It started out to be just a two-week respite period, but upon consulting with people in my life who I hold in high regard, it was recommended to make it permanent. This did not bode well with her, but we proceeded and moved her in July of 2022. Best decision EVER. It took a while for her to acclimate, but within a short period, she adapted and became quite popular with residents and staff. I affectionally call it "the Resort", because if they had a pool, I'd have moved in! The grounds are lovely and well maintained, as is the interior. Clean public restrooms, no odor, well-furnished and esthetically appealing. The dining room is very inviting and well-kempt and staff is very accommodating The staff is personable, caring, attentive and kind. There are a multitude of activities every day and weekly outings outside of the Resort, including dining out, shopping and the casino! My mom had to check her calendar to see if she was "available" for me to visit. They also have a salon for haircuts, styles, etc. as well as offering manicures on a weekly basis. There is transportation available every Tuesday and Thursday for residents to attend medical, dental and eye appointments. They also host events for families a couple times a year (summer picnic and Holiday luncheon) and they're very well attended. I'd be remiss not to mention "Happy Hour" every Friday at 3:30. :-) Green Tree is their pharmacy and can supply all of your loved one's medications pre-portioned, packaged and covered by Medicare. The meds can be self-administered or by the nursing staff, depending on the resident's capability and family preference. The use of Green Tree is not a requirement. ESL also has in-house PT/OT staff and equipment on the premise (also available to Legacy residents). They are wonderful and patient. They want to see the residents be comfortable, strong and thrive and will do all they can to make that happen. The nursing staff is on-point, as I received calls late at night if my mom was having an issue. My mom was hospitalized on January 8, 2026 and diagnosed with Flu A virus. She remained there for 10 days and sent to a skilled nursing facility (SNF), from which she was discharged on February 20. We moved her back to her own apartment at ESL under hospice care. She passed away on March 8 at the age of 90 1/2. Between residents and staff, it was like a parade in her apartment. Everyone was so incredibly supportive, it left me speechless. I have been back a couple of times and plan to do so at least once a month to visit with the friends we made there, both residents and staff. I have and will recommend Evergreen to anyone looking for an assisted living or memory care facility for their loved ones. Thank you ALL for what you do. Susan

Two days after she arrived, she was left unattended on the toilet and fell off the toilet and had to go to the ER. She became dehydrated while she was at Smith Crossing, which was confirmed by urinalysis. She tested positive for a UTI while at Smith Crossing but was not treated for it. I asked several times to speak with the attending doctor or nurse practitioner about the UTI and dehydration and was told they would call me but they never did. My mother has significantly declined while in their care and I believe it is due to the neglect, dehydration, and untreated UTI that she experienced at Smith crossing. Two days before she broke her arm, she walked around shopping with me for over an hour and now she can hardly walk 40 feet. Please do not send your loved ones here if they are unable to advocate for themselves. It may be beautiful on the outside, but the actual care is atrocious.

My dear mom finally made the decision to place my dad here after caring for him for years. His dementia had gotten to the point that my mom's quality of life was really awful and she was exhausted. Mom did a great job of keeping him safe at home even with stairs. He never had an accident. First night at Brookdale he "fell"and gashed his head. Several days later he "fell" again and broke his hip. This began the most horrific hospital stay where he almost was starved to death and ultimately hospice at home. We were led to believe they were able to handle all levels of dementia. They cannot. If your loved one is still really with it maybe this place would be ok. But otherwise do not send them there. The communication is horrendous, they are woefully understaffed and they do not watch them nor care for them the way you would at home... not even close. We thought he'd be safer, turns out mom was way more capable than a whole facility of "trained" staff. Dad suffered for weeks in hospice not understanding that he couldn't get out of bed with his broken hip. He fought so hard trying the whole time to regain his independence and dignity. But ultimately I think he realized he wasn't going to get better and gave up. He died after a long and extremely sad deterioration. We are heartbroken as it didn't have to be like this. Please keep your parents home with help if you can. I wouldn't wish this nightmare on anyone.

While I ensured her safety, I second guessed my decisions as she vocally struggled to adjust; it was a lot of change particularly for someone who rarely left her home. Almost 6 months in, a different set of doubts & worries took over. Did I make the right call, could she live somewhere more affordable, would she outlive her money, etc. I don't even recall what search term led me to Senior Living Experts, but I'm grateful. Rebecca listened to my story and made me feel less alone. She thought where my mom was at was the best option in terms of cost, desired location and care needs addressed, but would do the research and circle back. We also discussed the care she received currently and provided a few suggestions. As promised, Rebecca followed up and confirmed her initial assessment. Yes, I'll still worry about her finances, but I now have a better understanding of the landscape and what comes should her care needs increase.
