at home and his whole groin area was RAW. It was very painful for him while I cleaned him and applied an anti fungal ointment. He slept for a couple of hrs. , woke up, & said he had to pee. Grabbed his walker, stood up and his legs buckled. He was very weak. I called a neighbor who helped sit and lay down. He was shaking and sweating profusely. I called 911, blood pressure too low and back to hospital sane day by 7:P. M. Four days later Sunday Oct. 16 after tests done I had not spoken with any Dr. yet. Leaving for the night a Dr. appeared, asked if I was the patients wife. Then he spoke , " your husband is out of the woods, he had an acute UTI, the infection got to his kidneys he won't need dialysis but is sepsis." He continued to explain sepsis but I said I knew it was blood poisoning! Continued with " if your husband had been at that rehab (Trellis ) one more day he would have died. To say I was stunned and couldn't speak is an understatement. Dr. continued that antibiotics were being administered through a pick line, wanted him in hospital 2 more days then to a rehab for two more weeks of antibiotics. I chose Encompass rehab in Henderson. The nightmare got worse. I'm tired and may relay that place later. I'm 80 yrs. old & only by the grace of God still strong enough to fight. Today my husband is bedridden & in home hospice ordered by a dr. at Encompass rehab who professes to be a dementia expert. My faith in our present health has weakened but my faith in God won't! I chose to share all of this because if it helps just one person or family it will be worth my time. Grateful for having place to do so. Thank you. read more

Not thrilled by anyone in this place. The nurses barely come to your assistance when you are screaming for help even. They don't care at all if the patient checks themselves out because they will NOT give them the medications the doctors prescribed them, even if that means the patient will die without them. NOT THEIR PROBLEM!!! The director of nursing both time I saw her was extremely unprofessional from bursting in a meeting room to chewing like a slob when I was trying to talk to her. And doesn't care at all about any concerns you have. She just wants you out of her face as soon as possible. I wouldn't and will never recommend this place for the lack of concern and they claim to be 5 stars yea OK!!! Your story people. Don't fall for it!! Also don't expect to keep all your belongings either. Because two times the patient had to go to the hospital and they lost his stuff both times. When we requested to pick up his things two days prior they couldn't even bring everything back to him. Don't take anything expensive. It's just bad all the way around! read more

I just want to take a moment to share how great this facility was to my mother and myself. She was a patient here for over 60 days. She was recovering from a major surgery and also has a few other challenges including dementia. They have a great routine for the patients and the facility is clean as can be. I was always greeted by a smile at the front desk. I tried to be their everyday to remind her of where she was. She is very tricky to care for and they handled it like pros. Shot out to Mae the case manager. She helped answer all my questions and made many phone calls to help me out with the process. You are GREAT at your craft. Such a beautiful building. It's truly one of the nicest facilities I have ever seen. When you walk in you are greeted with friendly front desk personal. The facility is so clean and has a clean smell. I love the rehab gym because they have a huge wall full of windows that make it so bright and cheerful. This facility is definitely top notch and I recommend it. read more

They are amazing, they helped my father out in a very difficult situation with my father, they took him right in and got to work immediately on setting him up with proper care. Having a family member in the hospital is stressful and trying to figure out what comes after is even worse. Thankfully, we received a call from Jessica inviting us to come tour the facility and Giovanna came to visit my grandfather and I at bedside and provided us even more information. Upon admission, we discovered what a beautiful building Trellis is. They have all private rooms and the grounds are extremely clean. Case management was so helpful with resources. The gym is so spacious and bright. I can not thank the Trellis team enough for all that they've done for my grandfather. You can tell everyone there loves their jobs and what they do. It shows through the patients and the energy throughout the facility. read more
