What I find especially concerning is that this practice specializes in urogynecology, yet when the prescribed treatment failed, there was no effort to evaluate my condition further or discuss alternative options. My other healthcare providers routinely follow up after prescribing medication to ensure that the treatment is working and to address any ongoing issues. Additionally, the office changed my specialist without notifying me in advance, which was both confusing and frustrating. As a patient, I expect communication, follow-up, and continuity of care. On May 6, I attended an appointment with Dr. Jackeline Castillo. Before the appointment, I completed all required paperwork and provided my insurance information. At no point during scheduling, registration, or in any forms was I informed that I would be required to pay the full cost of the visit because of my deductible. No one explained what deductible amount triggers this policy or what my financial responsibility would be before I arrived. When I arrived, I was told I had to pay $203.30. I was already there, I was sick, and I needed to see a specialist, so I paid $203.30 and proceeded with the visit. The medical care was excellent. My complaint is not about Dr. Castillo or the quality of care. It is about the office's billing practices. After receiving my UnitedHealthcare Explanation of Benefits (EOB), I learned that my actual responsibility for the visit was only $14.92. I was charged $188.38 more than my insurance determined I owed. On June 19, 2026, I contacted the office and asked where this policy is disclosed and what deductible amount is considered "high." The representative could not direct me to any website, form, or written policy and simply stated that it was the office's internal policy. I am requesting reimbursement of the $188.38 overcharge and that this practice be required to clearly disclose these billing policies to patients before appointments are scheduled.

She was very thorough, empathetic, thoughtful and caring while she took down the details of my health history. She did not rush the appointment and she answered all my questions and concerns. That is so important! In my experience, sometimes medical doctors can rush you through your appointment. I have left their offices feeling like the visit was unfinished or unsuccessful. That is NOT the case with Dr. Scarzella and her staff. I have been a patient for 3 years now and I couldn't imagine going to another Urologist. The staff office are equally as professional and caring! They make you feel special and well cared for every time! Anytime I call with questions or concerns someone always gets back to me that day. I honestly can't say enough about this Dr. and her amazing staff! The office is beautiful, extremely clean and that is also so welcoming. So for new patients sometime there is a wait...BUT she is so worth it!! I highly recommend her and her staff.

When I asked to speak to someone other than the lowly "answering service," I was told that there is NO way to speak to any of the clinical staff at her practice without first getting "vetted" by the incompetent "answering service!" That should have been enough to deter me from setting up an appointment, but I'm interesting in seeing just how bad things are, and perhaps file a formal complaint with my Medicare Health plan, and/or AHCA. I have been struggling to find a doctor that would help me with my severe & debilitating period & ovulation pain. After hearing what my symptoms were we decided that he would perform laparoscopic surgery on me. Finally after 2.5 years of pain and not knowing what was going on in my body he gave me a diagnosis of endometriosis. I am still going through treatment with him but I can confidently say that I trust him and his recommendations with absolutely no doubts. Thank you so much Dr. Perez I am legitimately crying tears of relief while writing this. P.S. the wait time can be long, but that's because he makes sure his patients are treated properly. I acquired an acute kidney injury which has left me with compromised renal function and I had to have specialized surgery to remove damaged part of ureter because of damage the stone made on the ureter. I was forced to have my ureter resected and reconstructed because of the trauma from the stone and the length of time it was allowed to stay stuck . I would not recomendad Aventura hospital since the OR cancelled surgery abruptly because the ceiling was collapsing while I was on the surgical table. Just So you understand, the hospital called me the day before the surgery to cancel it because they lost the lazer to be used for surgery. Only to be called back 30 min later saying they had found the lazer and the surgery was back on. I know it sounds crazy but this happened to me a year ago and I never wrote a bad review nor sued the Dr. nor the Hospital but now I receive a bill in collections from Dr Levin for like $88.
