My brother was there for 2-1/2 weeks. It was an awful experience. Because he has dementia and has aphasia he was easily ignored. Surgeon's orders were that he was not to be showered, they did not follow orders. When changing dressing, they stripped a portion of the skin graft. I could go on and on. read more

My husband was at Gem for3 weeks after surgery from a perforated bowel along with final radiation treatments for throat cancer. Care was complicated, requiring wound care expertise, issues with a new colostomy which required training for family members and logistical problems for transportation to the radiation facility every day. Gem's staff was excellent--efficient, professional and always cordial. Family and other visitors were welcomed as an essential element of the patient's wellbeing. The facility was clean and the food was adequate, considering that my husband required a bland pureed diet. I understand that Gem is under new management so I would disregard some of the older negative comments on Yelp. Our experience was excellent. I'm going to start by saying I would recommend this place. But, anyone in need of something like this really needs to do their homework first. In the real world, all nursing homes, transitional care centers, and rehab centers are grim, depressing places no matter what. If you are considering a long term or permanent placement for someone, you might want to keep looking. If it is for short term rehab, then go for it. I give them 5 stars because they completely met my expectations. This is not going to by a gushy love love love review. I am going to include some points that come under the heading of constructive criticism. My father fell and broke his hip. At the tender age of 95 he needed rehabilitation. Prior to the fall he was living independently and our goal was to have him return to his baseline. This was no easy task. The physical therapist, the administrator, the social worker, all met with me and my family and communicated with us directly and professionally at every step. They called me on the cell phone. They helped us navigate a complicated insurance challenge. One nurse called me late at night to say my father had a rash on his backside, she called the doctor, and got authorization for an antibiotic ointment. I have some background in the realm of medical caregiving, and I could not identify any rash that she pointed out to me. But it was all in the best interest of the patient and I really appreciate that. All the care staff were upbeat, positive, attentive to his needs. They could not come running in immediately when the call button was pushed every time, but they got the job done. Admittedly, I was there a lot so maybe they knew I was watching. I tasted the food regularly and I rate it as good. My father rated it at very good to excellent. Upon admission the staff asks what are his likes, dislikes, and favorite things to eat. They tried to accommodate his wishes. My father likes hot tea, but there was a swallowing issue so doctor's orders were to add a thickening agent to all the liquids so there was no risk of aspiration. Hot tea and thickening agents don't work. The tea bag does not absorb the hot water enough to brew. It turned out there really was no restriction on his liquids, and it took a while before they actually stopped thickening his drinks. Sometimes they would bring just a cup of hot water and no tea bag. Sometimes they would bring just a tea bag but no hot water. They would eventually get it right. If my father missed a meal (usually due to a scheduled follow up appointment off site) they would get him something off the supplemental menu; usually a peanut butter sandwich. I have to point out something that Gem has no control over, but it is something you might want to take into account. This is December 2024 and there are two major construction jobs on the same side of the street as Gem; one is two blocks to the north and one is just south of Gem on Fair Oaks. This makes traffic on Fair Oaks an absolute nightmare. It can take you 20 minutes just to get a half a block if you are trying to get into their parking lot (plenty of parking). When scoping out Gem we noticed that it was a bit cleaner than the others we toured in the area. I also noticed they have good landscaping out front and a well-maintained inner courtyard. This includes some windchimes and bird feeders in the trees. They also have a salt-water aquarium near the front entrance and a fresh -water aquarium on site. Both are maintained by a service that comes in. The way I look at it, if they can afford to have these extra amenities just to make the place look nice, they must be doing something right. This was also one of the only places that has an actually well maintained, decorated, and comfortable waiting room right by the main entrance. There are signs all over the place saying that 9PM is quiet hour and I did see staff enforcing that. However, there were times when their own people could have done a better job following that rule. They would shout down the hall at one another about what a patient needs, or who is coming in tomorrow, sometimes way after 9PM. My father has hearing aids and they were getting misplaced. He was having difficulty putting them in and they would end up on the floor from time to time. I would search for them, find them either under the bed or in the track for the sliding glass door. One time, they were sitting on the adjustable table and a staff member almost put a tray of food right on top of them. They were very adamant about keeping track of every patient's personal belongings, but I would suggest they try a little harder with hearing aids. Finally, I have to say, I did see one cockroach. It might have been an anomaly, but there it is. The place still looks clean and smells clean overall. Again, do your homework first. Check all places out in person first. If the place feels right to you, then it probably is. read more

My mom has been a resident since June 2023. She has had excellent, loving care. Their care and concern for the patients heath has been unmatched w any other facility I have looked at. Until recently, all visitors had to COVID test every 48 hours and wore masks. The front office staff is excellent. Sherr is always friendly and genuinely concerned with how my day has been when I arrive. My dad visits twice a day and the staff is very kind to him. The nursing and CNA staff are attentive and very forthcoming with information. Even though I visit daily, I will still receive phone calls if they feel an update is needed before my arrival. GEM has been a blessing for my mom and our family. This has been a challenging transition for us but GEM's staff has made it as easy as possible, while treating the patients with respect and kindness. read more

This is place is hell. 1. HORRIBLE FOOD. Won't feed the food they serve to my dogs to be honest. It used to be better according to the staff I dont know what happened. 2. They DO NOT grant special food requests to patients because they say "It's not a restaurant". 3. The kitchen was instructed by the administrator to limit the food served to patients. 4. Physical therapy is good but nursing is awful. Almost killed my mom with mismanagement of meds. She ended up going back to the hospital with internal bleeding but only after I insisted. Woman in the next room was supposed to be on a liquid diet (recovering from a stroke) they gave her solid food. She almost choked. read more

Well my mother lived at GEM for about 10 years and she was very comfortable enjoyed the activities and staff especially Benny the chef. Everyone was so kind and thoughtful tried as best they could to help families deal with having a loved one placed there cuz it's such a difficult transition although my mother was so easy never made family feel bad/ guilty for having placed her outside of her home. Absolute nightmare! Checked out my mother as soon as I can! Admitted my mother in this hell hole in June and here's what happened: I see UberEats delivering patient's food! Run! Patients refusing horrible food! I don't blame the cooks coz I'm pretty sure this is the admin slashing kitchen and dietary budget, so the cooks are only working on what they got. I used to work for a daycare so I know admin and dietary supervisor are in cahoots! Run! #Shameful. read more

My mom was recently admitted at GEM Transitional Care Center for short term stay for rehab. She was discharged home this week and thanks to the team at GEM, she has improved a lot and almost back to 100%. Great staff, exceptional care and delicious food. Thank you team GEM!! read more

Care was good. Such a super hard job. Just because you have a title doesn't mean you can do your job. With that said majority of staff were very nice n accommodated with MOM. So i leave a 4 star. If I could give this place a ZERO! I would!!!! This place is a HELL HOLE!!! They have no compassion for the elderly and I'm willing to bet they abuse them! We placed my great grandmother in their care, my GREAT grandmother!! She's 90!!! They did show her any care whatsoever! We went to visit her on Friday, we found her ON THE FLOOR! CRYING! NAKED! In only a diaper!!! We immediately called for help! And asked them to call 911. When asked what had happened, how did she end up on the floor?! They had no answer! They don't even know how long she was left that way! they didn't remember who or when she was last check on! THEY KNEW NOTHING!!! But those beds have railings on them, so I doubt that my dear old sick and fragile great grandmother was able to do that on her own... which makes me outraged beyond words. Luckily she didn't break any bones but she has a hugeeee bruise on the side of her head!!! So disgustingly rude and unprofessional. We have filed a complaint, placed a police report, & are now seeking legal action. This place needs to be SHUT THE EFF DOWN! DO NOT TRUST THIS PLACE WITH YOUR LOVED ONES!!!!! read more

My father had spinal surgery a month ago. He had lost all strength below his neck and would soon become a quadraplegic. After a successful surgery, he was transfered to G.E.M. for physical recovery. Every time my siblings and I walked in to the facility we were greeted by extremely pleasant receptionists - Joanna and Sherr My family and I were received by Jeanette, the admissions coordinator, who made us feel at ease, answered all of our questions, and provided comfort during a stressful time. The team at G.E.M. provided mental therapy, not only to my father, but also to my family through their friendly talks, and positive attitude during our daily visits. My dad's CNA's (Yvette, Marci, and Jackie) provided amazing care and never made us feel like we were asking too much. They truly went above and beyond what was required of their job. My dad would like to say that his LVN's were always very sweet and he never saw them look annoyed. His stay at G.E.M. was also physically successful due to the diligent care that was provided by his physical therapists - Tina, Donna, Gabriel, and Claire. They pushed my father to do his physical therapy and he is not able to walk, and do most things on his own. My family and I want to thank the entire team for a truly amazing experience. I hope that others who need this care are able to experience it. read more

When it comes to caring for your loved ones, there is no perfection, however I will say GEM is close to it, nothing is 100% what makes this place so great, let's start with the administration, very hands on, sincerely wanting to make every day better then the next, the staff, extremely helpful, there is always one or two that don't meet the criteria, lucky for them, the rest of the staff makes up for it, the facility is clean, if can use some upgrades, but it truly is well kept, now to my absolute favorite part, the Physical Therapy staff, this could not get any better, the kind of care they gave to my mother, along with all the rest of the patients while we were there, was bar none the best, they are caring/loving/knowledgeable/helpful/friendly/accommodating, I can go on and on, can't say enough wonderful things about them. Overall my mom stayed there for 3 weeks, she came there in an ambulance, wasn't able to take a step without falling, she left there walking 100 feet without complaining of any pain. kudos to the staff. My grandfather loved this place. One disclaimer is there are different wings depending on your insurance. My gramps was in the Medicare, not medical wing. And everybody hates dealing with medical patients Bc a common complaint is medical never pays. The admissions staff were amazing. My gramps was in some awful places after his stroke and we were praying for a miracle. Gem was it but I had to change his insurance plan to get in. He loved the staff and it just gave him the hope and strength he needed to recover and come home when everyone thought he wasn't gonna make it. He went in there super skinny, unable to swallow and on oxygen with a big ass bi-pap mask at night. And a g tube feeding tube. He hated it but he fought on. He left eating regular food and walking at least 80 feet with assistance. He eventually got rid of the mask. He has passed since then but Gem was a savior and we are forever grateful. Gem was my second home and they let me cone and go as needed. read more

My experience with Gem Transitional Care Center has been has been very positive. My husband arrived for what we thought would be a brief stay after having hip replacement surgery, but due to his muscle atrophy & other problems before surgery, he has needed the ongoing care of Gem. It is a stressful, scary & overwhelming experience needing to select a skilled nursing facility & navigating the complicated insurance process & applications. From my first contact with Gem to my ongoing daily contact & visitation, I have found the staff to be be very friendly, supportive, helpful & have been impressed with the teams care of my husband. I have very mixed feelings about GEM: 1. The facility itself is beyond old & ugly; totally depressing. 2. The food is inedible. 3. There are some angels working there. Others, not so much. 4. My husband picked up 2 free-of-charge infections there. 5. Rocky, the PT guy, is the best ever, period. 6. Odessa, LVN: two big thumbs up. 7. The docs are not around very much. 8. They took GREAT care of my mom during her month long stay after she had her entire jaw removed and rebuilt after cancer. They were respectful, prompt and attentive. Due to the fact that my mom had her teeth removed, she was given a special diet of purreed food only. They made it as possibly appealing as they could for her. The case managers were very sweet and helpful. As for the facilities: they were a bit dated, but my mom wasn't walking around much. What we loved is that she had a room and they gave her the bed next to the sliding glass doors which looked out onto a patio area. It also provided a little more room for us when my mom had multiple visitors (rooms don't have much space for more than one guest)... so we would go out and sit right outside her room on the patio. It provided a nice break. I saw a review that said the physical therapy was a joke, which I totally disagree with. They were patient and VERY helpful to my mom, who had a bone taken from her leg to rebuild her jaw. They helped her relearn to walk, use the toilet and really started her off on a good foot. If needed, which I hope it is not, we would definitely return to Gem for nursing care. THE ONLY NEGATIVE we found was a nurse named Marsha, whom we asked that she would never be allowed to be assigned to my mom again... and they kept that up. We truly never saw her after I complained. She was not nice to my mom and refused to give her ice chips (one of the only things that felt good in my mom's mouth) because she said my mom had "had enough". Grrrr. Still pisses me off. But at least they kept her away after I complained. I am hoping they fired her, actually. Rant over. read more

My 97 yr mother has been very well cared for the week of May 2015 , she is recovering hip replacement . They have properly treated her with respect . They have her walking , she is clean and they are controlling her sugar levels. They have trimmed her nails & done her hair. Some of the ratings are older on this does not reflect today's employees and care. And some are not rated because they forgot to fill in stars. USC keck recommends this place. Horrible place, don't send your loved ones here!!!! The experience at this facility was extremely frustrating. Main problem is staff doesn't really care about what they do, which translates into horrible patient care. Someone dear to me was placed at this facility after knee surgery, and ended up in the hospital low sodium levels, with an ulcer on her leg because they never removed her leg brace. My experience was a nightmare. It is filthy, in poor condition, terribly understaffed, and poorly maintained. In my shared room, the sliding door to the patio did not close completely so when that area was hosed off, the water came in our room. The CNA who gives showers (only one per week is allowed) has to store her boots in the small closet in our room. Though some staff members were nice, several told me the pay is low and, as one CNA told me, the expectations of staff are low, as well. Staff members do not respond to call lights - I think that the high patient to nurse ratio makes it impossible to be attentive. A positive closing comment: at least I didn't get beaten up while I was there. read more

This place was awful. My father lived here for a few years and was almost dead before being transferred. I was always fighting with the staff there. His heels were necrotic, which means they turned black from loss of blood flow to that area. Unfortunately he was unable to move on his own, they kept him in the same position most of the time. He was given meds to keep him sedated most days so as to not have to deal with his dementia. No stimulation. They never got him up. There was mold in his suction machine. The Administrator, nurses, CNA's, and the doctor assigned to my father were the worst I have ever dealt with. He was finally transferred to the hospital after my sister demanded they do it. After his stay in the hospital for over a week, he was transferred to another facility where he received amazing care. He was doing physical therapy. He was so much more alert. The staff actually cared for him. He has since passed, but i'm grateful it wasn't because of GEM's negligence. Short term care there is different. The rooms are nicer and they take better care of you. When you become a long term patient they throw you in another room with two other patients and forget about you. Take your loved one there at your own risk. read more

My dad spent the past few months in three different physical rehabilitation facilities, and he loved being at GEM. He found the staff to be kind, attentive, and supportive. He raved about the delicious food. He got excellent rest and rigorous physical and occupational therapy, until he was well enough to come home. During the many hours I spent with my dad at GEM, I got the feeling that the staff members--the nurses, the assistants, the therapists, the chef--really like their jobs. read more

I wish I could give it 3 stars because of Kenn (the case manager liaison) and Rocky (the PT), but I feel that would be a disservice to others reading this review. My mom was brought here to do her rehab after she fell and fractured her left hip and the time she spent there was okay - to be honest I couldn't wait to get her home. However, before I complain, let me just say that - Kenn is great and great on communication. Rocky is AWESOME! What a God-send to GEM. However - the rest of the staff (minus a few LVN's who I can't recall names - sorry) are AWFUL!! Especially the RN, Mejer and the Health Care Partners Nurse Practitioner, Abiola. Omgosh - I can't express my feelings for these two, especially for Abiola. All I can say is if your parent, or loved one, is admitted to GEM and Abiola is assigned to them - RUN! Take them someplace else or request another nurse practitioner. She shouldn't be doing this job - she is rude and clearly shows she hates her job and patients. When you speak to her on the phone - she will hang up on you and avoid you like the plague if she sees you in the facility. But - like I have to say again - thank goodness for Kenn and Rocky!! read more

I was going to go for 4 1/2 stars if possible. My 88 years old father had 2 stays at GEM, each after hospitalizations. Although the facility could use an update, the place was clean and had no odor whatsoever (a real positive in skilled nusrsing places)! The staff was kind and compassionate, and our family felt at home with them while we visited my father. They are very flexible on the visiting hours, and allowed us as much time as we wanted and needed there. The bedding was always clean, and he received personal grooming daily. When my dad took a turn for the worse, the staff was so wonderful, and went out of their way to help in whatever they could. I would recommend this facility highly, although not perfect, we would use them again! read more

I found GEM Transitional Care Center online after my grandmother had a pretty extensive knee injury due to a slip and fall (torn ACL, meniscus and tibia fracture). We had gone to two other physical therapists, and found both of them either lacking in their attention to detail, or a bit zoo-y with patients stacked up behind patients. From my first visit with Gabe the PT, it was clear that GEM was detailed oriented, passionate about their work, and extremely skilled at what they do. My evaluation was thorough, specific and Gabe approached my grandmother's plan for therapy with a lot of thought and consideration. Now, four months after therapy, I have Gabe to thank for my grandmother's progress and ability to re-assimilate in to her everyday life that she love's and had missed. I strongly recommend these guys - they're head and shoulders above the other clinics I went to and their strengths are ones that matter - attention to detail and really loving what they do. read more

In the interest of full disclosure you should know that my mom spent her next to last days here. So I am definitely biased but trying to be honest and informative so that this is helpful to others in what is an excruciating and emotionally trying time. My sister and I had to quickly find an assisted care facility for my elderly mother (a retired RN) when Keck kicked her out after repairing (another story) her broken femur. Our experience occurred about 14 months ago. Keck recommended GEM along with a few others and out of the two we toured, we picked GEM. The recruiting staff we interviewed seemed professional, compassionate and competent. They give you a fair tour of the plant. Facilities: This has a very industrial feel to it, nothing soothing in the surroundings as it is in a busy commercial/medical area of Pasadena. The hallways and common areas are just ok-clean. The nursing home smell is just about everywhere but I expected that. Food service is unattractive and true to stereotype. In fact, patient food service at other hospitals I had the chance to observe around that same time were much better and surprisingly not-that-bad. Who decided on those salmon-colored plates you see in care facilities anyway? Access and visiting are good. You can stay just about as long as you want; however, seating in patient rooms is nearly none. If your loved one is mobile, you can visit in one of the common areas. TVs are early-90s vintage. It was mid-August and the A/C was not up to the task. A few requests to the nurses resulted in no appreciable improvements in air handling. Staff: The nurses are attentive, helpful and nice. The advertised doctor is barely noticeable, seems like this is a side-gig for them, we were never sure when they were going to show up. The physical therapy is a joke. Unlike the other review, the only one who did extra to get mom into therapy was the lead PT. The others went through the motions with little enthusiasm. Her plan (seems like most of the other patients were on a similar schedule) encompassed two 30-minute sessions - one AM and one PM - except on Saturdays (only one) and none on Sundays. So start with a regimen that has you sedentary for 23 hours a day. The therapy itself was of such low exertion, it is hard to imagine being able to build any strength and rehabilitate through it. This was not just related to my mom as we saw the other patients doing similar exercises. If you are working with the nurse or are indisposed when the PT comes for your session, it is likely that you will not go to that session because they did not come back, except for the lead PT. As I said at the start, mom was not a win for GEM; in fairness she was pretty weak coming in, although she was active until her accident 10 days earlier. After about 10 days at GEM, she caught an infection that elevated her temperature to 102+, whereupon she was taken back to the hospital. Bottom line: If you want to get better and have the will to do so, this might work for you. If your loved one requires more motivation and encouragement (like ours did), look elsewhere. read more

If I can give more than 5 stars to this facility I absolutely will. My father was a patient at GEM Transitional Care Center a few months back after a stroke. The hospital where he was told me to prepare for the worst because it was a massive stroke. I went to GEM because his doctor spoke very highly of this facility. The staff was absolutely very caring, professional, and knowledgeable. They met with my family immediately and laid out their plan of care. Being in the health care industry myself, I was very impressed. Their nursing and rehab team worked very hard and motivated my dad to the point of him regaining some of his functions. They were even able to wean him off his feeding tube! Now my Dad is back home with us. In case he will need this kind of care again, I will take him back to this wonderful facility in a heartbeat. read more

My father is 96 and stayed at gem for few months. In general the staff is good. However the so called social worker Ken takes sides (other than the patients) and is nothing of a social worker. There is an employee named Tony who blatantly stole $200 from my father by confusing him and someone took 5:20s from his drawer while he was sleeping. At least laundry comes back. read more
