I been to the Hyatt Regency San Diego a few times and the Piedmont Gardens are on par with the Hyatt. They are most definitely the best rehab center in the East Bay. The staff like the other care centers are the best, with their system of care and comfort they cannot be beat. Now I know why it's almost impossible to get in for care. Their kitchen menu is like gourmet food, and you can order ala-carte. There is a rooftop garden, with a fantastic view. An atrium with a lush garden I want in my backyard. A real gem in Piedmont in Oakland. I don't want to go home. Two weeks isn't enough to spend there. read more

My grandparents recently had to have a couple of short term stays at Piedmont Gardens to recover from a fracture and surgery. The members of staff at Piedmont Gardens were very helpful and friendly. They were always in contact with me during the stay and kept me informed of my family's status no matter what happened any time of day. Nurses were easy to reach and administrative staff was easy to talk to. Facilities were clean, well kept, and up to date. If there were any unfortunate reasons why we would have to go back, I wouldn't hesitate to reach back out to Piedmont Gardens. My parents both stayed at Piedmont Gardens several years ago for several years, and overall, they received genuinely good care. The nursing staff was caring and attentive, the physicians seemed on top of my parents' conditions, there were a few activities that my mom who had dementia liked, and the social workers were excellent in helping me navigate paperwork and logistics. We had regular care meetings, and communication was generally strong. There weren't a lot of bells and whistles - it wasn't fancy, but when either I or another caregiver visited four to five times a week, every time we visited, my parents appeared clean, comfortable, and well cared for. For example, the care team made a decision which is hindsight was stellar - my dad really struggled with seeing my cognitively decline and my mom was struggling with seeing my dad, and so they separated them - a few rooms apart. At the time I was advocating for them to be in the same room, but I recognize it was better psychologically for both of them to be apart and visit each other. For most of their stay, I felt confident that they were in good hands. The one exception, and the reason I am writing this review, was the night my mother passed away. On her final evening, she was clearly experiencing increased pain and discomfort. Earlier, I had spoken with her physician, who told me should probably pass that night and that there was a prescription on file allowing her morphine dosage to be increased if needed. However, late in the evening, the nurse on duty (who may have been new) when I asked for the dosage to be upped, she repeatedly said that no such order was on file and that she could not increase the dose. She also said that she had attempted to reach the physician but was unable to get in touch with him. In a state of panic, I left briefly to try to reach my husband who was a physician himself. When I returned, my mother had passed. The following day, I spoke directly with the physician, who insisted that there was an order on file to increase her medication and that he did not understand why he had not been reached. At the time, I was deep in grief and did not have the energy to pursue a he-said-she-said situation. In my final conversation with the physician, even though I appreciated his overall care and the respect he showed my family, I told him I would never forgive him or Piedmont for my mom's final moments, and to be honest, I haven't. Even now, years later, the experience has stayed with me. The idea that my mom suffered and I was not there in her final moments haunts me with sadness, because it was so avoidable. I'm not sure what to do with the grief; so I thought I would write this out and try to tell a balanced story. I'm sharing this not to criticize the overall quality of care, which for the most part was very good, but to help other families make informed decisions and advocate for their loved ones. Would I have sent my parents to Piedmont Gardens again, based on our experience at the time? Yes, with important caveats. My advice to families is this: * Double-check that all medication orders are clearly documented and on file - also know what those medications are. * If there are any conditional or "as needed" medication changes, confirm that nursing staff can see them in the system. * When visiting, especially during hospice or end-of-life care, don't hesitate to ask staff to confirm current medication orders so there are no surprises in urgent moments. I don't know what Piedmont Gardens is like today, but when my parents were there (about 5 years ago), it was largely a caring and competent facility. I hope this review helps others ask the right questions and feel more prepared during an incredibly difficult time. read more

My parent is here in skilled nursing and she is completely content and extremely well cared for. The staff are so kind, skilled, patient and responsive, there are daily social and recreational activities, the facility is clean with nice amenities, and they check in with me as needed. I was trying to check on her regularly when she lived on her own while also working full time and caring for my son but she fell a few times over the last few years and lost a lot of mobility, she also has memory challenges. I used to be in a constant state of worry about getting calls from the ER, but I never worry now, she is safe and happy. Thank you PG!!! read more

Piedmont Gardens is not what it used to be! The lack of quality Care with the shortage of staff has become an ongoing issue in the skilled nursing facility. The attention to detail and care are not there. ​ ​ It does appear that there's a gap in staffing which is causing a lack of attention and care for the residents. The food is hit or miss . They often don't have substitution when they send a plate that's lacking. If you're willing to go further away from the bay area he will get better care. Piedmont Gardens is just not what it was and what it should be. The supplies for short-term, long-term care, restaurant, Assistant living. All the way around Piedmont Gardens has declined. I would not recommend Piedmont Gardens to anyone. If you want real quality Care I would highly recommend Rossmoor /Golden rain foundation Walnut Creek California. read more

I give 3 stars because I can't see giving any Skilled Nursing Facility more. Overall, Piedmont Gardens was a negative experience, though I saw much worse elsewhere. Despite the criticisms below, Piedmont Gardens is a high quality SNF. SNFs simply are horrible places to be. Unlike other places that are overcrowded, filthy and neglectful, the main problem here was neglect, due to under-staffing and RNs that were unable to communicate problems to the doctors. I don't know why. Because of this, a mistake was made administering 2 of my mother's medications making them unavailable, so she had a hard time breathing. She was both under-nourished and dehydrated enough for it to be noticed by her outside specialists, but we all knew it could have been far worse. My mother was at Piedmont Gardens for 7 weeks. She had a broken hip. She never left her bed except for PT and to go to see outside specialists. Her only occupation was to look out the window. She was far-sighted & hard of hearing, so the TV was of little use. Daily, I put the things she needed back into place on her night table so she could find them, mainly by touch. My mother's bed was broken. They never fixed it. Visitors are restricted to 1/2 an hour per day. Visitors must email 24 hours in advance to arrange a visit. Because PG is a luxury SNF, I was permitted 1 hour a day, every day. Everybody is cared for by the same main doctor. I never met him. I think the RNs write in charts that the main doctor looks at. This may be how the medication problem arose. The RNs told me they prepare charts in their office and that I wasn't to talk to them there. One RN is supposed to be outside to field problems, but one was rarely there and no other replaced her. The main doctor prescribed an inhaler that my mother was unable to use. I asked the RNs to put that in her chart, but nothing changed. 10 days later I bumped into the assistant doctor & told him about the inhaler problem. He promptly gave the prescribed medications a different way that required 10 to 15 minutes of supervision. The CNA's literally run from room to room. They lost some of my mother's clothes that she wore to a doctor's office visit, taking them to be washed as soon as she got out of them. Their solution was that I hunt through the laundry and find other clothing. In order to save her clothes, I took to racing her transport back to the facility & taking the clothes with me. If I left something in drawers or the closet, they were moved or lost. The menu does not have a big range of choices. Due to cultural reasons finding her food became a problem. I had to fill in her menu every single day for all 3 meals, and my order was sometimes lost. Many times as I passed rooms, I was asked for the call button (also controls the TV), which I learned to find above people's beds, out-of-reach in the same general area, as though they had been removed, rather than having fallen or been misplaced. The place is fancy at the independent living level, but like many SNFs it was a nightmare for those who couldn't get out of bed or for some reason could not advocate for themselves. A woman who was dying was put in the bed next to my mother. Though they came together, her entire family was not able to spend time with her at the same time. They had to come in groups of no more than 3. There was enough space for one person to sit. After about a week the lady died. They closed her mouth with a towel tied about her head. Her family came in and found her that way. She lay there for part of the night, well into the next day. My mother knew. Other gut-twisting experiences: 1) meeting a woman that they sat in a wheelchair every day in the hallway near the nurse's station where they could keep an eye on her. If any of you have ever sat in a wheelchair for several hours, you know that it's not comfortable. She was sitting there whenever I came up the elevator, and she was still sitting there when I left. Since I passed directly in front of her daily, I would say, "hello." For 2 weeks there was absolutely no recognition of my greeting. I thought she was unable to do so... then suddenly, she smiled! Every day afterward, we would exchange a few words. She told me she didn't think that anyone was paying attention to her, so it took her the 2 weeks to realize that I was addressing her. 2) Another much younger, more mobile person would haunt the elevator door, so that she also could talk to me daily. She would have taken my entire hour if I had not told her it was impossible. My mother needed me to advocate for everything for her. What I report is not a complaint. As I said, PG is one of the better SNFs I've seen. If possible, try for home care, despite the price and lack of insurance coverage. If supervised and you find the right company, home caregivers provide a more humane and dignified experience. read more

I recently spent thirty days at the Piedmont Gardens (May) recovering from sepsis, and extensive surgery. This was post 9 days in the hospital. I had excellent care the entire 30 days I spent at the Piedmont Gardens. Every Charge Nurse (efficient, skilled and great) checked on and with me regularly and the staff was kind and caring. I had a daily infusions which after day fifteen caused bad side effects. Nothing medically could be done (we tried) One staff member would watch after my daily required antibiotic infusion and would whisk me into a cold shower even if I was not scheduled, this was the best relief. Those acts and many more were amazingly thoughtful and beyond their duties. Care was great-regular visits and access to internal doctors. Intake was easy, and fully explained. The therapists were wonderful with their personalized care. They took notice of the full mind and body and appeared to tailor the therapy just for me. After all that extensive surgery I came out of Piedmont Gardens much better than I was before the surgery. Food was fine and varied. Many choices offered. Kitchen/Dining staff were attentive to special requests. Food was served either in your room or in the communal nicely appointed dining area. Night staff was friendly and watchful as they should. Visitors were easily accommodated especially on the outdoor roof deck area (nicely appointed) Piedmont Gardens was clean, friendly and a great place to recover. My experience with Skilled Nursing and an Assisted Living apartment at Piedmont Gardens for my Mom is that they are trustworthy and kind. The staff are caring and conscientious. My mother required care in a skilled nursing facility after a health crisis in the early days of the 2020 pandemic. At the time, we weren't allowed to visit due to both PG's, and the State of California's COVID protocols. We appreciated all they did for our mom then, and we were able to have contact with her as soon as protocols allowed. I mention this because their facility is run in a conscientious manner and with the safety of their charges and staff in mind, all while adapting to the changes in health guidelines and protocols. Their mostly full-time staff not moving around the area picking up shifts in other facilities while being exposed to a greater number of patients seemed to help them from having serious outbreaks. Also, when members of the staff were infected, measures were taken to isolate the effects and families were notified in the email newsletter. So, trustworthy and well managed? Check. Later, my Mom moved to the Assisted Living apartments upstairs and the staff have been just as kind as those in Skilled Nursing. Activities offered give it a real sense of community among the active members of the building. The dining and visiting areas are clean and well kept, as well as being well used. The residents seem happy when I cross paths with them in the lobby on their way out to visit or walk along quiet Piedmont Avenue. The dining areas are well staffed and clean, with conversations among groups buzzing in the air. My mom gets the care she needs, and I get a peace of mind from knowing a nurse is a button-press away. She moved back to Skilled Nursing recently, and it was really nice to have a familiar facility that can take care of her, and transition between her studio apartment upstairs and the extra care and staffing on duty in Skilled Nursing. I appreciate the continuity even if my mom can't remember the staff from her stay in 2020, The same kind faces greet me at the front even though we're now using Skilled Nursing again instead of Assisted Living. Piedmont garden is not inexpensive. But, I benefit from access to a nearby facility. The 24-hour floor-nurse on staff on my Mom's assisted living floor provides me with an extra sense of security in terms of decisions made about my Mom's care. They are the nicest people to have in her corner. I visited the Skilled Nursing Facility my mom spent so much time in back in 2020 for the first time this month and things are really abuzz, and the staff are similarly wonderful to the nice voices I heard over the phone in 2020. read more

My father did short term rehab at Piedmont Gardens in January after being hospitalized for pneumonia. I cannot say more positive things about the experience. They went above and beyond to provide him the nursing care and rehab therapy necessary for him to go home safely. When we arrived, they accommodated my mother in providing a very nice bed so that she could stay overnight with him for the first 5 days of his stay. The room was spacious, clean, and beautiful. The nurses and assistants were incredibly attentive to his and our family's needs. We were always on the same page and were made to feel like we were a part of the care team. His care plan meeting was very thorough and helpful in guiding and understanding his goals. The social worker was very helpful in communicating with Kaiser's case manager as well which wasn't easy to navigate. Lastly the rehab team got him to be strong enough to be able to climb all 17 of the stairs in his house when he could barely go up 1 or 2 at the beginning. If anything ever happens to my father again, I hope that they will have a bed available. I know that I would be able to entrust my fathers care to them without worry! read more

Piedmont Gardens has a wonderful environment that will help with physical therapy occupational therapy and speech therapy. One thing That is great is that it really takes away mental stress because the environment is so beautiful that all you really have to worry about is concentrating on getting well and mental Concentration on the physical therapy, and all the therapies that are offered. The quality and consistency of nursing staff was especially good with the focus on my healing and preparation for home. The therapy staff , PT , OT, Speech,working together with nursing, were thorough, attentive and consistent in executing the plan for transitioning to home. I found the food and food choices varied, appetizing and consistently good quality. I especially enjoyed the soup . My kudos to the Activity staff who offered many programs to keep my mind and body busy with lots music and interactions to enjoy. Writing a review for Piedmont Gardens at this time is one of difficulty and of a profound sadness. My 97-year old mother was admitted to Alta Bates Summit Hospital Oakland Campus Sunday evening. She was not feeling well. Tests were done and the doctor at Summit told us on Tuesday that it was a matter of days that my mother will transition to higher ground. On Wednesday, January 24 at about 5:00 PM, she was transported via ambulance to this skilled nursing facility. Even though I did my first check-in on Thursday, I was not prepared to write a review and I did not have a chance to tour the facility to see what Piedmont Gardens had to offer to its patients and people who were residing here (assisted living). My husband was talking to one of the women who lives here to find out about her experience. She was informative and gave us a positive feedback. Yesterday afternoon I decided to walk around on the first floor. This residential community has a beautiful outdoor courtyard, plenty of artwork mounted on the walls, dining room, activity room and a sitting area (library type setting) for reading books. The nursing staff was helpful and caring. My family did not encounter any rude demeanor by its employees. They were professional in handling our inquiries. My mother passed away and transitioned to higher ground at 5:46 PM on January 26. The manager offered her condolences to us and showed us sympathy and compassion. A representative from Albert Brown Mortuary came to transport my mother. We said our good byes. There is a time to weep and a time to laugh, a time to love and a time to hate, a time for war and a time for peace. Rest in peace, mom. Eventually we will see you over the rainbow on the other side. Finally, I want to say thank you to the nursing staff and the manager for their understanding and their care of my beloved mother. The Piedmont Gardens staff and facilities are great. My mother has been in their rehabilitation wing since mid -march for a back issue. The process to move her into the facility was easy with the help of their staff. The on staff Dr. has provided good service and my family really appreciates the great nursing staff and physical therapists. Through this entire process the staff have kept us informed with her progress, provided all the detailed information for her stay. The facility has all the required modern equipment, nutritionists and support teams. The facilities are spotless and cleaned multiple times each day. My mother was accepted to Piedmont Garden after the spinal surgery recently, and the care she received here is so impressive that we feel it's important to write a positive review about this facility. My mother received competent and compassionate care at Piedmont Gardens. Unlike some facilities, I never smelled anything that made me think the place wasn't spanking clean which can really be a challenge with the elderly. During her 5 years there she was in independent living, assisted-living, and skilled nursing. I thought all levels of care did a fabulous job. The staff seemed very happy in their work and incredibly caring. I would like to go there when my time comes. I am so grateful that my husband was placed at Piedmont Gardens Skilled Nursing Facility after his fall and hip surgery. Not only is he getting competent care, but kind and compassionate care from all levels of the health care personnel. My husband is cared for with patience and gentleness. My father has been at PG since 2019, first in independent living and now in AL. From what I've observed, all the staff here are compassionate, kind, and skilled. Throughout the pandemic, they've been careful with their mask policies, and immediately responsive whenever there's been a single case. They also have a variety of activities and cultural events for the residents to stay engaged and active. And it's in a great location, just off Piedmont Avenue, where there are shops and restaurants, if anyone wants to leave the premises. Like most senior facilities, it can get costly, but there are so many places that are just as pricey without the high level of care. I especially appreciate Zinnia Koch, Stephanie Rose, and all the CNAs taking care of my dad. read more

I recently spent 2 weeks in PG following surgery for a complete Quad muscle tear. When I arrived only a few hours post-op, unable to walk, it was several hours after ordinary admitting hours because of transport issues unrelated to Piedmont Gardens. Nonetheless, I was warmly greeted by a staff nurse at the door, bringing a wheelchair. We realized that I required a leg extender because the slightest flexion was excruciating. When he couldn't immediately locate one, he walked alongside me for several hundred feet through the hallways, gently holding my outstretched leg completely straight, as my sister pushed the chair, maintaining a cheerful, helpful manner throughout. If he had let it bend more than a couple degrees, I had excruciating pain. He didn't This was emblematic of my entire stay. Nurses, PT, social service members, physicians, everyone - were consistently cheerful, skilled, friendly, and went out of their way to accommodate as issues arose. I noted consistently excellent care provided to every resident. Food was tasty, ample, and flexible alternatives always available. Discharge planning was excellent, accommodating emerging issues, and professionally done. Comprehensive arrangements for post-rehab home care for both nursing and PT were outstanding. I might note that as a physician myself, I do pay attention to the care, professional and personal, that staff provide. Couldn't have asked for better. I'm delighted to recommend the rehab program to anyone in need. read more

When my mother became very ill and needed a nursing home last year, Piedmont Gardens really came through. From the interview and intake process to the nursing and physical therapy, and at every other stage, they were polite, professional, and competent. But everyone was also very compassionate and supportive, which meant a lot at that extremely difficult time. If we ever need a nursing home, we would certainly return, and I highly recommend them. My mother was a resident of Piedmont Gardens. She was not the easiest resident and at times had difficult dietary needs. Rachel and Simon from the Dietary department provided excellent care for my mother's, sometimes difficult, dietary requirements. They also were diligent and provided the family with constant updates concerning her condition and ensured her weight remained constant. Piedmont Gardens had regular video family meetings with representatives of all departments to review my mother's condition so the family was kept informed during limited visits due to COVID visitation restrictions. My mother was very well cared for and I highly recommend Piedmont Gardens to anyone who asks for recommendations for senior care. read more

In the fall I suffered a broken hip and a broken collarbone, requiring surgery which I received at Kaiser Oakland Hospital. After the surgery I was taken to the Skilled Nursing department at Piedmont Gardens for rehabilitation. The care I received at Piedmont Gardens was truly exceptional. Their nursing staff is experienced and caring, the rehab services are comprehensive and most attentive to personal needs, and the facility itself is beautiful and inviting to both patients and their visitors. They offered physical rehab services, of course, but also a variety of activities suitable for their patients. Their dining room served simple and delicious meals, and the staff knew every patient by name. By the time I left, I was walking with support and capable of toileting myself unaided. I would like to commend in particular, staff member Georgia Upshaw, a kind of roving ambassador who was a particular help in encouraging me in my recuperation efforts and arranging interesting activities for all the patients. My mother, now 98, moved to Piedmont Gardens nearly ten years ago. First in the Independant Living section and later on to the Skilled Nursing Floor. She continues to enjoy her stay and is looked after by a most caring and dedicated staff who are always there and lovingly take care of her needs. In every visit, we find our mother cheerful, comfortable, well groomed, and attended to. Piedmont Garden's personnel always keep us informed of any special needs and put our minds at ease knowing that our mother is well taken care of. We are so fortunate to have our mother at Piedmont Gardens and cannot thank everyone enough for all they do. Skilled Nursing My mother and stepfather moved into Piedmont gardens 8 years ago. Getting them to leave their beloved home in Berkeley was very difficult but necessary as my stepfather was ailing and my mothers' physical needs had outgrown their ability to manage a home together. Within 4 days of moving to the gardens my Mother called to say how much she loved it there! The staff and residents were so welcoming and loving. The activities were stimulating and the location was within walking distance to shops, restaurants and even a local Irish pub for my Irish stepfather. Sadly my stepfather passed away from dementia within 3 years of having moved there. While a very difficult time for both of them the gardens had a means to accommodate the necessary care for him as he declined and my mother found solace and support from the community of residents and staff that helped her through this challenging phase of life. Both my stepfather and my mother have had to take advantage of the skilled nursing facilities when they needed expert nursing care. My stepfather had extended care in skilled nursing when he could no longer care for himself where my mother had to use the facilities after surgery when she needed nursing care for a brief period of time. The staff was excellent and the accommodations perfect for both my parents different needs. The family was kept up to date on their care and and neither was lacking for attention. My mother felt she received extra special attention while she recovered in skilled nursing and I know that helped her get better faster. I have no doubt the staff treats everyone with that level of care and attention and makes everyone feel special but I am so grateful to them for making my mother feel important and loved while she recovered. Angels work at Piedmont Gardens. From my mother: I woke up to find myself in a lovely room with large windows overlooking a sunny sky shining through the trees. I was in horrible pain with both legs infected, complications from vascular surgery. After pushing the call button there was an immediate response from the nurse. She introduced herself and proceeded to give me pain meds. She carefully adjusted my bed position covered me with a warm blanket. Prior to leaving she asked if there was anything else she could do for me, making sure water was on my table, phone by my side, assuring me to call at any time for assistance. This was the kind of service I received for 3 weeks. Nurses, caregivers, and doctors provided me with excellent care, assisting with proper meds, updating me and my family regularly on my condition and healing progress. The physical therapists were wonderful, patient and caring. They worked with me to relearn how to sit up and walk again during my painful recovery. I can attest to the incredible caring treatment of supportive, compassionate team members in the 3 weeks I was in Piedmont Gardens skilled nursing facility. My husband was released from hospital with two days notice to Skilled Nursing Care. We were given a list of which we had one day to decide. Without question, after checking out all the four and five star site in East Bay we applied to Piedmont Gardens and were delighted we were accepted. The facilities are very attractive and vibrant! The staff is caring and fastidious. The environ/gardens are wonderful and the patient is encouraged to take advantage (good pet dogs allowed)!!! Activities are numerous and the dining and outside deck is stunning. We have subsequently been moved to an Acute Rehab facility. If required in the future, we would put Piedmont Gardens as #1 as our selection for SNC. Just great. read more

Piedmont Gardens continues to be a superb location for my in-laws. I upped my rating to a full five stars! The skilled nursing facility apparently receive 5 stars on a nation review done by the Center for Medicaid and Medical Service (CMS). That's apparently the highest rating, and more importantly we have a friend there who is super happy. read more

I selected Piedmont Gardens from a list, provided by the hospital, of possible skilled nursing facilities that had a bed available and my wife's insurance would be welcomed. I visited 2 of the suggested sites. I was impressed by the overall facilities at Piedmont Gardens, the high level of covid "security", general cleanliness and the interest/care expressed by the staff member who conducted the tour. I liked the fact that Piedmont Gardens is a multi-care facility - senior living, assisted living, memory care, and short-term skilled nursing. I was able to spend 8+ hours each day visiting during her 2 week stay. The ratio of RN/LVN and CNA staff to patient is good. They were generally very responsive to all needs A difficult situation with a "roommate" who complained throughout the night, making sleep difficult, was handled very well the next day & a change of room assignment was implemented. However, a patient in another room who continually yelled for water or nurse, etc and could be heard up & down the floor persisted. The patient may not have been able to be reasoned with. So not sure what could have been done about it. All of the staff displayed a caring, interested attitude towards their patients. My wife looked forward to seeing who was on duty each day. The administration also takes great care to ensure that you understand all your rights. The food choices were generally good, although at times could have arrived hotter. There is a roof garden that is available to take your patient up to as well as nice patio off of a social room when the weather permits being outside. We were there in February, so did not use but once or twice. Parking could be a challenge but it is a high density area with a very small visitor parking lot. Unfortunately, there was a covid outbreak within a couple of days of her discharge. The affected patients were segregated but the number testing positive increased each day. Safety measures, such as n95 masks, were immediately implemented. After getting home, we both tested positive within several days. That just gave us one more hurdle to overcome as she was recovering from pneumonia and other complications. I do not fault Piedmont Gardens as their protocols were good. Believe it was just a fact of life in environments with many people. All-in-all, I would recommend Piedmont Gardens without hesitation. I think that comparatively speaking it is very good at providing a high level of post-hospital care before a patient is ready to return to their home. read more
