Staff dismissed repeated family complaints that their loved one wasn't right, refused to call 911, and by the time EMS arrived--summoned by the family--the patient's blood sugar had plummeted to a critical level in the 20s. Staff then withheld the EMS report because the family wouldn't sign an AMA form, despite the patient facing death within hours without that intervention. The physical therapy and ot are better than other snf's. They did a lot of wiping down of equipment in the therapy room but i wondered what the dwell time was and if it did any good. I wasn't 100% into therapy because i didn't know what we were doing day to day. The room is nice because the bathroom is 100% accessible. My room was at the end of the hall; i often smelled smoke in my room coming inside from windows. There was a small refrigerator & microwave; however, the sign warned the fridge was just a cooler and not for storing perishables. I asked for tissues and never received them so when i was crying i blew my nose on my sheet. There was one CNA & Nurse on duty at night time. There was a man down the hall late one night asking "Is anyone there? Is anyone awake? Help."So after a while i pressed my red button and someone came to check on me. I asked for something silly like ice and asked if they heard this guy talking. The answer was no, (she was wearing one ear bud). She thanked me for letting her know. Housekeeping came to clean and vacuumed each day....that was nice. But the rooms have carpet and i wondered how they were sanitized. Universal Precautions used by staff are questionable. Breakfast was great and hot if you went to the dining room. Okay, in room. Dinner & lunch options were not very flavorful and there is a few combinations of veggies that they use in recipes. A few of the optional meal items were....okay. I ate lots of pasta (it was great!). I was pretty much stuck in bed and my room was untidy. I would have lots of styrofoam cups that built up. It was difficult to find ice packs and they didn't have the ones with the plastic bags inside. Each day i may or may not have known who my nurse or cna were or did i know the date. It took me a while to catch onto how things worked during 24 hours. Fresh i water was brought in at 3 or 4 am and then someone would come to do vitals. This place is a joke and they give you nothing but lip service. All they care about is the money (which I understand because it's a business). If you don't agree with their discharge plan they will bully you out of the facility. If you have Medicare just know that you have appeal rights but they will make you feel uncertain about using your appeal rights. They will say that medical progress notes say this, that and the third but when you talk to the actual care aides taking care of you they will say something totally different so what this means is that their progress notes don't mean shit. The only thing that matters is the doctor that comes in to see you less than two minutes and what he writes down for notes. Stay away from this joke of a care facility! Just because it's in a nice clean part of town doesn't mean the facility is a reflection of that. All they care about is the money. It's literally a revolving door for Medicare funds. They will bully you so that you agree to discharge even though you are not medically ready and you can even have health care professionals vouch for you but mysteriously and conviently their input and notes don't get added to your medical profile or taken into consideration. The insurance doesn't want to pay anymore and that's fine because they will start paying for ER bills for my mother when she falls because she literally can't walk and she is a fall risk. So rather then ensure that she is at least stronger and safe to walk at least with a cane or walker to accommodate her balance they rather discharge her because they don't want to pay. Ok, well you can just pay emergency room bills then idiots and as far as the administration at this facility you guys can go to hell. read more

My husband went to Advanced Healthcare Glendale for rehab but died 12 days after I got him transferred back to the hospital. Within 24 hours of his admission there, I saw that he was declining, requested he be sent back to the hospital. His nurse, the Director of Nursing and the administrator minimized my concerns, did order some tests, made a med change and assured me they could meet his care needs. Bottom line: he continued to decline to the point of requiring emergency hospitalization because he had a new acute complication; the end result of which was his death. He didn't go there because there was no hope of recovery from complications with pancreatic cancer. We knew his life would be shortened by that disease, but I have no doubt that the 4 day delay before they agreed he should be re-hospitalized hastened that process and may have made this episode irreversible. Even though he didn't go there to decline or die, his decline seemed acceptable to them. The aide said that she didn't know what his baseline was when he was total assist, unable to swallow or make his needs known; all of which he'd been able to do 3 days earlier. Advanced Health Care is a lovely-appearing skilled care facility. The day nurse Alec and night nurse (Wanda?) were both very nice. No one ever seemed uncaring. Our family member was admitted to this skilled care nursing because they had an infectious process going on and was in contact isolation. He was in the facility for two full months, "being treated" with antibiotics that we told the physician didn't work when he was in the hospital, but the doctors insisted that they "were working and he was getting better" even though he looked worse than when he arrived. We suggested him to go back to the hospital but they said "he is getting better". We got notice that his illness was gone for a week and he was to be discharged. We made arrangements for a place for him to move to because he was in worse condition when he left that when he arrived (I thought you were supposed to leave in better condition?). He got in my car, immediately threw up all over it, then when we got to the home he was supposed to stay at he had explosive diarrhea all over the shower. We took him to the hospital WITHIN HOURS of being discharged from Advanced Healthcare to find out HE STILL HAD THE SAME ILLNESS HE WAS SENT THERE WITH 2 MONTHS PRIOR, THAT THEY INSISTED WAS GONE. AND he unnecessarily exposed many people to this contagious illness he had by not testing to make sure the illness was really gone. I have used Advanced Healthcare before for another family member years ago and had a good experience so I thought it would be the same this time. That was not the case. The hospital asked if when my family member was ready to get out of the hospital again if I wanted him to go back there and my resounding answer was absolutely not! I am just glad that no one ended up getting sick from the negligence of this facility. read more

If you or a loved one needs some extra care from a nursing facility, this is your place! My mom just spent a week at this place and I can't tell you enough about the wonderful treatment she received from all staff! The people here are so caring and always treated my mom with dignity & respect. She felt 100% terrible and scared when she arrived and by the time she left a week later she was hugging staff goodbye. She came in needing help to go to the bathroom, showering, and moving around. The OT & PT (Doug & Dana especially) staff gave her hope and once she learned how to use the wheelchair and then the walker there literally was no stopping her. The nurses were equally helpful checking on her daily, administering care, and answering questions (Patricia & Andrew!) and a couple full time staff LPN'S in particular really stood out...Montarra, Jimmy, Kyle, and Reina (registry LPN). THANK YOU ALL for the care & concern you provided to my mom! My mom loved the food and said it was delicious. They have a full-time chef whom my mom also got to meet. The decor at this place is pristine...a very beautiful place inside and out. Mom's linens were changed regularly and her private bathroom and room were also kept clean. They have a gorgeous gym with up to date rehabilitation equipment and sidewalks and patios outside for more exercise or to just get done fresh air. The place is open 24/7 and none of our family had any problems visiting my mom or staying there as long as we wanted. The staff was also always very nice to our family. I have to admit that when it was happening, I was skeptical about my mom having to stay at a place like this, but now I am grateful we picked this place and will keep it in mind if any of us ever needs extra care outside of the home. Also, this place provides in home health care as the patient improves their condition & can go home. This place is located on 63rd Ave/Bell Rd. in the NW valley not from Arrowhead Mall. It's a very nice location and not fat from the 101, so it is easy to get to. They take Medicare & Tricare for sure. I'm sure they take other insurance but you'd have to call to find out for sure. My dad has needed a rehabilitation center twice in the past 2 yrs. & as my family toured several facilities both times it always came back to Advanced being the place we felt comfortable with. The facility is clean, accessible, safe, professional yet made each patient feel like they were part of a community not just a "rehab." The staff in itself made him & our entire family feel cared for & listened to. The administrators, nurses, aids, wound care, therapists, drivers, kitchen staff & housekeeping were all apart of making his stay more bearable since he couldn't be at home. Everyone who was apart of his recovery team was extremely patient & understanding of his condition. The took the time to work with and explain everything to him and also included us. I recommend them 100% & if needed for him or any family or friends we will go there. read more

I gave Advanced Health Care of Glendale one star because it didn't give the option to give zero stars. My father was in this facility because he had fallen and fractured his pelvis in two places and suffered a burst fracture of his L-5 vertebrae. Less than a week after being admitted to the facility he fell in their bathroom and couldn't get up so he had to stay on the floor until someone found him. I asked for an MRI or x-ray to see if he had done any further damage and they refused, saying, "He appears to be alright". I asked how many people on the staff had x-ray vision and he laughed and told me nobody. My brother tried calling Wyatt, the facility manager to talk to him about getting treatment for our dad. Wyatt refused his calls. My dad never got an x-ray. He was in considerable pain after that fall and went downhill fast. My father also had Alzheimer's and one night when my brother and l went to visit through the window, my father was nowhere to be found. We went and rang the doorbell to tell them we were looking for our father and gave his room number. We were asked if our father was a patient there. Again, we gave the room number and she said she would go look. She was gone quite a while and when she came back she was rather annoyed because our father had made himself comfortable in the wrong room. She then told us he wasn't supposed to do that. She said he was in his own room so we went to visit him. When he looked up l held up my phone and told him l was going to call him. I told him we had been looking for him and he said, "l guess l was in the wrong room. I'm senile now, you know." We never talked about him being senile. After that, it became apparent that my father was dying. I called and spoke to the nurse in charge of releasing patients and told her we wanted to bring him home to die. She assured me that they weren't worried about him dying. I didn't share the same sentiment, but she asked me to talk to the gentleman in charge of my father's rehab. I did and my mind still didn't change. She told me that they needed three days notice. It had been three days but she didn't count the time until l spoke to the rehab guy. My dad came home Easter Sunday, April 4th. He was missing most of his belongings. We weren't allowed in to help him pack. I called and told them l wanted his stuff. When I gotta there to get it, l was told the cleaning staff had already cleared out his room but she managed to find his stuff. We never did get his toiletries. My father died April 14. I would not let my dog go there for rehab services. If you have a family member that you care about, please look elsewhere for your needs. read more

I can't say enough about advance Healthcare. The staff is considerate, concerned and knowledgeable in all areas of hospice care. My mother has suffered injuries from a separate falls. I can't tell you how grateful we are for a number of caring employees. My mother was in need of delicate care both at home and at a shared home facility. The RN assigned to my mother is Robin Anderson who has gone above and beyond the call for care. My mother took such a liking and trust towards Robin. The goal of the hospice team is make sure their patients are comfortable during their final chapter of life. There are so many others to thank Lisa Brewster, Tony Robis, Dave and so many more. BUT AGAIN,,, I AM SO HAPPY MY SON CHECKED OTHERS AND CHOSE THIS FACILITY!!!! no one can please everyone, & i 'm sorry for any negative experience some have had. This facility gets all the stars for appearance. I kind of wish the grounds keepers were the care givers for my dad. They would have done a better job. He was admitted from Mayo to finish an anti-viral med and get strong enough to go home. Instead, he was neglected, and digressed. I can confidently state that had we been allowed in, his circumstances would have been far better. I respect the fact that, due to COVID, we can not enter the facility. They should take this into consideration, and up the staff and care of their patients. We were forced to visit through the window, and again, I understand that is out of everyone's control. He was in a South facing room, and the outside temperature was usually over 100. (August/September) The first day he arrived, I called to ask how he was adjusting and to see if he needed anything. The only reason dad's nurse returned my call (over 4 hours later) was to inform me (she was obligated to do so- her own words) that dad had fallen trying to get to the restroom unassisted. She assured me he was unharmed, and safely back to bed. She would make the same phone call a few days later- from her personal cell phone. I don't answer unknown, out of state numbers, and I feel it's rather unprofessional for staff at such a facility to call family from their personal cell phones. She left a message at 5:30 in the morning, . I then called the facility directly and it sounded to me as if someone in the background was helping her to figure out how to check her messages. Dad had stated that the suppers were "barely edible" So we took turns (my sister-in-law) and myself bringing him foods we knew he would enjoy. We did have to remind the attending staff to open the blinds so we could see that dad had gotten the food we brought, and we had to remind them to put it where he could get to it. They were pretty good about setting it out of his reach. And he was tucked into the bed so tight, he couldn't get up for a fire drill if he wanted to. I question why the blinds were closed once we got to the window and they came to help him move from the bed to the chair by the window. One male nurse did NOT close the blinds, and he seemed very kind and gentle. Dad even said this nurse was very pleasant and helpful. He didn't remember his name We were told daily that he needed clean clothes- so we dropped off a few sets of every thing he might need. I promise you - he did not get into clean clothes on a daily basis here. Dad's last day at the facility, the blinds were open, and we could see the food we had brought for him, but we couldn't SEE him, so we called the front desk to see if he was out of the room for PT/OT. Heather told us they stay in their rooms for that, and he should be in his room. His restroom door was open, and we could see that he was NOT IN HIS ROOM. We peered in the window and FINALLY he was moved by wheelchair closer to the window. He was in very bad shape. He couldn't lift his head to see us, and he couldn't talk on the phone. We left, really discouraged, thinking he must have been pretty tired. Low and behold - the facility tried calling- three times. I called back and finally spoke to Regina, who was noticeably upset, stating that my dad was unresponsive and she was calling an ambulance to get him to the closest hospital - he needed a brain scan (her words) Within a very short time, the ER concluded that he was dehydrated. He was then transferred to Mayo for treatment of pneumonia and COVID. This was Tuesday afternoon. I went back the following Friday to get his belongings, being certain he would NOT be going back to this facility. I kid you not - we received enough clothes for him to go on a month long cruise and never have to wash anything and THE GRILLED CHEESE AND TOMATO SOUP WE BROUGHT HIM ON TUESDAY. I would give a few more stars for the folks that answer the phone, and the one male nurse that was attentive and kind. But I can not get past the fact that DEHYDRATION - caused at the hands of my dads caregivers, was misdiagnosed as a stroke. read more

Mom has been here for about 2 weeks. She loves it and honestly I'm thinking about moving in myself! :) Very clean! Staff is WONDERFUL! Mom loves the food! Physical and occupational therapists are encouraging, positive and caring. We researched prior to admitting mom. This place is like no other that I've ever visited. This place is a fancy $hit hole. If your loved one needs more than just strength rehab do NOT bring them here. My grandmother was brought here after a hospital stay and she ended up falling out of bed and breaking multiple bones in her foot causing her health to go from bad to worse. She was left alone for 10 hours and not checked on. She apparently tested positive for Covid and not one adult relative was informed instead they told a child that was in the room visiting. Then proceeded to reprimand us for exposing others. First off we're grieving the fact that our loved one came here for rehabilitation and is leaving in hospice care. Why was it up to us to figure out the medical aspect of this? The director Wyatt pretended to care to save face but he doesn't care one bit. Don't let the fancy walls fool you It's all a facade and they don't give 1 single damn about the patients. Once they realized she was basically gonna die they wrote her off and left her to suffer. Be very very careful when considering where to take a loved one. This place is NOT it. read more

I just took my 86 year old mother home yesterday after a 2 week stay. Her shoulder was replaced, and she feels that due to the excellent services of both in house physical therapy and occupational therapy she is weeks ahead in her recovery than if she had gone straight home. Staff was nice and attentive. Mom loved the food, I ate there twice, it was good. Mom hated the food in the hospital, and hardly ate. Once I got her home and settled she remarked that after the 5 days in the hospital, she was sure her stay in rehab would be longer than the 2 weeks. We both only have good things to say about Advanced Health Care. My biggest is thank you for such excellent care of mom. read more

This place is wonderful. They have been so friendly and helpful ever since we rolled through their front doors, and they've treated my grandpa like the prince he is. I've spent a good portion of my time in town with him in his room, including crashing on the comfy recliner chair a few times, and the CNAs, nurses and physical therapists have been more attentive and caring than all the other places been been seen combined. He loves how beautiful and comfortable it is there and is the happiest I've seen him in a long time. I'm glad to finally be able to trust he's in good hands. read more

Don't let the beautiful interior décor and meticulous landscaping fool you! The staff is rude, unhelpful and after my mother was there for 17 days, she was in the hospital ER 24 hours after discharge with three separate UTI's. She was admitted and hospitalized for five days. This facility will bill the patient for services, that under Medicare guidelines, are to be paid for by the facility. We were very unhappy with Dr. Patel. Advanced is not to be trusted. I advise you to look elsewhere for the care of your loved one. read more

Advances Health Care Rehabilitation facility is definitely a 5 star facility. It is like being in a resort. It doesn't feel like a rehab facility and certainly doesn't look like one. While it isn't a perfect place it comes close. My mother was a patient there for 3 weeks and got great care, great therapy and the people were really nice. We researched facilities online and then drove to our top choices to interview them before agreeing to release mom to their care. The place looks like a 5-star hotel. What got our vote was the staff. I was encouraged to speak with anyone, patients included. I heard all positive words and was given a thorough tour and another chance to ask questions. Mom loved this place and the people.
