My father entered this facility with documented "good rehab potential." Until only weeks earlier, he had been ambulatory and self-sufficient. Yet almost immediately, Dr. Cha repeatedly dismissed his worsening condition as simply "old age." What we witnessed felt less like compassionate rehabilitation medicine and more like an elderly patient quietly being written off. One physician later intervened with IV hydration and steroids after recognizing a serious decline in my father's condition, and we saw noticeable improvement. Yet shortly afterward, according to staff, the IV was removed despite my father struggling to swallow even water. Communication with the family was deeply inadequate despite staff knowing my father spoke no English and communicated only in the Calabrian dialect of southern Italy. He was therefore heavily dependent upon proper medical oversight, attentive observation, and honest communication with family members advocating on his behalf. The medical records later obtained by our family contained troubling inconsistencies, contradictory notes, omissions, incorrect dates, and even the wrong gender in one report. Most heartbreaking of all was the feeling that my father was no longer being treated as someone worth fighting for. Despite ongoing weakness and COVID symptoms, he was discharged to a skilled nursing facility instead of receiving higher-level hospital care. Two days later, he was dead. After reading numerous prior reviews of San Joaquin Valley Rehabilitation Hospital, I was deeply disturbed by how many experiences closely resembled our own concerns regarding patient care and Dr. Cha's medical oversight. Multiple reviewers described issues involving medication management, poor communication, dismissive behavior toward family advocates, questionable treatment decisions, and inadequate patient care -- concerns that strongly parallel what is documented in our records. My concerns are not speculative. I possess extensive medical documentation, medication records, physician notes, timelines, and related evidence that I believe demonstrate serious negligence involving Dr. Cha and, by extension, San Joaquin Valley Rehabilitation Hospital. What is especially troubling is the consistency between our documented experience and the accounts already publicly described by multiple other families here. Because these matters are supported by substantial documentation, I am willing to privately share relevant records with legitimate investigators, attorneys, licensing authorities, journalists, or families seeking information regarding this facility. If others experienced similar issues and wish to compare records or discuss concerns privately, they may contact me. Patients and families deserve transparency, accountability, and safe medical care. If my review were based on appearance alone, it would be five stars. However, that is where my compliments end. We have been searching for a memory care facility for my father, and with his discharge from a skilled nursing facility approaching, we were feeling the pressure to secure a placement. My first visit to Westmont was discouraging. No one was available to give a tour, and the front desk staff was extremely unfriendly. I was told that tours only happen on weekdays between 9am-5pm and that "most people take the day off work" to do them. When I asked for a pamphlet or business card, they had nothing to give me. On our second visit, midday on a Thursday, we again encountered unprofessional front desk staff. Two receptionists were chatting, and one was practically laying across the counter, barely acknowledging me. They seemed annoyed that I needed help. Again, the person responsible for tours was not there despite it being within the hours they had previously insisted on. The maintenance director ultimately met with us, and while that was unexpected, he was great, very kind, informative, and gave a great tour. The memory care unit itself looked well-kept, so we decided to move forward. I spoke with their Community Relations Director, completed paperwork, and provided our credit card for the refundable $500 deposit to reserve a spot. We were told there were two spots available and four families trying to secure them, so we pushed hard to get everything done quickly. The skilled nursing facility's social services manager moved mountains to complete the necessary documentation within days, and Westmont even came out to do the care assessment. And then...nothing. No updates, returned calls, or responses to emails. Eventually, the social services manager was told that Westmont would not be accepting my father. She requested a reevaluation because the notes did not match his diagnosis, suggesting there had been a mix-up, but Westmont ignored her as well. I made MULTIPLE attempts to contact their Community Relations Director and never received a single reply. Not even a courtesy call after taking our deposit (which they stated was refundable, but no one communicated the process for that either). Regardless of the reason, whether the spot went to someone else or they determined he wasn't a fit, the professional and compassionate thing to do would have been to communicate that directly to us. Being ignored after we completed all the steps they asked of us was incredibly unprofessional and disheartening. For a facility of this size, I truly hope this was an isolated incident. This experience made it very clear that Westmont is not a place I could ever trust with my father's care. That said, I believe things worked out exactly as they were meant to. The next place we toured was Paintbrush Assisted Living and Memory Care, and it has been the perfect home for my father. Their Community Relations Director, Reuben, was warm and helpful from the start, and their Memory Care Director, Traci, is an absolute angel. They treat us like family, and I feel my father is in the best hands.

Once you get your loved one in the facility the management team does not respond to phone calls and…read moreemails for several days at times. It has been necessary to hide phone number to get them to answer. The care team has a high turnover rate and some are not properly trained and behave unprofessionally. They are not licensed to administer a simple blood pressure reading or test a patient's insulin levels. The facility is very beautiful and clean and there are staff who are very kind and knowledgeable but improvement is needed. This review is not intended to reflect that all staff need to improve but to advise newcomers to address some of these issues prior to making your decision to place your loved one in the facility. I am more than willing to update my review when improvements are made.

The staff made such a difference in a very caring warm environment . I am heartbroken and outraged by the gross negligence of the skilled nursing facility where my…read more91-year-old grandfather was receiving care for his physical therapy. He was supposed to recover there and return home, but because they failed to properly monitor his catheter, he developed a severe infection. He was transferred back to the community hospital, and the infection entered his bloodstream. Ultimatley, he passed away. At one point, we spoke with the director in charge late in the day. While we voiced my concerns, she didn't even make eye contact--she packed her bag, shut down her computer, and got ready to leave. It was clear she had no real interest in addressing the issues, and that moment reflected exactly why the facility runs the way it does--with indifference and a lack of accountability. They never followed directions, and any time I asked questions, no one knew the answers. They blamed the previous shift instead of taking responsibility. No family should have to suffer a loss because of such neglect. I urge others to stay far away from this facility. Their inaction cost my grandfather his life.

I must say that Orchard Post Acute with a loving, compassionate staff made her stay easier to tolerate. The CNA'S were more than happy to help with dressing and personal needs. They and the whole staff truly went above and beyond to make her stay pleasant. The PT department was so awesome! They truly had a motivating attitude towards all of the patients. You can see and feel that they really care about people. What was special and touched me deeply was her red carpet send off. The staff gathered around and played her favorite music as she left the facility. It's these things that make a facility special. I was tearful as I witnessed the care my mom received. I highly recommend Orchard Post Acute for your loved one.
