UPDATE: We just received their letter indicating the…read moreoutcome of their 'so-called" investigation regarding my husband's lack of care in their facility. NOTE: I was there at their facility visiting my husband every day, sometimes, twice a day, for a considerable length of time. So, I DID see what was going on. 1. Their claim: "They went in to bathe him four times, but he refused two times." I KNOW this is false because I asked my husband and staff at the time about cleaning him, clean bed linens, and a clean hospital gown. He was bathed only twice in 7 days. (at the time I wrote my review, he had been bathed only once and the sheets had not been changed nor was his hospital gown changed, but because I complained, they finally gave him a sponge bath and changed his gown and the sheets at 6:30 am the next morning). 2. Their claim: "Our investigation revealed that the required therapy minutes by the (missing word?) provided by all disciplines." The following was his schedule: (I have the original schedules of every day except Monday, Tuesday, Saturday and Sunday of days where no sessions were given. Nor do I have a Monday schedule, because it was not planned for his day of release) a. Monday & Tuesday: no sessions b. Wednesday: one therapy session c. Thursday: four sessions (right shoulder was dislocated) d. Friday: One OT session and one PT session & social/exercise group e. Saturday & Sunday: No sessions f. Monday: One PT session g. Tuesday: One session before releasing him If the staff believes that the above scheduling is advantageous, perhaps they might consider having outside professionals experienced in physical therapy advise their staff members. Please realize that if the staff is questioned, they are going to be on the defensive and perhaps bend the 'real truth' Although I have a doctorate, two counseling degrees, and a degree in psychology, it doesn't take ANY degree to figure out that defensiveness, being deceptive, and perhaps changing records is only human nature, when their jobs are on the line. I've even suggested that they consider having someone who is not recognized by the staff do an incognito investigation as to what is really going on here.... with no success. In summary, because my husband was not allowed to leave his bed (loud alarm would go off due to his high risk of falling), he was worse when I took him home than he was when he came in. Therefore, it appears to me that Ms. Harding, Director of Quality Management and Ms. Walker, Chief Executive Officer did not take my concerns seriously but only did CYA form of gestures that were required to keep 'their' jobs.

When I signed up with them I was told they have no problem working with moms primary doctor, etc. However, they did not work with her doctor. When there was a question they couldn't/wouldn't answer or a concern they couldn't/wouldn't address with me I would have to go to my mother's primary for the answer/info. They were never "comfortable or compliant" with her primary doc's findings/advice. Especially when it came to meds. Also, when my mom had a stroke while we were in the park, of course my knee-jerk reaction is to call 911. I did, long story short they abandoned me in my time of need. No returned ph calls fr anyone. The following Mon I was called by their social worker telling me I did an unforgiveable no-no by taking my mom to ER & they dropped me just like that. NO ONE EVER TOLD ME THAT! I was told they would work with her outside providers! Their motto is "Compassionate guardians of comfort and dignity"...They were none of this to my mother & me!

The employee's that are there will talk to you as such as well to a new employee that is trying to learn so you ask questions and they will get upset of the training was up to par alot of questions would not be asked the med aids are clueless about giving prn's meds on hold that should not be on hold because they are too lazy to go check the med room and communicate with the D.O.N when I said something about weed smoke one of the trainers said that's what they do on weekends! How ever, I wanted to Thank each of you for what you did, I know it's your job but I also know its more than that, just what little time I had to play nurse/caregiver, I seen there is absolutely no way to do your job and not have heart and connection, no way! It's a hard job mentally and physically and definitely emotionally, you've got to be one extremely strong human being to do what you all do, and still go home to your families and home life and sleep. So Thank You all for being there and helping me with my mother. Im sorry it has taken me 6 months to Thank You all, it's been a rough 6 months, no one or nothing really prepares you for the loss of your mother, I've never really been the type for much emotion but this last few months I've turned into a cry baby and have no idea how to deal with these so called emotions haha. There is one person I do remember her name, and I wanted to make sure I gave her the biggest Thank You, she was the last nurse that came in to help me with my mother, and even after getting my mother turned and in a more comfortably position, gave her medicine, rubbed my mothers head and stood there by my mothers side with me as my mother struggled to breath, and after telling her it was okay to go if she needed to go, she didn't have to stay, she insisted on staying with me, and as I told Ms. Tammy King once more, if she needed to go it was okay she could go, My mother took her last breath. I just want to Thank Tammy so very much from the bottom of my heart Thank You for staying with me and Thank You for helping me afterwards with what to do and who to call to come get my mother as I was completely lost and had no idea. Thank You so much you have no idea what you being there meant to me. Thank You all ALL OF YOU for helping my mother stay as comfortable as she could, thank you for coming in to help me, thank you for showing me, and teaching me what I needed to know. Thank YOU. My mother was not completely there mentally but I know she would have loved you guys. Ok one more time Thank You all for everything you do. P.s. From November 2024 to Jan. 2025 I was taking notes on all my mom's hospital stays and things that went on. good and bad. and before my mom got to sick I had told her I was going to write a book about her and the infection, She told me "Do It. Finish It and Make The Big Bucks" well i'm not real sure about the BIG BUCKS but in March of this year I finish my book and I published it for the world to read. Communicating with anyone there is an absolute nightmare. No one knows what's going on and one knows how to do their job. This goes for the director and Brynlee the 'social worker' as well. The staff will leave their patients to lay in their own mess for hours before cleaning it up. They are very dismissive of their patients needs and concerns. It's a disgusting and vile place to be. I can't imagine how bad it is for people there who don't have any family to advocate for them and speak up. The place is ran like a circus and they treat their patients in the skilled nursing area like dirt.

I lost my mother from Alzheimers after being neglected and abused by this facility and their long…read moreterm care group, Incare. my dad continued to live in his home there independently after my mother passed. Prices kept going up while food and other services got increasingly worse. No one came to fix things wrong at his home including flooding in the house every time it rained. We lost my dad in Jan. of 26 due to illness. After being at St. Francis, Covenant told us they would take my dad for rehab. They even sent me a confirmation and room number. The next day they backed out because a higher up told the social worker they didn't like my dad and voided it all. This was done to my dad as a member of their "community" out of spite. Since there were no other options for rehab, we lost him a few weeks later. Please find another facility for your loved one or yourself, don't be fooled by how this place looks on the outside.

This is such a wonderful facility and a great resource in our community! It's rare to find a…read moreprogram that can accommodate children while their mothers participate in residential treatment. This facility provides substance abuse treatment services for women and they can bring their children (12 and under) to participate in their in-house children's program. I know there is a lot of promising research on a dual generation wholistic approach and it's great that we have this available in Tulsa. The building is spacious and clean and all the employees I've met are super helpful and friendly. I've had the chance to volunteer here during the weekends and have really enjoyed my experience.
