I was very scared about being a patient in this type of facility. From the moment I was brought in until my last day Cristina made my stay more pleasant. She goes above and beyond. Received a tour of the facility before transferring my dad there. They show you only the 1st floor, everything is clean and kept. Father gets transferred to the 2nd floor. Staff are rude, no instructions or welcoming just placed in a bed and left there. After 30 minutes of trying to call and check on status, I am told by his friend that his phone was taken from him. Beware of the shiny showroom and asked to see the 2nd floor before deciding what to do with your family member. read more

My mom had been in rehab for several weeks. I had heard nothing from the staff about her progress. There have been no rehab-home connections. Even when I call I don't get responses. That's not great customer service. The service in general was terrible. If I could give negative stars I would. One example is that the first time I came in to visit my mom, the nurses were yelling at each other. They also yelled at me about something they hadn't told me about earlier. In the end, they even billed me incorrectly, but they were okay with making it correct. That was the only positive thing that happened. My parents are my world and my worst nightmare happened and my Mom fell. She dislocated one arm and broke the other. We chose this place and I will say I was worried. Will they treat her right, will she fall again, will she get any infections, will they do physical therapy? I was so worried. I will tell you they were professional, it was a clean facility, they were attentive to her needs and I was extremely satisfied with the care and treatment she was given. I forgot to mention the size of the rooms were fairly large, even though you had to share rooms.Because of the size, you still had your own privacy and plenty of space. My mom was able to control her thermostat in her room adjusting the temperature to her liking. Glad to say she was not to cold or too hot. My Mom's doctor there Dr. Penmatcha was informative and thorough in keeping us informed, The PTs and OTs were excellent Gavino and Guillermina and the other therapists were excellent, sorry I couldn't remember the other names. Nurse Kim and CNA Anai, were exceptional; they were kind, great and they made my mom a priority which made me know she was in good hands. Precious the social worker was also very helpful and did a great job. I wish I would have wrote down everyone's names but everyone was fabulous. We loved everyone there. i couldn't believe the exercises she was capable of doing, they gave her the confidence she needed in herself to be able to get up even though she is still afraid of falling they really did a great job of working her core. The food although my mom didn't want to eat it, was actually delicious I did take a few bites of her meals. She was just used to her own cooking however; the food was delicious and if you didn't like the food they made for the day you were given the option to choose something else. The only issue that needs a little work is with who they contract for transportation when patients have to go to the doctor. My family knows that if anything were to ever happen to me I would want to go there and great job Bella Terra La Grange. Two Thumbs up! read more

My father in law was in Lexington after fracturing his hip. How my father in law went from being alive in August to being dead in November is totally confounding to me Our first problem occurred when a nurse was being too rough with him. Thank God he was still ok and complained. They took care of it. I will give them that. Then my father in law fell out of bed. How the f&$k that happened defies my imagination. When I got to Lexington and started asking questions - I was told, "it happens all the time." What?!?!?!?!?! Are u freaking kidding me right now?!?!?! Then we go visit him again and he is trying to get out of bed....my father in law's dementia was really bad by then and nobody had lowered the bed or was doing anything. Wtf?!?!?! The final indignity came 3 days before we brought him home to die. The social worker, Theresa, came into my father in laws room and announced like we were having a party, "are you ready to go into hospice?"" We had not told my father in law that he was going into hospice. I mean who says stuff like that????? My father in law went from having a fractured hip in August to dying in November. You do the math...I would not send my dog to that place. read more

I have to tell you my experience that Bella Terra was a 50-50 experience at first but after about two weeks I can see why the reviews were very negative. The CNA's were pretty decent. I had to tell you that, so are the nurses, but the food was horrible. I mean absolutely horrible. I lost 30 pounds in two months because the food was terrible. I had to order GrubHub almost every day to get my protein. I finally told my doctors I want to leave and get me out of here so they did that pretty quick and I went home and I went to another facility, I went to Meadow Brook up the street and was treated like a prince. What a difference of treatment and care I would never recommend this place for anyone even my worst enemy please clean up your act, change the food maybe more people will be willing to stay at your facility. read more

If I could give it zero I would. We had to hire care takers to watch over my mother in law. It's that bad. The nursing staff were so rude to our care takers when for us the care takers we hired were our angels on earth. They fed her as long as it took her. Her throat was damaged during the intubation. So with that she ate very slow. The purée food they would bring in was disgusting. I wouldn't give that to my dog. Which is really sad that you are in a place to help you when really didn't see anyone cared. My mother in law has passed and I thank our lucky stars we were able to have those care takers. I hope this review helps people to be more proactive on looking somewhere else. read more

My mom was here for a month for rehab. The rooms were large and the therapy good... The nursing were prompt with meds and aides were patient ( generally ) with my moms lack of mobility. But the food was really wierd. Many days they told my mom they were " out " of salad - which is what she had requested to eat. How can you be out of salad? Esspecially with a grocery store across the street! The food was also lots of cheese and processed food - I thought nursing homes would be healthy. The social worker also was not great... My mom ended up staying a few more days due to their inability to figure out getting a hospital bed and walker from her insurance. read more

I have nothing good to say about this facility. I know why they changed their name from Lexington Health Care, to many one star ratings. My wife was only in this facility from LaGrange Memorial Hospital less then three days after good test results, before her death. I was there almost the total allotted time during those days. She complained of back pain the time I was there. They did not have medication to lesson that pain. I finally received a call the morning of the third day, to let me know that they could not resuscitate her. Medication, as required by the hospital, was not available when required. Christina from food service at Bella Terra Lagrange was rude and disrespectful the care is great my mom was pleased for her 2 months stay and did benefit from theraphy and the care but Christina argued with me about outside food being brought in said the nursing staff told her we bring food in yes so she can eat some cooked food of course we did she needs to eat well cooked food which Bella Terra Lagrange doesn't provide as described when you call and listen to them describe how your still will be and the food is deliciouse i told Christina the food service Supervisor i was there on several times and seen the food wasn't done i was to by her im the only one complaining yes i said for my mom thats whats this is about but i have a aunt that does't eat the food and a inlaw who both live there and has been there longer than my mom. My 81 year old mom was placed here after shoulder surgery. No one ever responded for any requests; not for water,her scheduled medications or assistance of any kind. Food is served in bowls regardless of what it is. Her roommate sat in a soiled diaper for over 3 hours as no one would come help her. One day all patients were swabbed for Covid without explanation. I had to call to see why they were being tested and was told there was an outbreak. No one was notified. On a number of occasions a male long term resident came into the room in the middle of the night and tried to put covers on her. At 3am one day they came into her room to give her meds; telling her to take it with applesauce. My mom knew her roommate took meds with applesauce so thankfully my mom insisted they check and it was for her roommate and not her. Who knows what would have happened if she took it. I would never recommend this place to anyone. I only gave one star so I could submit my review read more

If you love the person your are trying to find a nursing home for, do not come here! There is little or no attempt to call family members after bring told I would when she arrived from the hospital. When my grandmother first came here, I had to get rather angry after my 6th call over a 4 hour period trying to contact my grandmother when no one seemed to know where she was. Another relative found her in the cafeteria at least an hour after they stopped serving food. She was on a special unit for elderly patients with dementia. She was still in a hospital gown from when she was in the ER the previous day. For the insane week that she was in there, I averaged at least 6 calls a day before I demanded that someone find her. The other days we would find her in random hallways where they "surfed" patients and made them sit in the hallway. My grandmother's condition actually worsened by being here. She was always a walker but her walker was taken away and she was forced to sit in a wheelchair as I was told it was for her safety. Yeah right, her legs began to swell and fill read more

The place is clean. That's about the only positive thing I will say about the facility. Walking in, my initial impression was that the place is understaffed and untimely. Additionally, they are rude, unprofessional and don't come across as understanding what they are there to do. Do not send your loved one here even for rehab. The medication schedule is not kept per doctor's orders, it takes forever to get a response, and they overestimate the "required" length of stay. Presumably, to keep the patient in longer, they do not provide such equipment as a walker for knee rehab patients. This after that's all the discharging hospital ordered to be used. In my opinion, they do not have the patient's best care at heart. This facility is not Joint Commission rated. The rating on medicare.gov is an absolute joke. Go find a location that's Joint Commission rated and you won't be disappointed. If you're local to the LaGrange area, driving all the way out to Marion Joy in Wheaton is a far better compromise than leaving your loved one in this facility. read more

My parent recently passed away at the La Grange facility. The night it happened my calls were all met with snippy, condescending, patronizing answers. I will never forgive myself for allowing my loved one to die here. I know it sounds a bit dramatic, but I am so upset. There are some good people, but compassion is not practiced here. I blame the owners at the end of the day. To think it would cost almost 100k a year to care for a loved one, only to find them left to die alone is criminal in my opinion. I understand no one wants to be in a place like this but this place is horrible - awful. And they do not care. Nursing staff is inept - I'm not sure how they still work any where but know why they work here - no hospital or doctor's office would have them. The place makes medication errors, nutritional errors, technical care errors, it goes on and on. Your loved one will probably get C-Dif being here for over a week or so - it is rampant due to no hand washing. (alcohol gel doesn't kill it and they don't know it or they don't care.) The nurse once actually placed a stool sample and the paper towels used to clean the patient on the bed tray table contaminating the surface, and they wonder how c-dif spreads! The staff let the room call buttons ring forever while they text and chat. Stop by late at night and you'll see for yourself. They wear leather jackets or hoodies that look like the grim reaper has arrived. This place has little professionalism. The doctor is drawing a paycheck and that's about all she does there. Awful, incompetent, horrible place. Older, less able, patients sit lined up in the halls in wheel chairs. At first I thought it was nice they got out of their rooms, but then I saw it was so there would be no call buttons pushed and I watched nurses walk past those trying to get attention to request something. I often went to the nurses station to get them assistance myself. Not related to medical care but they even run out of bread on days and tea on others. They just cannot plan ahead on anything even though they make the patients order a week in advance. Clean - but hey, that service is contracted out I suppose. read more

My mom lived here for about 2 years. I had her moved from the British Home after I found my mother in a coma sleeping so soundly we couldn't wake her. She had an untreated urinary tract infection. She was taken by ambulance to the hospital and after several weeks transferred to Lexington in La Grange. There I found the food excellent - when a patient had to miss dinner unexpectedly - I sometimes was given their meal. I sat with my mom at her table eating while a nurses aide fed her next to me. I found them very patient. And one of the staff whose husband came to bring her home offered me a ride if I was leaving at the same time. I was very appreciative of this because I didn't have a car and had to call a taxi. They used a major road which passed by my home to get to theirs. After my mom passed away I came back to say hello to the staff because they were so good to her and that nurses aide was gone. She had passed away. I wish I had known I would have come to the funeral home and paid my respects to her family. read more

My Grandpa has been at Lexington rehab center for almost 2 years now since his stroke. I believe the people who work there have good potential. But, they are very understaffed and over worked. A lot of people to take care of and not enough hands. Besides that, I believe this place is a good place to be and I wouldn't trust my Grandpa in any other place. read more

I don't understand the negative reviews. My husband has been to Lexington in Lake Zurich several times. Everyone there is wonderful. They make you feel like family; very caring. For the person who said there were no pain melds, talk to the doctor; don't blame the nurses, they can' prescribe. Everyone there was professional, courteous, friendly, caring and made you feel so welcomed. I never saw anyone left in the corridors -- EVER. My husband was treated as if he was family. La Grange Hospital in La Grange Illinois' Lab Department, for blood work is the WORST. I went in earlier this month and only half of the test results were sent to my Doctor. I called and spoke to so called Supervisor Sue was very Ignorant and Rude!! She refused to stop talking over me or tell my why my results were incomplete.she than proceeded to hang up on me. I called the Hospital Administrator's office and of course had to leave a message. I have not received a call back. These tests are important to my health or I wouldn't be getting them.They have the attitude that they are in charge and you have no recourse. Let alone billing insurance companies and not doing their job, someone should look into this practice. Plus the personal needs to on how to treat customers. Fortunately, we have other labs to go to "La Grange is Not the only game in town". I will never go to this hospital again, find another. read more

this is the place where the RN's who flunked out or the CNA's who are hapless and feckless ended up. The place is severely understaffed and overworked, but there is a lack of personal initiative in the workforce as a whole. this affects the resident in negative ways. It is not the worst place to be. The staff is RUDE and they stole 2 bottles of Italian wine brought to the patient by a friend. We were assured the wine would have the patients name on it and kept in a "safe" place. The patient was never wearing his own clothes, they dress the patients in whatever is available. Patient brought 3 pairs of pants and the pants were always "being washed". Unless monitored closely, patients are left in uncomfortable positions for hours. I would HIGHLY recommend finding another facility. read more

It is HORRIBLE to work here and if you only knew how your loved ones are treated here by Administration, you'd run them out the door with you and find somewhere else. I met a lot of marketing people at marketing events I went to and they all seemed so depressed and told me how the turn-over at LAKE ZURICH is horrible; how the Administrator acts like she's better than everyone (especially the needy residents who she never has time for). You will be asked why you want employment?, what type of pay are you looking for Scary . They tell you directly. After the nice interview that is actually fake, the young lady will come back into the room to retrieve you for a walk through angry Dangerous!!. Then she walks you coldly to the front door, indifferently tells you she will call next week, NOT welcome aboard, You 'll start when we set the schedule. They are looking for ruff workers to handle Activity Aide work. Just read the complaints of abuse from families about their loved ones. Awful! Therapists did not document accurately and could not justify the treatment they were rendering. The facility did not utilize a collaborative approach with working the patient and patient's family, instead they were confrontational and defensive. The orders from the Dr. where not implemented consistently unless the family was proactive to have the orders fulfilled. In addition, the therapists were not diligent about ensuring that weight bearing orders were maintained and carried out. I don't know if there is a worse health care center in LaGrange, IL. read more

I was transferred to Lexington following a knee replacement surgery. I was promised they would continue my pain medications but there were no pain medications for almost two days. That was completely unacceptable in my opinion. The food was terrible and I lost 16 pounds during my two week stay. The staff was ok...terribly understaffed and did not come for long periods of time when needed and called. The physical therapists were good and the housekeeping staff kept the facility very clean. That is the best I can say about this place. My mother was also sent to Lexington for rehab services. She called it "the hell hole." read more
