After a week of back and forth with Union Gas I was told the Abbey Air tech was just too lazy to fill out the form and that neither myself nor the retailer were authorized to do so. Abbey Air offered no apology and acknowledged no wrong doing but said they'd apply the $111 to any future work they do. How generous.

Rene was able to help confirm my suspicions about the leak and helped offer a couple cost effective solutions vs redoing the whole stall.
