    I am here to get rehab therapy for my broken left hip. It's a hairline fracture in the femoral…read moreball. This is not the rehab center that I originally chose! However, I am extremely happy that I am here! The whole staff looks like they are extremely competent and caring. The facility is very nice, clean, and well equipped with all the equipment that they need! They have many activities that all are allowed to participate in. I would definitely recommend this facility if you need in-house rehab!!

    South Port Square / Harbor Health promises a "seamless transition" to the next level of care. Their…read moremarketing claims "when there is a change in health, the resident can directly enter a care level that matches their needs". Don't believe this. My mom was in Assisted Living at Harbor Health when she suffered a UTI that caused dementia to take over her mind. It was clear she needed the supervision and locked doors that came in the Memory Care unit, but there were no apartments available. If you think South Port Square has a plan to deal with this situation, you'd be wrong. My mom was forced to stay in her Assisted Living apartment. She was required to pay a home health agency (owned by South Port Square) for 1-on-1 supervision at $33 per hour. Their instructions were to keep her in her room for her "health and safety". Administrators at Harbor Health then increased the amount she paid monthly for her apartment by $900 because her dementia was such that she needed more help and made their jobs more difficult! Mom's move to Memory Care didn't happen within days or even weeks.....she remained in Assisted Living for 28 days! The cost to her for the 1-on-1 supervision they required exceeded $10,000. During the month she waited for an apartment she was isolated, confused and depressed. Her hallucinations and delusions were at their worst, and she threatened to kill herself....that's how sick her mind had become. The help and counseling she desperately needed in the dementia unit was just down the hall from her Assisted Living apartment but was not available to her. Mom was so bad by the time she was able to move into Memory Card that she was admitted to Hospice and passed after 4 short weeks.

    Cannot say enough good things about this facility…read more I contacted Pam at Grand Care Assisted Living when I was searching for a location for an elderly relative to live. He had been in a skilled nursing facility previously, but was receiving sub-par care there. Pam was quite helpful; although this location is listed as "assisted living" rather than "skilled nursing," Pam questioned whether this relative really needed skilled nursing. After speaking with her for a bit, I realized she was right. This relative simply needed a change of scenery, a smaller facility in which to live, and one that would keep an eye on him. We made arrangements (along with my brother) to have Pam evaluate our gentleman relative at his then-current location and scheduled a visit to Grand Care while I was in town. Immediately impressed with the actual location: easy to access, one-story, small, pastel-painted and beautifully decorated rooms, multiple amenities and staff that has worked here for years. Cost quoted was quite reasonable. We made arrangements for our elderly relative to move here as soon as it could be arranged. The improvement in this relative's demeanor, health and outlook on life skyrocketed. Where he had been withdrawn, irritable, refusing to eat and in pain (he had multiple falls in the skilled nursing facility), within weeks we saw an extremely positive change. He's eating, socializing, exercising, moving more freely, no longer in pain and has developed multiple friendships here (both among the staff and other residents). Absolutely heartwarming for us to see such a marked improvement. This is a small facility. It is more of a home-like setting rather than an institution. Staff is wonderful. Pam remains a great communicator and motivator. Her handling of the residents in the aftermath of Hurricane Ian was stellar. I've interviewed and seen my share of home healthcare and assisted living facilities. Some are downright despicable, presenting either sanitized living space and/or caregivers that demonstrate little compassion. Grand Care Assisted Living is, in my opinion, top notch and probably one of the best locations for assisted living in the area. Highly recommended, as the the care that my elderly relative is receiving is nothing short of remarkable, and the transformation in his outlook, health and attitude has been extraordinary.

    I want to acknowledge that we've had numerous positive experiences at The Meridian, particularly…read morewith the memory care unit, where my mother constantly expresses her love for the care and environment. It is crucial to ensure the safety and well-being of all residents and staff, and sometimes that requires difficult decisions regarding individuals who may pose a risk to others. With over 32 years in the healthcare field, and leading large Emergency Rooms, I understand the complexities involved in caring for individuals with challenging behaviors. There are compliance regulations mandated by AHCA (Agency for Healthcare Administration), Florida's regulatory body for assuring the safety of patients and residents in all facilities who care for patients/residents. I encourage anyone reading this gentleman's post to not let negative experiences by one individual overshadow the positive aspects of The Meridian. If a resident harms another resident or healthcare team member, they must take action and assure this resident receives age specific care they need in facility capable of handling the aggressive/impulsive behaviors. I'm thankful the Meridian team took action in this case to keep my mother and the other residents safe. While they strive to create a nurturing environment for all families, they are always looking for ways to keep the memory care residents engaged and safe; go see for yourself, you won't be disappointed.

Administration disagrees and our family had an incident (s) as a result . The police called us to tell us of their concerns regarding neglect of my dad . I can't warn you more strongly . STAY CLEAR. If you have a parent that needs memory care, there are better facilities that do staff better and do care .

    My parents were at this facility for 3.5 years . Unfortunately, this facility has declined…read moresteadily in several areas. The food quality, variety and service has dropped precipitously. The general staff tries hard but as they appear understaffed , they seem to have significant turnover and in my opinion the rank and file employees morale has declined . They have had significant turnover in upper level day to day management , the current management staff seems destined to perpetuate the downward trend . The nursing coordinator struck me (I happen to be a physician ) , as woefully out of date on basic knowledge and in fact not only wrong but far worse headstrong and unyielding . They gave us mixed messages as to my mother's return after an extended hospitalization and resulted in great distress to my 86yo mom and significant logistical problems for us as a family . I found them callus as to the gravity of the problems they created for us but much more importantly for the emotional state of my mother . I would not recommend this facility in its current state , we have moved my mother to another facility and she is very happy and often remarks on how much better the food and cleanliness is at the current facility.
