    My Dad was a short-term stay resident in the rehab facility after a hospital stay. As soon as he…read morearrived there, I was received an email and a phone call to discuss his needs. The email had phone numbers for the front desk, the nurses station, and his social worker. The nurse gave me her cell phone number right away and was texting with me through the first day and evening to be sure everything was set up and to reassure a worried daughter. Everyone I came into contact with at Park Place was kind, helpful and caring. The daily COVID email updates as well as follow up phone calls regarding any cases were very appreciated. All safety protocols were followed and I felt confident that Park Place was doing their best to keep all of the residents safe and healthy. Dad was well taken care of and he was always very happy on the phone and during an outside visit. His therapy went very well and I was informed about his progress weekly. Everyone, including the director, nurse and social worker were very prompt to reply to any email as well as any voicemail. Upon his discharge, I was given detailed discharge information as well as all necessary medications and prescriptions. Park Place also calls the caregiver for a few days after discharge to check on his progress at his new living arrangement. I highly recommend Park Place.

Based on…read morewhat I observed during my loved one's stay, I believe the facility falls far short of the standard of care and professionalism that its name suggests. On the very first day of admission, there was a cloth chair in my loved one's room that appeared to have dried human waste on the armrest. I immediately brought this to the attention of the staff. After investigating, I was assured that the chair would be removed right away. However, three days later, the chair remained in the room. Throughout the stay, the level of care, responsiveness, and bedside manner displayed by many of the nursing and CNA staff was unacceptable. The facility appeared to be understaffed, and there was a noticeable lack of urgency when patients required assistance. On numerous occasions, we used the call button to request help for my loved one. Not once did a nurse or CNA respond to the call button in a timely manner. Each time, a family member had to physically walk to the nurses' station to request assistance before anyone responded. My loved one shared a room with another patient, and I frequently overheard that patient calling out for help. She could often be heard moaning, groaning, and asking for assistance for extended periods. When staff eventually entered the room, I was shocked to hear them scold the patient, telling her that she was "always calling" and that she "didn't want anything." Witnessing this interaction was deeply troubling and demonstrated a lack of compassion and respect for a vulnerable resident. Another concern was the apparent lack of engagement with residents. Many elderly patients spent entire days sitting near the nurses' station with little to no interaction from staff. They appeared isolated, disengaged, and unhappy. The most disturbing issue occurred when I noticed what initially appeared to be clumps of dirt or lint along the wall behind my loved one's bed. Upon closer inspection, I discovered that the area was covered with hundreds, if not thousands, of ants crawling along the wall and throughout the room. I immediately reported the issue to staff. One CNA responded by saying, "Well, I just got here." A nurse then acknowledged that she had already informed the day aide about the ant problem. This indicated that staff were already aware of the infestation, yet no action had been taken to resolve it or relocate the patient. When I expressed my concern and disgust regarding the situation, the nurse rolled her eyes and told me that I should contact maintenance about the ants. I found this response unprofessional and inappropriate, especially given that the issue involved patient health, safety, and sanitation. My final negative interaction occurred during the discharge process. The nurse manager reviewed my loved one's discharge instructions and medications. When I asked questions regarding follow-up medications, I was met with visible irritation and an unwelcoming attitude, as though asking questions about my loved one's care was an inconvenience. I strongly believe the facility should reevaluate its hiring practices, staff training, accountability measures, cleanliness standards, and responsiveness to patient concerns. Patients and families deserve compassionate care, professional communication, and a clean, safe environment. The one positive aspect of our experience was the therapy department. I did observe meaningful improvement in my loved one's therapy progress, and the therapy staff appeared dedicated to their work. They were the bright spot in an otherwise disappointing experience. I was relieved when my loved one was discharged from Ignite Medical Resort Dyer. Based on my experience, I would not choose this facility again. I encourage families with loved ones in nursing homes or rehabilitation facilities to visit frequently and remain actively involved in their care. This place gave a very convincing tour…read moreof a well ran facility full of light, love & activities to create a home like environment. None of which was true. Ice cream parlor, hair salon, movie theater.... In 2 months those doors were NEVER open until the day we were removing our loved one. Convenient right? The amount of neglect, mental/physical abuse & disregard for the well being of the elderly they are employed to protect is despicable. The nursing staff was unorganized & unprofessional. The administration & leadership staff was unhelpful & dishonest. And genuinely behaved bothered & irritated when asked for assistance. Our family member went in for small fixable issues & came out no longer able to walk, only using diapers, dehydrated, bed sores & a full body rash.... All of which were NEVER an issue before. Left to sit in their own waste for an obscene amount of hours to the point of straining a large area of the skin, cold showers (rare if at all) & sanitation of the room was very poor. IF the necessary medicine/treatment was being given is unsure, but doubtful given the declined condition of health upon leaving this facility. Director & social workers close their office doors on you once they realize you are not going to tolerate their BS. A member of the admissions staff even said.. and I quote "I wouldn't even put my own mother here & I only work here cuz they offered me more money." THAT right there should of been enough of a red flag. Day of departure... Staff members stood nearby gawking as we struggled to help our family member into the car. We asked for help & got told they were "on break"... But they kept watching. If you love your family members, do not place them here. They may not be so lucky.

In my very own…read moreopinion from my experience this past month, this facility should definitely be evaluated/inspected by state agencies. The care and services I witnessed were horrendous. I am an out of town grandson of a recent resident of this facility. My family member suffers from Alzheimer's and Dementia. She has also had mobility issues and was sent here, recommended by insurance company for REHABILITATION. Rehab DID NOT happen. In fact, my family member worsened while in the care of Dyer Nursing and Rehab. Here is what I witnessed in just a few days: - Poor security. While they did have someone at the front concerned about checking temperatures, this person did not guide guests to the residents room, just pointed in the direction. I wandered the facility looking for my family member's room. Each area had it's own nurses station with plenty of staff around. Not one of them greeted me or even asked what I was doing or who I was there to see. I could have easily wandered into any residents room. Eventually I found my family member. - Horrendous behavior from staff. While I was in the room with my family member I could hear an employee (who was was in scrubs, in the hallway) cursing left and right. "Mother F*** this and that, F*** so and so etc." This was not just one sentence. This was on and on and very loud. I walked to the hallway to see who she was speaking to like that, and it appeared she had an ear piece in and was talking to someone on the phone. Meanwhile, my family member as well as other residents and guest could hear this nasty language clear and loudly from their rooms. This made my family member very anxious. - EVERY time I visited my family member, she was soiled. This told me they were not routinely checking/changing her. - At one point a nurse came in the room and handed my family member a cup of pills and said, "time to take your medicine." Didn't greet me or my family member. I asked a question about my family member's mobility and capabilities as I was visiting and I wanted to know her progress. The nurse responded in an extremely rude manner saying, "Don't you know who she is?" "Don't you know her capablities?!" I replied, "Yes, she is my grandmother, but I am visiting from out of state and want to know her restrictions with mobility." The nurse didn't say a word. Walked to her chart, turned around and said, "SHE'S A ONE ASSIST." I reply, "I'm sorry ma'am, what does that mean?" This aggrevated her and she replied, "IT MEANS SHE NEEDS ONE PERSON TO ASSIST HER." That was the end of that conversation. - While visiting, I heard a resident across the hall yelling, "NURSE... NURSE... I need a bed pan." She yelled this for 20 minutes. Bottom line is the staff does not show their residents basic dignity, care, or respect. - My family member was only here for a few weeks and left in worse shape than she arrived. Bruises all over her body. - I witnessed lunch/dinner being dropped off. It was delivered COLD. Cold meat, cold vegetable and cold coffee. - My family member would be in the same attire for days in a row. She was only changed and cleaned up because I did so. If you have a loved one, please look for other facilities to put them in. From my first hand experience, this place is a MESS! The lack of BASIC care and respect for the patients/residents is mind blowing. Extremely unprofessional. I'm grateful my family member was moved to another facility in hopes for better care. I DO NOT RECOMMEND! I highly encourage county and state agencies to investigate the ongoings of this nursing/rehab operation. - A concerned family member.

    This is not a family-friendly facility. My 14 year old tried to come with me as a guest to try it…read moreout (my 16 year old had come once). They said the 14 year old had to buy a package for kids which was $30 for 10 visits (no smaller packages just to try it out). When deciding on it when she was already with me in her workout clothes, we told the worker that she was going to sometimes come with her friend's family, so we would get it. Nothing was said about any rules against this. A month after that, I decided to cancel my membership because of several reasons (class times not convenient for a teacher. Rigid rules about teen memberships, etc). When I canceled, they called to tell me Tessa's membership would also be canceled since she has to come with me. I told them my understanding was that she just had to be with an adult. They would not budge and told me absolutely no refunds for any reason. This is very poor business. Also, their policies are so rigid...a 14-15 year old must work out with a parent. I don't know many teens that want to be glued to their parent's side when working out. Other gyms have much more flexibility. Other businesses would be more flexible with miscommunications. I'm glad we are ending our membership there.

They used to take your blood pressure by just noticing you sitting in a chair. Today, they're too lazy to get up off their asses! Don't blame Covid, that's any easy excuse and a cop out. No mouthwash in the locker room any longer! We didn't forget! Fitness Pointe did! Poor leadership that would rather blame someone for complaining about a problem person, in hopes one of them would cancel their membership so Mr Coward manager didn't have to take a stand. Missing leadership! Arrogant! Excellent at being pompous and arrogant! Then there's the therapy pool piranha and the school of disgruntled, mean followers that believe this is their own personal warm water swamp. The pool piranha feverishly splashes about from side to side hogging up three lanes is a message to everyone...I OWN THIS POOL! I COME FIRST! You must kiss the ass of the selfish pool piranha and follow it around, back and forth like a good little obedient fan. No respect for others! You can't enjoy the therapy pool alone because the pool piranha lacks attention and needs your constant validation. Then there's the employee with the strawberry blonde brush haircut that hates everyone including itself. Mean, belligerent and can easily grimace, flip you the finger or tell you to SHUT UP! Excuse me....Is one of it's tired, dated lines. When management lacks the balls to use their authority properly, you end up with sacred cow staff members, laziness and a few angered members calling the shots over the majority. Fitness Pointe isn't the best place to work out or enjoy your leisure. DON'T WASTE YOUR TIME HERE! Not worth the aggravation!
