The level of professionalism and scope of care administered to my dad is phenomenal. Felicia Boakye, the admissions counselor, was truly a gem. She went above and beyond with admissions and kept me updated every step of the way. If I would trust anyone with my dad's care, it is definitely this nursing home. The nursing staff is first quality and treat patient's like family. The therapy team is the best I've seen. Food is okay. I saw a lot of negative reviews and I was apprehensive of going there. I'm so glad I did. read more

Love Love Love the Physical Therapy. Push, understanding, patience, push, push push are the words that I think of first. Seemed to be short staffed at times, and did not like the food! Did have a roach in my bed on one occasion but I chased several roaches around the bathroom spraying them either Lysol. So bring snacks, and bug spray, but enjoy the PT group they are fun to work with. read more

My mother was a patient there. The facility was pretty on the surface, but there was things that needed fixing. The bed controller constantly froze and had to be fixed addressed by the staff. The TV in the room did not work. There was leaks in the ceiling, ants, and issues with the water.Though most of the staff was pleasant, I felt they were not staffed adequately to provide the care needed or even the care they wanted to provide. The staff to patient ratio seemed to keep the patients from getting proper care. I felt the administrator, Chido, was condescending to family members when approached with issues. Being in management myself, I feel the leadership in this facility has failed its patients and the staff. read more

Lack of care killed my partner. He went to Pruitt after surgery for a broken back and left with a stage 3 tunneling bedsore. He could walk with a walker when admitted but could not bear his own weight when he left. He was severely impacted with bowel movement because the aids would not get him up to the bathroom. He entered hospice care as soon as we pulled him out of the facility. He would try to get out of bed so they put mats on the floor and kept food and water out of his reach. You are putting someone's life in danger if they go to this facility. The sad part is that hospice caregivers were not surprised to hear this about the facility. read more

I'm sure Pruitt doesn't care, but hopefully, this might keep somebody from taking their loved one to this facility. My grandmother went here after having surgery for a bowel obstruction. She was weak as she had not been eating or drinking and was experiencing nausea for a few weeks leading up to a hospital stay and then the surgery. She's in her eighties, has cancer, vision problems, hearing problems, and dementia. My family and I were there often, here are the things we experienced: They would hardly ever come check on her, sometimes for up to 7 hours. We basically had to take care of her and do their jobs while we were there. Constantly, we would hear other people calling for help or hear the sound going off after they pressed their call buttons.. This would go on for sometimes up to 30 minutes. During this time the nurses and CNAs were too busy sitting in the hall (sometimes even with their shoes off) talking or on their phones. We talked to Katrina, the activities director about her or somebody coming to take my grandmother to the activities they offered, as she couldn't see the calendar of events in her room, didn't remember events were happening, and even if she did, she was too weak to get herself there. Katrina assured us that they would get her and take her to activities and that she would come get her before she (Katrina) got off work for the day and take her outside. NONE of this happened, not once did anybody come get her for an activity or take her outside. When this was brought up, there was always an excuse, "We didn't have any activities today" or, "I didn't have time to come get her for an activity". When we tried to take her to activities, they were not going on at the times the calendar said.. I get things can change, but it always seemed that there were hardly ever activities going on and when there were activities, they couldn't be bothered to come and get her. We were told she would be given meal choices each day to choose from. There were many times that she does not like. We're not sure if she didn't have her hearing aid in and couldn't hear what they were asking her, if she was confused what they were asking her, or if they were even asking her at all, as we could never get the Nutrition director on the phone and she never returned any calls/messages. We also requested that she have soup with her meals, she would sometimes get it, but the soup was either cold or so watery it didn't taste like soup. We mentioned she loved tomato soup, they said they didn't always have that, but they would put down for her to have some when they did, she never got a bowl of tomato soup. We showed up one day and the nurse mentioned that she had been asking to go outside for most of the day. When we asked her why they hadn't rolled her in her wheelchair outside, she said there weren't enough people and they didn't have time to take her outside. Therefore, she had just been left to sit in her room, other than the few hours of therapy she got that day, until we got there. The CNA, Tia is rude and unprofessional. When we took my grandmother to the restroom one day, around 4pm, we noticed that the diaper she had on was completely soaked through.. When Tina was asked about this she claimed that she had changed her when she got out of bed (they apparently didn't get her out until 10 or 11 that morning). We know from experience of taking care of her before and after this hospital stay, that she would not be soaked like she was in a matter of a few hours. Even if that were true though, we got there around 2 and Tia didn't come check on her up until we took her at 4, so not the 2 to 3 hours that they're required to come check on their residents. The whole time she was there, her hearing aid was kept on charge and was put in by a staff member each morning, she could not reach the charger on her own. On the day we showed up to get her and take her home, we found her broken hearing aid laying on her nightstand. We were told by the CNA, Dana, that they had found it in bed underneath her that morning. She does not weigh enough to break her hearing aid, and even if she did, it doesn't explain the multiple cracks in the plastic of the hearing aid.. Almost like somebody dropped it and broke it. Again, she couldn't reach the charger to get it off and hadn't tried to put it in on her own the week that she was in the hospital or the two weeks that she was in this facility, up until they claim that she did on this day. There was one day where the CNA assigned to my grandmother left several hours early. They did nothing to assign a new CNA to her or any of the other residents that the CNA who left early should've been taking care of. The residents were left to either take care of themselves, wait for another CNA to come check on them, or have their families do everything for them. She was supposed to to be there longer, but after all of this, we took her home after 2 weeks. read more

After reading the reviews, our family was very skeptical about leaving our Mom here. She was admitted to the rehab side for 4 weeks. We were pleasantly surprised. The nurses taking care of her really seemed to care. They were very personable and made our Mom feel safe. The food could be better, but that is not enough for me to take away any stars. 5 stars is a joke. My friend of almost 40 years started out in rehab and is now a long term resident. While on the rehab side, when questioned about the food; I was told by her nurse they could not accommodate a restricted sodium diet. This was critical to her health as she had been hospitalized for congestive heart failure. Only after she went back to the hospital and then returned did they acknowledge her dietary restrictions and said what the nurse had told me was not true. She is also diabetic and was frequently given sweet instead of unsweet tea and packets of sugar and salt. In the past few months she has called me several times and asked me to call them to get the nurse back to her room as her blood sugar had dropped so dangerously low; she was on the verge of passing out. Her call button had failed to bring anyone after 20 to 30 minutes. One of her doctors had told her to stay hydrated. One day recently, I began talking to her on the phone just before 3 pm. She said she had asked for water about 1 pm and again about an hour later. While on the phone with her, she asked 2 different people 2 times each for the same. even begged for it. I could hear their responses promising they would get her some. When I got off the phone with her just before 6 pm; she still had no water. Somehow they managed a cup of ice. Even offered her a soda (which she should not have) and encouraging her to finish some lemonade (hard on her kidneys and her last hospitalization was for kidney failure). Her cell phone is placed out of her reach most of the time. Due to hand tremors, she might need help but can often manage to answer a call. IF she has access. She can't get out of bed without assistance so she can't get her phone by herself. And that's not the end of the issues. Just the tip of the iceberg... It's often extremely difficult for her son (who lives out of state) to get anyone in admin to return a phone call. I don't know if there are any facilities anywhere who actually live up to a 5 star rating, but I know for sure this one doesn't. read more
