My mother and her husband moved to Walnut Place from their home last August. My mom's husband had the beginnings of dementia and my Mom was unable to walk too well or feed herself independently. They had to be moved several times due to his issues but the staff was always caring and pleasant. I learned so much what these nurses had to endure daily, I have a new respect for what they do at Walnut Place. The head nurses communicated very honestly with me and my brother on what steps would be best for both of them. It is always hard to move your loved ones to a nursing facility from their home. But I truly feel that Walnut Place was the best place for them. read more

My dad was a resident at Walnut Place from July 2021 through December 2021. At the time, my dad was 85 years old and had been diagnosed with dementia for approximately 2 years. He was initially hospitalized at Presbyterian for a UTI in July and the doctors there recommended to my mom that he be transferred to Walnut Place for further treatment/rehab. I am unsure if there is an affiliation between Presbyterian and Walnut Place but it seems likely that there is. My mom took Presbyterian's recommendation at face value and had my dad moved to Walnut Place.  My dad lived on 3 different floors while he was a resident there. It was in his first room on the 2nd floor where he tried to get out of bed. This resulted in him falling and fracturing his wrist. To this day, it is unclear to me why he was in a bed without bars since this would have likely impeded this activity. The staff on this floor was fairly hit and miss. Shortly after this, my dad moved to the Rehab To Home program. These rooms are located on the 6th floor, and this was his best experience. The rooms were all clean with small kitchenettes, the staff was friendly, and my dad's needs were attended to, such as cutting up his food & feeding him, changing him regularly, clean bed linens etc. His last and final move to the 3rd floor is the primary reason for this rating. This floor always smelled like urine. As soon as you stepped off the elevator, it hit you in the face. I saw at least 3 roaches in my dad's room during his time there. Face masks, paper towels, and wet wipes were scarce if existent at all. We never found one of the foot pedals for his personal wheelchair after it mysteriously disappeared.  I would do my best to come to Dallas 2 weekends a month. It never failed that when I would arrive on a Friday or Saturday morning that I would find my dad with food all over his face and clothing. My dad could not cut up food or feed himself at this time and there were instructions for staff to assist with this. When I would ask staff about this, they would get short and defensive with me and tell me they did not have the time. Even more disturbing than the food was the fact that my dad was often soaked in urine and occasionally soiled with feces. My dad was still lucid at this point and he was very embarrassed about this. I can only speculate that my dad was in similar conditions on the mornings that I was not there.  My dad was moved to this floor because staff had determined that he required 2 staff members to assist him, and that was not a "service" available through the Rehab To Home program. Good luck finding 2 staff members on the 3rd floor who were not only available but also willing to help. I would track someone down and either get told, I'll be there soon (and not see this person again), or I'd be met with attitude about how busy they were, my dad's room wasn't in their section etc. One aide practically yelled her schedule in my face. I would offer to help and sometimes that would result in him getting cleaned up sooner but then there was no reason for him to be on this floor if the 2-person requirement couldn't be met with current staffing. It was $6000/month for this "service." Do your due diligence now, folks. If you have an aging parent or family member, start looking at facilities now. Have a list of questions. What is the turnover rate? How many staff are there during busy times such as meal services? Is there a daily checklist for each resident? Who maintains that checklist and ensures that each item is carried out? What accommodations do they provide and how does the facility ensure that these accommodations are implemented?  Ask for recommendations from trusted friends and family. Do not just take your loved one to the first facility that a hospital or doctor recommends. We were lucky that my mom could sit with my dad a lot of the day to ensure he received the care he needed. Not all residents were that fortunate.  There were some very kind staff who worked at Walnut Place, and I always made sure they knew how much we appreciated them. The arts & crafts ladies were so sweet and you could tell they loved their jobs. The front desk staff, the physical therapists, and the doctors were all knowledgeable and helpful. But the aides here, who are the backbone of this facility (and this industry, in general), were overall unreliable, unpleasant & not helpful. If they are indifferent and/or negligent, it is your loved ones who will suffer as a result. read more

My mother-in-law has just been transferred to Walnut Place from another Texas facility. So far, interactions with the Walnut Place staff from practitioners to receptionists has been extremely positive. A recent incident occurred in which staff (Ms. Alex, Ms. Esmeralda, and Mr. Justin) went above and beyond their job description to help resolve the issue. Care quality for your mom, dad, etc., will suffer. This place is unacceptable when it comes to caring for your loved one. starting from the professionals at the office to the nursing care is very poor! The nurses do not do their jobs, they are annoyed if you question the care your loved one is receiving, their head down at the nurses desk so you do not bother them. Your hope for your loved one is to get better when you place them at these facilities, instead they suffer and do not improve. Horrible!!! read more

Friends mothers is in Asst Living. Most of the time there's only one person on the whole floor. His mother has fallen several times and lay on the floor for two hours before anyone knew she has fallen. They mean well but there is not nearly enough help. read more

My sister-in-law has stayed at Walnut Place (3 South wing) for the past five years. We could not have been more pleased. The nurses and other staff members were attentive, kind, knowledgeable, and timely. Safety measures were enforced, and staff communicated promptly with family members if any concerns arose. There is a good variety of delicious meals, and the food is served promptly. There was also a nice variety of interesting activities (kudos to the activities director!), and a family feel there. I always felt welcome to ask questions or express concerns, and these were answered promptly. Walnut Place is located directly across the street from Presbyterian Hospital, so appointments with doctors have been very convenient. And although there was never a need for an emergency visit for us, we knew help was available in minutes if needed. The nurses were mostly very good, the food was cold and awful (to me), the bed was so uncomfortable I could not move, or pull up as in other hospital beds. The floor would have been about the same, really. They got ripped off on their beds, IMHO. I was left for 2 hours on several occasions with my call button on. I think Covid has hit this place, as it has this world, and they are understaffed, over worked, cannot be attentive. Several temp aides, who just came for the $200 bonus to show up. It is operating on a shoestring and that is apparent. I did not sleep the entire time I was there, I never saw my "doctor", just a quick Face time. Once in 8 days, just FaceTime, that is it The air is either freezing or so stifling hot, so I was dripping with sweat. I have been in 4 other rehab facilities in the last 7 years, and this was by far the worst, in terms of comfort, support in every way, the physical therapy is fine, I am sure, they are all pretty much the same. Bottom line, I was miserable, felt completely unsupported by staff. If you have other options, I recommend anywhere but here. Think a long time before placing your loved one at Walnut Place on Glen Lakes in Dallas. The staff is completely inattentive and unavailable. They do not answer patient call buttons. They do not answer calls from the family. They do not attend to the patient. My mother was left in her bed for 7 weeks without exercise or movement. She is now atrophied and unable to do anything for herself. It is criminal. read more

This is my second time to have interaction with Walnut Place as a rehab facility for an older patient . Both of the times the team of Doctors and Medical staff have all been really attentive, caring and supportive along with great care of the patient. Angela has been extremely helpful in keeping me informed of deadlines and updates on the patient. We have had and continue to have a great experience with this facility. read more

VERY Disappointed with this facility and rushed to get my father out in less than 24 hours from admittance. There isn't a receptionist to answer the phones after 7:00 pm so we literally called for 32 minutes and no one ever answered the phone. We had to drive to the facility to check on my dad as he called in distress. The nurse patient ratio is extremely POOR , no matter what they tell you. We asked for a bedside commode NUMEROUS times and was told he needs to push the button for help.... But that goes unanswered as well. We took a tour prior to admission but the "show room" is probably the only clean room in the building.. I have pics to support this. The odor is stale, patient ice is retrieved from a public igloo type cooler and they fill your glass from the tap water.... PAM rehab in Allen and Richardson is the absolute BEST and they got my dad OUT of Walnut and into their facility in record time on the Saturday of the Labor Day holiday weekend. read more

A resident for 13 1/2 years, my mother thought of Walnut Place as her home and the staff as her family. Many Walnut Place employees -- in particular the nursing, activities and physical therapy staff -- went above and beyond to make sure my mother felt safe and well cared for. The clinical staff seem great however... The social workers seem to hate their job and exude pure apathy. The billing office seems to have a theme here. Unscrupulous billing practices mixed with disrespectful customer service. I dealt with an extremely unprofessional woman today and plan to visit with her supervisor. This facility looks nice and presents well and honestly the doctors attending and nursing staff were above average. The front office business and billing staff are unfortunately unprofessional and displayed the most unscrupulous and questionable billing I have seen in three years of health care services for my father-in-law. Even going so far as to threaten collection against others in the family without legal grounds. It took involvement of our family lawyer to resolve the situation. When loved ones are in care situations this type of business practice is unacceptable. This is unfortunate because we so so many positives in this place in the nursing staff levels. read more

Walnut Place is by far one of the finest facilities for skilled nursing around. My father has been placed here for wound treatment and rehabilitation. The staff far exceeds anything I could imagine. They absolutely believe in making you feel like part of the family. They aren't just treating patients, they are caring for you like you are apart of their family. I want to shout out to the Walnut Place Family that has been taking care of my father!! Sherrell you're awesome! Mia we all love you! My elderly cousin has had the misfortune to need a skilled nursing facility several times in the past years. We were very lucky to find Walnut Place. The staff , especially the Physical Therapy department are really professional. The caregivers have often been there a very long time which is a good sign. They actually show intelligence and competence as well as compassion. That is unlike a couple of other places which were newer and more beautiful but the care was incompetent and inadequate. The facility is not as new as some but still very comfortable. Thank goodness we were able to get a spot at Walnut Place. read more

My stay at Walnut Place has far exceeded my expectations. The new management and ownership have certainly made great strides in training their clinical staff, and administrators. The nurses and aides have been doing very well, and easy to communicate with if there is a problem. Renovation of the facility is taking place now with new carpet, paint, and wall covering. Therapy has been done every week day and also on Sunday. My PT is Tif and my OT is Neha. They consistently work with me and take time to understand the pain while providing the push to improve my agility. The staff at Walnut Place stop by frequently to see if there is anything I need. My follow up appointment with my surgeon was done on time and very efficient. I am in a wheelchair with a full leg extension. They had their own shuttle van driver deliver me directly into my doctors office. They followed up with my return to my room. I would recommend giving this facility a tour if you are needing skilled nursing and rehab. read more

The medical staff and therapists at Walnut Place are great at their job. However, the billing practices are not professional. They have threatened to put my wheelchair bound daughter out because her insurance has ran out. They offer no assistance to help you with your SSD and case workers doesn't seem care about your needs behind if you can pay or not. Management needs to retrain the non medical staff or the state will close them down read more

When my mother was here in 2010 for a 2 month rehab stay, it seemed mostly okay. It was clean and the staff was friendly and helpful. The food was bad then - pretty sure a lot of it was institutional frozen entrees. Nothing really bad happened. But when she went back for another rehab stay in 2012, it was a different story. Still friendly and helpful administration. But a night shift aide took some serious shortcuts with transferring to the bathroom, and my mother ended up with a significant cut. They used a piece of equipment that was wrong and dangerous, instead of taking the time to go get the right one. read more

Everyone from their friendly admissions team to excellent nursing and therapy staff made sure that all needs were taken care of immediately. It was very clean, the food was good. We were on the Rehab floor and the nurses were very attentive.

My mother lived at Walnut Place for a year and four months before passing away in April. Her care and attention to her obvious health problems before having to be transferred to the hospital were severely lacking. The wait for a doctor to see her to recommend she be transferred to the hospital took five hours. My mother passed away two days after reaching the hospital. I could have forgiven the slow response, but after she passed, I never heard another word from anyone at Walnut Place. No phone call, no condolences or even a sorry for your loss. It was as if my mom had never even been there. It hurt me so much that I called the administrator to voice my concerns and even after that, I never heard another word from Walnut Place. I wouldn't recommend this place to anyone looking for a place for their loved ones. read more
