Staff are nice but some workes smoking cigarettes a worker walked out with 2 packs of cigarettes

Does not value its workers, bare minimum care for residents. Administration just assumes the residents questions and concerns aren't important, constantly brushing off the residents. read more

This place is so inconsistent with there rules and regulations some of the Staff can be rude and treat Patients like imbasilse and children even though they have no Cognitive disabilities so you can imagine how they treat Patients that do have Cognitive Disabilities and cannot advocate for themselves The place is rundown and old and the food is hit or miss as they overcook things so Patients with no teeth have a hard time eating it Some Staff take forever to answer call lights They never Scheduled my follow up Appointment with the Vascular Doctor which was supposed to be two weeks after my Hospital Surgery and I was at this Facility for two Months read more

Do not let your loved ones stay in this facility. Abuse, neglect, incompetence, filthy conditions. My grandmother was placed here to recover after a surgery- she was denied meds for 4 days. Even her daily heart and anxiety medications were not given to her, nonetheless her pain medication she was prescribed. Her "bed" which was a small cot, reeked of urine. She was not given a pillow. The nurse did not check on her until she requested (demanded) to leave the facility early...again she was here for 4 days- no medical treatment was given or offered. There's no charts or information on patients, "medically trained staff" had no idea why she was in the facility, so she went through multiple examinations that were completely unnecessary. If there longer, I'm sure she would have experienced bed sores, infection, and God knows what else. Terrible screams were heard from patients at all times of day, they were unlawfully restraining patients who wanted to leave. Patients in wheelchairs were left in the hallway for hours. No one helped her into a wheelchair to use the bathroom for 12 hours during her first night there. I am reporting this place, and I have contacted the Florida Department of Health, as well as my attorney. I am only giving you a small idea of the horrible experience my grandmother had, just trust me- do not let your family members stay at this facility. It's straight out of a horror film. She was terrified to call us to explain the conditions there because she was afraid staff would hear and she would "get in trouble." Her surgery was on her leg so she wasn't able to walk, she was terrified that she could not escape. When we picked her up, they would not let me inside (I am vaxxed and was wearing a mask) I understand covid policies, but I have a strong feeling they are milking the "no visitor covid policy" to prevent family members from witnessing the current conditions. If you want a family member to receive terrible health care services please send them here, the staff doesn't care about any of the patients or concerned family members, they are rude and unprofessional. My grandmother want given oxygen until I made a big deal about it and then the staff acted as if I was causing a problem by having them do their job. This place is unsanitary and needs to be shut down immediately. read more

My dad was sent here after being hospitalized with serious health problems. He has been there less than a year and has had to be rushed back multiple times to the hospital because of this place failing to constantly provide accurate care and diagnosis of patients condition. Mulitple times upon showing up for a visit, all of a sudden they realized something was wrong or he was non responsive and 911 was called and he was taken out and days later returned back to the facility. He is a Vet and his insurance only covers this place. After today's visit and once again 911 call and removal, I honestly don't know why I don't have a long list of 1st degree felonies after today. These people will lie to your face and protect each other to get a situation calmed down dont give a damn... Even my dads roommate stated he dont get his pain meds like he should. Don't see how nobody has made the news on this one about people loved ones being neglected. Needs a clean sweep of certain staff ( Angela ) and who ever is in charge needs to step down. I have had long experience with Bartow Center. It is a good facility and they work hard for the patients in their care. They have done a good job caring for my Wards and they have gone above and beyond what a lot of homes will do to take care of the special needs of the people in my care who live there. There is a lot of activity in the home when I visit and people are engaging in sponsored events and other activities provided by Bartow Center. I appreciate all they do to take good care of their patients. read more

BEWARE OF THIS PLACE! On Day 1 that I went there, which was on Sept 8th, I met with Mia, the social worker. It was then that i in person, saw the Power of Attorney form in his chart and briefed her on my dads' condition, as well as our wishes, and the condition of his home. It was also then that i sent her a copy of the psych consult to her email address which she received as i sat there with her, and witnessed her print it out and place notes into the computer. Days later, i get a call from home health that BartowCenter is discharging my dad because insurance will not pay any more days and he has completed his care. Come to find out, it was Mia that made the call to the insurance in the first place to tell them his care was complete and he could leave if he wanted to, which prompted them to stop services. I immediately called and talked with Mia's assistant, who told me that she doesn't have any POA and it was never received, nor was there any record of a Pysch consult. I argued with her and then she hung up on me, telling me she cannot talk to me and will not talk to me, that my father doesn't want me involved. I then made threats that by law she has to talk to me or i will get an attorney and sue them. I called back and spoke with the financial officer, who said that she would look for the forms and I could send them to her which she received. A couple days later, my wife and I receive a call from Mia that my dad is again placed on discharged and he would be discharged that day, and that i had to come pick him up immediately. If we did not, we would have to pay $10,000 up front immediately. We immediately contacted DCF who put a hold on his discharge. She told us she "didn't care if he got a taxi to a crackhouse or lived on the streets, it wasn't [her] problem." My father had a severe stroke in Nov of 2016 in his right basal ganglia that compromised his reasoning and judgement which led to him becoming septic hours from death and above the knee amputation. His foot cut off back in Feb. My father has had two psych evaluations that both deemed him unable to care for himself and was documented by several health professionals. Mia, the social worker has put my dad at risk of his life so many times. She also has went of her way to manipulate my dad, the nurses, the staff, and the doctor, Dr. Dean, who she told him and the Administrator, not to talk to me and my wife until he met with her and the administrator. Dr. Dean then 45 minutes later, went in and spoke to my dad for 5 minutes, deeming him capable of making his own decisions with the malicious intent to revoke my power of attorney. We had an intervention which once again halted the discharge but no disciplinary action on Mia who lied saying she never talked to me and has no idea of a consult or documents. We insisted on another consult in which only a nurse practitioner came in and said to us that she was not qualified to determine capacity, it had to be a psychiatrist or psychologist, while telling Mia, she didn't want to make waves. Mia has now refused to send the transfer requests to Jacksonville as ordered and is refusing to adhere to knowledge that concerns my father's well being and safety as he has no place to go, and his home is unlivable and unsafe, as well as he has no leg and doesn't know how to get around yet. She also convinced the dcf investigator to let her come along, and manipulated her also that i get yet another call from my brother telling me they called him, he is being discharged, upon learning that Mia convinced Maia, the DCF investigator that he could leave. Maia, lied and said i was supposed to be there and i never showed, which days before gave her the numbers and info of family members to call to be present as well as the company in charge of cleaning his home and repairing it. No call was ever made or received. The supervisor was called and complaint has been filed. Mia is now giving my father legal advice to revoke my POA. IF YOU HAVE HAD PROBLEMS. If I could give this place 0 stars that's what I would give. But since I have to give it something I will give it one star. This place is a joke. I was sent there after one of my surgeries (leg amputation) that required three surgeries. After the last surgery I was sent to Bartow Center for aftercare. First off, when I got there my room wasn't ready so they put me in a bed in the hallway for hours. Their CNA's suck. There was one, I wish I could remember her name, but she would help me turn digging her nails into my skin. I told her she was hurting me but she said VERY rudely, "oh, I am not" then continue doing it. I had to tell her to never touch me again. There was only 1 caring nurse the others were old crabs. I was there for about 4 days until I woke up in ICU at the Bartow hospital. What happened? I still don't know to this day. From there I went to a different aftercare in Tampa. I asked my husband to retrieve my belongings and all they had was my purse. All my other stuff was gone,.an ipad, clothes with tags still on them, a new bag my husband bought me for those cloths, a beautiful plush housecoat, and some other stuff that they stole from me so they are also thieves.Their social worker is the worst. Very, VERY rude when I called to get my things back. I wouldn't send a pet rat to that place. STEER AWAY, PLEASE! read more
