I reported these issues to the director, who assured me he would address the problems and follow up. After I posted a negative review, he asked me to remove it, promising immediate action. Instead, I spent more than a week trying to reach him, and nothing meaningful was done. Only when I made it clear that I would file complaints with city and state authorities did the facility make any changes--and those changes were minimal. During her stay, my aunt contracted COVID‑19 and had to be rushed to the ER. Thankfully she recovered, and we transferred her out of Chapin as soon as she was discharged. My family member entered this nursing and rehabilitation center needing care and support, and instead left this facility with sepsis, injuries, and severe neglect. Residents are left sitting for hours in soiled diapers filled with urine and feces because administration refuses to properly staff or provide adequate supplies. Many of these residents cannot advocate for themselves, especially those suffering from dementia, and instead of helping them, staff close the room doors and ignore their suffering. What we witnessed was beyond disturbing. There is a complete lack of empathy, accountability, and professionalism. Staff members laugh and make jokes when residents who do not speak English are trying to communicate. The discrimination and disrespect shown toward vulnerable elderly people is disgusting and unacceptable. This facility also mixes drug rehabilitation patients with elderly residents, creating what feels like a very unsafe environment for people who are already fragile and vulnerable. Families trust these places with their loved ones, expecting safety and care -- not fear and neglect. The food is terrible, often cold, and simply thrown in front of residents who cannot even feed themselves. No one seems to care whether residents actually eat or not. The level of neglect is horrifying. To make matters worse, the Director of Nursing Aurimel Buenviaje took everything as a joke and never once responded to escalations or concerns brought forward by our family. Complaints are ignored, accountability does not exist, and residents continue to suffer. The worst thing that happened was the transfer from Fairview to Resorts Nursing Home. Since her placement was short-term, after 100 days she had to be transferred to a nursing home. It was a traumatic experience. The family was not told about the transfer until the day of the transfer. The family was told that Resorts was the only nursing home that accepted her. When the family balked at the forced transfer, we were threatened with a $25K bill for the following month if my sister stayed at Fairview and was not transferred that day. We finally relented with the transfer to Resorts Nursing Home located on the Rockaway Peninsula. We are now working on a transfer to a nursing home closer to family members.

My grandmother was in "floor 1" (up one flight) and the place was a hot mess. It smelled like poop and piss AT ALL TIMES. My grandmother was showered only once after I persisted on requesting it. My grandmother clothes was stolen, and finally in the last couple of weeks my grandmother had two horrible falls because she called and screamed for staff to take her to the restroom and no one would. I really thought Nurse Ratchet was a thing of the past- BUT I was wrong. The nurse was rude, and resistant. I believe by the nurse's behavior was a demonstration of a standard and not an exception to the rule. In my protest I was visited by the administration- (DON probably) but that wasn't helpful, but it was packed with an immediate response. I didn't ask for an apology, or any other actions, I just wanted to be informative. The main reason I engaged in the awkward conversation with this administrator, was to reveal something serious but I didn't feel satisfied that it would create change or cause an immediate address. Afterward I felt a pit in my stomach and uncomfortable leaving my aged loved one, defenseless, and feeble in the care of those who didn't mind shouting at me being healthy, robust, and able to voice my concerns. I could only imagine what type of treatment my grandmother had experienced in my absence and how much she'd be willing to tolerate if left alone with unsympathetic staff. I questioned to what extent this behavior and bad customer service presented. I would still recommend it to anyone, because the atmosphere is calm and clean. You can tell it was created out of an abundance of concern for the sick and shut in but the staff is not as compassionate as the facade suggests. I would only recommend it for families that will be an active and present while their loved one stays at Ozanam. Create a shift for loved ones to show up all day or even hire a trusted person to sit and help your loved one while they stay.

My family didn't place dad there blindly, it was by choice, after weighing all the options. We have a family member who is a long term resident there currently as well as a long time family friend who is there too. They love the care they receive and the staffers they receive it from. Their recommendations were well received. From the moment I entered the front door and was greeted by the front desk attendant Lana, I knew dad was in a great, loving, caring and Holy place. I went up to dad's floor, was greeted by every staff member I passed. Not just on my first visit, but every visit. No matter how busy they were, every single person said hello. As time went on, those greetings were by name. It truly felt like dad was being cared for by loving family. My visits only lasted a week or so as I live about 700 miles away, but my experience and my opinions of my dad's treatment are fact based. He could not have received better care anywhere else. The facility was spotless, well maintained and the staff all love why they do. It takes a special kind of person to do this work. Ozanam is blessed with the best. Ozanam is even updating the rooms one floor at a time. It's nice to see the investment back into the facilities. The office staff was extremely helpful, kind and very communicative. They assisted in every way possible, making the daunting logistics of dealing with the insurance companies a less daunting part of the experience. I could not ask for better! I'd like to name everyone I met there who cared for me dad and assisted my family throughout, but I would have to name every person in the public eye as well as everyone behind the scenes there. By God's plan, my dad, Donald B. declined due to advancing medical conditions. He received great personal care and was treated with dignity up to and beyond when he took his final breath. He passed on Sept 22, peacefully in his sleep. The staffers who knew him were as sad as we are. Thank you to everyone at Ozanam Hall of Queens for all you do for every person in your care every day. I was a rehab patient at Hollis House of Horrors for about six weeks after spinal surgery…read more I have nothing positive to say whatsoever about the administrators, social workers, and other managerial staff who make their money providing the substandard care that patients are subjected to day after day. Anyone who is unable to advocate for themselves will surely suffer. To the extent management takes any interest in the clientele it is only when a sufficiently loud complaint is made, repeatedly if necessary. One morning, a nurse was so quick and rough changing my shirt that my neck was yanked hard enough to require painkillers and x-rays to ensure no damage had occurred. I was also awoken in the middle of the night when someone was taking my blood. The administrator told me it was regular practice to draw blood, and the service comes early in the morning hours. I also blame management on the complete lack of communication between night staff and day staff, as well as staff taking breaks and other staff that remain on-duty. Requests or information conveyed to day staff will likely never be known to the night staff and vice versa. The same holds true with morning and afternoon staff and vice versa. The lack of safety protocols was also evident after I began to feel ill and needed to insist on getting a Covid test.Had I not insisted on the test and the medicine, I likely would've been allowed to interact with staff and patients for days, the majority of whom don't wear masks when interacting with others -even after it was obvious that there was Covid in the facility because I had not been outside in six weeks to contract it elsewhere. To make matters worse, after some investigation, I was advised that the last dose of my covid medicine was used on another patient while they waited for medicine to arrive for that patient- the poor record-keeping and the need for me to insist that I had not finished a full course of the medication was completely unacceptable in a facility charged with administering medication to patients. Also, because of a lack of dexterity in my hand I was supposed to receive a weighted spoon with every meal, which allowed me to feed myself. At least 50% of the time the weighted spoon was not provided to me with my meals even though it was noted on every meal slip given to me with each meal. In another example of management disconnect with the patients in their care, the facility is undergoing renovations that began at 8 o'clock in the morning and continued past 8 PM-Despite the law requiring construction to stop at 6 PM. It makes me physically ill to think of all the patients unable to speak for themselves that likely must endure that construction till 8:00 PM. There was no safety grab bars in the bathroom in my room. When my dirty laundry continued to sit for 2 weeks, the excuse I was given was when there's not enough laundry, it's not taken- Zero acceptance of any responsibility by management. Overall, my stay at the Hollis horror house was almost as traumatic as my spinal injury itself. Being subjected to diaper changes at five in the morning despite my insistence that it's not necessary, the crappy food, the lack of safety protocols and absence of sufficient staff that allows for proper recordkeeping and effective communications between staff to be standard practice makes this place as close to hell on earth as I would ever want to come. I don't believe this facility should be allowed to care for patients at all, but if you are considering this facility for yourself or for a loved one be prepared to vigorously and aggressively advocate on a daily basis for yourself or your loved one. Understand that complaints made to anyone other then the administrator and which you dont document are likely going to be lost if not ignored
