My assigned insurance claim adjuster advised that after months of back and forth and proof, all they needed was their report from Dr. Hess. Mass Mutual reached out, and I reached out many times (moved to California so I could not easily physically drop in to talk). No response. This cost me several thousand dollars. I love NIM and don't know where I'd be without this clinic! It's one of the only allergy clinics in the country that does end-point-titration allergy-therapy, which makes such a big difference in successfully controlling my allergies. (I used to have to fly from Massachusetts to Dallas for end-point titration!)
