I called several home care providers who told me it would take as many as five days to find care givers. I needed someone to drive me home and begin care the next day. We Care could provide what I needed. They helped me complete the necessary forms, line by line. I was extremley anxious about being home. Diane and Sonia were able to support and encourage me through my anxious moments. They provided both physical and emotional support as I adjusted to being home. In addition, all my laundry was done, my bed changed, dishes washed, and my meals were prepared. They both went "above and beyond". I valued their companionship as I adjusted to being on my own. They became like surrogate family members, and I appreciate their caring and kindness above all.

However, based on the experience and patient outcome I cannot honestly recommend this facility for after stroke care. 1. Nursing staff is not friendly/approachable or empathic to family members concerns or simple requests for patient updates during visits. Their language, facial expressions and speaking tone is that of annoyance and inconveniences. And nursing staff residing at the nursing station ignore visitors; they do not acknowledge or greet their visitors. 2. Nursing tends to place calls to the patient family members for non-emergency situations at late hours of day (after 11:00 PM) to inform they are documenting their notes for test results which creates panic. 3. Patient care is not proactive, and the medical staff responds reactively when presented with patient status concerns. Example, running blood work when family members note the patient appears lethargic and non-responsive. Or, when a support staff member makes note of a patient's condition to the medical staff who doesn't follow-up. 4. Patient's that are non-ambulatory or become non-ambulatory are left for hours to sit or lie in the urine and facies that causes a break down in the skin that evenly leads to bed sores. 5. "Family Meetings" within for first 30-day stay of a patient's admission is finance based and not a medical update of the patient's status of improve or decline. During this meeting any type of medical update is provided. As a matter of fact, if a patient is admitted for a 90-day admission, there is no family meeting that involves a consult with the physicians or nursing staff present. Only a family meeting at least two weeks before a patient's expectant discharge date that includes the auxiliary staff, i.e. rehab, social worker, pastoral care, financial officer. nutrition, etc. 6. Patient items may be stolen/lost from the patient's room such a clothing, phone charges. 7. In my experience (there is so much to say regarding the care received for my loved one), my family was given the false hope that our family member was stable enough to discharge home and would require additional home care nursing, physical therapy and a nutritional high caloric diet for the significant weight loss for continued care. Sadly, the outcome was not a positive one.

Other that that Meadow Park needs a complete overhaul! The staff appears lazy and unconcerned about my mom's needs. They do not answer the phone at the nursing station. However, when you go there everyone is sitting at the nursing station on computers. They did not change my mom often. Most times she sat in her own mess. The linen was often dirty and the rooms were unkept. The nurses lack poor communication skills and they are not compassionate. Elizabeth (a nurse) told me that my mom was not special, because I asked that she be changed asap. The social work department appeared disorganized as well. This facility is deplorable and requires help. The health department needs to do a visit and an inspection! The patients are only allowed two showers a week. That is nasty, dirty and disgusting! Bypass this facility at all cost, unless you plan on spending time there on a daily basis.

    Waterfalls is the sort of place you WISH you would find for your senior loved one, and can't…read morebelieve you actually have. Purpose-remodeled by owner Matt Decker, who lives upstairs with his family, this beautiful, sparkling clean Assisted Living and Memory center is set in a grove of lovely trees, with a deck and koi pond in back. The facility has 8 large bedrooms, each with a beautiful view and en-suite or hallway bathroom access, plus a long-time, dedicated, and very caring staff. They plan enrichment activities for the residents (e.g., chair exercise, game show time, small art projects), and cook meals and snacks (like freshly baked cookies) in their lovely kitchen. We could not have been happier with the care my MIL received, and she was appropriately sent to a hospital when the staff noticed that she was eating poorly and began complaining of stomach pain, in what ended up to be her final illness. I hope you are blessed enough to find space at Waterfalls when your loved one needs it.

We do have wheelchair bound residents that we transfer daily utilizing a Hoyer lift. All showers are wheelchair accessible. We have a delegating nurse plus two on-call Nurse Practitioners. Our staff is specifically trained for dementia residents and on average have 15-years experience in the industry.
