My mom, who requires full-time skilled nursing and is fully dependent, had a crucial 9 a.m. hospital appointment. I arrived at 8:25 (as I always ride along to advocate for her), and the pickup was set for 8:30, but she was still in bed. Due to short staffing--only one inexperienced CNA for a Hoyer lift--I took action. Being fit, I lifted my mom myself, leaving at 8:50, and we arrived only 10 minutes late. I did not put anyone at risk--I'm fully capable. Yet Steve accused me of jeopardizing jobs. I'm not trying to get staff in trouble--I hold management accountable for this recurring pattern of short staffing. Hoyer lift patients require more support. I urge management to ensure proper staffing so families like mine aren't forced into this again.

No matter which nursing facility you place your family member.. if you can, please make sure to visit ..and visit often. Not one nursing facility is going to be everything you are looking for ...budget cuts/lack of staff/difficulty hiring and retaining staff unfortunately is affecting many nursing facilities. It would be naive to think one facility must be a good place due to a trusted friend or neighbor saying they had a great experience in said facility. Too many variables not mentioned that affect one's perception of care that need to be considered. Is your loved one mobile or fully dependent? Is your loved one able to communicate their needs? Is your loved one going to the facility for temporary after-care or for long-term care? Seen this facility (6840 W. Touhy Niles branch) through its transitions from when it was known as Forest Villa to Avanti Nursing Home and now Elevate Care. Do not let photos and the main floor deceive you as it does not speak of the entire facility. Offices on main floor seem nicely updated while the residents' rooms are left needing work. The first floor may seem somewhat welcoming and is mainly for their temporary residents which is where my dad first started. The rehabilitation/physical therapy room is on the first floor and if you are lucky to come here for that and go back home, great. The physical therapy staff were the highlight of my dad's days. Unfortunately, my dad's condition progressively worsened and he was moved up to the second floor which is where they place their long- term residents. This place as my dad described it was literal hell on earth. Sadly, after scouring every review and driving from one facility to the next searching for a place for him we quickly realized there is no such thing as a good nursing facility...We ruled out the nursing facilities that reeked of urine at first entrance and landed on this option as one of the "better" ones and chose to visit as often as we could. I cannot stress enough the importance of doing so. There just is not enough staff to provide the type of care you will want for your loved one...PERIOD. Add to that if your family member has a severe physical disability or is completely immobile and needs assistance feeding, brushing teeth...to even hope for any sort of consistency in that regard or think that any complaints will be rectified...serious waste of time and energy. Cannot say how painful it was to even have to wonder if your loved one has eaten or not. Trays left untouched, water cups left out of reach, call light strings left out of reach...for someone with limited mobility, 100% know that there will be neglect. As our dad declined, this place simply was not able to meet his care. His teeth went unbrushed, lack of care and being diabetic...literal teeth lost in matter of months. Whether he ate or not depended on if a kind soul paid enough attention to notice he wasn't able to reach his food or water, let alone help himself due to his limited mobility. Nearly full shifts w/ not one staff changing him out of his soiled clothes/bed until we would come and ask for it...even then call light response was horrendous. Peeking out to see if there were staff nearby to help...either no one or if there were I would often see them on their phones or talking amongst themselves while call lights were left unattended and often hearing residents yelling "help, help!"...Countless concerns from both my dad and family dismissed...Responses to rectify but no follow through...The feeling of defeat, hopelessness and frustration and knowing my dad had to live this daily...Eventually my dad ended up receiving hospice while here and even with the hospice nurse coming in weekly we did not stop our multiple days/week visits due to the neglect here. There are some kind souls working here but kindness simply does not fill the need for additional staff...with that said there are also many working here who are completely apathetic. It would be great if this facility had some time dedicated to educating their staff on elder abuse as some may not even realize that their dismissals is a form of abuse...compassionate elder care and how their small acts can really make such a huge difference. If the people running this place experienced even ONE DAY of their loved one being here or if they found themselves suddenly immobile and dependent on care, I question if that would bring about any change? Sadly the problem here is too great that I don't believe the situation here is going to get any better. My wish is this review will help someone going through the difficult situation of placing their loved one in a facility and for whoever reading this review that they do not have to go through what my dad did. May he now rest in peace.

The day we were to…read morediscuss assisted living with her doctor, I found myself calling 911 and sending her to the hospital for what would be 51 days, sepsis, 2 surgeries & 2 rehab stints. Having her return home was no longer an option; which led me to quickly get to up to speed on senior living options,mostly on my own. I found her a place & moved her in all before her release. While I ensured her safety, I second guessed my decisions as she vocally struggled to adjust; it was a lot of change particularly for someone who rarely left her home. Almost 6 months in, a different set of doubts & worries took over. Did I make the right call, could she live somewhere more affordable, would she outlive her money, etc. I don't even recall what search term led me to Senior Living Experts, but I'm grateful. Rebecca listened to my story and made me feel less alone. She thought where my mom was at was the best option in terms of cost, desired location and care needs addressed, but would do the research and circle back. We also discussed the care she received currently and provided a few suggestions. As promised, Rebecca followed up and confirmed her initial assessment. Yes, I'll still worry about her finances, but I now have a better understanding of the landscape and what comes should her care needs increase. If I had a do over, I would have started with Senior Living Experts, they are amazing!

The facilities were clean and easy to find parking at. There is a ton of extracurricular activities for the guests and residents too, such as gardening, painting, and movie nights. The food was standard, but there are also separate restaurants that offer food that's a little more high end. The only reason I gave 4 instead of 5 stars were because the rehabilitation bedrooms are a little dark and drab. And we also ran into one transport issue trying to bring my mom to Presence for a doctors appointment. She was late getting to the hospital and the nursing home claimed she was stuck in traffic. However, because I track my mother, I knew she had yet to leave the nursing home. I was disappointed that they lied instead of just saying that they forgot about her. Other than that, I would recommend touring the facility yourself to see if it's the right fit for your elder. In comparison to other nearby facilities, we were pleased and would send our parents back there for a stay if need be without hesitation.

They brought all new staff in and let go of some really great people without even knowing what they did. They let go of all agency people to save money yet that left them so short on staff in every area, this should have been phased out instead. Prices are rising yet quality is falling. Everybody in the neighborhood has known somebody that has been here and has always recommended the place but not anymore.
