I will say that all of the staff were very supportive and attentive to his needs. Every staff member was so good with my husband. The quality of care he received was very professional and caring. My husband was very appreciative to the staff. He never experienced any problems with staff. Nursing staff and aids were great. Physical and occupational therapists spent a lot of time with him encouraging him and they were very patient and pleased with his progress. The facility was always clean. My husband could be difficult sometimes and with the encouragement he became more confident and willing to work hard. Thank you to all of the staff. They were so pleasant and supportive to not only my husband but to me as well. Now mind you my husband had spent time at two other facilities and I can honestly say one of them I had to have him moved out of it. It was filthy and the staff just left him laying in his bed everyday. Avalon encouraged him and I cannot thank all of the staff. I never had any problems with anyone nor anything. Now the food was ok, not too bad but could improve a little bit. If my husband ever had to go to another sniff it will definitely be Avalon. Thank you to everyone that helped my husband with his recovery.

They don't make you feel like you're bothering them when you need something. No matter what you are having them do. Cleaning you up, getting ice water, or just opening the blinds. There is 1 nurse on the night shift which is very grouchy and rude. She gets off at 10:30pm and i called to be changed and she said "Your couldn't wait for 10 minutes to push your button. Excuse me if I dont know your schedule and I can't control my bowels. Maybe she shouldn't be taking care of elderly patients.

I am allergic to most perfume, so discussed it with the supervisor. She apparently thought I was the only person who was reactive and did nothing. A couple of days later, I smelled her wearing perfume. I was also disappointed there was no decaff coffee. I can't be the only one with high blood pressure. It seems that a healthcare facility should always have decaff for patients.

On June 23, my family was getting back to the house due to traffic and a host of medical procedures, medicine pick-up, and camp for kids with special needs. I sent a text message to the ResCare employee to go ahead and enter the house, it was extremely hot that week and the house has air conditioning. Karen Moutain ordered the care provider to not go in, but to leave. On June 24, I text the care giver that my son might be late coming back from camp, once again, I offered her the confront of waiting inside. Later that night, I received an email from Karen Moutain, that ResCare was canceling services with us, because employees are not allowed in the house when clients are not home. After this event, whenever one of us had a doctor's appointment or had to be at work, Karen Moutain would delay or/cancel the assignment; thus interfering the our ability to make a living and confounding our healthcare. We contacted ResCare Corperate with the ethics complaint, we asked for a continuation of services with a new manager, but they have not contacted us at this time. They did agree to continue services until we have a new agency, but changed their mind after they sent a caregiver to the house 2 and a half hours early and blamed it on us despite the fact that I had text the caregiver of the change in time a week prior and emailed the supervisor. We I attempt to contact the supervisor I discovered they had gone on vacation.

One day you may call her and she's happy to hear from you, then the next day she has a horrible attitude and makes you feel stupid for making a mistake. Example? We are assigned task sheets as a caregiver that the client has to initial (basically showing what was done that day), I ACCIDENTALLY marked essential shopping instead of medical transport when I took my client to her appt, instead of just calmly explaining to mark medical transport next time, you get attitude. I know 3 other caregivers personally and they all have the same complaints. But other than that, clients are taken care of which is all in all the main concern. Just don't work there.

While I was there I noticed his call button go unanswered so I brought it to their attention. While I was there - they were fine - as soon as I left they started ignoring again. Things are just in such disrepair that he needs to call for assistance. The remote on his bed doesn't work. The toilet seat slides off the bowl... then there are the two screaming men across the hall. It's terrifying for anyone to listen to that. I just want them to fix their things, and answer The call buttons. I know patients abuse it, but in this case he got hurt because of the neglect.

On top of that she refused to help her get dressed because she could "do it herself". She had then apologized but only because I had informed one of the managers. It's sad to see almost all the residents being treated like they're animals. Tell me, would you feel comfortable seeing so many residents being yelled at and talked about as if they're not in the room? As if they aren't a human being? They place has now changed ceos or top managers I'm not sure, and I can tell they want the place to change for the better but like I have stated before, the nurses there do not care about the residents. I tried my best to help residents in any way I could even though it wasn't my job. Most of the residents would not receive their correct food which was such a big issue because of food allergies and difficulties with solid food. There has been many times where patients wouldn't get their food at all! And the nurses would refuse to get them what they asked or get their correct food. There needs to be more activities for residents who cannot attend in person groups! It's sad when I would walk by rooms and patients would be staring out the window because they were tired of watching tv all day. I can tell the activity department only liked the residents who were able to attend the group in person. There is so many residents who would complain to me about not being included! Lastly, I advise if you do send your loved ones here to keep in contact with them to find out what goes on. There has been so many times where staff just like to do whatever they want when visitors aren't around.

Staff is attentive and very pleasant PT and OT were very good and made sure they gave my husband the attention needed to regain his strength Cons Parking is horrible. I spoke to the Administrator about when I saw at least six staff members parking in front rather than around the back so there was no room for visitors, He was almost rude and pretty much stated it wasn't his problem. Food was less than stellar. They were supposed to take orders so you didn't have to eat the meal of the day, sometimes that happened and sometimes not. Way over salted but when my husband mentioned that he was told they didn't salt anything. Might want change who the food is sourced from. Beds were so uncomfortable according to my husband. Activities were not something most would want to participate in, It was better than some but there is still room for improvement and if the higher staff members don't see a problem then nothing changes

The only reason I am giving one star is because a zero or negative rating is not an option…read more My 94-year-old mother was admitted to Avamere Transitional Care of Puget Sound on a Saturday evening with casts on both arms extending from her fingers to above her elbows. Upon arrival, she was served a meal consisting of a hoagie roll with two very thin slices of turkey, a piece of Kraft cheese cut in half, cold fries, and fruit that was unrecognizable. This raised immediate concerns about basic care and nutrition. The goal of short term rehabilitation is to help patients regain strength, improve their health, and return home to their normal activities. Unfortunately, our experience was the opposite. My mother became weaker with each passing day. Given her condition, it should have been immediately clear that she required assistance with basic tasks. However, it took until the following day for staff to recognize that she was unable to feed herself. When assistance was requested for toileting, a CNA stated in front of me that my mother could use her diaper and would be cleaned later. This appeared to be standard practice, as she was frequently left in bed until the afternoon and not regularly assisted to the restroom. As a result, her hygiene declined significantly. A physical therapist later expressed concern about her condition, noting that at 3:30 PM she was still in bed and had not been properly cared for. Communication throughout her stay was extremely poor. There were constant staff changes, and no clear continuity of care. Nursing and physical therapy appeared to operate in separate systems with little coordination. Despite multiple requests, we were never able to speak with a physician at any point during her stay. During this time, my mother developed pneumonia. We have since moved her to another facility. Within the first 30 minutes of her arrival at Orchard Park, she received more care and attention than she did during the entire two weeks at Avamere. I fear for patients who do not have an advocate, as they may be left with little to no care. Overall, the level of care, communication, and coordination fell well below expectations.
