After my mom had suffered a minor stroke and a fall resulting in a head injury, the Dallas hospital had recommended that she be admitted into a rehabilitation facility. I wanted a place that was close to my home, and we chose Carrara Skilled Nursing Facility in Plano, TX. This was a huge mistake .. on so many levels. First .. there were some positives .. a few of the nursing staff - Rebecca & Angela were wonderful, caring, and helpful. Their therapy staff was wonderful as well .. striving to improve functionality, and were tremendously positive. Also, the exec director John was great - engaged, apologetic about the staff's numerous failings, and ready to jump-in when needed ... even at late hours and on holidays. Thank you John! Then .. there the nursing staff. The worst. Nasty. Lazy. Disengaged. Possibly even .. evil at times. Most clearly hate their jobs, and it shows. Some have really bad attitude problems. They are so incredibly lazy. The call buzzer will ring for 1-hour +, with no response. My mother was there for 5 weeks .. I visited almost every day and saw call lights on, but nursing staff were scrolling on their cellphones in the nursing station while the buzzers were ringing with room call-lights lit-up. They chose to not respond. This is a clear case of negligence. Two days ago, I witnessed 5 call lights on in multiple halls .. no help. Nursing staff were at the nurses station chatting and staring at their phones. I found (4-nights in a row) my mom's call buzzer laying on the floor under the bed (I have photos), with zero accessibility to my mom. They would leave the buzzer there, turn off the lights, and close the door. My mom was screaming for help .. but there was no help to be found. Until I'd get a desperate call from her cellphone to drive over there and help. This happened many times. Nursing staff doesn't care. I would complain to John .. he was super apologetic and would get people moving to remedy this .. but why should I have to bother him for this stuff every time? Nursing manager is named Ta'Wanda. I had a conference call with her. She made clear that "the real problem was that my expectations were too high. My mom had been transferred from a hospital with a much higher staff-to-patient ratio, and she said that I needed to adjust my expectations". I called her out on that claim .. she didn't like it. That was the last time we spoke, and the executive director requested that I no longer speak with Ta'Wanda. I will spare the reader of this review the graphic details of all of my mom's experiences, but the unpleasantries of neglect for many hours on end were pretty bad. I witnessed my mom become angrier and angrier by the day. I repeatedly requested only female nursing staff to help my mom to the restroom, but again and again, that was not the case .. with a large man from another country watching her pee. The laundry staff lost my mom's nightgowns - I asked them to track-down her clothes, but apparently (according to their staff) it was my fault for not having properly nametag-labeled her clothing in advance. Those articles of clothing are gone. The food was inedible .. absolutely disgusting. The first night .. dinner looked like vomit atop a pile of rice. And the dessert looked even worse. My whole family took it upon ourselves to supplement my mom's food in order to keep her alive amid a veritable barrage of inedible piles of slop on a tray presented to my mother. I entered that building almost every day for 5 weeks .. and amazingly, I never had to sign-in or out even once .. gee .. nice security. I was even advised by one staff member (& thank you to that wonderful staff member) that it would be a good idea to remove my mom's purse from her room, in order to keep her personal belongings safe from the other staff members. The kitchen staff was quite memorable. There was an instance when I brought my mom a delicious slice of chocolate cake (she loves chocolate) .. and then I went to the kitchen to request a plate. I said, 'May I please have a plate?" .. The kitchen staff member said .. "You wanna plate"?, then handed me a styrofoam box. So I said, "No, please .. may I please have a plate for some cake for my mom" .. She then handed me a bowl. At this point, I had graduated to the mocking phase .. picked-up a plate right in front of her .. held it in front of her face and said .. "Plate"! It appeared as though the IQ level of some staff members might not even register - - As a kitchen staff member.. the first thing to understand is ... "What is a Plate?" This is a facility that I would never recommend for anyone to use. At first, it appears to be a nice rehab place with some decent aesthetics shown on their website .. but once you understand their nursing culture .. Do yourself a huge favor and run for the hills. FYI - there were multiple incidents which were reported to their management to be a problem, that were never properly remedied .... STAY AWAY! read more

Do not put your family here. This place is a disaster! My husband has been here for 2 weeks. Terrible neglect for hours and on weekends is really bad. My husband stayed in dirty diapers for hours. There is no leadership here. The physical therapy is hot and cold along with occupational therapy. They are never on schedule or consistent with therapy. My husband is not getting better. He's worse. This place should be investigated by the state of Texas. He doesn't complain much and is a nice considerate person. The staff is lazy and blames each shift. Or says it's the next shift's job. It's ridiculous. The help hate their job. For one Gigi is always mad and has no personality. Couldn't smile if her life depended on it. I'm sick To death of these lazy People. David received wonderful therapy from Pratibha Bahamas and Mitra. He is learning to walk with a walker again.

Wifecwas discharged yesterday. She was only there because they advertise as a wound care facility. Just had an independent wound care specialist come to look at her wound. Wound is horrendous and infected. Specialist says she should not have been discharged home. They also sent her home with a urine catheter. Home nurse and wound specialist say the catheter is filthy and should have been changed. This is total neglect in her care. read more

This is the worst skilled nursing facility. I wouldn't even put my dog in here. My uncle was placed here after him stay in the hospital for 3 weeks. Great when you tour and the nurse gives you her cell phone and says you can call me 24/7. Well that lasted a couple days and then I was always pushed off to someone else. My uncle had to have a bag changed 1-2 times a day and there were several days he would call all day and someone would Come and say nurse will be here soon. At 9pm at night someone would come and change it. Physical therapy would come and keep him in his wheel chair and let him do a bike sitting in his wheelchair. Never got him up. Needless to say he never walked in this place. Discharge was even worse. They discharged him with nothing in place and we tryed to get a bed and no one ever did anything I called everyday and the rude receptionist always told me she's in a meeting. Come on all day. That's ridiculous. I would never recommend this place to anyone. I will also be doing a post on the Medicare website. People need to know this. Oh on top of it no one told me we had to pay $300 a day after he was discharged I didn't know I set it up for Sunday and his last day was Friday. So they sent us a bill for $600. We were told my the financial lady early on its $172.00 a day if insurance didn't pay for it. So where did the $300 come from. Never again. read more

In my experience, Carrara employs very shameful business practices. I pre-paid a certain number of days for my mother to stay at this facility. She passed away at Carrara, with five pre-paid days remaining. When she died, Carrara admitted a four-digit refund was due to me and promised to process same within a few weeks. More than a year later, after multiple follow-up requests for the refund, including a formal demand letter, I have yet to receive the money. It boggles my mind how an organization can operate in the medical provider space and utterly lack a conscience! Perhaps Carrara intentionally withholds refunds from customers to increase revenue? read more

Ok. If you care for your loved ones and their possessions. DO NOT let them stay there. I lost my mother just recently. They gave me her clothes, but couldn't seem to find her phone. The one thing that was important to me. Family photos etc. The staff is rude and uncaring. After getting upset w/ them. Her phone magically appeared. In her EMPTY medicine bag. But someone tried compromise her phone. So now it useless. I lost irreplaceable photos. What they stole is more than anyone can put in their pocket. Addition to my other one star review: Workers are still not interacting with my mom. She's a delightful woman, but she's getting depressed from her horrible social treatment. PT, Steve, is still awesome. She hasn't been offered a shower in the 15 days she's been there. She can't stand or walk, but she could transfer to a shower chair if someone cared enough to offer. She's there due to a bad, long-lasting UTI. She came from the hospital. She has a permanent catheter. Workers didn't empty her bag, and it backed up into her bladder. She'll probably end up back in the hospital with another infection. Her carpet was cleaned. Room smells better. Cloth recliner smells of urine. Toilet paper holder was put up. We chose Carrara rehab because they were close to the hospital my dad had hip surgery at. We had my dad in two different rehab centers due to two different injuries. I was very disappointed with the communication with Carrara. The only time they'd call me is if my dad fell out of bed or some kind of injury took place. Every time I called them to check on my dad I was placed on hold for a ridiculous amount of time. Out of the blue we received a call from a speech therapist that told me my dad was having trouble swallowing and this was due to osteophites forming in his throat. She said she was so sorry to have to share this news with me, which I still didn't fully understand the severity of the problem. This occurred during the COVID-19 outbreak, so we weren't allowed to see him. Then I received a call from the staff doctor saying because of this, there were only two options for him. Either have a feeding tube installed in his stomach or have Hospice take over. Had we known this at an earlier date, we could have explored our options in more depth. The hospice team took over on a Wednesday, and no one called me to let me know. In short, the communication was horrible with both the Carrera nursing staff and the hospice staff. In addition to this, I received a call letting me know they were offering video chats so we could at least speak and see him during the COVID outbreak. I only received one short call and the gentleman that was offering this service suggested I text him my brother's information and my dads siblings info so they too could talk to him. This never happened. Unfortunately since hospice didn't let me know they took over until late Friday night, we had to wait until Monday to get the management staff at Carrara give us approval to come visit him. I called on Monday, was put on hold until the hospice nurse could answer and advise us on when we should come. They finally said "come now" so I immediately called my brother and my dad's siblings (because they live about 2 hours away) and before they could get halfway there I received another call from the hospice nurse letting me know my dad just took his last breath. Had we been able to visit him on the weekend we would have known the severity of his condition and be there with him when he past away. Two months later I received another bill from Carrara. They never even bothered to call me about picking up his belongings or anything. The worst service I've ever seen from a rehab center and I wouldn't recommend anyone use them. Not to mention I couldn't even understand the staff members when I was able to talk to them. Horrible English that I had to have them repeat multiple times. Do not use Carrara rehab even if a nearby hospital recommends them! So very frustrated with all of them! read more

Back again after a few months of misery for my dear husband. He recently passed away after a month stay in the hospital after leaving Carrara, and having to go back into the hospital 3 days later. Because of a deep tissue wound that was not taken care of in the 3 weeks he was there. Only cleaned 2 times in that time period and 2nd time was the day he left there. When getting to the hospital ER in such pain in the heel of the wound, it was found that it was infected and also the infection had went to the bone. Osteomylitus (mispelled). Was never able to walk, or get out of his bed, and had other tissue wounds and needless to say he ended up on hospice care passing 3 days after that. Never never trust that facility again, it may look good but that is all that is good about it. read more

I tried to get my family member transferred after the first day & they created a huge block. I'm sure to continue collecting money from Medicare. WARNING DO NOT LET A LOVED ONE STAY HERE!!!!!! Staff is lazy, unfocused and they all negative attitudes. He was forced to wear diaper or use bed pan when he first arrived. Often took hours to come change him or remove bed pan. Whomever came to give shots, draw blood, put in IV they would throw alcohol used pads, needle covers anything medical that was THROWN on floor, and left there for weeks. Sheets changed once whole month he was there & room was never cleaned. If Nurses came in middle night, they would flip all lights on and wouldn't bother to turn them off when leaving. He would place order for cereal but wasn't given milk, most food was so heavily salted it was uneatable. Most times nothing to drink with meals. His last meal there he was given half a chicken breast with a lot of rice, and every meal was always cold. Due to my health issues, I was unable to travel and be with him. Rarely did any of the staff come in during the day to check on him or see if he needed something. I was horrified about what I was told and wish he had said something to me. It's all so shameful but the worst thing that really bothered me was when HE TOLD ME ABOUT LITTLE OLD SPANISH GUY THAT WOULD GO IN HIS ROOM IN THE MIDDLE OF THE NIGHT EMPTY ONE TRASH THEN GO LIFT HIS GOWN PULL ON FRONT OF HIS DIAPER OR UNDERWARE AND ASK, "ARE YOU WET". My husband felt sure he was a pervert. I was allowed to travel to pick him up, when I walked in front door place smelled of urine and I could hear someone yelling for help. I moved my mother from a facility in Austin to Carrara and couldn't be happier! From my initial visit and tour of the facility to the actual moving of Mom up here, they were helpful, supportive and made the transition as easy as possible for myself and my mother. If you have a loved one who needs care, especially long-term care, Carrara is the worst possible place to send them, both for you and them. Regrettably, my father contracted Covid-19 at another long-term care facility in July and after a brief hospital stay, he was sent to Carrara to convalesce because the previous nursing home did not have a COVID-wing. My sister and I were finally able to see him in October after the governor loosened visitation restrictions in nursing homes. The lack of communication at Carrara is bar none the worst I have ever experienced in the six years my father lived in long-term care facilities. Every time I called, the receptionist, Esther, made me feel like it was a huge inconvenience, and I was a real pain. I called to ask questions, and someone would say so- and-so would get back with me, but nobody would call me back. I got email addresses and began communicating that way, but the nursing staff never responded. The most responsive people there were Nancy, in account management, and Stacy, the social worker. Unfortunately, they were powerless to help me address my father's health concerns. I felt that I had to beg for somebody to pay attention to our concerns regarding my father's health. I had to call, email, again and again for a nurse or somebody to address serious issues my father was experiencing. We asked for blood work to be done to check kidney function and was told that x-rays were taken of his arm, for edema of all things, and of his head after a fall, but nobody reached out for results and when I asked multiple times for paperwork none was provided. The level of care was basal and neglectful, the incompetence rampant; however, if you need somebody to change a diaper or insert a catheter, or put a cannula over your loved one's face, they are very good about doing that. I called to their attention that my father was beginning to hallucinate. It took weeks for them to finally find out that he had a UTI. On Tuesday, the 22nd, my sister went to visit and found our father unresponsive and on oxygen. They had just changed his diaper though. Michael, an aid there, told my sister that my father had bad diarrhea and asked if she was sure she wanted to see him. My sister asked for the nurse when she could not wake him up. The nurse did not come. I called and talked to Esther-again-but the nurse showed up an hour later without a sense of urgency but to simply administer his insulin without bothering to ask about why I needed to speak with her. Furthermore, she was not truthful about him being fine a few hours prior to her showing and could not tell her why he was on oxygen. When asked about the x-rays and why no one had called us, she said they were fine. After continuing to press the nurse for answers about our dad's current state, she decided to call 911, and they were there within minutes. When she walked out of the nursing home facility, no one said a word to her and acted as if nothing had happened. Sadly, our father passed away during the early hours of Christmas Eve. When we picked up his belongings at Carrara, Michael handed over several bags, turned his back and muttered something under his breath thinking he had not been heard. He didn't even have the heart to acknowledge our grief. That is the level of care, sensitivity, and professionalism this people demonstrated even after my father had died. You do not have to go through the same thing; you don't have to choose Carrara. Choose to give your loved one a place that will treat him/her humanely and with dignity. read more

No Rating, (MINUS 5 STARS IF AVAILABLE). Carrara is a shit show and DO NOT SEND YOUR LOVED ONE'S THERE!!!! My Parent contracted COVID 19 from another Assisted Living facility. My Parent spent 1 month at Presbyterian Hospital in Dallas, (ICU, then regular room). My Parent spent 3 weeks at Carrara for recommended rehab. I Chose Carrara for rehab because they are located close to my other parent; BIG MISTAKE! While in COVID area of rehab at Carrara, nurses were great. PT Betsy was fantastic! Nurses were very responsive to all of my daily calls. When moved to non-COVID room, everything hit the fan. Some nurses were great, and some not so much. PT was great. But soiled clothing was not changed, clothing was not washed, toilets were not cleaned (inside was diarrhea for 4 days and the outside rim was not cleaned where crusted poop remained for 4 days). At this point, according to Administration (Nancy), they were very short handed because of Dallas "winter weather" and staff was afraid to travel into work at Carrara. Also, pipes burst so there was "no water to clean clothes" and "no water for residents to drink." I brought MANY LARGE bottles of water to my parent to drink because Carrara did not have any clean water to give my parent, (although I was able to purchase bottled water at Target). Also, PT told my parent that they needed to sit in the hospital chair in the room, but nurses would not assisit my parent in doing so. So, in the bed my parent sat for days when telling nurses that they needed to sit in the chair. I delivered 8 pairs of pajamas, 8 shirts, and 8 pairs of pants to my parent upon entering Carrara. Upon release departure, my parent had none of the clothes that were delivered. My parent was supposed to be released at noon, but Carrara changed the time to 4:00 pm because my parent wanted to leave with all of their clothes. Carrara had 4 hours to find and wash said clothes and my parent returned to their home with 0 pairs of pajamas, 3 shirts, and 2 pairs of pants. Carrara is still trying to locate these missing clothing items. My parent also received a tall kitchen bag full of of someone else's clothes and a bag full of perfumes, creams, and medical information that do not belong to my parent. Also, a well-labeled bag of hearing aid batteries were delivered to Carrara for my parent in which they never received. Nancy Loft, so called Buisness Manager is TRULY all about buisness. When she needs business info (ss info, insurance information, etc.), she is all about business. But since she is the only person besides nurses that you actually speak to from Carrara, you would think that she would be your go-to person. NOT THE CASE! I have gone through SO MUCH HASSLE in trying to get Carrara to find my parent's missing clothes! Miss Nancy Loft now says (because she is through with getting all financial info from me), that she is not responsible for finding my parent's missing clothes, and that I need to speak to Social Worker, Stacy. Well, I have attempted MANY times to speak to Social Worker, Stacy and she will not return my calls. Miss Nancy Loft also said that I need to speak to the Housekeeping Supervisor but she will not give me her name. Miss Nancy Loft is someone that is the BUSINESS MANAGER at Carrara, but has NO IDEA on how to conduct business. Miss Nancy Loft decided that it would be best to pass the problem onto the next person. Miss Nancy Loft is zero help. And, Miss Nancy Loft also told me that Carrara's ED will contact me regarding my parent's missing clothing, this never happened. Daily receptionist, Amber, is also a piece of work. She is a pawn for all Carrara Administration. Every time I called to speak to Miss Nancy Loft or Social Worker, Stacy, her response was always..."she's in a meeting," "she's on another call," "she's on a conference call," "she's at lunch," "she left for the day" (at 4pm???). She also purposely hung up on me. Not cool! I'm just trying to find answers as to where my parent's $1000 or so clothes are. Again, BEWARE OF CARRARA! NOT RECOMMENDED! read more

Don't judge a book by it's covers! BEWARE! After two weeks, we couldn't take it anymore and asked for early release for my father who was recovering from sepsis. His IV antibiotics were usually several hours late, and at times the IV was left in him several hours after the treatment was done. There was a major problem with staffing shortage and training especially on the weekends. My father said the nurses rarely used alcohol swabs to clean his IV line and heparin afterwards to prevent blood clots. One of the IV's had to be taken out of the refrigerator at least 45 minutes to be administered room temperature. Again, rarely did the nurses read the instructions and tried to give it to him cold, which he had to correct often. He had to constantly tell the nurses how to do their job. His medications were misplaced, lost, or administered incorrectly. The call button which lights up a light outside the room is a big joke. Good luck finding someone to help you in a real emergency. Someone would check on you maybe after an hour or two. Thank God my father was mobile. Often he got so fed up, he went to find help. I can only imagine what would happen if someone was sedated or immobile what kind of care that patient would be receiving. The physical therapist, Steven, gave a great speech at the first meeting saying we would see him 3 x's a week, we never saw him after that meeting. The only positive take aways was the occupational therapist was great and worked with dad several times a week, clean facility, and the food was decent. Other than the 3 positives, the experience was frightening, and you would not want your loved one at this place. Unfortunately, I think these facilities are all about the same or worse. Best to have in-home care, and pay up for better service. read more

Whoa, people. First off, therapy personnel are AWSOME, top notch. After that, this place is abhorrent. As all things corporate, everything starts at the top. Facility is newer, rather nice but don't judge a book by its cover because after the facility, it goes downhill fast. Director does NOT have control of this place. The staff is nonchalant at best, overall. I have been visiting a friend in this facility who is there for the rehab and she is doing well as far as rehab is concerned. When she first came in, a wedding band went missing, off of a hand that it had never left in 71 years !!!!!! WITHIN 24 hours after being admitted!!! That is just low rent! Administration starts the usual bullshit of " are you sure they had it when they came in?" Showed her a picture of the ring on the finger TAKEN IN TH FACILITY THE DAY THEY ARRIVED ! Minimal to NO staffing on the weekend, food staff fighting in the kitchen loud enough to heard by the residents....... Do yourself a favor and go anywhere else, your loved ones will thank you for it read more

DO NOT LEAVE YOUR LOVED ONE HERE! Although the weekday staff is great and the physical therapy is quite good, the night and weekend staff are NOT. There are a few very competent capable people on the night and weekend shift, but they are so horribly understaffed they cannot be effective. After mutiple incidents of taking more than 30 minutes to respond to a nurse call, I started timing the response and waiting an hour was not unusual, and the worst response was one hour, 47 minutes, FAR below TDHS legally required response. Management made multiple promises but nothing improved. To make matters worse, the food is inedible (there is a refrigerator in the room so we stocked our own) and there is a theft problem here. Initial tour was positive. OT and PT were great as were a few (very few). Facility is largely under staffed and under trained. Turn over of qualified nurses is high. My family highly recommends Carrara as both a highly effective temporary rehab facility for those needing extended care for rehabilitation of broken hip, and fractured vertebrae, for example, and for extended long term care for those family members who are unable to care for themselves independently any longer. Our mother has been receiving excellent care at Carrara in Plano, TX for more than a year. This care center is not your typical nursing home. It is bright and airy, the staff friendly, caring and skilled, the Management team accessible, engaged with the patients and their families, and have a strong commitment to the highest level of physical, medical and personal enrichment for their residents. Issues that arise are quickly handled. Family members are encouraged to visit and participate in activities. Our mother is 92 with multiple challenges, and is not always an easy patient/resident, but she stated months ago, "This is my home now, and I know they care about me". My grandmother was there and from my experience, I would say it was professional at all. Some of the employees were rude and disrespectful. Before My grandmother went there I explained in detail what I needed and they give false information. My grandmother was there for a full week before I meet anyone. Michelle was supposed to be her social worker acting like she was going to assist and she didn't. I don't recommend that you put your family here at all. All the bad reviews I read, I agree with them. They are understaffed and they allowed my grandmother to be in pain for hours before they give her pain meds. They dont give meds as they supposed, and the fastest reply I received from them was telling me that she needed to leave based on the lies that they put in the paperwork for Medicaid/medicare. If I can give this place a ZERO I would. The night staff dont give a good damn about the people in there. I heard a lady screaming for help and nobody replied to her. I had to go off on the head nurse because he thought he was going to talk to me like he was crazy. This place is nice but the people on the inside are awful and rude. Please dont send your loved ones here, because all that nice shit they talking about is fake. Don't judge a book by its cover because, after the facility, it goes downhill fast. read more

I have been employed with Carrara going on 4 months now and can honestly say this is a great place to work. Our leader is hands on and always willing to get out on the floor and help anyone that is in need. Under her great leadership and from the support of all of the other management team I have truly had a positive experience. The residents seem very happy to be here and over all it is just a great place to work at and to live. read more

I would like to thank the staff at Carrara for their help and hard work. I am grateful for the work being done. It is not an easy business. We all would prefer to be at home and active, but when your health takes a turn for the worse, this is a good go to place. I would recommend this facility to my loved ones and friends. read more
