Prices kept going up while food and other services got increasingly worse. No one came to fix things wrong at his home including flooding in the house every time it rained. We lost my dad in Jan. of 26 due to illness. After being at St. Francis, Covenant told us they would take my dad for rehab. They even sent me a confirmation and room number. The next day they backed out because a higher up told the social worker they didn't like my dad and voided it all. This was done to my dad as a member of their "community" out of spite. Since there were no other options for rehab, we lost him a few weeks later. Please find another facility for your loved one or yourself, don't be fooled by how this place looks on the outside.

UPDATE: We just received their letter indicating the…read moreoutcome of their 'so-called" investigation regarding my husband's lack of care in their facility. NOTE: I was there at their facility visiting my husband every day, sometimes, twice a day, for a considerable length of time. So, I DID see what was going on. 1. Their claim: "They went in to bathe him four times, but he refused two times." I KNOW this is false because I asked my husband and staff at the time about cleaning him, clean bed linens, and a clean hospital gown. He was bathed only twice in 7 days. (at the time I wrote my review, he had been bathed only once and the sheets had not been changed nor was his hospital gown changed, but because I complained, they finally gave him a sponge bath and changed his gown and the sheets at 6:30 am the next morning). 2. Their claim: "Our investigation revealed that the required therapy minutes by the (missing word?) provided by all disciplines." The following was his schedule: (I have the original schedules of every day except Monday, Tuesday, Saturday and Sunday of days where no sessions were given. Nor do I have a Monday schedule, because it was not planned for his day of release) a. Monday & Tuesday: no sessions b. Wednesday: one therapy session c. Thursday: four sessions (right shoulder was dislocated) d. Friday: One OT session and one PT session & social/exercise group e. Saturday & Sunday: No sessions f. Monday: One PT session g. Tuesday: One session before releasing him If the staff believes that the above scheduling is advantageous, perhaps they might consider having outside professionals experienced in physical therapy advise their staff members. Please realize that if the staff is questioned, they are going to be on the defensive and perhaps bend the 'real truth' Although I have a doctorate, two counseling degrees, and a degree in psychology, it doesn't take ANY degree to figure out that defensiveness, being deceptive, and perhaps changing records is only human nature, when their jobs are on the line. I've even suggested that they consider having someone who is not recognized by the staff do an incognito investigation as to what is really going on here.... with no success. In summary, because my husband was not allowed to leave his bed (loud alarm would go off due to his high risk of falling), he was worse when I took him home than he was when he came in. Therefore, it appears to me that Ms. Harding, Director of Quality Management and Ms. Walker, Chief Executive Officer did not take my concerns seriously but only did CYA form of gestures that were required to keep 'their' jobs.

I just want to start out saying that I have a degree in political science, while studying at OU in…read morepursuit of a second degree in sociology with a niche in criminology. My child attends this place and has been going on two years, give or take. I have had several instances of occurrences with this company regarding communication and covert discrimination. My child's mom and I have a 50/50 custody arrangement through the district court, and he attends on days that I do not have him in my care/custody. To balance this, I requested that I receive updates regarding treatment and his behavioral progress, with emphasis on any negative or new behaviors. I have been in direct contact with the director of this place regarding these issues/concerns of mine and have been lulled in to thinking we were on the same page. Just recently (last week), I heard about him urinating on one of their walls of the facility. How did I find this out? Through my son (9 with developmental disabilities) and my daughter (7). I didn't receive any calls messages from their mother, but more importantly I didn't receive any kind of calls or messages from the facility. I called today and asked to speak with the director, asking questions about what transpired and why I wasn't reached. She told me she didn't have the full details and that that's not how they do things at that facility. This contrasts the communication between us previously which she explicitly let it be known that Forever Learning would be transparent with me about my sons treatment regarding any negative or new behavioral occurrences while he attends treatment. My child's mother whipped my son, whom is diagnosed with 3 types of neurodivergent disabilities (autism, adhd, and moderate intellectual disability), for the urination incident he had while attending there. I let it be known that the way he is and has been disciplined by his mother is causing outbursts of benign behavior and overstimulation, and I was interrupted by the director telling me to take that up with his mother. This was also the first time I was told that he has been having behavioral issues daily. After being told by the director "that's not the way we do things here", regarding regular updates on my sons treatment while there, she subsequently followed up with that if I didn't agree with the way things were that I could "take him out the program". Again, this contrasts what we have discussed through email and over the phone regarding his treatment. When I mentioned the previous communication, I was met with a dismissive discourse, a lack of understanding ethics policy as defined by the BACB, and accountability. I told them I would be reporting the hostility to the BACB, and here on google. This is not the first occurrence as it is the final one of several, including a hostile email I received from their receptionist regarding attendance. I have been extremely flexible with them because it is a small black owned business, but that has only led to an increase in hostility towards my inquiries, a lack of respect, and a diminished sense of trust. The smiles and gentle disposition quickly faded when I started asking questions, then I was told I could speak with their lawyer about it. So that's what I intend on doing after filing with the BACB and any other behavioral business association I can find to hold them accountable. PS: If you value accountability and transparency, I would highly recommend you find somewhere with a little more history underneath their belt as a facility treating children with various types of neurodivergence. I know finding a reasonable place is challenging but there are some out there.This is not about grown adults, it's about the well being of our children and I take that very seriously. Shertaurus, I am deeply disappointed in the stance you took with me on the phone today and hope you don't come across a person with the resources to legally hold you to account. I'm sure you don't treat everyone that way, more importantly the mothers that trust you and your staff to uphold the values and ethics that come with the position you signed up for and are contractually obligated to. I truly believe you undermine my presence because I'm an educated African american male and you favor my son's mother because she isn't as inquisitive. We've had some really decent conversations as two educated parents of children with developmental disabilities, and as a father, I am truly disappointed this is what it had to lead too.

How ever, I wanted to Thank each of you for what you did, I know it's your job but I also know its more than that, just what little time I had to play nurse/caregiver, I seen there is absolutely no way to do your job and not have heart and connection, no way! It's a hard job mentally and physically and definitely emotionally, you've got to be one extremely strong human being to do what you all do, and still go home to your families and home life and sleep. I just want to Thank Tammy so very much from the bottom of my heart Thank You for staying with me and Thank You for helping me afterwards with what to do and who to call to come get my mother as I was completely lost and had no idea. Another time he was in bed all day and no one checked on him. He was always up by 5am or 6am and ready for the day. Why no one checked on him all day is beyond me. The food was not good at all and the residents had to eat with cheap plastic ware. We finally started taking his meals to him. We moved him to another true assisted living facility where they are extremely caring and helpful and he loves it there. Very disappointed in the care at Convenant, we feel that sales person sold us on care we did not receive. Unless your loved one is more on the independent side I cannot recommend this place.
