On many occasions I have requested very complex alterations and have without exception found the service considerate, diligent and of the highest quality. That might seem pricey to people that can do it themselves, but if you don't want your garment to look like a dog's breakfast (I am referring to my own button sewing abilities here) then it's not too much to baulk at.

If you want a shirt in your size, with your preference of cuff then there is a very limited range. Service. Initial service when buying shorts was from a member of staff who had little knowledge of their product. Their assistance was limited to occasionally hovering and helping us pay for the shirt I chose. When I returned the shirt (due to it having a mark in the centre of the back) the first person I talked to wanted to help until they found it was a return, once they had verified that it was indeed a valid return they handed the shirt back to me and wandered off without another word. The second person I spoke with apparently didn't know how to help. I ended up finding a replacement myself, waiting in line for the store manager and then finally doing a switch. My initial view dealing with this company was to have a quick look when they get new shirts/ties in to see if there is something interesting.
