The facility is overall a good facility. It is very clean, has good food, they have many good staff members and offer a fair number of services and activities. Like any other place of business, there are several areas that need improvement and several staff members and permanent and temporary employees that need to be replaced. They are also short staffed and at times disorganized because of these staffing shortages. That being said, there are some exceptional, caring, friendly, professional and knowledgeable employees that I would like to recognize. Jasmine Roberts is a fantastic CNA (certified nursing assistant). She is extremely attentive, knowledgeable and professional while being friendly with all of the patients. She never complains about dealing with very difficult situations, circumstances and people. She never misses her rounds, never avoids the difficult tasks like bathing patients, never complains about coworkers, calls patients and their family members by name, never shows anger even under very stressful conditions and is an example for others to follow. Christine James who oversees the kitchen staff does a phenomenal job. She always greets the patients, asks for their opinions on the food, asks if they need anything else and genuinely cares about their wellbeing. Patty Matlick in the business office also does outstanding work. She answered all my questions and she handled all of the paperwork to get Medicaid approved for my wife and kept me up to date and well informed during the entire difficult and stressful process. read more

This place is amazing the staff in the kitchen and the restaurant meet next seed any kind of rehabilitation program I've been to. The staff is always here it's under a new management and these guys are taking it serious for making everybody comfortable and able to talk to staff anytime for any problems. The nurse is here happy and willing to help all the time I asked for my medicine they're always willing to give it to me and ask me my pain level and if there's anything else that they can do. This place has changed from the last reviews. I haven't experienced any of those things the staff and nurses and restaurant are on point they always have a lot of coffee to drink too. Also they have any time menu so you can pick your healthy diet what is it for weight loss or just to eat good food to keep healthy. The kitchen and restaurant though I need to really give a five-star to these guys three meals a day for this many people and making the food that good it's like a real restaurant in there and it has a real good atmosphere and it's always really clean thank you for reading my post. read more

Clean, spacious, with engaged staff most of the time! My father was cared for well and the facility administrator Mr. F. Pratt was exceptional with his follow through. Their Social Events are engaging and well planned, and PT/RN staff knowledgeable. Upon our first day admitted to this place, my dad's tech was moved to tell us about his history. I should have run then. The guy said he was a convict and former drug addict. When is it okay to allow individuals with this type of history to work with vulnerable people such as children and the elderly? He seemed to be a nice guy, just lazy, and loves to talk too much. My dad was at this place for three weeks and during two of these weeks, he was admitted to the hospital with a severe infection. During the last hospitalization, I did not take him back. He was actively bleeding and the techs did not report it to the nurse when I reported it to Emma the DON she said it happens sometimes when the techs pull the catheter line while working with the patient causing drama where the bladder would bleed. She got upset because I wanted Dad to go to the hospital. The ER doctor said that if I had not brought Dad to the hospital he would have died. Shame on you Jason calling yourself an administrator. We had a care plan with Jason to inform him that my dad was not receiving proper care from his care tech per the doctor's orders and HHSC regulations. Jason wrote me in an email stating that it was his policy that they did not follow those regulations nor doctors' orders, and if I wanted to move dad I could based upon my asking for basic care. I called the state department once Dad left this hell hole and they got cited on one of the worst citations a nursing home could receive. YOU ARE WELCOME! This facility should be reported to adult protective service and human services. They are not only under staffed but lack of trained staff. Recently my grandfather was left unattended after transporting from the hospital unattended for 24 hours with oxygen and his insulin- 2 life saving medications. Please do not take your loved ones here!! We had such a traumatic experience I could write a novel. My 92 year old grandfather was left alone in his room with no food, water or assistance. It was 10 am and no one had been in there yet and he was sitting in his filth in the dark. We asked for help and they said he wasn't assigned to a nurse or tech. He was not given any medication for over 24 hours after arriving. We asked them to check his vital signs and had to beg for oxygen and insulin due to the lack of care and attention. Other elderly patients were yelling out for help and my mother had to go assist because no one was there to care for them? After receiving PT (that we had to beg for) they dropped him off and left him in the wrong room where he remained until we found him. The facility blamed him for not having pictures/decor in his room to make it distinguishable. This is absolutely inexcusable and I beg you to not leave your loved ones in their care. After reading SO many other people's reviews and seeing the pattern of abuse and neglect I urge you to please report this to the Health and Human Services department. Please call 800-458-9858 to file a claim. Your family member may be out of there (Thank God) but this place is still full of many elderly patients sitting in their feces, rotting and starving to death. read more

Accel Willowbend is Not the facility for your loved one should they have Alzheimer's. Your loved will be dismissed, neglected and mistreated. Even if your loved one is easy, meaning our family member simply cries easily, you don't want them here. Due to Alzheimer', our family member obviously couldn't remember she was not physically strong enough to stand on her own after her hip replacement surgery. Staff will call you consistently telling you to "get up here" to help; "we cannot handle her; she keeps trying to get up!; you need to hire a private sitter for her!" I personally didn't sleep while she was in Accel's so-called care. With the exception of the receptionist, staff will not even acknowledge your presence. You would think that staff would at least make an effort with family members. When we took her out, her doctor's nurse looked at her medications and was stunned to see how much they were sedating her. Her nurse immediately cut her medication in half and, within a few days, took her off it completely. We called Jason Vaughn, the director, three times. He left three messages and Jason did not return one call. She lives in a nearby Memory Care facility. The facility's director and nurse visited her and were shocked at how staff treated both them and her. Barely talking to them, ignoring our family member and completely disengaged. Do not be thrown off by the beautiful building and decor. Accel has no business accepting Alzheimer's patients. They apparently are struggling to pay that slight bill. Her bedroom floors were sticky and the cabinets were filled with dust bunnies along with scraps of things left by former patients. We are reporting Accel Willowbend to the state of Texas senior living administration. We took our family member back to her memory care facility, and had both physical and occupational therapists come in. Once she was back where staff loved her, opened her blinds, took her to the activity room and basically treated her like a human being, I slept through the night again. read more

This place will undoubtedly neglect your loved one and their health will decline because of the lack of care they will receive. They left our grandfather unattended without basic necessities including food and water. Please please do not make the same mistake we did and avoid this place at all costs. If I could put zero stars I would. Every time I place my father into this facility for rehabilitation, he ends up staying there for weeks due to other issues that arise. I know it is their fault because: 1. He fell in the shower when only one supervisor was with him when he needs 2 (he's a 98 year old war veteran), resulting in sutures placed in his head and a broken shoulder 2. He has a severe UTI that has not been taken care of for weeks 3.obtained pneumonia from this place 4. As well as obtained sepsis. If you love your family, DO NOT send them here. read more

This place is horrible and the care and staff is identical. my family member arrived on a Thursday and three days later, no towels in toom, and no sheets of on bed. No consistent food schedule, not clean. Past pretty front desk, it is disgusting and I would not recommend this place. Smells of urine and unclean. This place is horrid. Dirty dishes. And they don't care. I talked to, I guess, the kitchen manager, Ron. I asked if he was going to take accountability for the dirty dishes and he said "I said I'm sorry". I asked if he planned to check the dishes and he told me no. He said they roll over 100 silverware a day and he isn't going to check it. He had an attitude. My Dad knocked a glass of water over and the water stayed on the floor until it dried. My Dad can't eat cheese. They brought him cheese on almost every meal. The staff are unfriendly and do not listen whatsoever. Please do not take your family here. Its awful! And upon leaving the nurse came into the room and started talking about my Dads meds to someone other than himself. Acted like he wasn't there and was giving his medical information to someone else. Someone she didn't know and didn't ask who he was in relation to the patient. And my Dad is of sound mind and she should have talked to him. When I first arrived to Accel, I was very nervous and scared because I had my dad at previous facilities and it was a horrible experience. Upon entering I was greeted by the front office staff which was very welcoming. The Director of Nursing, answered all my questions and made me feel heard and understood. The care staff were very nice and accommodating to my family as well. The community was clean and the staff were visible. This place is an outrage! Do not send your loved one here! We are now having them investigated by 7 different agencies! My husband who is a Vietnam veteran was so mistreated and abused, it is criminal! When asked for help to go to the bathroom, they threw a diaper on the floor and told him to "pee on the floor"! When asked for help to get his injured leg back up onto the bed, they said, "man up and do it yourself" and walked out of the room. They administered the wrong meds which set him back. I had to call and threaten to have the police come and meet me there to see to him. The crazy thing is, that I was listening to this all over the phone when he called me.I got there as quick as possible and had an ambulance bring him back to the hospital. This place needs to be shut down! Just read the other reviews posted. Imagine them treating a veteran like this, much less someone with any type of dementia who can't properly communicate the issues with their loved ones? No one deserves this type of abuse and total disrespect and disregard for humanity. If you don't care, don't get into this career! read more

Our family has had our share of these places. I must say this place has been the best. It's clean, people were amazing to my mom and after several weeks she got out today. They had a nice celebration for her. Very organized from the day she came in until her leaving today. I was really surprised to not see more great reviews on here for this one. Also on food... Most days she thought food was good to great. Sometimes not so good but overall I can't say anything but good about this facility and all of the staff. Big thanks for taking care of mom who had 5 broken ribs, pneumonia and infection recovery. You all are great. read more

Accel at Willowbend - Mom getting out of skilled nursing after several weeks. People here were absolutely great to her! - Skilled Nursing Near Me - Plano, Texas

My husband was a patient at Accel for 27 days. The purpose of skilled nursing care was to prepare him for knee replacement, surgery through strength, training, physical, and occupational therapy. These services were excellent however, the facility lacked sufficient number of aids to provide care sufficient to meet his needs for personal care the facility did not provide a heart healthy diet and finger foods as requested. Preop instructions provided in writing from the surgeon were only partially followed. Examples: care for incontinence was in adequate. On a daily basis, request to have the diaper changed were delayed, sometimes, for as long as three hours. As a result, he developed a diaper rash that was not treated until he was hospitalized for surgery. On a regular basis, staff left the room without putting the call button, pull string for the light, and over bed table within reach. And several days he was left in bed all day except during therapy. Thus he lost strength. Upon admission he could walk with his walker upon discharge he could not do so. The staff refused to provide a shower with. Hibiclens The three days before surgery. It is possible this is the reason he developed cellulitis around the incision after surgery.and read more

This place is fabulous! It was our second try at rehab with the first being at a place with amazing reviews that didn't live up to the hype. Don't bring your loved ones here! Nursing staff screws up meds, doesn't answer call lights. Dad in for rehab which he didn't get. He leaves the facility still unable to stand or walk. Mom has had to be his nurse, cleaning, bathing and changing his diaper since he isn't able to walk. Total waste of time, resources and money. read more

My wife has been here 3 times since Jan 15, 2019. And she's only been out of there a total of 12 days since Jan. She broke her hip, then 6 weeks later broke her leg. So I'll give you 6 months of review. Overall 4 stars; nothing is perfect but these folks listen and fix problems that I bring to their attention. This is NOT a hospital, and they are not staffed like one. My biggest complaint was that the response time to a call light was too long. My wife cannot walk yet so has to be assisted in every move she makes. They were having 10-15 minute response times....unacceptable to me. They have a new director and a new head of nursing, and the changes are all for the better. Now the call light is answered in under 5 mins. I am spending most nights in room with my wife, so I can see a lot that goes on late at night. New management has made nights much more responsive to patient needs. Physical therapy is good. They plan well and include family members (me!) in plans. Occupational and speech therapy are also very good. There is no urine smell in this facility. Let's face facts - accidents happen. The proof is in the effort they put in to keep this place clean and odor neutral. 5 stars for this. My largest complaint is with the Psychiatrist who makes rounds at this hospital. This doctor is uncommunicative with family- and my wife has Parkinson's as well, so I need to be in the loop. They need a new doctor for this. Other doctors and PA's are good and include family at all times. Food is average to poor. I tend to bring dinners in as I wouldn't eat their menu. Nursing staff and aides are very very good and are the highlight of the facility. They form a bond w patients - this is often the person that the patient sees the most. Always remind the staff - "Don't walk past a call light that is on at a room". They are much more responsive since new Director has been on board. I'm taking with my money here. If I thought for a moment that my wife was not being cared for, she'd be outta there. I have been there when patients fall because they have disobeyed orders and gotten out of bed without assistance instead of using call lights. Let's place responsibility where it belongs. The business folks here are great - they work with us to make sure insurance and billing are all operating right. In summary, I would use this facility if I fall and break a hip or need recovery after my back surgeries. [They may not want me, but that's a different matter. ;-). ] Stuff happens - let's judge them by what they do right, and how fast they fix wrongs. No one is perfect. My mother has been at Accel at Willow Bend the past 9 months. I have been pleased with her care. The past couple of weeks I have had a few minor concerns. Today, I brought my concerns to a lead nurse's attention. Shirley, the new Director of Nursing was made aware of my concerns and within 30 minutes she gave me a phone call to discuss my concerns. This is what I call excellent customer care. Shirley recently became Head of Nursing and both of my interactions with her have been positive and professional. Thank you very much!! Don't consider this place for your loved one if you have anything but simple rehab. This is not a facility for individuals requiring high levels of attention. While the facility is clean and the staff is friendly, I am not sure why Medicare would rate this facility so high. The patient to nursing staff ratio is about 21 to 1. Employees are overworked and do not have the time to attend to patients. For example, my mother was taken to the hospital and we only became aware of this when the hospital called us 5 hours later. read more

1 star because my mom did participate in the movie activities and this won't let me give anything less. Other than that, this place is a wolf in sheeps clothing. Other patients said that they doesn't like it, but at first it seemed ok, until it wasn't. My mom was neglected. She fell and they took forever to get to her. She had oxygen that was supposed to be at 2, but they had her do therapy for an hour with it at 0.2. I could go on about what they did wrong. I insisted that she go to the ER, and they refused, insisting that they knew what they were doing...until they realized days later that she did have to go to the ER. The hospital was shocked as to why the place didn't send her there earlier. I had begged this place to send her, and they insisted they knew best. My mom never made it out of the hospital. I called for my mom's things, my calls were not returned. I went to the place to get them and I was told they couldn't find them. I was told by the lady to "go ahead and sue". It's like they knew that the tort reform would save them. I could have sued, but would have spent more money and heartache than we had to win. Don't trust this place. read more

I'd give negative stars if possible. My father was recently a patient here. I am a medical professional and I am appalled at the level of incompetence displayed throughout his stay. Some of the staff were friendly, but overall no one seemed to know what they were doing from the Nurse Aide level all the way to top administration. There was no one to go to for problems because the people in charge were also clueless. So much confusion regarding meds, treatments, diet, therapies. So much mis-communication and lapses in minimal care responsibilities. Frankly, I'm surprised this place is able to legally remain open and providing care. Do not send your loved one here. read more

OMG!!! Do NOT put your loved one in this joke of a facility. My friend was sent here after hip replacement. A technician did not assist her getting out of bed to use the facilities and she heard a 'pop' sound ad felt extreme pain. She protested to anyone who would listen about her pain...no one helped. 12 hours later, her pain was so severe she was crying. I asked the nurse to get xrays and she would not. This nurse insisted that it was 'normal'... no one gave a damn about her, so I took her out of this place. I called 911 and they took her to an ER. She had a broken femur and cracks in her pelvis. The hospital thanked me for getting her out of there. I wouldn't put my dog at this facility. read more

My experience at Accel was great! The staff were friendly and attentive to my loved ones needs. Therapy was great, the staff was very thorough and my nurses and care team were amazing! I can only speak on a few things as I have a friend that currently lives there and I've only visited her a few times. She is not as mobile as she used to be but is sharp as a tack and told me about an experience she had where one of the "healthcare" (I use that term loosely) workers taunted her and told her that they were going on a break, nah, nah, nah, nah, nah, so we won't even answer if you call for us. I hope your mouth dropped as much as mine did. Thankfully this was reported but I can't imagine how much goes unreported, and who knows if there were any consequences. Also anytime I've walked through I see all the employees slouching in their chairs looking like they don't give a damn and looking like they're hanging out at their home, not at work! I will never truly understand why someone would work with elderly people if they have no love or respect for them. It makes me sick to my stomach to think they can treat humans like this, and also makes me worry that if this is how they treat patients that can defend themselves how are they treating patients that can't. This breaks my heart and I hope that the management sees this and does an overhaul, and only hires people that want to be there. read more

I hope owners send mystery shoppers at nights or install cameras to see how long lights are left on and how many patients are crying , banging on walls, and need help. Let's start off by saying I would give this place no stars if I could. My father was there in reprise care for a few days to give my mother a break. He came back home worse than when he left. I was there everyday but one and I can tell you specific examples of the horrible care that is given. The staff does not care at all about the people in their care. They announce when I would arrive so that everyone would get up off their butts and pretend to work. I would try to ask questions about how my father was doing and not one person could tell me anything. The staff would literally walk away from me and not give me any answers. The nurse didn't administer my Fathers medication right once. I know it's not that hard as I can do it myself. My father also has to be fed by hand so please tell me why his mouth wasn't wiped clean?? He also came home with a huge blister on his foot because he was not bathed and the staff never bothered to check to see if he was to tall for the bed. The nurses took away my Dads button to call for assistance because he pushed it twice in an hour. They told him that they weren't going to help him and to stop it. He was also told that he had to go to sleep at 745pm. Give me a break. I not only witnessed the horrible treatment of my father but other residents as well. The nurse at the front desk yelled at a woman because she was having a bad day and he needed to take her blood sugar. He yelled across the way to her saying I told you five times I had to check your blood sugar. In my opinion if he would have taken five seconds away from sitting on his butt from doing nothing and walked over to her and spoke to her kindly maybe she would have been in a better mood. By the time I left over an hour later the same guy made the comment to the resident see that didn't take long now you can go. Where does he get off speaking to anyone like that let alone a resident that is in his care? If You want your family member or friend to be mistreated and neglected please put them at this place they will be sure to be ignored and not taken care of at all. I have also reported them to the state has this is abuse and neglect of the elderly. Horrible. read more

Do not consider putting anyone you love or care about here. My father walked into this building on a Monday evening and on Saturday was transferred back to the hospital via ambulance with a raging UTI, swelling in his brain from the shunt, electrolytes out and on and on. His mental status has greatly declined over several days to where he was hallucinating and no one seemed to care. I called just about every single person on the leadership team including leaving messages several times for the Administrator and not until we were loading into the Ambulance did he show up in the parking lot. Only one person ( Lisa- Nurse Manager ) seemed to care. My dad was left in the same clothes for 3 days, told he can only shower MWF and I have pictures of the feces left on the shower floor for 4 days. STAY away from this place. We will never be back. My dad is now in the hospital and stable after only 20 hours. read more
