My brother has been a patient/resident at this facility for quite some time. I am extremely happy with the level of empathy, care and compassion he and the other residents receive. From the CNA's all the way to the Director of Nursing (DON) Anytime there is a slight change in his medical condition, someone from the facility is quick to inform me. I meet at least once a week with Nurse Martinez and we discuss any questions or concerns I might have regarding my brother's medical care/condition. My concerns have always been promptly addressed and or corrected by the medical staff. My brother is very happy there. I have asked him several times if he wants to come home and he says NO. He enjoys the different activities they provide from playing bingo, dancing, coloring movie night and his favorite, candle light dinner. My aunt was admitted to this facility after having a major stroke. She was unable to walk, use the bathroom, or eat on her own. The therapy she received here was life changing. After a few months of intensive physical, occupational, and speech therapy she was able to get well enough to safely come back home. My grandfather was in this place and it was terrible, they didn't do anything to improve the help when we would speak to them about what needed to be done and the way they feed them and just let them be and not wanting to move them or doing any type of activity for the the patients or wanting to change them when they needed to be changed and let's not even talk about the fact that my grandfather was attacked by another patient and they didn't wanna do anything about it at all. This place is no good! I would never recommend it for anybody to put any of their loved ones in this facility. They do not care for the safety. They do not care for the physical therapy. They do not care for the patient whatsoever. They just leave you sitting in your bed and do not do any activities for your mobility to come back they leave you sitting in your feces And in your urine pamper you get bed sores. They do not do anything about them. They just say oh it's just a cut. I visited College Vista as a possible place for my grandmother. It was a difficult visit for me obviously but the nurses reassured me and were nothing but helpful and compassionate. I had the most contact with nurse Vanessa Cruz. She was very knowledgeable and showed me around the facility. Overall the facility is very clean, with many hard working CNAs. My grandpa was here at College Vista last month and wow Am pleased with all the people I meet everyone was very nice. This was the best team of medical staff I have been around in a long time. We are grateful to have my grandma at this Rehabilitation Center. She has improved a whole lot from the time she went in. The staff there are amazing and have a lot of patience. They have taken good care of my grandma especially since she's so delicate at the moment. The type of care they do to the patients there is amazing. They take her out of her room every day. They do a lot of activities with them to keep them distracted. What we also like about College Point Acute is that they are bi-lingual speaking nurses. My grandma only speaks Spanish and we are glad that the nurses there are able to understand her. read more

My mother has been in this facility for two yrs now. The facility is strategically located in the Eagle rock area which is accessible to the neighboring cites. First let me say that the Staff was not only professional, but made me feel like family. The nurses and CNA's were very helpful and catered to all my needs. I was here for Therapy for my recent back surgery. The Therapist; Physical and Occupational were excellent. They made me work hard, but got me to where I could care for myself. My mom is currently staying at College Vista since August 2022 . I toured different nursing homes before in 2013 for my Dad, and CV is one of the cleanest facilities I visited. My Dad passed away a year after at CV. The former owner was very kind and treated us like a family. I used to go to meet different shifts of nurses and caregivers, and they were all great! That's why when Mom needed to go to a nursing home, I chose College Vista again. I didn't know that there's a change in ownership and, of course, and with new regulations, things were different. I'm happy to see there are still some CNAs working there now from Dad's time. They're very caring and compassionate. All of the CNAs, now with the exception of at least one that I know from current experience, are very caring and compassionate. The nurses are great. One thing, though, some visiting families don't understand that they get busy when they're doing their medicine rounds, so they may not be on their stations at certain times. Morning shifts tend to be busier than late shifts. Late shifts , I think, are more attentive to patients & visiting families because some patients are already sleeping or resting and not so chaotic. I'll give a shout-out to some who has bent over backwards for my mother especially when she was isolated (non Covid related), Nurses Alicia & Francisco ( they helped us see Mom through video chat few times and give us updates), Justine (front desk), DON Angelica, and the superstars are the CNAs ,a lot of names to mention but you know who you are, they keep Mom clean and warm, feed her when she couldn't by herself ( only pureed), and very attentive to her needs. I still choose College Vista any day from any other nursing homes I've seen and heard. Thank you very much, and on behalf of my family, we're very grateful to all of you. read more

I am so pleased to donate some of my personal photographic art to College Vista. I toured and loved talking to residents and staff who were amazed that I was the main photographer for the new art that was showcased. My visit really touched me when a resident, Flora, pulled me aside to give a complement of the staff and the Administrator Mike. She wanted to show me her room and her new furniture and TV she got last month. I was at Glendale Adventist today visiting a friend when I heard about this Nursing Facility & decided to tour. The neighborhood it's in is very charming. The building itself is much bigger on the inside than it appears on the outside. The first thing that stood out to me was the cleanliness & pleasant smell. "Alyce" gave me a tour & introduced me to the Director of nursing and the Administrator as well as the rehab director. This staff is very attentive & the building is in great shape the floors look brand new. I've seen enough good things here to make me feel comfortable in having them care for my loved one. It's not too far from the freeway & It's still very close to Glendale Hospital. read more

When I toured this facility, I was truly impressed. The Director of Nursing gave me a thorough tour, answered all of my questions, was very friendly, and really shared her feeling that it was like one big family. There were no smells, the residents were up, and engaged in activities, and each staff member smiled at me. A great feeling when you walk in to a nursing home. I have toured many nursing homes and you can really get a sense of how they care of patients, with how they treat a stranger coming into the building. The staff was protective and stopped me, but pleasantly asked how they could help. If my mom were in this facility, I would want every visitor to be stopped before randomly entering. I felt like this was a safe and secure facility. I toured the rehab room and had many questions. Their director of physical therapy was very knowledgeable and actually sat down with me to answer various questions, some of which were not about rehab. Everyone is very welcoming and friendly from the cna's to the receptionist. The environment is lovely and they actually make a effort to tend to all her needs, which is such a breathe of fresh air. I love how the activity department keeps her involved and moving which shows me how they truly care (especially the young lady in activities). Interstingly, this skilled nursing facility is a, "Diamond in the Rough" in Eagle Rock, CA. I say this because my father had been diagnosed with prostate cancer a couple of years ago. Unfortunately, he has passed. To get to the point, upon his transfer from Verdugo Hills Hospital for a few days, College Vista was the place. At first greet, the front desk staff is always cheerful, hospitable and open to hear anything. I really like the home - front feeling that a patient recieves. Better yet, it's more of a familiarity type family-oriented mood that floats in the air here. This is quite important as most of the patients are on the latter journey of life. There are all sorts of care treatments available. The staff who specialize from administration, skilled nursing, senior health-care, out-patient sourcing, to the overall cooking team, all indeed have great chemistry. This is a good reason any and all new patients should look into at a skilled nursing facility. The time left I had with my father and his stay here is a lasting memory in the sky. He's with me mind and spirit and I can truly give credit where credit is due, College Vista is again a truly sound place. 5 Stars in my book! read more

I am the emergency contact and medical decision maker for a good friend with mental illness who spent several months at College Vista. The staff there was first rate. Dr. Bassuk and Blanca, the social worker, always took my calls or returned them promptly to keep me in the loop. Communication is often a problem in these situations, but College Vista was amazing. I had to drive a long way to visit my friend and a nurse or aide always called to let me know if there was a change in his schedule or routine. The nurses always took my calls and provided the information I needed. The CNAs were so friendly and loving to my friend. It was always clean, and well stocked with supplies. This Skilled Nursing Facility is absolutely horrible. DO NOT SEND YOUR ELDERS HERE. The staff here was extremely disrespectful to my grandmother when she was admitted here. They did not do a wound check on her, which is essential to perform on seniors with diabetes. When she was released, she had a ankle wound that became infected. When they sent their doctor to assess the wound, he dismissed it. We went to three other doctors and all said it was an infected wound. It seems like DO NOT care about the physical nor the mental health of their patients. My grandmother shared stories of when they would mock her and our family when we would try to check in with her. They're extremely rude and they have their own staff like jin kee M. write reviews on Yelp to boost their reviews. I'd give zero stars if I could. read more

My experience.... My first impression of College Vista was very good. My husband needed rehab and at the time I felt comfortable having him there. I was able to visit with scheduled visits and he was not far from home. Unfortunately, my husband's health declined while there. He entered able to walk with the help of a Walker, but left unable to walk. He also got Covid 19 while there and was admitted back to the hospital. He got weaker and weaker. My visits after that became harder to book and the Activity Coordinator made it harder for me. She seemed too busy. She complained that she had so much to do, but I was on the outside looking in. I had to see my husband even though it was limited to 20 minutes to -1/2 hour a visit. I was only given three days to find a new place for my husband because Medicare would not and could not continue with his physical therapy. I became very stressed from this ordeal. I feel that I was not given ample time to find an appropriate place for my husband. I wanted him nearby. The Social worker sent me too far to look at possible new places. I finally settled on one nearby, but the cost was very expensive. I know that Covid 19 played a huge factor on everything from restrictions and visiting. I hope that the nursing skilled facilities can get more experienced help and that no one goes through this. It's been hard. Dementia is hard to deal with, along with the other medical issues with my husband, but I just felt that College Vista gave up on treating my husband. Note: I did meet some exceptional nurses there. If you are looking for a solid option for short term rehab, this is a good place to start :) Close to Pasadena and major freeways for easy family access. The registered nurses here are also unskilled and do not know how to assess the patients. They are here to "practice" their skills. Don't let them practice on your family. Dad has been there since December 2013. I have to say College Vista doesn't have the bells & whistles compared to other facilities but its gem is the staff. All nurses, the kitchen staff, housekeeping who maintains this facility so clean everyday. The nurses are so friendly & patient. My Dad suffered a minor stroke and went to Rehab here, mainly because it was closer to my house and I would be able to visit daily with ease. From day 1 I received warm greetings from Somony, the Admissions rep who was informative, kind, and took time to discuss my concerns with my Dad's condition. The nursing staff there were all attentive, patient, and compassionate. From the head of nursing Mark, to the Charge Nurses Carrel, Marion, Aaron, Alyssa, and Kevin to the CNA's Ricky, Daniel, Papito, and Lupita. The Physical Therapists Maureen, Oliver, Monica and April all were really encouraging and great working with my Dad. The Occupational Therapist Jeannie was super kind as well and their support made my Dad's stay at CVPA much more comforting as I knew he would be cared for properly. A big shout out to Ann, the Social Services Manager, whose hard work, tenacity, patience and support helped us transition my Dad to long term care at the time when he was ready. So grateful she was there for us! Although the facility is small, this is the characteristic that provides the family type of atmosphere that eases one's mind when a loved one is staying there, while on the mend. Last but not least I really appreciate the Activities Director Yakelyn, who really takes her job to heart as she works really hard to keep the residents engaged, entertained, active and smiling!
