Checking into the facility, I noticed it looked a bit old and had walls being repainted BUT it looked and smelled very clean. Despite this, I has my reservations about leaving my mom. It was the first time that we had been in a situation where we required extensive nursing care outside of our home. Then, I met Lela. She is a nurse manager at the facility. Immediately she sensed my apprehension. I expressed that I didn't want to leave my mom. She encouraged me and offered reassurance. She said, "Maybe the building needs TLC and we're working on it but, I guarantee you, the people here are the best at what they do and you're mom will excel." So, although I was hesitant, I left my mom, and returned the next day and the next day and the day after that.... What I witnessed was truly remarkable. Having been a patient and being a Nurse Practitioner, I can tell you that medicine isn't always kind...except at Sonterra Health Center. Every single staff member that the facility has amassed CARES for their patients. They take their lunch breaks in their patient's rooms if they notice someone feels lonely, they come in with a smile at the beep of every call bell, they gladly change out food trays if the least bit of disappointment is expressed. Every single person has gone above and beyond to support my mother as she heals. They even provided my mother with a volunteer opportunity to do some social service, which lifted her spirits. The testament to the staff was that when it came time to go home, she didn't want to leave! I will never use another facility for skilled nursing. Hands down, the best place!!!

I want to be fair: the CNAs, nurses, and therapists were some of the kindest, most compassionate and hardworking people I have ever met. Many work extremely long shifts doing physically and emotionally demanding work. My criticism is not directed at them. My concern is management. I became especially concerned about an elderly resident who told me she was terrified of her roommate, who has Alzheimer's. She reported being physically attacked and waking at night with her roommate standing over her threatening her. She became so frightened of her room that she sometimes preferred sitting outside in the Texas heat. I personally approached the facility director, explained the situation, and asked him to intervene. He promised to look into it. Before I was discharged, I confronted him again and asked whether this woman's situation would actually be resolved. After leaving, I checked on her and learned that the problem still had not been meaningfully resolved. Sonterra later contacted me about my evaluation. I again explained the situation and was assured someone would investigate. I checked again afterward and was told that nobody had come to see her as promised. This particularly bothers me because this resident appears to have nobody consistently advocating for her. I cannot know why her concerns have received so little attention, and I will not accuse anyone of motives I cannot prove. What I do know is that a frightened, vulnerable resident repeatedly asked for help, I personally asked management to help her, and I still saw no meaningful resolution. During my stay, I also had concerns about the food, insects in resident areas, staffing levels, and what I personally observed as a lack of meaningful interaction between upper management and residents. My advice to families is simple: there are wonderful frontline caregivers at Sonterra, but advocate strongly for your loved one. Visit frequently, ask questions, pay attention, and don't assume management knows what is happening simply because staff have been told. I have written commendations for Sonterra employees who provided me outstanding care because good people deserve recognition. But vulnerable residents also deserve someone willing to speak when something is wrong. Every resident matters, whether they have money, family, influence, or nobody at all. They are human beings deserving dignity and safety. I tried repeatedly to address this internally before writing publicly. My mother was encouraged to come to this facility to recover after knee surgery. She was told she…read morewould get excellent care here and that there were nothing but strong reviews. We didn't have that experience at all. The rooms were small and shared. My mother's roommate said that she had been waiting for batteries for her tv remote for 4 hours and that the tv was stuck on sports and on a high volume. She was still waiting for a walker as she had already been there for a couple of days and had asked for bedrails but they would not be there for a few more days. She also mentioned she had been waiting for hours for pain meds. When my mother asked about bed rails they also said it would be three days before she would have them. Upon arrival in her room there was a pair of dirty socks on the nightstand and a nebulizer that had a used mask and medication. We pointed it out to the staff and they shrugged and left them where they were found. My mother was in pain and we asked about medication. They offered Tylenol around 7pm and we had to request an ice pack for her knee. We asked about her evening meds and they said they hadn't received anything- we were concerned because my mother is diabetic and has other health conditions that require regular medication. The staff seemed unconcerned and dismissive. We received a call from my mother at 1:30 AM that there were bugs crawling on the walls, the room was hot, and they had not provided her with pain medication or another ice pack. She felt feverish around 3:00AM and we went to the facility and they hesitated letting us in as hours of visitation are only 8am-8pm. They had the nerve to tell my dad that my mom did nothing but complain. We were shocked at the lack of compassion. We felt so sorry for the people who rely on this place for rehabilitation and also for their long term residence. We strongly ask case managers who recommend this place to actually go visit and see where they are sending patients for rehabilitative care.

My father was a resident in Pathways from October until his passing in May. He was only 63 years old. During his time there, we encountered repeated and deeply concerning issues that other families also experienced. 1. Financial Mismanagement Despite repeated requests, we were never provided invoices during my father's stay. Charges frequently increased without explanation, and management was consistently unresponsive to emails and phone calls. We at one point were told we owe thousands of dollars without ever knowing why. Yet after their audit, it was confirmed that we did overpay by thousands of dollars--yet we still lack full transparency or assurance that all overpayments have been identified and reimbursed. We never figured out what was going on. 2. Medication Misuse and Chemical Restraint We witnessed multiple incidents of medication errors, including staff administering the wrong medications during my father's panic attacks--sometimes giving him medications meant for other residents. Even more alarming, there were repeated reports among families of chemical restraint practices, which are illegal in the state of Texas. We are one of those families. Families were often told their loved ones were "aggressive" and required additional medications or private caregivers--an added, unexpected financial and emotional burden. We were part of a family council where these issues were openly and frequently discussed by multiple parties. 3. Living Conditions The condition of my father's room was unacceptable. Food would be left on the floor, his incontinence accidents were not cleaned up in a timely manner, and the room was consistently dirty with sticky floors, gnats, and flies. This was not an isolated occurrence. We had to clean his room every day. 4. Falls and Neglect Falls often went unnoticed and undocumented. If not for the camera we installed in his room, my father could have remained on the floor for hours after some of his falls. I had to visit at night on multiple occasions to address why he was not dressed, not medicated, or lying on the floor without assistance. 5. COVID-19 Mismanagement During the height of the pandemic, Landon Ridge failed to report COVID-19 cases to the state as mandated. My father, among others, contracted COVID during this time and his health rapidly declined thereafter. Several residents, including ours, suffered serious falls linked to post-COVID complications such as vertigo, and there was no increase in supervision despite known fall risks. 6. Retaliation and Administrative Negligence There is a clear pattern of retaliation from management. Families who raise concerns often report decreased attentiveness to their loved ones' care. In our case, the head of nursing barred our hospice nurse from seeing my father on the very day he was transitioning--reportedly "due to a personal grudge." We were never consulted or informed of this decision, which left us blindsided during an already devastating time. This individual has also been known to expel families from the facility with little or no warning, particularly those who raise concerns or question care practices. Families are advised to have a backup plan if they advocate for their loved ones. Families have reported to us that they were given as little as five hours to vacate. 7. Corporate Indifference When we escalated our concerns to corporate management in Fort Worth, we were dismissed. I was told, with little professionalism, that the reported issues "never happened" and "couldn't happen." Days later, corporate staff arrived at the facility and openly criticized me to other staff members. If you are considering Landon Ridge Pathways for your loved one, please do your due diligence. Many families who experienced similar issues are unable to speak publicly but wish they could. If you would like clarification or more information, I am open to sharing our experience. Families deserve transparency, safety, and respect. We did not receive that at Landon Ridge.

This review focuses on this assisted living facility and its absolutely amazing staff. First off, you will know that you have been blessed or have received amazing good fortune if you meet and work with the Sales & Marketing Director, Ms. Lauren De La Rosa! Ask for her by name if you go visit. Lauren is the kind of person, that just genuinely cares about everyone! She is gifted with the ability to really listen and understand your concerns and needs. Lauren goes well above and beyond all expectations. In example, during our meet and greet conversation I vented a little about several challenges we were having with other entities and organizations related to our aunts new medical challenges. Things which were absolutely not related to Lauren's position at Landon Ridge. Our meeting concluded, and my wife and I left, feeling very happy and grateful to have found Landon Ridge. Lauren shocked us a few hours later, when she called us after her work day, to let us know that she had done some research online related to the unrelated challenges I had mentioned to her earlier! She explained what calls she had made on our behalf and also what resources she had found for us online. She then sent us an organized, easy to understand summary via text. The information which Lauren provided would have taken us days to find ourselves, that is, if we were even able to find it! I can not emphasize enough what an amazing human being Lauren is and what an ASSET TO LANDON RIDGE she is. By the time our aunt moved in, everyone on staff made a genuine effort to go meet her, and to let her know they were always ready to assist her in any way. Our aunt said she needed to have her personal TV and cable system put up, and literally it was done first thing the following morning! The staff made sure to introduce our aunt to the long time residents that liked helping new residents transition into assisted living, and from day one she had lunch buddies, and our aunt was smiling and ecstatic! Tragically the pre-move in medical results have now come in and we have discovered that our aunt has a progressive, condition which will require full time medical services, well beyond any assisted living facilities scope. This is tragic on a physical health basis for our aunt, and also on an emotional basis because she absolutely loves the Landon Ridge community! It is such a special place, that our suggestion, for anyone in the San Antonio area, who is able to choose an assisted living facility, is please go meet Lauren, take a tour through the facility, (it is more like a resort than a facility), stop and chit chat with the residents you see, and you will find that it is absolutely the place to be. The Executive Director of the facility is Madison Parker, and she runs an amazing operation! Every time we visited Landon Ridge, Madison was actively engaging with employees and residents throughout the facility. She is an executive who leads by example, and who is always easily found if you need to speak with her. At one point, I spoke with one of the staff members that distribute medications, and asked her "off the record" if she liked working at Landon Ridge and under Madison. This staff member literally lit up with a giant genuine smile and said she loved working there and that Madison was amazing. I later asked another staff member the same question, and he also immediately lit up and smiled confirming that it is great group to work with. Clearly, Landon Ridge Alamo Ranch Assisted Living & Memory care have made a commitment to amazing resident service, and they invest in quality staff and leadership.

I really thought this was the place I would live the rest of my life. I learned about Blue Skies in 2015 and compared it to other similar communities. Blue Skies remained on the top of my list over time. There are several things that prospective residents must consider before committing to signing a contract. The organization's business/finance practices are not at all good. Poor business practices include lack of transparency to residents regarding pricing of apartments/homes (it is a secret), and lack of coordination between various departments when moving in or out. In my case, I requested and was allowed an in-house move to a different apartment. I did have access to the "secret" apartment price list and assumed I would be paying the listed price for my new apartment. Imagine my surprise, after spending $2000 for a move, and just over a month AFTER my move, I find out the price of my new apartment is $257/mo MORE than what is listed on the price list. I never received any documentation before or during the move that informed me of the new rent rate - a violation of the contract that states a resident must be informed of any rent increase in writing at least 30 days prior to the new rate taking effect. Unfortunately for me, I did sign a lease addendum that showed the new price. PLEASE - do not do what I did! I almost didn't sign the addendum because the rent amount was much higher than I anticipated. I should have consulted an attorney then. I did do that later and found out that I should not have signed the addendum because I was never informed in writing of the new rent price. Also, there is significant financial mismanagement of resident accounts. I received 2 outrageous debits on a monthly statement. It was impossible to speak to or email an individual in the Finance Department to discuss my concerns. My voicemails went unreturned and emails never received a response necessitating my going to the assistant resident director for help. An issue regarding a $1369 erroneous charge was corrected, but only after the assistant resident director contacted the Finance Department - where he received an immediate pick-up of his phone call. Additionally, I had to confirm it was, in fact, an erroneous charge after the person from Finance asked: "Is she sure this is incorrect?" I was livid! The assumption from this Finance Department is that every entry they make on a resident's statement is correct, and it's up to the resident to prove them wrong. Currently, I am in the midst of attempting to get a refund of MY money for an incorrect rent overcharge for November 2025. Despite disputing the amount of this debit via my bank, Blue Skies Finance states that I approved the amount. I DID NOT! Because of their response to the bank stating this, my bank closed the dispute. I'm currently exploring other options for getting this corrected. And just know that rent money is not all the money they actually owe me. Please, just be VERY CAREFUL if you decide on Blue Skies as your next home. Look at other continuing care communities in the San Antonio area. There are also very nice 55+ communities in the area, with more being built. Blue Skies prices are the most expensive, or close to it.

Driving into the complex there is a gate guard so you have to have a specific purpose or appointment to even enter the premises. As you walk into The Mission, which is the building where my mother is, there are more security measures in place. The doors are always locked and opened by ringing reception. Then you must stop at reception and sign in electronically so you can be issued a pass. You know what there isn't!?! Adult diaper smell! Hospital smell! Old people smell!!! It is set up beautifully with separate halls dedicated to different attend to different issues. The level of care is second to none. EVERYONE is kind and pleasant. I have seen more humanity and patience here than ever before in my life. The level of dignity they treat the patients with is heartwarming. Their attention to detail is such a welcome quality... there are not enough wonderful adjectives to describe my experience with my mother here. I know there is independent living, assisted living, memory care, skilled nursing and rehab here.. I think they also offer palliative and hospice care. If you have a loved one that you find yourself needing help with, I highly recommend this facility!!!

My wife's grandmother smells nice. There are nurses constantly checking on her and they are very informative. My wife and my Mother in law asked me to sit in with them for a meeting they have every three months. I did not speak much. I just listened to the nurse and the head administrator explain how things were going with her grandmother. Medications she's taking, her progress, etc. I am relieved. My wife had been stressing about finding a good place for her grandmother. So if she's stressed, well all the husbands know, than I'm stressed. Lol. Anyway, this place is full of life. I started to take pictures, but I was not allowed to take pictures of the residents, I was told by a nurse, but they had no problem with me taking pictures of anything else. It was for the protection of the residents, which I totally understand. Good for them, protecting the identity of the residents. Everyone there seems so happy. They even have a beauty salon in the home!!! They have activities and they encourage the residents to do whatever they want. They don't force them, they let them go at there own pace. I am relieved. When I saw grandma sleeping, snoring, lol, I knew everything was going to be alright. Oh and I asked some questions about employment tenure. Almost all of the workers there have been working for the nursing home for 10 years+! That's a great sign, because you don't stay at a job that long if your not happy.
