I've reached out to the Case manager whom my mother receives information from made me feel like I was less than a human being because I have behavioral health issues yet kind of joked about it. I called (like I said earlier) to coordinate my mother's Dr appointment upon her discharge day. Aside from the case manager treating me bad I called to confirm my mother's must make appointment and someone answered. I expressed my concern and was put on hold. I wasn't put on hold and I hear EVERYTHING the nurse or whomever say terrible things about me and she figured out that the phone was not on the hanger yet she hung up without acknowledging I heard every word and just hung up. I called back and there was no answer. The care. I'll give a 5 just because my mother has been treated right. But as far as giving information and being polite to family members whom has loved 1s in the facility and showing respect. Just from my experience. They get a ZERO in my book. Creekside Health and Rehabilitation was a huge disappointment to our family. Our sister's initial…read morestay in the rehab wing was okay, not great and not quite subpar. Due to her health restrictions we were forced to make the decision to admit her as a resident. We greatly regret that decision. The majority of the nursing staff was dismissive of her health needs to the degree that vital medication was not dispensed in a timely manner. She was frequently left sitting in excrement for hours, once almost 24 hours, resulting in wounds on her buttocks and back. Her dietary restrictions were regularly ignored, resulting in the family ordering food being delivered to her. Unfortunately we were operating from out of state so we couldn't monitor her care in person. She has since been released to hospice care where her aides are appalled by the condition her many wounds. The greatest travesty is the lack of oversight for a location recommended by Medicare & Medicaid agencies. The management staff need to be held accountable. If at all possible, avoid this place at all costs.

They recently moved from Madison Avenue to E. Countyline Rd, in Greenwood located conveniently across the street from the new Costco. They are part of the Franciscan Physician Group. Note their new name is now Madison Family Medicine (previous name was Madison Avenue Family Practice). I paid a recent wellness visit to my primary care physician, Dr. Shelley Stiner, DO, who practices at this facility. Upon arriving at their beautiful new location, I found the facility to have a large waiting area with plenty of seating, magazines and flat screen TVs to enjoy while waiting for my appointment. The staff has always been very friendly from the front desk to the nursing assistants. Dr. Stiner always listens carefully to my concerns and offers healthy alternatives to prescriptions where possible. She is very pleasant and professional. I have always had short wait times on all of my visits and scheduling for appointments or a referral has been very easy when working with their office staff. Lastly, I love their "My Chart", web portal....you can schedule appointments see your past visits, Billings, request a prescription refill and various other things related to your healthcare experiences. They have an excellent response time to their emails that have been submitted to this portal. After each visit I receive an emailed survey of my most recent visit to complete if I choose to do so. I have always seen Dr. Stiner, but I believe they have at least one nurse practitioner and possibly more and other doctors available. For this large of facility I'm sure they have to have other physicians, but I don't have that information. Actually as I was walking out the door I saw sign for a new physician. (See photo attached) If you are ever in need of a Doctor of Osteopathic Medicine, I highly recommend giving Dr. Stiner a try if she is accepting new patients. I have been with her for approximately 1 1/2 years due to my doctor retiring and wanting to try a DO for my healthcare needs. Due to her high demand, I had to wait initially about six months until she was able to accept a new patient. (I had to keep calling every so often to see if they were accepting new patients). I called 2 days in a row and left a detailed message with admissions, specifically Megan, the second day, asking for someone to call me back. I had spoke with a very nice person in the rehab center who said she was in a 1 hour meeting and would be out at 10am. SILENCE again. Very disappointed with the customer service, hopefully this is NOT indicative of their patient care. I did call several rehab centers on the southside of Indy and everyone of them called me back within 2 hours! My dad easily found a nice place and we are all happy.

It was late in the evening and I was very stressed and not feeling well. The gentleman who does the intake and triage (I wish I could remember his name!) was amazing - he calmed me down and assured me I would get help. It is required of me to receive these vaccines and it is something that should be common knowledge for anyone working in a hospital--regardless of whether you're a nurse or not. After I asked my question, whoever was working reception had zero idea what I was asking for, which is fairly alarming considering rabies is one of the most lethal viruses we have. She asked me if I was a current patient, I said no I don't have insurance. This was when she really stopped helping me. I understand prices are high, but they're going to be high regardless because of what I'm asking for. She told me to ask my school what do, to which I told her they told me to contact hospitals because they are all equipped to give these vaccines. She just kept saying she doesn't know what I was asking for and that she can't help. She then directed me to Jane Pauley, who is closed on weekends. I understand not being versed in everything that happens in a hospital, but not knowing what a pre-exposure vaccine is, and being unwilling in helping access that, is a terrible look for any healthcare facility. Please educate your staff on basic healthcare and train them to not just push people off the phone.

Based upon recent experiences with my mother and mother-in-law, if you are looking for assisted…read moreliving for a loved one, consider the following: 1. Don't just talk to the sales staff. Seems obvious but I failed here. Their job is to get you to sign on and obscure the less positive aspects of the facility and operations. 2. Talk to the Executive Director. Ask if this is their full time position or do they have responsibilities outside the facility that take their focus off the facility you are interested in. Find out how often they visit the residents in their apartment (ours never did). If they don't do this, ask them how they stay engaged in the operation to understand the resident's needs. 3. Talk to the Nursing Director. Ask the same questions as the Executive Director. Also ask how they assure resident safety. Listen for rote practiced answers with little sincerity. Ask how they deal with conflict resolution when a resident and an aide just do not see eye-to-eye. My experience with this was maddening. 4. Talk to the Chef. Ask them if their budget meets their needs. Ask how they deal with quality control so that food is not overcooked and therefore tough for old folks to cut and chew. 5. Ask if the facility has a nutritionist, and if so talk to them about how they assure vegetables are a part of the choices available. Ask how often they work with the Chef to ensure nutritious choices exist. 6. A big one: ask to see written policies defining levels of service and if they use a points system (I think most do), ask for written policies that define this points system. We didn't understand the importance of this until health started failing and then were refused anything in writing. I now feel this was unacceptable. 7. If your loved ones have limited mobility ask how the facility works to engage them into the activities and get them out of the isolation of their room. With this facility and the requirements of the building codes, the apartment doors truly encumber resident visits between apartments. Ask those you talk to how their operations work to neutralize this. 8. Ask sales, the Executive Director, and the Nursing Director how they track the activities of the aides so that you are assured you are getting the services your loved one is paying for. Found out there is no such thing at this facility (and maybe that's the case at others as well). But, if you ask, you'll at least know it going in. Also ask if they are fully staffed (all facilities are struggling with staffing). With tight staffing how do they ensure resident safety never waivers as aides may want to hurry residents that no longer can physically do this. 9. Of course, visit the Indiana State Department of Health website to understand any violations or reports tied to the facility you are interested in. Please know that while I was extremely frustrated with a couple caregivers and what I saw was a lack of correction and accountability, there were several wonderfully caring CNAs and nurses.

For one, they constantly forgot to give her her meals, and for 2 they were mistakingly giving her someone else's meds. A lot of the staff there (not all) seemed like they didn't care about the patients well being and were just there to collect a check and go.
