He was supposed to recover there and return home, but because they failed to properly monitor his catheter, he developed a severe infection. He was transferred back to the community hospital, and the infection entered his bloodstream. Ultimatley, he passed away. At one point, we spoke with the director in charge late in the day. While we voiced my concerns, she didn't even make eye contact--she packed her bag, shut down her computer, and got ready to leave. It was clear she had no real interest in addressing the issues, and that moment reflected exactly why the facility runs the way it does--with indifference and a lack of accountability. They never followed directions, and any time I asked questions, no one knew the answers. They blamed the previous shift instead of taking responsibility. No family should have to suffer a loss because of such neglect. I urge others to stay far away from this facility. Their inaction cost my grandfather his life.

They are the only facility in the Fresno area that would accept someone with his diagnosis. When I removed him from his previous facility that was rated 4 stars by Clovis Community, he was so weak that he could not sit up or stand because he had lost nearly 30 pounds because of their poor care. The receptionist at Sierra Vista was very welcoming. The staff was friendly and I could ask any staff member about his condition. At his previous facility I was only allowed to speak to one person and she never gave me answers to my questions regarding his weight, BP, etc. Sierra Vista's dedicated Physical Therapy staff was able to get him up on his feet and walking again. I am extremely thankful for them doing that! When he was discharged to go home many of the staff hugged him goodbye or gave him a kiss on the cheek. They will be happy to know that he is still up and walking. Thank you Sierra Vista!

I was there three months b/c of a car accident. If I had to put some one in this home I believe I would feel good about the general care. I of course would have to bring meals for my loved ones because honestly their food was not edible. HOWEVER **** if you are there with a wound this is not the place to be. My wound nurse decided to oil up my wound then proceeded to tightly bandage my wounded leg. This was not the surgeons orders. Wound nurses are suppose to be a specialized skill. Well this one wasn't. I went home thinking I was finally on my way to recovery. Three days later my wound had opened and I could see the hardware in my leg. Hugh hole. Returned to E.R. Turns out the infection had turned to MRSA and I had to be readmitted to the hospital to open the wound, clean it out, remove the rod in my leg. So now I'm back to square one. After the hospital I went to Willow Creek. Not my choice but because I now had MRSA I needed to be in isolation. DON'T GO HERE. Everyone is lazy. You rarely see your CNA, your nurse is NEVER on time with meds, and sometimes my call button would be on for 1 1/2 hours. Not the kind of place I'd send my elderly. This place is ABSOLUTELY THE WORST! This is a long account and I'm skipping the next 4 weeks. The long and the short of it is: I loved the people at Sierra Vista. Watch out for the wound nurse, make sure you can deliver meals to your loved ones. And remember to stay away from Willow Creek. I would have given this a 5 because I loved the people. I subtracted for the terrible food and the unskilled wound nurse.

Every one of them looked happy, healthy and fully engaged. They chatted happily about the lifestyle there, and were justifiably proud of their art, some of which boarders on museum quality, and all of which was expertly done! We got a tour of the property, and oh, my gosh! It is gorgeous. Well kept, clean Mediterranean style design. The amenities were many and varied from restaurants to a gym, from a residents' garden to clean, spacious, well equipped common spaces. There is also a facility tram, much like a combination community bus and taxi, which can be hailed on demand from convenient stops throughout the community. We noticed some negative Yelp reviews for short term convalescent care and we asked about it. Apparently some short term patients come expecting hospital level care for which The Terraces is not equipped. Understandable from our perspective. In general, this community of seniors is a wonderful facility. All of the staff we met were caring and friendly, and all of the residents we met seemed wholly satisfied. If this is a service you're researching, we highly recommend including this facility on your considerations.

All of the cna's were sweet and always willing to help. The PT staff were very good and caring. There is one person who really stood out, it was Kelly Hadley, she was amazing, she's not only the wound nurse but she was so helpful to me, she was always willing to help me if I had any question's or if I needed help finding doctors for my mom. Although Keystone really is old and does need some tender loving care, the staff makes you overlook that.

I was pretty nitpicky about the room, the temperature not being cool enough when it was 100 degrees outside, the lack of immediate attention. I am writing this review to let you know that things do get better. You need to give it some time and be patient. Whenever I had an issue with the care my mom was receiving, I would talk to the staff, and immediately it was rectified or looked into and the issue resolved. There was staff that would stay in my mom's room at night so she would not be alone. They were so kind and truly made the experience lovely. My mom would be very emotional some days, and they would call me so I could speak with her or reassure her that everything was OK. I would like to personally thank Jessica, Raj, Paula, AJ, Mike, Michelle, Peggy, Sherry, Kelly, the wound care nurse, and the rest of the staff. I did visit her everyday and could see that her needs were being met. Finally, my mom was ready to come home so she could watch her hummingbirds on the front patio, pet her cat again, and sleep in her own bed. Sadly, my mom passed away October 1. It was a difficult 4 months leading up to her discharge, but I am grateful for the care she received while at Keystone. I definitely would recommend them to anyone.

    My grandfather was suppose to be here for 5 days, but we moved him after 2 days because of the…read morehorrible treatment he received there. They basically kept him doped up on morphine so they wouldn't have to deal with him, and denied ever giving him morphine. They were told that he could only eat soft foods, and the first night I got there to visit, they had a pork chop, a hard roll, and veggies on his plate. He managed to eat 2 spoons of peas off the plate. The next morning, they told my parents that he ate almost all his breakfast and that he was greatly improving. My grandfather's last remaining kidney is functioning at less then 5% and he has been refusing food for the previous 2 weeks. Barely eating 100 calories a day. So to say he ate as much as he did was clearly a lie. Also, they left water out for him, but he was too weak to hold a glass. I never saw a nurse helping him at least drink water. His mouth was bloody and cracked from dehydration. When we checked him out to transfer him to a better facility, the medication charts showed they never gave him morphine, when clearly the bottle had a good portion removed from it, and he was clearly heavily medicated. They also put in his discharge papers that his health had greatly improved on his stay there, even though it is easy to see his health is rapidly declining. Also, right before the transport came to move him, a nurse walked in with his breakfast (again with foods he couldn't eat), sat the tray down, turned and saw the transport, and then said "oh thank god" at not having to try and feed him. My father, grandmother, and I all heard her snide comment. If you have loved ones, that are really ill and can't advocate for themselves, this is NOT the place for them.
