If you are looking for a true rehab center-don't expect to find it here. Therapy is very limited. The workers there claim they are short staffed so it cuts down on the attention you may need-like a simple cup of ice water or help going to the bathroom- the wait is very long-only to find you have messed the bed. then you are given a wet wash rag with no soap & told to clean up. d some nights no wash cloths or towels were available we were told! really?? how would you like ants crawling in youf room-even in your bed! only to be told they would enter a ticket on it. claimed they had no bug spray to use on it. how about waking you at 8:30pm while sleeping- to ask if you want to take a shower??!!! then have trouble getting back to sleep! the food is mostly disgusting -once In awhile something is edible. servings are small & no snacks available. only serve sweet tea- can't get unsweet tea. some nurses are nice but that number is few. So please look elsewhere for a true rehab center. read more

Hello, my name is Troy Tomczak. I recently had the extreme displeasure of visiting , avante at ocala. I was brought in by hospital transportation for rehab ! The entire facility smelled of urine, feces, feer and death! Upon reaching the room tgey attempted to assign to me , there was a large mess on the floor , in the entrance to my room ! One of the staff said. I told them to clean that up earlier, now what does that look like ! Being on a gurney , I couldn't see so I said , tell me what does it look like ? No one answered. My friend later told me there was what appeared to be blood and urine on the floor. The room was about 80 degrees and looked like it was furnished in 1975. Also in my room was what looked like someone in a bed with several blankets that may or may not have been alive. I saw no movement or anything spoken from this victim of avante. Needless to say , my visit was extremely short . On the way out the staff actually tried to bribe me with a hamburger to stay ! I would not bring my goldfish to this facility. worst place ever for care you must be dropped off on a desert island. You have to ask for everything nobody does anything on their own to help you how is that care you have to ask for water you wait you have to ask for C PAP water you have to ask to get washed do you have to ask to get your bed changed? Shouldn't that be part of their job? How are you supposed to sleep in a room with a neighbor that has oxygen machine on 24 seven talks in his sleep and awake burps out loud like he's in a bar wants the window open to make it like a sauna so you'll be sweating. Avante of Ocala met my needs for rehabilitation after acquiring Rhabdomyolyesis, a life-threatening condition that causes the muscles to deteriorate. The staff were phenomenal from direct care to physical and occupational therapist. Along with being patient and caring, the nursing assistant were extremely attentive. I was encouraged by the consistent manner that each staff person demonstrated day after day. The only improvement that I would suggest would be with the Food Services. I would be dishonest if I did by not mention that the culinary offerings were shocking and void of nutritional balance. I did not look forward to the meals until after having a come to accountability meeting with the Patient Advocate. Change! That was what the entire facility needed when it came to food services. Thus is the reason I rate Avante st Ocala 4 out of 5. I"ConsiderIng the rehabilitation I needed, I left Avante with greater strength than when I came and grateful that this facility was my daughter's choice! I rate the Physical & Occupational Therapy department 5/5. If the need ever arise again, I pray that it diesnt, I would definitely make this Avante At Ocala my choice, even being 45 miles away from home. I pray that this first visit will be my last! Hear ! THE WORST PLACE!! If you place your family members here, shame on you. The whole place smells like piss, the floors are constantly sticky, the nurses don't give meds on time or ever, I was able to walk in and walk out with my Uncle and no one stopped me - no one asked who I was - I was shocked. My uncle had extensive neck surgery and he fell in the middle of the night, after pushing the nurse button for 10 minutes with no one coming, he called the main office and THEY called the nurses office and STILL no one came to help him. He had to call a SECOND time and finally somebody came to help him. he was on the floor for 20 minutes. He could have died in his fall and Avante and it's staff wouldn't have given a shit (excuse my language I'm still fuming mad). This place should be shut down. Its a fucking disaster. read more

Very good place for rehab good health care and great administration clean .and good food nice people nice facilities.

This place is horrible! I would not feed their food to my dog! Some nurses were nice, most were rude and very unpleasant. The physical therapist were very nice and helpful. DO NOT GO THERE!! read more

Father was admitted for 23 days. He did not have any family in the state and our only means of communication was reliant on the staff facilitating us talking to him. Yet, it took two weeks for them to even realize both phones in the room didn't work and then another week of calling everyday constantly asking for the phone to be fixed and then Sue the North Wing Nurse supervisor physically had to get the maintenance man to come fix the phone. In weeks of calling multiple times a day to speak to my sick, frail, alone elderly father I got through to him maybe 5 times. On top of that when finally are able we have him transferred to near us he looks like he's been shut away in a prison camp, haggard and unkempt with severely cracked lips and tongue caked in dry skin which was accumulating at the back of his throat. We were told this was due to a basic lack of oral hygiene care. I shudder to think what else they were negligent about but I will never know as my father passed away 4 days after being transferred to a different facility. Shame on me for not checking their Facebook page before my father was transferred there. They have absolutely have no transparency since they do not allow vistors to post to their page or write reviews. The only option available is to recommend them. On top of their disregard for customer feedback the pictures they feature on Facebook are mainly focused on their staff showcasing how they treat them to catered meals, parties, and desserts. With my father looking as he did I am sure he wasn't considered good "Facebook" material. read more

This facility is deplorable. This care is not even sub-standard. I took care of an elderly lady that has thankfully passed away. I was lucky enough to be able to get her out of there and into hospice care at an end-of-life facility. To think of her dying in that place was more than I could stand. Some of the staff was very caring, but she sustained serious injury after serious injury and the last one was not even reported to me. She had bed sores and when she was unable to feed herself, it depended on how much personnel was available if she was fed or not. They are routinely understaffed. The facility is rundown and unclean. Clothing is lost in the laundry and never returned. I made daily visits at many different times during the day and the conditions were the same whenever I visited. Management is surly and uncooperative. If you have any kind of choice when you need nursing home care for yourself your loved one - choose another facility. They show up at the hospital and talk a good game, but they can't deliver the goods. Once you are there, it is almost impossible to get out. read more

The only reason this facility gets a one instead of a zero, is that it looks nice and some of the staff was REALLY helpful. Others, not so much! My mom was admitted to Avante in Ocala, FL for rehab services after major surgery. The staff was nice and helpful at that time. Mom was so sick with acid reflux so bad that she was gagging and vomiting. Upon arriving they had to process her in I guess, therefore she could not get her needed med's, for which she has an RX. Mom had a really miserable night. She began having an anxiety attack and it took literally hours and practically an act of congress to get her a dose of her anxiety medicine. The room was hot, the doors were left open and and hallway was loud and bright; it goes on and on and on. SO, the next a.m. when i arrived she was almost in tears, saying she was not staying there an night,; and began to tell me some of her issues. I did not learn of all of them until later after we got home. When I notifified the floor nurse that we were taking her home, she said Medicare would not pay for her stay there, her hospital stay or for any PT and nursing care as she was leaving AMA (against medical advice) I asked for a supervisor, who came into room later and told mom, myself and my husband that Medicare absolutely would NOT pay for her to have any at home or outpatient PT. This was shocking and surprising, and mom was so upset she said she didn't even care, she just wanted out of that place. To make a long story longer, medicare DOES in fact pay for any services her Dr. orders for her, and this facility supervisor is either grossly misinformed or just flat out lied. We are highly upset at the level of stress this facility caused my 86 YO mother. Would never recommend to anyone. Jo B. read more

I was very impressed with the facility when we went here on tour. Everything was new and clean and I was assured by the tour director that they could accommodate someone with dementia. I was not happy with this facility once my mom was a patient for short term rehabilitation after major surgery. She was not supervised as well as someone with her level of dementia needed. Her room was placed far from the nursing station for the first couple of days until I requested that it be moved closer. The physical therapy department would not allow my mom to use a walker (this has been her mode of transportation) so they placed her in a wheelchair. Since she was not familiar with the wheelchair, the first night she was there, her roommate woke to see her walking out into the hallway and had to call for help. the food served was a 'regular' diet when we emphasized that my mom needed a wheat-free, diabetic diet. The CNA's, PT's, OT's and Speech therapists who worked with my mom were very good. I was impressed with the rehab program but the supervision for my mom's level of dementia just wasn't there. We discharged my mom on the 5th day. read more

My father is receiving therapy at Avante, he has been in and out of many hospitals and says this one is the best. The food is good, your able to choose from the menu and service is great. My dad says they go out of their way to make your stay comfortable, anytime he asked for help they were there. The physical therapy is great and they answer all your questions and last but not least the facility is very clean. read more
