To the laundry staff who helped to find my mother's lost blanket which was a gift to her from her granddaughter, thank you! To all the therapists who worked with my mother, thank you. The first thing one notices at Sunrise is the happy disposition of all the staff. Everyone says hello with a smile. This must flow down from the top. Happy employees take good care of the patients. When it came time for my mother to move to another facility, Jenna & Cari did all they could to help & follow up. The facility is clean & doesn't smell. There are serene places to sit outside. All this is in contrast to the horrible experience at the vaunted Woodlands at John Knox Village. My mom was there twice and it was the same. Unhelpful & uninterested staff. Annoyed all the time. The Woodlands at John Knox Village is the dark night to the Sunrise Health & Rehab's bright day. Basically there are about 8 rider type machines a small walking track and stair rack and a machine to aid standing up. All these machines and tracks are very close together A few of the bicycle machines need repairs to the timing and difficulty ranging. I have had a very good therapist. I have not seen all the rooms but I assume they are generally as roomy as my room. Service is spotty. There are some very good CNA'S BUT IN MY OPININON NOT ENOUGH OF THEM. I have waited as long as 2 hours for service. i can honestly say that the upper level medical staff in my case and in my opinion are not at all competent. I have a script that requires 3 specific antibiotics Monday, Wednesday and Friday. I had to contact the head nurse because they could not do that correctly. We had come to an agreement that they would dispense the parts of that prescription noon day on the Monday Wednesday and Friday so i could verify the drugs. That lasted about 2 weeks. Right now It is 9:00 and I do not have my proper script from my Wednesday order. They have bee telling me that a nurse is coming. The last time I called they hung up the phone. I hope that my opinion of this place helps you make a good decision on you choice of rehab facility

I personally drove her to the facility and provided a 30-minute arrival window, followed by a 10-minute advance notice before we arrived. Despite this, no one was prepared to receive her. When we arrived, there was no staff member to greet us, and it took over 20 minutes for someone to locate a wheelchair. During that entire time, my mother-in-law--an elderly patient--was forced to wait in the car, in the Florida heat, which was completely unacceptable. The care only worsened once she was inside. When my mother-in-law asked a tech nurse for assistance to use the bathroom, she was shockingly told to "go urinate in your diaper." This comment was not only degrading and cruel, but deeply concerning for a healthcare facility that is supposed to care for vulnerable patients with dignity and compassion. I raised these serious concerns directly with the CEO and requested accountability. Although we had a face-to-face meeting, I never received any follow-up, explanation, or response afterward. Based on our experience, I would strongly advise families to place their loved ones elsewhere. The lack of preparation, basic care, and respect shown here is alarming. No one deserves to be treated this way. The only positive is that the rehab technicians were very good.

My father was temporarily admitted to Spring Tree for rehabilitation and continued treatment of an existing foot infection. He is visually impaired but cognitively aware and communicates clearly. He was there to recover and eventually return home. We did everything possible to establish a positive relationship with the staff. We communicated respectfully and even brought coffee and donuts as a gesture of appreciation. We were not looking for conflict. We simply wanted our father cared for properly. Unfortunately, I felt I needed to constantly monitor his care. There were times I found him dirty, problems involving meals and medication timing, and poor communication between nursing shifts. We were told that many nurses were per diem rather than regular full time staff. In my opinion, staffing arrangements should never become an excuse for inadequate care or communication. One night, my husband and I arrived unexpectedly and found my visually impaired father on the floor. We later observed bruising on his face. We have photographs documenting his condition but will not post them publicly out of respect for his privacy. My father also told us he felt he was being mistreated and abused. We took his statements extremely seriously. Our family was not the only one raising concerns. While I was in his room, a social worker came in to address and apologize regarding grievances that had been filed concerning his roommate's care. Witnessing that interaction while experiencing our own problems was deeply concerning. Then we learned my father's foot was fractured. According to the medical information provided to our family, imaging performed before his admission to Spring Tree did not show this fracture. It was identified afterward. When I questioned Estelle, the head charge nurse, about what happened, she suggested one possibility was that his foot had been fractured during ambulance transport. Our family witnessed that transport and watched the paramedics handle my father carefully. I will not speculate publicly about how the fracture occurred. The medical records, imaging, photographs, and other documentation have been preserved. The timeline speaks for itself, and our family wants answers. After leaving Spring Tree, my father required hospitalization and surgery and was treated for sepsis. Thankfully, he is now recovering. My experience raising concerns with management was equally frustrating. I felt talked over and dismissed by Estelle. My experience with Marion at the front desk was also extremely negative. Sandra was one of the few people who genuinely listened and tried to help, and I appreciate her for that. Overall, our experience left me feeling that residents were too often treated like numbers rather than human beings whose families had entrusted this facility with their safety, dignity, and care. If someone you love is staying here, PLEASE stay involved. Visit unexpectedly. Ask questions. Pay attention to their medications, meals, hygiene, injuries, and changes in their condition. Listen when your loved one tells you something is wrong and document your concerns. There are additional details regarding our experience that we will not discuss publicly. Based on what I personally witnessed, what my father reported, and what occurred during and following his stay, I WOULD NEVER ENTRUST SOMEONE I LOVE TO SPRING TREE AGAIN.

From the Therapists, to the Nurses and Pysch, they take a heartfelt interest in their clients wellbeing. As for the rest of the staff, there is a lot left to be desired. Aside from a few shining stars the majority of the administrative and tech staff behave as if they don't want to be there and its just a paycheck to them, to outright treating the Clients like inmates, cussing at them, threating them ("if you don't like it, pack your shit and get out") ignoring them by not following up on requests and grievances to total and utter disrespect. I've heard it said "you don't pay for this your insurance company does". I for one take great exception to that as I worked for over 45 years and paid into the system to be able to take advantage of programs I paid for (How dare they say that). Also, when asking a question I was often told "that's not my department or its above my pay grade)??? I would then asked to whom do I need to speak with to get my answer, their response was mostly "I don't know" How do you work somewhere that you don't know? The buck gets passed so much you can't recognize the bill any longer. Which brings me to ownership, The Husband and wife team of Mr. and Mrs. Gedale Fenster, a self proclaimed, "successful podcaster" and "motivational speaker" expounding his wisdom as if he were the next Ghandi, throwing out concepts that have been around for hundreds If not thousands of years. I for one found the man to be boastful, arrogant and disingenuous. They don't have any compassion or empathy for their clients, their entrepreneurial opportunity in this business is their primary reason for entering into this field. Complaints of living conditions go unaddressed, from inoperable washers and dryers to basic maintenance items. They hire drug addicts, exconvicts and alcoholics whom have little to no management experience, other than they were clients, in a field where that should be a priority. They allow clients to "Audition" for jobs instead of going to group to work on themselves or maybe its their way of getting cheap labor. They think by putting "Lipstick on a pig", by purchasing new furniture and putting a fresh coat of paint on it will solve a lot of the issues, think again. Just last weekend one of the units on the second floor of the IOP residence water lines broke and flooded that unit. When the client staying in that unit offered to go buy the necessary parts to fix it and have one of the clients whom are auditioning for a job, fix it the client was told no because the director of facilities said "I'm not going to allow a client to fix a repair, it will be taken care of Monday, and to have the Clients living in that unit put a bucket under it and dump the bucket when it was full, well you can imagine what happened, the downstairs unit got flooded and did a lot more damage than was necessary, if the director would have just let the Clients fix it. That's what you get when you hire incompetentence to run your facilities. Then there's the human care aspect of this, or the lack thereof. They treat clients as insurance payments. I was directly affected by this. I was told this past week that unbeknownst to me I was automatically enrolled in Medicare as I turn 65 on November 11th and Medicare trumps my current insurance "Florida Blue" and was unceremoniously discharged, with no resolution from Evolutions other than "No Dice" you have till Thursday latest Friday to pack up and get out. An unconscionable way to run a Mental Health and Substance Abuse Treatment center. Also, when I was leaving yesterday, I had to leave my bags there for a period until I got settled into a hotel, when I retrieved my bags one of the bags was missing that had all my toiletries and medication in it, after I informed a tech he went room to room looking for it, then my bag suddenly appeared, however, when I got to my destination and opened the bag one of my medications , and some of my toiletries were missing. After calling back to the IOP residence and informing them that some of my things were missing, they did nothing about it. They market themselves as "South Florida's Premier Treatment Center", it's nowhere near that, it's a run of the mill treatment center. I would like to caution anyone, if you are contemplating Mental Health or Substance Abuse Treatment, to do your due diligence and find a more suitable Treatment Center. I would not even rate this with a star, that would be saying they have a glimmer of hope. They also have a sober living facility in Fort Lauderdale where they let former male and female clients live together in the same apartment as couples all the while discouraging coupling, and whom also happen to work for Evolutions in supposedly high ranking decision making positions. Ownership holds no one accountable. What do you expect.

Lastly when I left after 24 hours they held my valuables hostage, phone, wallet, electronics etc. I had to come back a few days later to retrieve it. Mind you if I was a young girl in Miami Gardens walking around with no way to call for help what an awful situation to put someone in, and don't forget about the rash and bed bug bites all over my body. 0/5 wouldn't Recomended if it was the last rehab on earth.

He has fallen multiple times and was left lying on the floor--naked and alone. Not only is this undignified, but it's also dangerous. He cannot feed himself, yet staff routinely drop off his food and walk away. The meals sit untouched and grow cold because no one assists him. On one visit, I was there for over six hours--not once did anyone come to change him or check on him. The nurses? Sitting at the desk. The aides? Overwhelmed or indifferent. When food was delivered during my visit, the aide casually said, "You can feed him." When I asked who would feed him if I wasn't there, she responded that she had other residents to tend to. Unacceptable. The room smelled horribly of neglect, and when concerns are raised, the excuses pile on: "Our backs hurt," or worse--suggestions that we hire a private nurse on top of the Medicare, Medicaid, and Social Security funds they are already receiving for his care. This is not just poor service--this is neglect. Nspire is failing its most vulnerable residents while collecting substantial payments. It's time for lawyers, advocates, and health care authorities to take a hard look at what's really happening behind these doors. Our seniors deserve better. My dad deserves better. Enough is enough.
