My pain and discomfort was addressed by the staff and my stay was pleasant considering what I was at the facility for. My caregivers, Arsinio, Carmen and Molly were EXCELLENT. My case manager kept me advised of my coverage and my rights. At a time in my life, when I needed help the most, this was the right place to be. Thank you all for your dedication and knowledge. Administrators who are easy to contact, understand the nuances of insurance and Medicare & Medicaid. 10/10 8. Location is central and near highway access in the NW valley. 9. Staff, I have to mention them again, a rehab is only as good as its staff/ 1000/1000 I'd love to mention them all suffice it to say we only had great care. 10. My husband is in great shape he's has been a Freedom Plaza twice now, both for Cardiac rehab. He's here smiling he knows what to do to keep him in good shape, Educating him was very important there were a lot of misconceptions. 11. My mother was 89 when she was there, and we're about to celebrate her 91st birthday she had done a job on her femur bone, they helped her learn to walk and strengthen her leg. Thank you so much. Again, it's awful when you're given a sheet of paper and told your insurance covers these facilities pick... IF YOU PICK FREEDOM PLAZA you've made a great choice for your beloved family member who is counting on you to be their advocate.

There are not enough care givers which means long delays in responses. Then when they do respond, it's like you bothered them. I'd only recommend this place to someone I didn't like.

There are a few good nurses but too many are lazy and they aren't well staffed. My mother and aunt were here at the same time. My aunt had a stroke but they didn't want her to go to the hospital. My mom kept saying that she'd wait for hours to get help with anything. I talked to other patients who said that they'd been sitting in their own urine for hours. I also noticed that they leave the dinners sitting in the hall for way too long time before handing them out. The dinners were there when I showed up at 4 and I left at almost 6 and they just started handing them out. You have to stay on top of them to make sure they're giving medications. I heard one of the nurses say she got confused and must have given pain medication to the wrong patient. My mom had been there for over 5 days and they didn't shower her. I saw a couple of times patients that voice frustration are met with more than 1 nurse at their bedside using authoritative voices and it almost feels/looks like intimidation/borderline bullying. This place shouldn't be open but bare minimum needs to management. The photo I am sharing is of all of the doors with lights are patients needing help, dinner cart sitting out full of dinners, and filthy hall. There's 1 nurse and 2 CNAs per hall. The CNA's were MIA and the Nurse was just standing at the end staring at a computer screen ignoring the lights and dinners.

Impact Physical Therapy has been amazing so far. I was in an accident that left me with several…read moreinjuries, and being in my third trimester of pregnancy has made getting treatment extra tricky. A lot of places either don't have experience treating pregnant women or are super limited in what they'll actually do, which can be really frustrating, even with my OB's approval. I was going to another PT place before this and honestly felt kind of hopeless. They weren't very personable or confident, and I felt like they were too scared to touch me, like I just had to wait until after I gave birth to get real help. I knew I was capable of doing more and wasn't improving at all, it was starting to take a toll on my mental health. Deciding to switch to Impact was the best move I could've made. The care here is night and day. You actually work with physical therapists one-on-one, not just techs, and everyone is super friendly, professional, and knows what they're doing. They offer a bunch of holistic treatment options like cupping, massage, dry needling, stretching, and strength work. I've only been going a little over a week, and already I feel so much more hopeful. I'm willing to do the work, I just needed the right guidance, and they've absolutely given me that. They don't treat me like I'm fragile or extremely limited just because I'm pregnant, and that means so much. I'm excited to keep going and finally feel like I'm on the right path to recovery.
