All the ladies in Physical Therapy (Karen (care Bear), Nina, Mary, Danielle (red), Danielle are the best rehab in the state of Ohio 5 stars. I could not walk when I came in and I'm up walking 3 days later. Yvonne, Miguel (big Mig) made me feel like at home from day one. Shelby Customer Service is the best. Alex the NP very good too. The care is absolutely horrible and if you have an issue and want to talk to upper management or corporate they do not give you the time of day My mom had a camera installed in her room due to chargers and hearing aids missing and the staff still didn't care about checking on her and taking care of her. She was vomiting black liquid (which they assumed was due to an internal bleed) and did not check on her for 13 hours!!!!! I stayed there day and night for 9 days and not once did the bathroom or floors get cleaned and her roommate was taken to isolation during this time frame due to an infection read more

Went to visit my friend. The ladies at the front desk are nice and friendly and gave good directions on how to find my friends room. The place appears pretty clean and no funky odors. While visiting the nurses or aides came in a few times to check on my friends diaper, provide meds and see if she needed water or anything since she is immobile. The nurses also asked if I needed water or anything and brought an extra chair over when someone came to visit, which was nice. read more

If you value the life of your loved one, don't send them to Parkside Villa. My sister was there for two weeks, during which time she barely saw any staff. I visited regularly but only saw staff members when I went looking for them. My sister was on oxygen. They were supposed to send her home (Friday) in an ambulance with oxygen. They did not. By the time she got home, she was gasping for breath. She died the following Tuesday. That's the way Parkside Villa cares for their residents. read more

This is the worst place to send your love one. My mother was sent home with an infection she had to be rushed to the hospital. After she got home or she would have died. The nurses don't know what they are doing either. They have a worthless doctor who only comes around twice a week. And doesn't do anything. But try to get the patient to lie there and be sick. They keep his people sedated so that they can't do nothing but be helpless. This should be noted waiting for them to die. That's a shame. read more

"THIS NURSING FACILITY NEEDS TO SHUT DOWN IMMEDIATELY!!!" My mom was placed at this facility straight from the hospital in March 2024. She was in this facility temp until she got better from intestinal infection and received antibiotic through IV. My mom was in this facility for more than 2 1/2 weeks. Poor services, understaffed, no cleaning people, 1 STNA and 1 RN in each section. They Hire young people that in the kitchen that have poor services and skills. My mother received breakfast, lunch, and dinner with nothing but cheese on on meals. The way cheese was poured onto the food looks gross, nothing but cheese can't tell what you are eating. No STNA to come change sheets. No housekeeper to clean floors, bathroom, take trash out. The RN has been taking trash out. On April 7th, 2024, my mom never got her antibiotics as schedule, but the RN put on her chart that she gave it to her. This is considered negligence! I filed a compliant to case manager and head nurse. April 9, 2024, I also called the Ohio State Insurance to file complaint. April 10, 2024, the Nursing facility magically was fully staffed. Many Stna's, Many RNS, Housekeeper, Full Kitchen staffed, because the State showed up today. This tells me that the nursing facility purposely understaffs because they don't want to pay!!! I will never put my mom in this horrible places and WILL NEVER RECOMMEND TO ANYONE.... PLEASE KEEP YOUR LOVE ONE FROM THIS FACILITY! read more

STAY AWAY! Parkside Villa is hands down the worst place to trust with your elderly loved ones. They show no medical professionalism, and they have absolutely no compassion for elderly people. My mother was there for almost 4 weeks because she had multiple broken bones from a fall at home. By doctor's order, my mother was not allowed to stand or walk unassisted, much less use the restroom by herself. But since the nurses (and I use that title loosely since they are very unknowledgeable about anything that has to do with nursing) and aids refused to respond to their patient's call button, she was forced to get out of bed and go to the bathroom on her own multiple times. Most of the times, an aid would come in her room approximately 1-2 hours later and ask what she needed because her button light was still on. She would tell them she no longer needed help because she went to the bathroom on her own, then they would get angry at her and upset her. So the next time she decided to wait for the aid to assist her to the bathroom, and she ended up soiling herself while waiting. Even though she was not able to move on her own, they would never check on her to see if she needed anything. They don't have phone numbers to call directly to the rooms, so we had to keep calling the front desk multiple times and hope someone would answer the phone to get connected to her room. There was no sense of urgency by the nursing and aid staff, they only moved in one speed, slow. They are very good at dismissing you and your concerns. Every time I asked a question, I would be told exactly what I wanted to hear even though they never followed through on their end, or they would tell me that they will call me back within an hour with an answer, but never did. She was under doctor's order to receive oxy on an "as needed" basis for pain if she wanted it, but it was administered to her almost around the clock, even if she said she didn't want it and didn't complain of pain. This pain med was causing terrible side effects that I am not going to discuss here, but they did not care about that. This place is only concerned with charging Medicare as much as they can, and it showed in the way they run their business. On a positive note, I have to commend the PT and OT department on their therapy. They did a great job working with my mother, especially Nick in OT. I must also say that Denise, which is one of the social workers, was amazing. She listened to my mother's concerns as well as mine, she was always smiling and upbeat, she explained everything she needed to, and happily answered all of our questions without dismissing and ignoring us like the nurses and aids there. read more

Keep your loved one FAR from this place! Upon my mother's admission to Parkside she was alert, cheery and fully participated in her therapy. By her 3rd week there she had become lethargic, unenthused, always sleepy and stopped eating. I went to visit my mom late one evening from the normal time which I would usually visit. Found my mom naked on her bed covered in her own feces, some of which had even dried on her hand which tells me she'd been unattended to for a while!!!!! Most days when I visited my mom she had dried tears on her face, her lips and mouth were dry, and she was often thirsty, saying she wasn't getting water when she asked for it. She also said that the nurse aide, (name withheld), would not come back when my mom would tell her she needed to use the restroom or wanted water. My mother also said this same aide had HIT her and on one occasion and even said to my mother "you get on my nerves." INTERESTINGLY, upon finding my mom laying in her own feces, this same aide was returning to the building with food in her hand, while my mom lay there suffering!!!! Thank God I went there when I did!!! Also, we had to bring mom food often because the food there is horrible. It's sad because I chose Parkside based on it's location and ratings but after my mom's experience I would not recommend it to a dog!! We have since moved my mother from Parkside, and I never want to be reminded of that place again! read more

Our experience with Parkside Villa was an unmitigated nightmare beyond belief. My dad was sent there for rehab because a year of going no place during the pandemic made his legs too weak to walk comfortably without assistance. As they admitted to me, he made zero improvement at their facility and thus the insurance cut off payments after two weeks. So then they asked me if we should try one experimental week (WEEK, NOT MONTH) in their assisted living section at $214 a day. According to them, my dad had agreed. Then after only two days in the $214 room, they transferred him promptly to a $297 room informing NO ONE IN THE FAMILY. Also, when I called them after he was in the assisted living section for a couple of days to ask how he was doing, they admitted he was highly upset and wanted to leave. So I said, "ok, failed experiment, so when are you discharging him?" That's when the first bomb dropped. "Oh we're not discharging him at all. He's a danger to himself." And they promptly began billing us for an ENTIRE MONTH IN ADVANCE, not week by week as they had told me they would. Oh, and "danger to himself"? Days after we spoke, he fell out of THEIR bed so hard that it gave him a massive bruise across his back, with huge areas of skin filled with blood bubbling up to the surface. And when we scheduled a visitation two days after that fall, with guests being a family member, close friend, and notary, Parkside STALLED THE VISIT FOR HALF AN HOUR. Also, he was wearing long pants that day, which was unusual because he always likes wearing shorts. Remember that; there's more to that part of the story. During the half-hour stall, a staff member came and took our notary to speak to her in private. They showed her a document and said they had gotten guardianship. She did not recognize the document because she is not a lawyer, but she asked them if they had informed the family of this. Their answer: "That's not our job!" And according to the document, they were planning to take over his entire finances. And they tried to get away with all of this behind the family's back! The notary then left and the family member and close friend were made to wait over three hours for Parkside Villa to release to them a copy of the document they showed the notary. After that, god bless dad's family member and his close friend: they helped to get him out of his room and they all left. He was shaking and saying that if he ever ended up back there, he would kill himself! When they got him home, they changed him into his shorts and discovered a huge scab on the front of this leg from a cut that was covered by his long pants so we could not see if when we were there. Oh and also, once he got back home, he was at first barely able to walk at all. But within a couple of days of practice, he was walking MUCH better than he was while at Parkside Villa! This experience with Parkside Villa was unimaginably horrific. Beware! read more

The food at this place is absolutely horrible! Complaints go unnoticed! Some of the staff is great but most of the aids are new and don't care. They train new people for 3 days and put them alone for 20 patients. My mom in the assisted living area. We are very unimpressed and have made phone calls numerous times. They do try and get the issues fixed but doesn't last long, all goes back to the same old issues. Again the food is the worst. The state needs to check their food out. No one should ever have to eat it! read more

Mom arrived at Parkside Villa mentally alert and though on heavy pain meds was improving daily at Southwest General after hip replacement surgery at 83yo. Within about a week she'd regressed, quit eating or drinking & was delusional. I asked the RN if she had been tested for a UTI/urinary tract infection. They said no, her white blood count was ok & so were her other vitals. They didn't feel a UTI was the reason for her rapid decline. As her POA I asked they take her off the narcotic pain meds, see if it helped the delusions. It didn't change Mom's mental state. By the next evening I was so worried about Mom I told the RN I wanted to admit Mom back into the hospital ER to see what the problem was. The lead RN stepped-in to talk to me, said she'd give my Mom IV fluids, get a UTI test done, which would save my Mom the trauma of being moved back to the ER at Southwest General. Low & behold, sure enough, she had a raging UTI!!!!!! She needed 2 days of IV antibiotics, plus a week of pills to get rid of it. How come I can diagnose a VERY common UTI in an elderly patient but the staff who WORKS WITH ELDERLY PEOPLE DAY IN & DAY OUT CAN'T?!!!! Incompetence, that's why. Also, during her stay my mother slept sitting up, sliding sideways in bed or her chair, so eventually her neck went out, causing spasms. It's still out & we're trying to get that remedied now that she's out of PV. But while she was in PV they would not give her Tylenol more than 2x a day. We'd ask & they'd refuse us. My mother had agonizing headaches from the neck pain, tears running down her cheeks, yet the staff would not give her another dose of Tylenol (not extra strength either, just regular Tylenol). Cruel, just cruel, not doing anything to change the order for the last 3 days she was incarcerated here (Medicare wouldn't pay for her stay there unless she stayed the entire time, receiving a doctor's discharge.... conveniently when the insurance ran out...... Grrrr........ ). We, her children did what we could to help her (ice her neck, change position, massage) but what she needed was another dose of Tylenol!!!!! The aides were generally nice & as efficient as they could be, but they're aren't enough to go around. The dumb-shit staff wrote an order for stool softeners for a woman who wasn't eating or drinking due to their undiagnosed UTI. The stool softeners were enough to give the poor old girl diarrhea! Could the aid show-up soon enough to get her to the toilet or a bedpan? Nooooooo....! So over time my mother bottom became raw. WTF?! I asked they take her off the stool softeners & was told it's under doctors orders. Idiots. Dangerous idiots. Please, please, please, if your loved one is too weak to feed themselves, or disoriented due to medications, PLEASE be there with them for meals. My mother was too weak to eat her own food, and in NONE of these places do they have staff that helps patients to eat. So you MUST be there to assist. Worse yet, whether at Southwest General Hospital, or here at PV, the food service people put the tray on the table and then LEFT THE TABLE ACROSS THE ROOM NOT AT THE BED! How can a patient eat when the tray is across the room! Arrrrrrgh!!!!!!!!! If you need to get your loved one some good soup, or other food, there is a Panera Bread Cafe in the mall complex at the end of the street (Bagley Rd). We went there several times to get healthy soup my Mom would eat. On the upside, the place appears quite clean, and the aids are nice and seem to really try, so I'm giving them a 2nd star. read more

Have you seen one of the SAW movies? That is what it is like to stay there. If you love yourself or family member I beg you not to go or send them here. Here are a few of the issues I experienced during my 10-day stay. It may look good if you are visiting but when after guests leave and the lights turn off the horrors come out. For the first 3-days Parkside did not have some of my medication. One of the prescriptions was my pain medication. I had an open wound! Some posters on other sites say the food is amazing? I am not sure where they ate but it was not Parkside. Over 10-days I think I has two good meals out of 30. Every night, nurses took hour to bring me medications. One nurse acted stoned each night. My vacation was continuously losing seal and being loud. When I reported it, I was told that it was holding its seal and that was the way vacs work. This is not true. The vac was quiet when I was in the hospital (29-days) and was 99% silent when I got home for three months. I went for days when I could no sleep because of the loud noise. At one point over a three-day period I slept for six hours. I would run a fan on high, turn on the rooms heater/air conditioner and wear a very good pair of noise canceling headsets to try and lessen the noise but it did not work. April 8X 2019 - I was released by Parkside to go home and receive home healthcare. The nursing home sent me home with a wet-to-dry dressing on my abdominal wound and said it should last 3-days which was when they said I should receive my home VAC machine. I spoke to numerous other nurses and my doctor after about this and none of them could understand why Parkside sent me home with only a wet-to-dry and that wet-to-dry dressings typically only last eight-hours and never 3-days. As a result of being sent home early with a wet-to-dry dressing on April 8th I had to go to the ER twice and Southwest General wound center once to receive help with changing the dressing. On or about April X - I received my home VAC because I called my insurance company. I managed to get the VAC approved with a 10-minute phone call on or about April 11. Parkside could not get it done after a week. On or about April 15 Parkside called me and wanted to do an "exit" interview. I was unable to speak with them but told them I had concerns on how I was treated at Parkside and how Parkside fumbled my discharge. Parkside said they would call me back but never did. I believe I called twice to speak with someone and left a message as well but of course they never called me back. Food/drinks Parkside kept in the refrigerators in my "wing" was for the most part expired. I told at least three nurses four or five times and I was told each time they would check into it. The food/drinks was never replaced. I had to continuously ask for my bed and room to be cleaned. The air/heat unit in my room has to be replaced because it was either 100F or 60F in my room for four-days. I will say the PT and OT people were great. They were caring, knowledgeable and on point every time. It is almost like they are in a different universe from the nursing staff. read more

My mother was at a lovely facility in Strongsville,OH. Unfortunately a fall put her back into Southwest General Hospital and then to Parkside Villa for rehab. She decided to stay there in the assisted living area of Magnolia....rather than go back to her other residence. Bad decision. She never recovered. The aides were great and showed real care and concern. The nursing staff was horrific. They were unkempt, wore dirty uniforms and just didn't care at all about my mom. She rapidly declined, to the point of refusing to eat and no one cared. They don't care if their patients eat or not eat. Get bathed or not, wear dirty nightgowns or not, lie in puddles of urine or not, etc. All they do to justify their actions is blame the patient saying she doesn't want a bath, she won't let us change her nightgown, she won't let us feed her, she won't take physical therapy. Her room was dirty, and the time we did spend there, an aide came in maybe twice over a 4-hour period. It was completely unacceptable, and we should have moved her to another facility, but she was beyond recovery at that point. Do not send your loved ones there, it will not end well. read more

Do not send your family here, listen to the reviews! The admins and the Social Worker treated MOTHER total horrible!!!!! Never seen such horrible disrespect toward an elder adult. Plus, not to mention how terrible the Social worker treated us her daughters and caregivers. My Mom was there for Physical therapy!!! The Social work treats your family like they need to have a mental evaluation down! Seen my mother without my knowledge when they had strict notes to have me present first! Once we tried to remove my mother from there Care The Social Worker called the police on US. In the 6 days my MOM was there she only received physical therapy once! After this experienced, I feel that this Social Workers need to have more training, professional, ethical and responsible for her actions. Mom was admitted to hospital with 103 degree fever, pneumonia, bladder infection and leg infected! The same leg I was demanding that nursing home have a doctor examined the day they called police to have me removed for video recording just 10 days prior. Horrible. I will forever regret my father being there. Out of all the staff, there was only one RN that was awesome. I believe she actually resigned shortly after my father was admitted. Not only did my father's room catch on fire due to an electrical short in the wall, they kept him so damn drugged he couldn't even hold a spoon! The physical therapy staff is substandard and the Occupational therapist was horribly unqualified! Please do not put your parents in this nursing home! It's so sad that our elderly have to suffer as they do in facilities such as this. My father went in to recover from a broken hip and was told because he is not progressing as they believe he should be and they stop therapy! Put him in the Medicare section of the facility and He died nine months after he was admitted. I believe this is a place to thin out the elderly! Oh, by the way, don't even try to bring any issues up to the DON or administrator. All they do is argue and try to discredit your argument. My father was put on such heavy medications that he didn't even know what day it was. I happen to visit him one morning before his meds and he was totally lucid and completely alert and that was the last time I actually had a great conversation with him! read more

My father in law was here for rehab, the staff is amazing and us family. The nurses were so nice and attentive to his needs. I can't put into words how wonderful the therapy department is. They even taught us how to help continue his exercises at home as well as teaching us the best way to transfer him into his wheel chair. My father in law also raved about the food and said " I never thought I'd get a home cooked meal in a nursing home" I would recommend Parkside to anyone having to get rehab or unable to care for their loved one at home. read more

It's a shame that only people with bad things to say will take the time to write these reviews. I have worked in the nursing home industry for over 25 years and it doesn't get much better than Parkside. My father in law, mother in law, and mother were all patients here at various times. The care was stellar, and I know how to judge care! I chuckle at the reviews regarding food, as this is the number one complaint in ANY health care setting. Can you imagine trying to please 200 people? I can't even keep my family of 5 happy! I was so impressed with the care my loved ones received that I came to work here. And I commute 70 miles every day because I CHOOSE to work in a great place. I pass a lot of nursing homes to get to this one. If you need a nursing home, do your homework and look around. You will find Parkside Villa to be one of the best. read more

My mom is currently a patient at Parkside Villa. If you cannot devote 10 to 12 hours a day sitting with your loved one and babysitting the staff, please DO NOT place your loved one here. My 85 year old mother came here after hip surgery. Not even a few days into her stay she was yelled at by an aide because she was to weak to assist in transferring. The aide said she had to try harder or she was going to fall, my mother is afraid of falling because that's how she broke her hip. There have been missed medications, they put her on Flexarol without even telling me along with her Tramadol which made her so confused, she stopped eating and was totally out of it. She had aspiration pneumonia which took them a couple of days to diagnose. She came in with 2 bed sores which they treated for a short time and never healed. One broke open and was bleeding so I had to take it upon myself to get the wound nurse to treat it again. The other day they assigned the aide sho had yelled at my mom to care for her and when I said no, was told they were short staffed. The aide is extremely rude and made a comment to me, when I responded she told the supervisor I threatened her, they called the police, made me leave the facility, caused my 85 year old mother to almost hyperventilate. After I went to the police station I was told by the police I was allowed back in the facility and it was not a threat. There is some good staff but the second shift aide, Linda on the Garden and the nurse Susan should NOT be working with the public. I have heard the aide disrespect her supervisor and coworker. I have kept a log and I encourage anyone who has to place a loved one here to be diligent and watch everything. The staff does not like to be held accountable. read more
