My father-in-law was sent here from the hospital to recover. He has diabetes, hypertension, severe gout, and celiac disease, all of which were noted on his chart. This place ignored his medical chart and fed him the same cheap food everybody got, all of which was in violation of his medically prescribed diet. His first meal there was grilled cheese sandwiches with turkey, french fries, and juice made from corn syrup. Turkey for gout. Bread for celiac disease, fried salty foods for hypertension, and corn syrup for diabetes. I actually brought this to their attention on and they still fed him the same stuff so we ended up paying to bring his meals for 5 days. If you have any sort of dietary restrictions at all, I'd say their food is pretty much terrible and you should make arrangements to feed your loved one who is unfortunate enough to be there. We also experienced no fewer than 10 different instances where the ear piercing alarms went off on the exterior exit from people push pushing the button and exiting the building without using the code. I understand why the alarm is there, but why even have it if nobody's going to respond? Every time it went off (with a few exceptions) it would go off for anywhere between five to as long as 15 minutes. At one point while we were there visiting and had guests I had to go searching for a nurse to come and turn it off and most of the nurses don't even know how to turn it off because they're not given the code. The manager on duty can do it, but if that person happens to be on the other side of the facility they can take a long time. Meanwhile, the patients are being disturbed. Cannot sleep cannot rest. Can't even have a conversation believe me it hurt my ears and I'm 60% deaf from service related hearing loss in the Marine Corps. The final straw, though really was the PT, which was the whole reason he was supposed to be there. It was not being performed regularly and was not being performed in a meaningful manner other than a couple times when a physical therapist named Chad came. He was awesome. I would classify him as the one true caring person I saw working with my father-in-law. He was friendly. He took the time to actually converse and did a meaningful PT session of 15 to 20 minutes.. on several other days when Chad was not , my father-in-law was given PT that consisted of nothing more than a person coming in for about five minutes and making him get out of bed... which has absolutely zero value, especially when you consider he could do much much more than that at home in a much more comfortable environment. In any case they had them scheduled to be there for two weeks and I could see the writing on the wall, but not only was he miserable and absolutely hated it there. He was getting nothing from it so we insisted on him being discharged after only four days. I would like to express my gratitude for the people in the facility, who actually did provide good care because there were some, and in fact, I don't blame the people who work there I blame the owners of these kinds of facilities that understaff and underpay these hard-working people. Heck, the TV in his room didn't even have a working remote. I had to drive to Walmart and buy a universal remote to get his television working otherwise he would've been in an isolated room with no Wi-Fi because nobody knew the password, and no television and no phone that was within reach because it's on a nightstand behind his bed where he can't reach. So, all in all I'd say The Place is a disaster and not good for somebody who truly wants to get better and go home. Id say you're much better off just going directly home and arranging for in-home hospice because Medicare will pay for it and your loved one will be much happier. read more

No one knows how to do their job here except for some nurses. Annette Garcia from social services at grand oaks and a few other admission/discharge people are extremely rude and unprofessional. They want to kick my 81 year old mother out who is an amputee, is confined to a wheelchair and has dementia. They called a meeting TWO days before her discharge. Conveniently, that's when her 100 days are up for when she fell off the toilet and broke her hip that her insurance was paying for! Now that we are waiting for Medicaid they want to shove her out the door. They think that an 81 year old amputee with dementia, confined to a wheelchair can take care of herself. They even said she can get her own apartment!! My husband called THREE times to let them know that my mother spent the night with her husband last night. He spoke to the director of nurses at 3:20 pm yesterday and informed her that it won't be today that she returns. Annette Garcia calls this morning and says that she was supposed to be back last night! We called yesterday (twice) and AGAIN last night when no one returned our phone call to let them know she wouldn't be coming back last night and STILL, Annette called this morning stating that she was supposed to be back last night. These people are incompetent. If you care about your loved one's safety, do NOT bring the en here! These people have no idea what they're talking about and it's pathetic you think an 81 yr old amputee, confined to a wheelchair with dementia can get an apartment by herself! Stay away! They even told my mom that if we didn't pick her up (because she can NOT take care of herself and we do not think it is safe for her to be discharged) that they would call her an UBER to get her!!!! INSANE!! Annette Garcia and the lady with the sunburn long black hair (both have a severe attitude) are cruel and heartless! I made a call to ombudsman. This place needs to be investigated. The facility needs to be investigated and then condemned. From patients being left unattended, to patients not being cleaned after accidents, to sub human treatment to the kitchen running out of food and serving peanut butter sandwiches or grilled cheese for meals. To complaints about piping backing waste up in bathrooms not being addressed. I was a resident and my bathroom because of waste backing up every night would smell so ban, I would have to leave my room and sleep outside. And nothing was done. The staff here are beyond cruel but that's the culture of the place set forth by management. Like overlooking the Christmas tree that's still up in June and Happy 4th of July banners from last year. No one is allowed to make any decisions and no one will go a step beyond to help. They constantly run out of food and also meds. They ran out of pain medication one time and didn't explain that my cup of various meds didn't have the pain meds. They just hoped I didn't notice. When asked I was told that it's on order. That does me no good at the moment. And the nurses who rotate thru don't know the patients and wrong med dispercenent is not uncommon. I have only been a resident for three weeks and what I have seen there is inhumane. Most if the patients there are much older and can not stand up for themselves. They just lay there in agony. Malnourished, unkept and ignored. I would never in a million years bring a loved one to that facility. And I would personally choisew death over residency. read more

Aspire at Grand Oaks took wonderful care of my father during the 3 weeks he was there following a bout of Covid that left him with severe dementia. The staff was so kind to dad, and they helped me so much with grasping this new reality of my dad's mental incapacity. When it came time to move dad to a facility in Palm Bay so that he could be closer to me, Shelley, the Director, arranged to transport him and was so patient with dad and his paranoia and distrust with being moved. The 2 gentlemen that actually traveled with dad from Aspire in Palm Coast to Aspire in Palm Bay were wonderful and made my dad feel at ease in what could have been a very stressful situation for him. Thank you to Shelley and all her staff for your wonderful caring and compassion for my father and our family. I am forever grateful. read more

Very simple put, my father begged me to just leave him in the parking lot, so he wouldn't have to deal with them. They really should just shut t his place down. No one cares at this place. If you take your loved one there know that they will be neglected. I have now made two health department complaints. Some of the staff are inappropriate and say things out loud about the patients. They don't care about how they make them feel. They pretend not to hear them. At this point I feel like I need to take legal action or go to the news to expose this uncaring, horrific place that has turned a blind eye to the neglect of their patients. Heather the Administrator cares only about money, and nothing about the welfare of the residents or staff of Grand Oaks. She is single handedly responsabile for the continued uncontrollable outbreak of COVID-19 that started on July 4, 21 and her hands are covered in blood of those that have died during this outbreak, which have been way too many! It all started when she thought it was a good idea to bring several COVID-19 positive residents from a memory care unit to come stay at Grand Oaks while they were sick. The residents were unfortunately unable to be isolated to their rooms. Thus spreading the virus to our other patients and employees. Since then she has continued to take more and more outside COVID-19 + patients including patients that again cannot be isolated due to wondering behaviors r/t dementia, while we are still unable to control the spread in the facility. However, her biggest concern was to make sure we can bill appropriately for the isolation, so that it is at a higher rate and that we fill all our empty beds. Never has she cared about stopping the spread. Ask employee there and they will tell you the same thing. Employees are going out left and right with the virus, but those that had to stay home got paid, with exception of the one that actually worked the entire time she was isolated! How legal is that! She is salary, she works the entire time she is isolated, and they refuse to pay her! If they do that to someone in leadership, imagine what the will do to someone who is not. If you are considering working here or placing a family member here, please don't for now. It is truly a hazard to your or your loved ones health! read more

Some nice people work here. Some diligent people work here. The food looks alright. Today, again, Grand Oaks is way understaffed. Patients said they were not allowed to get out of bed because they were short of staff. Other patients said they were not allowed in the dining room because of staffing shortages. Last week I heard from more than one person about a staff member who refused to let a patient use the toilet prior to their shower. The people who show up for work are working and seem to be doing a good jobs however this place seems to not have enough people to handle their patience's needs. Several patients have not been bathed in a very long time. Some have matted hair. They are fed and given their meds. Some of the overworked staff should get a 5 Star review. Those few employees who don't seem to work quite as hard might do well to remember they may reap what they sow. That being said most of the staff is fantastic and they are Grand Oaks saving grace. Otherwise I would give this place a 1 Star review. This is not an easy review to write because so many good people work here but they are severely understaffed and patients' needs are not always being met. read more

My wife is currently in Room 201-A, and she is not getting the best health care compared with being in the hospital. She has a bed sore, it hurts, but do the nurses give her pain pills and/or cream when needed? No, figure every 6 hours when the pain hits every 4. At least once, while she paged the nurses and waited in pain, she could hear the nurses discussing non-nursing stuff (aka lazy). Does she get a drink with her meals or when needed? Occasionally. Since she got there a week ago, she is not allowed visitors for another week (14 day quarantine due to Coronavirus prevention), which is not a complaint, but when I bring stuff to her, they first tell her there is nothing for her, then bring the box to her a few hours later. Three times a week, she is scheduled to go to DaVita for Dialysis (call it a monopoly and my insurance calls Davita out-of-network), they don't get her ready to go until maybe a half hour before the bus goes, meaning they're hurrying to get her ready. When she returns from Dialysis, her food is cold. She also gets PT, which is fine, except one of her therapists called her "evil" and yelled at her, the nurses said "oh, she's 7 months pregnant" (bad excuse for harassment). If the management wants to fix things, I will not talk to you, you shall go talk to her and FIX IT! If we could go to a better rehab hospital, we would, but Aetna (Group Insurance) said this is the only one they would pay for. So, do I recommend? Not one single bit! read more
