Three and one-half stars. I had been going to this office since 2013 when the powers that be changed their tune about primary care doctors prescribing opiods. I saw both doctor, Dr. Karamian and Dr. Ilbeigi. I did not care fot Karamian as her injections caused me pain and I also found her to be too clinical. And a bit rude. I loved Ilbeigi as he was kind, did pretty painless injections and fixed a problem that the staff messed up. once I told him That they inadvertently changed the dosage of my meds. Sadly Dr. Ilbeigi retired abruptly giving us all 2 weeks notice. Needless to say this was a problem. My body decided to have a sciatica flare up during this transition that hit the 10 level for a couple of weeks. I managed to use a muscle relaxer to help with the pain and allow me to sleep a few hours Intermittently throughout the day. Pain meds do very little for nerve pain. Finding a new pain management Dr that will give me the medication I was taking so far has been difficult . I had to wait a month before I could even make it to lay in the back seat of my girlfriend's car to get to these appointments. I thought I was okay with the first doctor but they shorted my prescription and never bothered to explain why nor how to go off these meds. when I called they told me to wait the month to speak to someone. I tried their main office as this clinic has various offices throughout Southern California, guess it's a money maker, however I got nowhere with her either. The next Dr berated Dr Ilbeigi and myself for even being on these meds. He was rude condescending and kept me waiting for 2 hours prior to seeing me. When I returned to Dr number one who is not a Dr but a rotating staff of nurse practitioners that one sees monthly she was under the assumption that I was told about the decrease. I wasn't. I was told she would keep my meds as is but that my next appointment the doctor might change them. No one mentioned nurse practitioners. Their main concern and to be honest both drs concern was following the regulations and abiding by the criteria set by the FDA or the powers that be with little concern for the patient. They wanted to cover their ass more than aid the pain patient. It has become pretty much a one size fits all. Since I had been in pain for years I tried various treatment. The one that changed my life, the pain meds. I went from being home bound to getting certified to teach water fitness which I did for 12 years until my next decline and sadly had to qui Teaching and working out. At that time I went all out trying injections, acupuncture, chiropractic (legs went numb) nerve blocks, mindful meditation, yoga and Pilates. The final thing that ultimately worked was massage which I paid out of pocket for over a year until my masseuse moved away. So if you want try Karamian go for it. Oh, she would not see anyone of us who had been a prior patient which I believe demonstrates her attitude and "first do no harm" way of practicing Medicine. I had hoped to find a group of prior patient so we could exchange notes on who best to see. I'm going to be 80 in April and I have been in pain since 1987 going on pain meds in 2003 when the policy was, "there is no cap on how much you can take of these drugs to this". I simply don't have the energy I had 13 years ago when I had to quit teaching and was in such good shape to Dr shop. Pain erodes your body and more so your spirit. Being told to detox myself without guidance is malpractice in my books but I know it's common practice in theirs. I have always been very careful with how I take these meds. After all they changed my life. I respected them unlike many of the providers who prescribe them. Good luck to those of you looking for pain management. I hope you find one you like read more

10/2025 Office staff leaves a lot to be desired. Long wait times and a general feeling of nonchalance.

Dr. Karimian is professional, brilliant, kind and compassionate. I've been to almost every pain mgmt Dr in the High Desert, they all treated me like I was a junkie trying to score pills. Dr. Karimian found out through her thorough exam process that I had over 17 herniated disc's in my back. She has helped me tremendously, I've been with her 3 years! The office staff is remarkable. They are kind, efficient, and easy to deal with. My only issue is getting a call back after leaving several messages. That doesn't happen often. read more

If you can under pain management you know how difficulty it is to find the right person. The right doctor. They must be in my opinion my humble opinion as a respiratory therapist, the only way to go see an Anesthesiologist. They're highly equipped in school and giving proper pain Management management! In addition he's a high-risk you urologist. They do all the procedures in house. You are lucky if he shows his high desert pain management. I got what she called her 'concoction' shot in my back and got a spine infection. I ended up in the hospital, not being able to walk for 2 weeks and had a pic line for 6 weeks for antibiotics. An approved referral was sent in January and I didn't get my first shot until August. The person that took care of referrals for her is a joke. The whole experience with her was terrible. read more

Dr. McDaniel is an incredible surgeon. He has been my gynecologist for several years now. I would highly recommend him. I was nearly in tears when I came to him because I was in so much pain and my hormones were all over the place. He has taken the time to listen and solve all of the issues I was having. My only regret is not knowing about him earlier. read more

This is a very nice and friendly place to go for pain management. Dr. Karimian is very caring. She takes the time to listen to you and explains everything in detail. When she gave me an injection in my lumbar spine, she did it so good that I could not even feel it. The receptionists are all really nice too and the medical assistants are all very nice. There is not a better place to go for pain management. There are other services here too. Urology is one of them. I would definitely recommend this place. read more

High Desert Pain Management - Pain Management Near Me - Apple Valley, California

I'm not here for treatment, rather as a driver for a friend receiving treatment. Not too nice of a place. Nurses were discussing with patients, there urinary tract infections and treatment options in the middle of the lobby. The music they play is terrible and does not match what the clients might listen too. Floor and doors were dirty. Approx 45 minute wait WITH an appointment! The air conditioner worked well. I'll give them that. Hope to not come here for treatment. Not personable and/or friendly staff. Maybe management needs to take a look!? read more

Loved Dr. Ilbeigi! He took the time to explain things to me and was very thorough with his testings. It's not easy finding a personable doctor who actually cares about their patients now-a-days. Front office, Daisy, was super nice and accommodating to my scheduling needs and was able to get me in around work hours. I've had good pain management in the past for a chronic condition. Since starting with Dr.Ilbeigi ive been treated like a child. He asked what has worked for me in the past (since 2001), only to prescribe something totally different and at a trivial dose. He most recently wrote for something that isn't even on formulary so I've been without pain medication since Thursday and it's now Monday and I am still without pain control and waiting for a callback from HDPM. read more
