Made an appointment. Made calls to providers to have prior…read morehearing tests faxed to them. It took two phone calls and two emails to get the pre visit questionnaire sent to me. Filled it out, ins info, medication list, etc. Was unable to submit. Was told I would have to come in and do it. At which point, in the phone call I made to them, I was told that the office would be closed on the date that my appt was scheduled. So, I now have no appt. They offered me another appt a month out and it won't work w my schedule. My level of confidence with this place is very low.
