I wanted a place that was close to my home, and we chose Carrara Skilled Nursing Facility in Plano, TX. This was a huge mistake .. on so many levels. First .. there were some positives .. a few of the nursing staff - Rebecca & Angela were wonderful, caring, and helpful. Their therapy staff was wonderful as well .. striving to improve functionality, and were tremendously positive. Also, the exec director John was great - engaged, apologetic about the staff's numerous failings, and ready to jump-in when needed ... even at late hours and on holidays. Thank you John! Then .. there the nursing staff. The worst. Nasty. Lazy. Disengaged. Possibly even .. evil at times. Most clearly hate their jobs, and it shows. Some have really bad attitude problems. They are so incredibly lazy. The call buzzer will ring for 1-hour +, with no response. My mother was there for 5 weeks .. I visited almost every day and saw call lights on, but nursing staff were scrolling on their cellphones in the nursing station while the buzzers were ringing with room call-lights lit-up. They chose to not respond. This is a clear case of negligence. Two days ago, I witnessed 5 call lights on in multiple halls .. no help. Nursing staff were at the nurses station chatting and staring at their phones. I found (4-nights in a row) my mom's call buzzer laying on the floor under the bed (I have photos), with zero accessibility to my mom. They would leave the buzzer there, turn off the lights, and close the door. My mom was screaming for help .. but there was no help to be found. Until I'd get a desperate call from her cellphone to drive over there and help. This happened many times. Nursing staff doesn't care. I would complain to John .. he was super apologetic and would get people moving to remedy this .. but why should I have to bother him for this stuff every time? Nursing manager is named Ta'Wanda. I had a conference call with her. She made clear that "the real problem was that my expectations were too high. My mom had been transferred from a hospital with a much higher staff-to-patient ratio, and she said that I needed to adjust my expectations". I called her out on that claim .. she didn't like it. That was the last time we spoke, and the executive director requested that I no longer speak with Ta'Wanda. I will spare the reader of this review the graphic details of all of my mom's experiences, but the unpleasantries of neglect for many hours on end were pretty bad. I witnessed my mom become angrier and angrier by the day. I repeatedly requested only female nursing staff to help my mom to the restroom, but again and again, that was not the case .. with a large man from another country watching her pee. The laundry staff lost my mom's nightgowns - I asked them to track-down her clothes, but apparently (according to their staff) it was my fault for not having properly nametag-labeled her clothing in advance. Those articles of clothing are gone. The food was inedible .. absolutely disgusting. The first night .. dinner looked like vomit atop a pile of rice. And the dessert looked even worse. My whole family took it upon ourselves to supplement my mom's food in order to keep her alive amid a veritable barrage of inedible piles of slop on a tray presented to my mother. I entered that building almost every day for 5 weeks .. and amazingly, I never had to sign-in or out even once .. gee .. nice security. I was even advised by one staff member (& thank you to that wonderful staff member) that it would be a good idea to remove my mom's purse from her room, in order to keep her personal belongings safe from the other staff members. The kitchen staff was quite memorable. There was an instance when I brought my mom a delicious slice of chocolate cake (she loves chocolate) .. and then I went to the kitchen to request a plate. I said, 'May I please have a plate?" .. The kitchen staff member said .. "You wanna plate"?, then handed me a styrofoam box. So I said, "No, please .. may I please have a plate for some cake for my mom" .. She then handed me a bowl. At this point, I had graduated to the mocking phase .. picked-up a plate right in front of her .. held it in front of her face and said .. "Plate"! It appeared as though the IQ level of some staff members might not even register - - As a kitchen staff member.. the first thing to understand is ... "What is a Plate?" This is a facility that I would never recommend for anyone to use. At first, it appears to be a nice rehab place with some decent aesthetics shown on their website .. but once you understand their nursing culture .. Do yourself a huge favor and run for the hills. FYI - there were multiple incidents which were reported to their management to be a problem, that were never properly remedied .... STAY AWAY!

Everyone staff member was fantastic including case workers, nurses, aids, PT, OT, dietician, and housekeeping. They offered a rigorous Physical therapy approach that I was welcomed to because of their encouragement and consistent appreciation for my efforts. Of course I had pain during the exercises, however, the care that they provided was essential towards my recovery. Being an Open-heart surgery Patient was excellent as the PT/OT team was always quick to act on my pain and other diagnosis that they could assist with. I wanted to recognize as many professionals as I could, however I couldn't remember all of those wonderful names so these are the valued professionals whom I remember. Case worker Marci for working with my daughter to coordinate my stay. PT/OT Cathy, Mckaly/ Mickie, Kim, Victor, Arthur, TeTe, Natalie and some others that I can't remember, Nurses Lorean, Maria, Crystal, Kekna, Sini, Racheal, Bidhria, Joel and so many others. CAN's Kusum, Heather, Lizzy, William, Alicia I met a lot of great people and I shared some great life experience with most of these folks. Tonight she is lying in urine as she doesn't have control of her urinary/bowels since her stroke. Most times I call to ask them to help her with the phone, however, not to long ago one of the people who answer the phone hung up on me and kept my number so whenever she is on shift I can't get hold of anyone and tonight is the case as well. This is frustrating when you have a disabled parent and noone is there to help them. Momma turned the light on to get hel;p but she has waited over 15 minutes. I know that sounds petty but when elderly people skin get broken down it can be hell. I lost a parent my father due to some of the same issues (thank God not at this facility). What's the chances the facility will not answer then Jordan from social services will not answer either. I spoke to Jordan (214-6863-1320) last night after 10PM last night and he tried to reassure me but we have the SAME ISSUE again today. I am afraid they will try to start mistreating my mother as they already will not answer the phone when I call to get her help, she's lying in urine and we cannot find her clothes that I know we have provided. Where the heck can elderly people and their family get help. The social services person came before and they step and fetch it and lie to fix the so call problems. I need someone to speak to me about my mother's care and we already had a care plan meeting so Jordan suggested that again last night, but hell nothing changed from the last care plan, so this is there way of trying to say the family is not doing what he need to do..bullcrap.

I made a good faith effort to have a real conversation with their office manager. It was IMPOSSIBLE. Everyone in the world has insurance today. But, he refused to accept my new insurance which is actually one of the top three nationally. He said, "Cash basis only!" When I asked him to put that in writing and/or email, he refused! That's not just a problem. That is a HUGE RED FLAG! IMHO. His "cash basis only" seems like a highly unlikely scenario in 2026! Then, you should also be aware of his inability to articulate in a clear & concise manner. He says the same thing repeatedly...four of five times each!!! For example, "We are no longer independent." (five times) "We are owned by Methodist hospital." (four times) Either he has dementia and repeats things over & over again or his English is so limited that he is unable to elaborate beyond a few key phrases. If you are expecting him to actually listen to you as the patient, your expectations are misplaced. He will continually speak over you...not to convey anything new...only to force you to listen to something that he has already said. REPEATEDLY! He enjoys the sound of his own voice so much that he fails to listen at all. Failure to listen to the patient proves that he has his own agenda. That lack of respect that he shows towards the patient's input is so unsettling that I have chosen to look for another physical therapy office in our area. That won't be too difficult. There seems to be one on every other corner. I suggest that you look elsewhere too.

Like any other place of business, there are several areas that need improvement and several staff members and permanent and temporary employees that need to be replaced. They are also short staffed and at times disorganized because of these staffing shortages. That being said, there are some exceptional, caring, friendly, professional and knowledgeable employees that I would like to recognize. Jasmine Roberts is a fantastic CNA (certified nursing assistant). She is extremely attentive, knowledgeable and professional while being friendly with all of the patients. She never complains about dealing with very difficult situations, circumstances and people. She never misses her rounds, never avoids the difficult tasks like bathing patients, never complains about coworkers, calls patients and their family members by name, never shows anger even under very stressful conditions and is an example for others to follow. Christine James who oversees the kitchen staff does a phenomenal job. She always greets the patients, asks for their opinions on the food, asks if they need anything else and genuinely cares about their wellbeing. Patty Matlick in the business office also does outstanding work. She answered all my questions and she handled all of the paperwork to get Medicaid approved for my wife and kept me up to date and well informed during the entire difficult and stressful process.

She laid in bed all night in feces while continuously pressing the nurse call button, but no one came. She even tried to sit on the side of the bed thinking the bed alarm would go off. The morning nurse came in and the call button was not plugged in and the bed alarm was not set. Mom had trouble breathing and had to request a chest X-ray which determined she had pneumonia. No breathing treatments were provided and several times her oxygen wasn't even on. I removed her against doctors orders on a Saturday and transported her by ambulance to a hospital. The EMTs said this is common with this facility. Do not let your loved in suffer here.

Mattress with…read moreholes in the middle where it has been broken down with many other sick people need to be replaced. The room was poorly cleaned each day, just wiping up what was in the open. The corners in the back looked like they had never been cleaned. Because of the mattress, remember therapy only three hours a day and the other 22 in bed, our loved one has needed extensive chiropractic care because of that. PAM care is poor. I don't know what else to say. Therapy was just nothing - on a child's level. No aggressive therapy. They say if it hurts don't do it. If you went by that philosophy a person would remain an invalid. You know, everyone - literally everyone is just going through the motions and can't wait until it's time to go home. If you don't have someone who can stay with your loved one at least 12 hours, I pity your loved one. Many things will not get done for them. Also, and this is very important - the most important thing is that they offer no financial assistance to anyone - no one!! Most hospitals have "charity care" for those under the federal government 200% poverty level. But NOT PAM. They receive every penny from insurance and still want more. I have never seen such a greedy, non-caring hospital like this one. Now I know why the care was poor. They are vastly understaffed - they save money there. The hospital is not clean - they save money there. Staff almost offended if you ask for help - could be poor wages and they are saving money there. Someone's making a killing! Oh, one plus for them. The only clean thing there is the entry hallway. It shines like a diamond, but the rest of the place - not so much. I don't know if it is a non-profit or profit classified hospital but they do not - absolutely refuse to help the poor with any financial assistance whatsoever. I remember one man who walked the entry way hall a lot, greeting people, saving how's thing going and all that - like he really cared. What a farce! From every thing we saw, a few people are making a fortune, the rest not so much which is why you get mediocre care at P.A.M. Rehabilitation Hospital of Allen, Texas.
