On Tuesday 9, 2026 at approximately 10:00 AM, Mr. Roth visited our family to discuss hospice care options. At that time, we informed him that we needed additional time to discuss these options as a family before making any decisions. Subsequently, another member of the care team met with us, and after further discussion, we decided not to pursue hospice care at that time. We assumed this decision had been communicated to all relevant members of the hospice and care teams. However, on Thursday at approximately 8:45AM, Mr. Roth returned and again began discussing hospice care. Because our family members were not all present, my father called my sister so she could participate in the conversation. During that discussion, we clearly communicated that we intended to continue pursuing treatment, including chemotherapy. Despite this, Mr. Roth continued emphasizing that my father's illness was terminal. While we fully understand the seriousness of his diagnosis, the manner in which the information was presented felt unnecessarily pessimistic and dismissive of our family's decision to continue treatment. Rather than feeling supported in making an informed choice, we felt pressured toward hospice care despite having already communicated our wishes. Additionally, communication barriers appeared to arise when Mr. Roth attempted to speak Spanish. Given the importance of discussing complex medical decisions, we believe professional interpretation services or communication in the language in which he is most proficient would help prevent misunderstandings and ensure patients and families receive clear, accurate information. We understand that hospice is an important option that should be discussed when appropriate. However, we believe patients and families should feel respected and supported regardless of the treatment path they choose. We hope this feedback can be reviewed to improve communication and patient-family experiences moving forward.

I am eternally grateful for each and every single one of them for helping my husband and I walk his precious mother to heaven. They have provided the most professional guidance I could have asked for. Always available. Always supportive. Always ready for a hug if I ever needed one. Following discharge from a five day stay at Sunrise Hospital for acute pain from spinal & leg…read moreissues, I was transported to this "skilled nursing facility" for care and PT rehab at the recommendation of Judy at Sunrise. Upon arrival at the facility I was taken to a room with a bed which was motorized but wouldn't work and looked decades old with an extremely uncomfortable plastic mattress. The room's furnishings were very dated and the recliner so old my husband covered it in a sheet so it could be more sanitary. The bed, the recliner and the AC unit were all very noisy from age. The room itself had damage on the wall and had the look of a much needed update. Staff was kind and courteous but my husband and I were immediately concerned when we asked if my meds had been acquired as I'd been told they would before I left Sunrise. I was also told that I'd be seen by a doctor they'd assigned to me within 24 hours. As the day progressed and still no meds were there and we realized they were past due and one anti seizure one must not be late, we asked but still they only offered excuses. My husband went home and got my meds there because we couldn't get a straight answer. I was non-ambulatory and used the call button after my husband left for help to my wheelchair to use the bathroom and kept pushing the button and no one came. This went on for half an hour and I finally tried to get to the wheelchair and didn't make it and went down when my left leg gave out. Pain was excruciating and my blood pressure was steadily going up, still no meds. CNA & Nurse were there and tried to help but it was explained to me that the ratio of patient to CNA was 13 to 1 and patient to Nurse ratio was 15 to 1. They had no time to spend with one patient. The next morning my husband came very early and still no meds or visit from the doctor they'd assigned and my blood pressure was so high the CNA couldn't even get a reading on her portable machine. We asked that I be seen by the doctor but were told that would not happen but he was sending his Nurse Practitioner since now my BP was at 191/110. The Nurse Practitioner was very knowledgeable and concerned and ordered an emergency medication to get the BP down quickly . My husband was very upset that we still had no meds nor had I seen a doctor since I was in acute pain and my BP was so high. He started to pack my stuff up and asked to have the resident Case Manager, Melissa Maguire, come to speak with us. She was very helpful and arranged medical transport from the facility to Boulder City hospital (we live in BC) where I was stabilized from the high BP and pain. I never returned to this facility and the lack of proper care is something I'll never forget. They're terribly understaffed and incapable of acute care because although they tell you doctors come each day and the paperwork you receive says you may choose your doctor, when we requested to see the doctor we never did. I asked to change from the unseen doctor Jason Gubler to any who was present and I was told no he was assigned to my care. After perusing his reviews on several websites I should count myself lucky. Don't bring a loved one here if they require acute care nor one who isn't mentally cognitive because they'll suffer the consequences. My family took me home to care for me and we employed Home Health Services and I'm so happy with my progress.

We had been anticipating home hospice and the 4th of July weekend did not look to be a good time to start. I made the phone call anticipating a Voice Mail session. Surprise, Michelle answered and took relevant information. On the Friday Holiday 4th of July I received a call from Harmony liaison Joe Trujillo - he did a DocuSign via email for legal documents. On Saturday the July the 5th Nurse Mia arrived to complete the intake. On Sunday July 6th Nurse Mia did a follow-up visit. This all happened so rapidly and on a big Holiday weekend. There was a following cascade of RN's, CNA, Social worker and religious support. The care and compassion that was delivered was superb. Many many thanks to our dedicated care giving team- Joe, Michelle, Mia, Nicole, Christine and Reverend Solomon who were with us to our dear wife and mother's passing. Watching a loved one's demise with dreaded Dementia is nothing short of agonizing and Team Harmony surely helped relieve some of the burden. Thanks again and a very Well Done!

Well I called and got an answering service. I don't want to leave a long winded message, so I waited 5-10 minutes and called again. This was at 4 in the afternoon on a Friday. Second time I called back, a recording asked me to leave my name and reason for calling first before the person on the other end would accept the call. Really? They're screening calls? That doesn't seem caring and compassionate to me, so they lose my business, and yes, it is a business as they get paid very well by insurance for each patient they take on.

The facility is beautiful in a Tuscan theme. The staff is amazing, they are the nicest empathetic group of people. The food is a good variety. The only qualm I have is my dad isn't able to feed himself and has been going without food. He won't ask for help because he doesn't want to bother anyone. My 92-yearl-old dad stayed and rehabilitated at AHC (Sunset and Jones location) for a couple of…read moremonths. He had suffered stroke-like paralysis on one side for both his arm and leg. We thought his recovery was an extreme longshot at best. However, thanks to the truly superb staff, he was able to slowly but successfully regain his ability to walk as well as a lot of his abilities to be somewhat independent. This would not have been possible without the amazing medical assistants, nurses, physical and occupational therapists, wound care specialists, case managers and even their maintenance man. When dad first arrived, and for most of his stay, he was completely dependent on AHC staff for bathing, toileting, getting food, and all. The staff was so attentive, patient and understanding (he did get grumpy and stubborn at times as he was dealing with a lot of medical issues;-) They never missed a beat and motivated him to keep working hard and improving. Without hesitation, I highly recommend this AHC for rehab. A few names I must recognize: Himani, Jordan (therapists), Mary, Janet and Chanel (the CNA's), Luz the head nurse, her daughter the woundcare specialist (forgot her name:-) and Arnelle the maintenance man. Arnelle didn't even have anything to do with patient care, but he had such a positive, smiling and motivational presence and got to know our dad and kept him smiling and laughing on a daily basis which is hard to do when someone has just lost their independence and is in pain. A huge thank you to the entire team; they are really like a family.
