I've reached out to the Case manager whom my mother receives information from made me feel like I was less than a human being because I have behavioral health issues yet kind of joked about it. I called (like I said earlier) to coordinate my mother's Dr appointment upon her discharge day. Aside from the case manager treating me bad I called to confirm my mother's must make appointment and someone answered. I expressed my concern and was put on hold. I wasn't put on hold and I hear EVERYTHING the nurse or whomever say terrible things about me and she figured out that the phone was not on the hanger yet she hung up without acknowledging I heard every word and just hung up. I called back and there was no answer. The care. I'll give a 5 just because my mother has been treated right. But as far as giving information and being polite to family members whom has loved 1s in the facility and showing respect. Just from my experience. They get a ZERO in my book. Please reflect and learn. Respect your patience loved 1s with respect as you would have done to you. Dennis S.

And no one recognizes you until you want to leave because of insurance oh and the recycle razors. Gave me a razor with someone else's hair in it. I'm just telling the truth. Go across the street to Fairbanks. Oh I will post more until they pay me off. Worst rehab ever, it's good if you're homeless but if you have insurance stay AWAY

For one, they constantly forgot to give her her meals, and for 2 they were mistakingly giving her someone else's meds. A lot of the staff there (not all) seemed like they didn't care about the patients well being and were just there to collect a check and go. The facility also had a horrible smell and the decor was extremely outdated.

They recently moved from Madison Avenue to E. Countyline Rd, in Greenwood located conveniently across the street from the new Costco. They are part of the Franciscan Physician Group. Note their new name is now Madison Family Medicine (previous name was Madison Avenue Family Practice). I paid a recent wellness visit to my primary care physician, Dr. Shelley Stiner, DO, who practices at this facility. Upon arriving at their beautiful new location, I found the facility to have a large waiting area with plenty of seating, magazines and flat screen TVs to enjoy while waiting for my appointment. The staff has always been very friendly from the front desk to the nursing assistants. Dr. Stiner always listens carefully to my concerns and offers healthy alternatives to prescriptions where possible. She is very pleasant and professional. I have always had short wait times on all of my visits and scheduling for appointments or a referral has been very easy when working with their office staff. Lastly, I love their "My Chart", web portal....you can schedule appointments see your past visits, Billings, request a prescription refill and various other things related to your healthcare experiences. They have an excellent response time to their emails that have been submitted to this portal. After each visit I receive an emailed survey of my most recent visit to complete if I choose to do so. I have always seen Dr. Stiner, but I believe they have at least one nurse practitioner and possibly more and other doctors available. For this large of facility I'm sure they have to have other physicians, but I don't have that information. Actually as I was walking out the door I saw sign for a new physician. (See photo attached) If you are ever in need of a Doctor of Osteopathic Medicine, I highly recommend giving Dr. Stiner a try if she is accepting new patients. I have been with her for approximately 1 1/2 years due to my doctor retiring and wanting to try a DO for my healthcare needs. Due to her high demand, I had to wait initially about six months until she was able to accept a new patient. (I had to keep calling every so often to see if they were accepting new patients). I hit it lucky I guess and on one call was told she was taking on new patients. At this point in time, I'm very happy with their services and the new location of this practice. Review #28 08/11/16

This is the IU Health - Glendale Town Center practice, a practice devoted to primary care and also…read morepediatrics. There's also a lab available in this office. For those familiar with Glendale Town Center, this is located on the second floor of the offices section of the complex on the side directly off of East 62nd Street. There's a small lobby/storefront area directly next to Mattress Firm that has an elevator that will take you to the second floor. There's also a set of stairs available for those who prefer. I needed to establish care with a new primary care physician as my previous one relocated to Shelbyville and is just a bit too far away for me. I'd previously looked at appointments here, however, they were a couple months out and I skipped it. However, because I attend an IU Health clinic elsewhere they were able to get me in within a couple weeks. They do have automatic doors for those who need them. That's a plus. I also noted very quickly that this was one of the better looking and cleaner IU Health clinics I've experienced. I arrived a good 15-20 minutes before my scheduled check-in time and checked in at the front desk. Because I'm an established IU Health patient, I didn't really need to do much else. I waited in the waiting room until I was called. I was taken back in a timely manner. They asked me how I usually got weighed (I don't usually as most clinics are not set up for wheelchair users) and I did note they didn't have any of the modern tech that would do that without a scale. I was taken into an exam room - very small room that would definitely not be considered ADA compliant. It was so small I had to move quite a bit in order for the employees to get around me. I did like that they had an exam bed that was lower to the ground and adjustable. However, this was definitely not an appropriate room for a wheelchair user. My doctor was Dr. Boyce. We had a nice conversation about my needs for the day. She only works part-time in the clinic and we also discussed that. It was a fine appointment - not outstanding, however, there was really nothing wrong with it. I was somewhat hesitant to establish with IU as they are not the top tier of my insurance, however, Dr. Boyce seems nice and this is very close to my home. So, it should work. My hope is that in the future I can utilize a more appropriate exam room. However, overall, this was a solid experience with friendly staff and a warm, comfortable atmosphere.

Going through anything that involves surgery I believe is always an an anxiety producing…read moreexperience, in my opinion. I am thankful when I find staff that are warm, helpful and reassuring. While recently coming to the surgery center for my husband's procedure, I experienced just that. The staff were attentive to us while my husband waited to go back for his surgery, and they even kept me updated with his progress by text which was very reassuring. I didn't know that would be an option. Before surgery, my husband's surgeon even said a quick prayer for us. Because we are religious, it really meant a lot to us that he did. I feel that Community always delivers on their motto and I would not hesitate returning here again if need be. Kudos.
