Home Care: After 30 days in the hospital and 30 days in a rehab facility, Horizon Health came to rescue to help my Mom regain her independence in the comfort of her own home. The home care staff took time to create a plan that fit her needs. They provided Mom with emotional and skilled support 2 days a week for an additional 7 months. And for me (the daughter) I was able to spend time with Mom doing things we both enjoy. read more

Director at Harmony House of Brainerd. This woman plays favorites for family members. Residents are neglected and she doesn't correct this, even though multiple people have reported issues. read more

History

Established in 1991.

Horizon Health, Inc. was created to help seniors remain living in their homes and communities as long as possible. Our four assisted living facilities focus on memory care, with one facility for gentlemen of whom most are veterans. The programs continue to grow in order to serve individuals with disabilities, transitioning to independent living, and those who need income-based rent options. Horizon Health, Inc. has added caregiver support groups and specific sessions for those with memory illnesses and their caregivers. Continuing to add services, our Faith in Action volunteer program provided over 1800 rides in 2021 to seniors who are homebound and need transportation to doctor appointments and the pharmacy.
