Without a Dr's approval moved our mom from assisted living (AL) respite care to memory care FOR WALKING HER DOG DURING QUARANTINE! When we were finally able to visit, we found the memory care building to be a completely different environment from AL; dirty carpeting, roaches, old furniture in common areas and the kitchen in disrepair. From Aug '20- Jan '21 she was literally in her bed 98% of the time; no activities worth mentioning unless you are a child, no stimulation, or interaction. They are completely understaffed and have had their activities coordinator cook food, and managers act as med techs. Nadja Mobley the marketing person committed to engaging with my mom in German, and showed up twice during the entire time. After repeated requests for a reassessment that went unanswered since the initial move to memory care and having to prepare my mom lunch and dinner EVERYDAY because the food is inedible (which Regional Director Jouvana Gobin admitted to!), we moved her. I tried reaching out to corporate to literally no avail, I had one conversation with Chris Terry, Director of Operations, who then ignored FOUR calls over a three week period. Even the CEO, Alex Hurst didn't have the decency to respond to a letter. This organization is unprofessional from the top to the bottom. DO NOT PLACE YOUR LOVED ONE HERE OR ANY OTHER AVENTINE PROPERTY!

We have even had several meetings and conversations with the Administration (many times with no response). We would like to know if anyone has experienced issues with any of the following: getting responses from administration ; administering of medications, such as overdosing, not being given on schedule, or/as prescribed ; making changes to Physicians Orders (meds, changing original prescribing doctors, changing number of refills as prescribed by original doctors) without notifying family and original prescribing dr's. ; the Emergency Call Wireless Pendent not working for extended periods of time and on many occasions a lack of adequate response time ; ordering of prescription drugs and over the counter meds without family approval ; lack of adequate staffing to provide the services and care they say are included in the rental charges and level of care fees ; being overcharged and or not responding to billing questions and questions regarding auto withdrawals & checks from Dad's bank ; getting resident, who they knew could not see, to sign document(s) ; experience with Licensed Nurse not being on call 24/7 (as stated in literature provided) to ensure oversight of issues at night and weekends if needed. These are just a few of the concerns we have had. In my opinion, the techs try their best but the understaffing and lack of scheduling, availability and support from the administration is pathetic.

These therapists are really excellent, genuine, and really enjoy what they do. They also helped recognize the need for Occupational Therapy and provided us documentation for Tricare. In 19 years as an active duty Soldier this has been one of the best experiences of healthcare I've received for my family members. With over six months of bringing her here three times a week I have also observed a high level of professionalism of their staff and the care each therapist provided to the kids here both in the waiting room and during therapy. The first time I visited the office with Christiana I carried her in because she couldn't stand or walk after surgery. I have a bad back and they could see the pain it caused me. It was at that point I noticed the love and care from TLC by helping us with a wheelchair and gifting us a shower chair. Although there were other children's therapy closer to Cibolo we decided to continue with TLC because of the excellent first impression and quick rapport they built with my daughter. Christiana has gained so much confidence and her success here has boosted her self-esteem and enriched her mental health (especially during this pandemic). The therapy was challenging but they always introduced fun concepts into the mix both there and with the at home assignments. The snacks and drinks at the end of each session is of course something my child would look forward to the most. Thank you so much to the entire staff we will always remember TLC Kids as a positive light with so many great memories.

Checking into the facility, I noticed it looked a bit old and had walls being repainted BUT it looked and smelled very clean. Despite this, I has my reservations about leaving my mom. It was the first time that we had been in a situation where we required extensive nursing care outside of our home. Then, I met Lela. She is a nurse manager at the facility. Immediately she sensed my apprehension. I expressed that I didn't want to leave my mom. She encouraged me and offered reassurance. She said, "Maybe the building needs TLC and we're working on it but, I guarantee you, the people here are the best at what they do and you're mom will excel." So, although I was hesitant, I left my mom, and returned the next day and the next day and the day after that.... What I witnessed was truly remarkable. Having been a patient and being a Nurse Practitioner, I can tell you that medicine isn't always kind...except at Sonterra Health Center. Every single staff member that the facility has amassed CARES for their patients. They take their lunch breaks in their patient's rooms if they notice someone feels lonely, they come in with a smile at the beep of every call bell, they gladly change out food trays if the least bit of disappointment is expressed. Every single person has gone above and beyond to support my mother as she heals. They even provided my mother with a volunteer opportunity to do some social service, which lifted her spirits. The testament to the staff was that when it came time to go home, she didn't want to leave! I will never use another facility for skilled nursing. Hands down, the best place!!!

I spent an extended period at Sonterra Rehabilitation Center, so I saw this facility from the…read moreinside. I want to be fair: the CNAs, nurses, and therapists were some of the kindest, most compassionate and hardworking people I have ever met. Many work extremely long shifts doing physically and emotionally demanding work. My criticism is not directed at them. My concern is management. I became especially concerned about an elderly resident who told me she was terrified of her roommate, who has Alzheimer's. She reported being physically attacked and waking at night with her roommate standing over her threatening her. She became so frightened of her room that she sometimes preferred sitting outside in the Texas heat. I personally approached the facility director, explained the situation, and asked him to intervene. He promised to look into it. Before I was discharged, I confronted him again and asked whether this woman's situation would actually be resolved. After leaving, I checked on her and learned that the problem still had not been meaningfully resolved. Sonterra later contacted me about my evaluation. I again explained the situation and was assured someone would investigate. I checked again afterward and was told that nobody had come to see her as promised. This particularly bothers me because this resident appears to have nobody consistently advocating for her. I cannot know why her concerns have received so little attention, and I will not accuse anyone of motives I cannot prove. What I do know is that a frightened, vulnerable resident repeatedly asked for help, I personally asked management to help her, and I still saw no meaningful resolution. During my stay, I also had concerns about the food, insects in resident areas, staffing levels, and what I personally observed as a lack of meaningful interaction between upper management and residents. My advice to families is simple: there are wonderful frontline caregivers at Sonterra, but advocate strongly for your loved one. Visit frequently, ask questions, pay attention, and don't assume management knows what is happening simply because staff have been told. I have written commendations for Sonterra employees who provided me outstanding care because good people deserve recognition. But vulnerable residents also deserve someone willing to speak when something is wrong. Every resident matters, whether they have money, family, influence, or nobody at all. They are human beings deserving dignity and safety. I tried repeatedly to address this internally before writing publicly. The front line (meaning the caregivers on site) are fantastic !!…read more The back office, meaning the ones actually scheduling the caregivers, are not responsive and require multiple emails and/or texts to get them to confirm. This does not work well when the family member responsible for the resident tries to schedule service in order to travel. It does not matter how good the caregivers are, if the family member cannot get confirmation of service, they either have to risk not getting service or cancel their travel. My loved one is too important for me to take that risk. I fired Family Tree after my third time having communication challenges.
