This is his FOURTH stay here at this location. He has vascular dementia, is blind, and has Alzheimer's as well as a few other medical issues. Each time he ends up in the hospital and then we request for him to come here to this facility until he is strong enough once again to come back home so my husband and myself can take care of him. My FIL has been gradually declining in health, mainly in his mind and getting more difficult to care for and a lot of times makes it hard to want to care for him but everyone here at the facility is very good with him and have so much patience with him. He is not always easy to take care of and regardless if Don is having a good moment or bad someone is always here to lend a hand. I know some people would say it's "their job and what they get paid to do" however we can see these people care about their patient's needs. The center has always been clean and nurses have always been available anytime we have questions to ask whether it be lab results, doctor appts and getting the transport assigned for them to take Don to these appointments outside the facility. We have recommended this facility to many of our family and friends and will continue to do so. My MIL was also here at one point while my FIL was here as well. We have been a few places, and this by far has been the one place where we can leave from visiting our dad and feel safe knowing when we aren't here, he is getting cared for and we need not worry. I myself have been in the medical field for years and see these cna's ,nurses, PT/OT, doctors concerned about everyone and treat them as people and not just numbers. Even down to admissions, kitchen staff, and janitorial workers, everyone has been caring and helpful! The food here has been pretty good too. Also they have daily/weekly activities for everyone here as well and celebrate holidays for those that are here and can't be home with their loved ones. There are patios to sit outside and get fresh air as well as a large room in front of the facility where if we have a few visitors at once we will take our friends and family into for more private visits. This is also the room that is used for celebrations and pot lucks. The rehabilitation room is also decent size and has everything to assist someone to build up strength etc. (See pictures included with this post) Don will be coming back home soon. However, we know he will need care like this again at some point (unfortunately these visits are getting closer together as his disease progresses) and both Jason and I find extreme comfort in knowing we can call Lodi Skilled Nursing & Rehab again for our father's needs when the time comes.

Ashliegh did not give up when the insurance company said no and she knew it was a yes. The physical and occupational therapy was on point, they help me build my strength and taught me how to navigate stairs so I could go back home. The Director of Nursing, Hansen Khan, RN has a great team of RN's, LVN and CNA's that work at station 1. They took pride in the care they given to their patients. Mr. Haneff made sure he let the patients know they could come to him if they have any concerns with the nursing staff. I want to mention the womb care staff, the kitchen staff also was on point. I really liked how the patients can get their hair,and nails done, i love the daily activities they have. I thought it was cool how they included all the patients in celebration by walking through the halls and in patients rooms like Halloween for example they dressed up.My issues was the food was not great and certain times of the day it gets cold. Overall the care I received was nice. Would I go back if needed, yes I would.

    This was the WORST experience. If negative stars were an option here on Yelp - Clearwater is a…read more-10. That's NEGATIVE TEN. My Mom had surgery at St Joes and was discharged to Clearwater Healthcare Center. She was sent there to help her infection clear, maintain dialysis treatments and keep her diabetes in check. Here's out it went: - They repeatedly failed to give her meds. The first time this happened, she was sent BACK to St Joes with a blood sugar reading OVER 400 and blood pressure reading over 200. St Joes managed to get her back on track, and she was returned to Clearwater for medicine management and recuperation. - Our second go-around with Clearwater, and both my sister and I explained the need for Mom to get her meds (insulin, BP meds and dialysis) at each and every prescribed time. Again, they failed to give Mom her meds. I called both at 5pm when she arrived and they were "working on the orders". I called at 5am the next day and she had no insulin whatsoever. - My Mom calls me and explains that a man is vomiting and screaming across from her room and asking for help, and no one from the facility is helping this man. I can hear him screaming on the phone. - Someone tried to climb into my Mom's bed in the middle of the night. Literally, I am without words. - I called on the fourth day to ASK THEM to do their job and administer meds, they left me on hold for THIRTY MINUTES. They came back on the phone and said, "We explained everything to another family member, we're not going to explain again, call them." And they HUNG UP ON ME. Terrible service. Terrible response. You literally have one job, and it's taking care of people - and they end up BACK in the hospital because you won't administer the meds that are prescribed. WHERE DO THOSE MEDS GO?

    Do not leave your loved ones here unless you want them to suffer! Staff take patients personal cell…read morephone for weeks at a time. So family is unable to communicate. Nursing staff hang up on you if you can get someone to answer. Patients are not getting proper breathing treatments. Nursing director and social worker are a joke. Neither have responded to repeated phones and letters. My family member has had to call the ambulance service several times because of breathing issues. Nursing staff at night won't give breathing treatment or oxygen. Nurse told my family member they would finally be released if they stop calling ambulance to come out. This place ONLY CARES ABOUT MONEY NOT THEY PT. When insurance stops paying Clearwater. Clearwater brings patient to hospital to be admitted for 10 mins and returned back to Clearwater to started billing insurance again.
