My beloved 97-year-old Papa has been a guest at Aviara for the past month, and likely will remain for some time. Our family chose Aviara among several long-term care facilities in San Diego County upon serious consideration, but I hope that my testimonial will highlight Aviara's amazing support and services to its clients. First and foremost, Aviara involves my family in Papa's overall care; WE are part of Team Aviara! I was with Papa 24/7, and got to witness the sensitive care from Team Aviara (around the clock) that validated and accommodated his physical and mental challenges. The entire team at Aviara - from the front desk, to the fabulous nurses and nurse assistants, to the food service personnel, and all of the therapy professionals, among others - really care about my Papa; and listen closely to my family when we offer suggestions to accommodate and assist him. When Papa had a medical emergency last week, the Aviara Administrator and an entire team responded to him within 30 seconds, and did everything needed to assure that he would be okay. The facility is very lovely and clean, with beautiful outdoor spaces for their clients to enjoy. Team Aviara keeps re-arranging Papa's room to make it easier for him to call for assistance, watch TV, make phone calls, AND arrange for family visits that involve many of us. My family gives Aviara a 10+ rating. While I hate that Papa is not home, I know he is safe, sound, and well cared for at Aviara. read more

BEWARE OF AVIARA HEALTH CARE: ENCINTIAS. With great heartbreak and sorrow, I share this story in the hope that no other family will endure what we did. My stepmother was admitted to this facility in 2024 after surviving a hemorrhagic stroke and a severe UTI that led to meningitis at another rehabilitation center. She had fought for months in the ICU, slowly regaining strength, and was finally beginning to eat, talk, and participate in therapy. The insurance we had left us no choice but to send her here for continued rehabilitation. On her second day at this facility, our family was gathered around her bedside -- my dad, my brother, his wife, and me -- celebrating her remarkable progress. Then, without warning, she went into diabetic shock right in front of us. Her eyes turned gray, and she couldn't speak or move. I immediately said, "Something's wrong!" and my dad and I ran down the hallway screaming for help. When the head nurse finally came in, she looked at my stepmother -- who was clearly in distress -- and told us to wait for a doctor and not call 911. Her advice was horrifying and negligent. We ignored her and I called 911 myself. The paramedics arrived quickly, and a male nurse checked her blood sugar -- it was 43! My dad was the one who ran to his own bag, grabbed orange juice, and tried to save her with his own hands. It was a traumatic, devastating experience for all of us. After being rushed back to the ICU, my stepmother never fully recovered. My father eventually brought her home under home care then hospice care, we were her full-time care givers, and she passed away just 5-months later. We filed a formal complaint through Kaiser, but no words can undo what happened that day. I understand that many healthcare workers are overworked and underpaid -- that's a systemic problem -- but failing to give a diabetic patient their medication and then telling the family not to call 911 is inexcusable. Please, do not send your loved ones here. There is no excuse for such negligence, and no family should have to go through what ours did. read more

Terrible terrible terrible!!! Most awful place for nursing home!!! Staff was horrible to my mother. Many many many medicine mix ups. If just relying on staff to be in charge of medicine it could be fatal. Not tending to hygiene needs when needing to be changed, which led to constant UTI'S. She sat in her own filth sometimes for hours! Needing to see her facility doctor and was never there! Very poor care! This place is just horrible! Very noisy. Better make sure patient has noise canceling earphones so they can sleep. Food was always cold and not suitable for people with no teeth. Extremely bad communication. Staff and doctors never returned phone calls. Maybe the rehab side is decent but definitely not nursing care. DO NOT HAVE YOUR LOVED ONES COME HERE!!!! DO NOT SEND YOUR LOVED ONES TO THIS FACILITY!!! My husband was in this facility and it is a complete dump. Nurses are often hanging out at the nurses station chit-chatting while patients such as my husband sit in their soiled diapers for more than an hour. One occasion his catheter bag broke and drenched his bed. He hit is call button at 3:30am (and several more times) and was not attended to until nearly 8am the following morning. The food was horrible and lukewarm at best. You had no way of contacting the nurses station in his wing and they couldn't receive outside calls. Patients are rarely given showers/bath. My husband was transferring to a new facility and started requesting a shower 3 weeks prior to transfer. I even spoke to his case worker Katelynn on several occasions about the shower. He never received one. It had been so long since his last shower that by the time he reached the new facility and was given a shower (within 24 hours of arrival) the skin on his legs peeled off and he was left with raw areas on his legs. His room was filthy! Sand sat on the floor under his bed (photo #1). It was like someone had gone to the beach and emptied their shoes. The bed rail had food and hair stuck to the side (photo #2). The floor at the head of the bed hadn't been mopped in weeks/months (photo #3). The most disgusting part was the foot of the bed floor where dead skin had fallen off of the sheets when I complained he was sleeping in filth (photo #4). At one point early on they lost all of my husbands clothing. He was moved to a new room but this personal items never made it to the new room. His headphones were stolen within 24 hours. To the facility's credit they did FINALLY reimburse ALMOST everything upon his leaving the facility. Again, PLEASE THINK TWICE ABOUT SENDING YOUR LOVED ONES HERE! read more

Trying to find a skilled nursing facility for a loved one is not an easy or enjoyable process. And the way that Medicare is set up makes it particularly frustrating if your family member needs cancer treatment. My sister and I have been in a stressful loop trying to help our father find a place to recover after his recent hospital stays. Aviara has come through for us. We appreciate the friendliness and hard work of Thrina, especially as she helped sort out a room readiness issue for our dad. We've also been impressed with the nurses, Brittaney, Emily, and Evelyn, as they've already helped us feel more at ease. And the food management staff member who came and spoke to our dad about his diet needs a shout out as well. Our father has a sorely limited pallet, and we appreciate any help we can get in finding solutions to get him more nutrition. There's no way to feel "good" about a situation like ours and no matter what SNF he ended up at, we knew we would need to stay involved and be present when we can to advocate for him, but that being said, Aviara has been open and inviting to us. Currently, Aviara is going through a refurbishing process. There is a wing of the building that has not yet been updated and it looks pretty rough, which is why I am so glad to know that they are changing things. Our father was in one of the older rooms the first time he came in, now he is in one of the refurbished rooms and it makes us all the more grateful that they are making this investment. That effort is more than can be said about other SNFs we looked into. It's our sincerest hope that he can recover and go home, with our continued support and the assistance of the staff at his new temporary residence. And even if he doesn't recover as we hope, the biggest help is knowing that he can receive some consistency and kindness where he is. read more

I have a family member who was recently sent to Aviara for rehab after a major health episode and surgery. Don't be fooled by the remodeled exterior! After a short time she developed a cough from lying in her back 24/7. Typical of poor care! Turns out, the attending physician never met with her...Ever in the first 3 weeks. Never laid eyes on the patient! We called him out on it, along with numerous other issues. He was initially arrogant and rude, but ended up seeing the patient that same day. What a mess. Thankfully, allied staff are kind--But can do nothing without doctor's orders. Called Social Worker who never called us back! She should be written up! Don't send anyone there unless you have a knowledgeable advocate that can stay on them. At that rate they should pay you! Sadly, I believe most of these rehab, long term care facilities are less than average. A Medicare/Medicaid boondoggle without even average care. That being said-- hold them accountable! Hound them! Make sure your loved ones get the need care to truly rehabilitate. read more

No. No no no. My step dad is here now. He will be moved as soon as possible. They have the incorrect name, bed location, and MEDICATIONS. Life threatening situation. Livid does not even begin to describe what I am feeling. They tried to give him someone else's medications. And they are insisting that his name is someone else's even though he gave them his ID. They "corrected" it, and it is STILL wrong. The staff told him that the director Brian will not allow him to have his prescription motorized wheelchair in the building. Wait, WHAT???? He needs that to be mobile in his home. They have him using a regular wheelchair with a seat that is too low and won't allow him to push off to stand. They are feeding him hot dogs and cheeseburgers. His bed is broken, and they MAY try to fix it tomorrow with him in it. It takes one hour for a nurse to come for him to be able to urinate. Not OK. read more

I went to visit my uncle and was.....uh surprised, confused, disappointed. You pick. The facility is easy to find and the receptionist was okay, not pleasant but not rude. I was given directions to my uncle's room. The hallway was riddled with patients lining the walls. One or more needed to be changed. My uncle's room was not horrible but could use a cleaning. We had a nice visit. My uncle's roommate needed a nurse and had hit his call button at least twice while we were there. We visited for almost an hour and in that time the nurse never came. My uncle even pushed his call button in hopes to get a nurse to the room. On my way out, I stopped at the nurses station to let them know they were needed. There were 7 nurses chatting & laughing. I remained at the nurses station until someone actually left toward the room. So since then my uncle has called several times (he calls us because the operator has not once connected us to the correct room) lately complaining about the food and asking for more clothes. His clothes disappear little by little during the laundry. I mentioned the food, I guess you can decide if it's food. This was dinner (see photo) pizza & pineapple for a 180 lb. man. He claims that was one of the meals he could actually figure out what it was. The pizza (2" wide at the top) was barely warm but a lot of people like/eat cold pizza. I came here to visit a friend recovering from surgery. Easy to find and close to the freeway. I think parking can be an issue. I just happen to find a space immediately, but I saw a few cars waiting for spots to open. I was greeted immediately at the front desk. The entrance has a welcoming feel. I was expecting a hospital look, especially walking through the halls. But I didn't get that as I walked towards my friends room. It had a fresh scent throughout the building, could be overpowering to sensitive visitors. The place was very big to my surprise. It was clean and by the looks of it, well staffed. Staff everywhere it seemed. My friend was in a two person room and it seemed spacious. My friend said the staff has been wonderful and that the food is even great! I'm happy to know my friend is in good hands. read more

This facility really has no order. The staff hang around the nurses station while residents call lights go off. My mom cll light was on 25 minutes and then someone came and I told them that my mom needed to be cleaned and they said they were going to tell someone. All and all it took about 45 minutes before my mom was cleaned up. At the nurses station, they are playing loud music and carrying on with conversation like they have nothing to do. You ask them to help and they say it's not my room! So happy to get out of here!!! read more

My mom has been in the short term section here for 3 weeks and I have found the staff and team to be extremely supportive and eager to help my family. I do visit a lot. I think is helpful to develop a relationship with the caregivers who are looking after our loved ones. The entire team from registration to CNAs, PTs, RNs are all actively updating my family but more importantly, they are on the floor tending to patients. While the food is not hotel quality.... It is helping my mom improve dramatically. Lunches seem to be the best meals. I read previous reviews that no one's answers the phone. However my experience has been different. They have answered when I called the office but again I physically visit to ensure proper care. The facility is continuously being cleaned and maintained from what I can see. it is not brand new but they go above and beyond given the resources they have available. I would have given 5 stars but we could all improve overall. Lastly, my mom will be here another 3 weeks so I will post again when we leave. We have been to other facilities over the years and the people working here are amazing. read more

They don't pick up the phone when trying to reach a patients room. My husband was discharged and admitted to Scripps hospital. I left a message for the Social Services director so I could pick up my husbands belongings. No call back, I called the next day and no answer at front desk. I show up 2 days later and no one can find his 2 cell phones and chargers, electric razor, they gave me someone else's clothes. This place is completely unorganized and no accountability. read more

Regarding Aviara Healthcare Center, I have never been to a skilled nursing center that had such overflowing kindness and courtesy. Each individual staff member made it a point to put the patient's needs first as well as extended themselves above and beyond to make sure that patients needs were satisfied. It's psychiatric and psychological staff was knowledgeable astute and attentive and we're very helpful in promoting psychological well-being of their patients. All departments such as physical and occupational therapy began bright and early each morning to manage one's personal recovery. The social work in case management staff took great interest in one's placement and even arranged transport. The maintenance department was prompt and efficient such as not only fixing one's TV but replacing it with new one in a timely manner and willingly fixed lights, and helped with other technical issues by extending themselves selflessly. Even the medical records staff provided one's needed documentation to other institutions quickly and efficiently, and again, went above and beyond their call of duty by graciously making printed copies for their patient keeping. The food was hot and delicious. I can't say enough about Aviara. All in all, I found it to be a buzzing joyful and electric environment that created a vibrational energy for health and healing. To sum it up concisely, my nurse stated: "I just love coming to work and I love my patients. It gives me such purpose and joy". My brother has been there 2 weeks and they have not given him a bath. 2 hours he had nursing light on with no response he finally started screaming nurse to get one. He needs to see his surgeon and staff doesnt do it, oh did I say my brother is immobile after spine surgery. I hear staff also steals things from pts. I recently spent 5 weeks here beginning early May 2023 following back surgery and was desperate to leave. Food was terrible for the most part and the menu not designed by some nutritionist but an institutional cook. Most work was done by DNA's who were tightly scheduled around mealtime service with lots of diaper changes. The rooms for two occupants were very small and outdated. No care was given to the reason for admission other than males were paired with males and females with females. I didn't sleep well a single night as my roomie insisted on having her tv on all night. Aides were afraid to confront her. But every night loud talking by the nurses station came down the hall. I was not given a walker when released which resulted in me falling the next day and having a trip by ambulance to Emergency with a broken toe. It was a horrible experience. read more

Do not go to this home if you want to talk to anyone! My father went to this home after his surgery. At first, the person calling to get him in initially was great. She was kind and helpful. However, my first sign should have been that when then the admitting nurse called me with questions. She was rude, snappy and short. When I got firm back then she apologized as she had several checks in that day. Well, I don't know why that should be my problem as it was my birthday that day and it was 1 am (as I live on the east coast) and I wasn't mean to her! After that for about a week or so, things were great. Dr. Kyle and his PA, Jamie were awesome. They had my father evaluated and it was determined that he has dementia. They were kind and caring. However, beyond them it was a nightmare. Every time you have to call the home you have to try calling at least 2-3 times just to get someone to answer the phone. When you did get someone on the phone, and had to be transferred you never got the person that you were looking for and the process of calling repeatedly would start all over! The next breaking point for me is when my dad became confused and upset over being in the home and became aggressive with the staff. He was taken to a hospital to be evaluated. Aviara reassured me that once he was calm that he would be returning to the home. However, when the hospital was ready to release him, Aviara refused to take him back and kicked him out. No call, no warnings to me. I once again had to try and find somewhere for him to go. I tried calling for a refund for the days he did not spend in the facility. One month later, I finally got my refund. Then I get a call from the social worker asking me how he was doing. Now mind you that I realize that she wasn't the person that made the decision to let him back but excuse me??? You kick him out, don't tell me, never even get me to a person when I'm upset and yelling because I have tried calling so many times with no answer and you now want to check up on him. No thanks. Here were are over a month since he's been discharged and now trying to get his medical records. The lady left me a message stating that she has questions. I tried calling her back and guess what??? You guessed it, after 2 attempts someone did answer the phone, got transferred and got nowhere! If you have nothing better to do then dial a phone all day and have all the time in the world on your hands, then maybe this is the home for you. Otherwise, you won't even be able to check up on your loved ones because you won't get thru!! I would never recommend this home to anyone! read more

No one wants to go from the hospital to rehab instead of directly home but sometimes it is simply needed. I was in the hospital for 4 weeks after 2 colon surgeries. My friend settled on Aviara and she couldn't have made a better choice. The rehab portion of the building has been upgraded and looks just fine. But the most important is the employees. I moved from a local high profile hospital system that had me constantly waiting for long periods for pain meds and was very understaffed with people that did not seem to care very much. Aviara is packed with staff w​ho are caring warm and responsive and in the end that is what really matters. read more

Aviara is the rehab center that my father landed after two hospital stays. It seemed like a good place based on the reviews, but our experience sucked. Upon admission they took an inventory of his belongings and medical situation. He was still recovering from MRSA so they put him in a room with a quarrantine sign on the door. I visited at 5:30 pm and the night staff told me that he was fine and that I did not need to wear the protective clothing that the protocol specified. We sat in his room and his IV was coding for several minutes. My brother finally tracked down the caregiver responsible for this situation and she fixed the problem, temporarily. It did start coding again when we were leaving. At this point, the first thing that we wanted to do was to decontaminate our hands with soap and warm water. We asked staff where the washing station was and they told us to do it within his room. Seriously folks, you can not decontaminate until you are OUTSIDE of the infected area. After telling them that was an unacceptable solution a staff member led us to an employee break room and let us clean up. The administrative staff were very prompt when it came to getting his payment information completed, they were on us for money out of the gate. Every day that he stayed at Aviara we had a personal caregiver onsite and what she saw is unforgivable. Another patient was wearing my father's favorite shirt and we asked staff to make sure that this item was returned promptly. That never happened and was in fact an indication of what would be coming. For their next trick, they called me to ask that I go onsite to complete more paperwork. I told them to have him sign it, but they had the nerve to tell me that he was in quarrantine and that their staff was not allowed to go into his room. This is wrong on so many levels. First off, they put my family's health in danger by letting us walk unprotected into this quarantine zone. Secondly their staff should not have to fear becoming infected to fill out paperwork if the room was set up to standards. Their final failure is still causing me a great deal of stress. They sent my dad off with a transportation service and upon departure they once again failed. This time nobody checked his list of personal belongings and he went home empty handed. He lives in a senior home and most of his clothes were never returned. He now has only 2 pairs of shorts to his name and lost his favorite shirt. They have received multiple calls from me, my brother and or caregiver and have not located a single item of his missing clothing. They do not return our calls and have not even offered to purchase the items that they lost. Every experience is a different beast, if you or someone in your family plan to visit Aviara then be cautious of infection control, mark and watch out for your belongings. --------- Update, they are now looking to replace our missing items which is a step in the right direction. read more

My mom was placed here after hip surgery. She also has dementia. The staff were supportive and caring. The front desk was always pleasant (I saw them everyday) and the care was great. My Aunt who has dementia has been rehabbing at this horrible place for three weeks. I found out during this time they have never washed her hair. When I try to speak to someone about it, they are never in. I never get my voicemails returned. Knowing what I do now, I would of never allowed my Aunt be transferred to this place. read more

Wow. Following my first review, this place blew me away with their response to my review and what took place immediately following. Took literally a few hours. 1. Christina is the care director. She reached out to me via cell phone and was very concerned. She professionally dealt with my complaints and even gave me her PERSONAL cell phone that I could reach her on 24 hours a day. She answered and made any and every request happen. Christina is the jewel of this operation and truly CARES about her patients and making sure they are receiving the utmost of care. Christina.... I couldn't make it thru this traumatic experience without your level head and professional help. I cannot Express my gratitude. Julie.... my mothers case manager made sure I was constantly updated on the plan for my mother. Thank you Tiffany...the social worker for my mom, keeping us well informed during the care meeting Dr. Kyle.... amazing with my mom and again gave me his cell phone to ask any questions as they came up Shirley and Gracie and Maria... the nurses that I spoke to the most that tended to my mothers needs no matter how difficult she is. I am forever indebted to the wonderful people who work here and work hard to help your loved one get acclimated. Again, christina was my light at a dark tunnel when I thought I would have to pull my mom out and try to make it at home. My recommendation is when bringing your loved one in, weather it's short or long term, get to know the staff. If you reach out, they will help you. I have worked with the team at Aviara for 5 years now as an outside resource for their patients. I would recommend this facility to anyone needing skilled services after a hospital discharge. I believe one way to recognize a superior place such as Aviara is by the staff retention. Most of the team there has been there for as long as I have worked with them. They are tenured in their roles, and in the community, and know exactly how to go about providing the best care possible for their patients while in the facility. However, their efforts go beyond their patients care in the facility. In my opinion, good skilled facilities make the staff and administration want to be there and happy to do their jobs with the maximum effort for every patient.
