She was just just discharged against medical advice, came home weighing just 94 pounds - 40 pound weight loss in 7 months for someone that was a healthy weight. She looks like a holocaust survivor. She is a quadriplegic and cannot feed herself. She was basically left to die there. The food is terrible, as they have a vegan chef. They are understaffed, so patients wait for hours. There is a resident that screams, "Help!" Pretty much around the clock. Her first doctors appointment after leaving the facility, led to her immediate admission to a hospital for malnutrition. Her hair is coming out in clumps. I would only ever go here if I were suicidal.

In fact the complete opposite. Call lights are answered within minutes, the patients I see laugh and joke with the staff even though they are busy, they spend the brief time they have making sure needs are met as best as possible. I do what I can to help communicate and I follow up. So far all of the requests and doctors orders have been met or exceeded! My mother is in this facility. It is clean (I always see someone cleaning!) and though an older building it has all that my mother needs. She is a long term patient currently living here and has since Sept 2025. She's had many appointments and starting with my rockstar Brooke (Admin Assistant) that makes rides possible and any other communication details, to Jen the Queen of activities and things interactive, to ALL of the CNA's, nurses, social workers, doctors and therapists (who am I missing??) that look after her. She has really thrived here. She loves to sleep and would all day, but they get her up and interacting to keep her mind working. She has come out of her shell more. They change her often because she is bed bound. She has NO bed sores. They arranged for a more functional wheelchair and do everything they can to make her comfortable and get her up. If she stops eating or doesn't eat very much, the staff lets me know right away. I will say, if you don't advocate for your loved one, or yourself you may not have as good of an experience. I show up several times a week even if it's brief to check on her and communicate her needs. Everyone is so busy doing their best and it's hard work. If you show appreciation, BE KIND (even if you're hurting). Be curious, not rude, about the care you (your loved one) are getting, ask questions and communicate your needs. You will be thoughtfully cared for. Have patience you're not the only person in the building! Anyway, I am so thankful to God for finding my mom this facility. It's so much better than where she was! We love it here.

I'm in here for my follow up, and was a few minutes past my scheduled time, and the lady checking me in was like, "your almost late" and we almost need to cancel. Really? I feel this is a way to grab money and bill me directly instead of insurance. I would have been really upset. There was a ton of traffic on interstate and I was barely late. There's nobody in the lobby waiting. They aren't busy. Just annoying to even mention it to me. I knew I was a little late, but it wasn't a big deal. It is now an hour past my scheduled time, and they have not called me back though they want to mention I was 5 minutes late? Double standard? They are quick to charge me a "no-show" fee, but don't get me back for over an hour past my scheduled time. I'm lowering this from a three star to two star.

My moms case is Very complicated as she metabolizes medications extremely slow & Leanne was so sweet and caring to my mom; always figuring out ways for my mom to be as pain free as possible without compromising her condition. Now Dr.Platt has taken over my mom's care for the most part; since she received her pain pump. He's such a sweet guy! We've been so very lucky with the Docs & NP's there. Definitely be ready to keep up with Dr.Platt though, lol! He's a super fast talker I timed our visit once for a pump adjustment & no joke... 12minutes .07seconds! And that's With the nurse at the time, checking mom in & doing blood pressure! The Only reason I gave 4 instead of 5 is because about an hour ago I called to schedule a pump adjustment, after being on hold for nearly 10min someone literally picked up & slammed the phone back down. Of course I called back & the girl that picked up was very nice at scheduling my mom. However before I hung up I told her I'd called like 2min ago & after waiting a bit, I was rudely hung up on. The girl immediately shot icicles through the phone. Her tone completely changed & she was snippy. Her reaction to my complaint was "uhh, huhhm, I don't know what to say about that"... So I just said "ok thank you so much for scheduling my mom so quickly, have a nice day" & she swiftly ended the call with "Yeah! Bye!*CLICK*". I thought about making it 3 because two weeks ago I tried scheduling my mom & I'd been on hold for over 40min. That's not where I had the problem though. The problem was, I was given the choice to leave a message so they could call me back so I wouldn't have to spend (probably) another 40min waiting on the phone, but still keep my spot in the queue. So I did that. I did Not ever get a phone call back though. I tried 4 more times on 3 different days to schedule & waited over 35min each time. I left the message & waited for my call back each time, but nuthin. ‍ I believe they just Really need to revamp their phone system & possibly even the two girls working up front/on the phones. The past few times we've been there for my mom's appointments, the blonde in particular, has just been so snappy & rude with a pissy aura to her quite honestly. I'm sorry, I don't want to be rude because I adore the doctors & nurses there, but when you're working the front desk at a pain clinic, you really need to have a certain sensitivity with these patients. These people are having to navigate life while being in constant pain to some degree. I just think they need to really think about that part.

This should really a zero. My wife was here for almost 4 weeks following a very traumatic femur…read morefracture injury. This place added more trauma to an already traumatic experience. The nurses were so overworked and overwhelmed that they couldn't possibly manage the number of patients they were assigned. When we met with administrator he had no accountability and was completely dismissive. My wife had to wait hours for pain meds on arrival that was pure torture. And then several times she had to wait for over an hour for medication and bathroom assistance since she was confined to her bed. They mismanaged her pain meds causing severe pain med withdrawal symptoms that were horrific. The food was absolutely inedible. Fortunately we as a family surrounded her and brought her meals so she had proper nutrition. We tried to advocate for her but there little response. She has diabetes and a UTI that the medical team completely mismanaged. They lost 3 urine samples over 8 days before diagnosing the UTI my wife knew she had. Most of CNAs were very young and completely untrained. The system is completely broken. The silver linings were Nurse Caitlin, Nurse Jackie and CNAs Debbie Jo and Matthew. In PT stay away from Anthony. He is one rude dude! Not what someone needs when recovering from a serious injury. And if that's not bad enough, there were layers of filth in the bathroom that were absolutely disgusting and was never cleaned the entire month she was a resident here. There was no water for OK several days and no hot water for 10 days. I could go on and on with more horror stories about this place All in all if you love your love ones stay away from this place.
