First I would like to take a moment to acknowledge Ashleigh, Haneef, Pedro P, Sopanny, Maunavie, Stacy A, Cyrus, Amera, Yulissa, Manila, Ranveet, Abid, Jessirene, Lisa, Mandeep, Paul, Tina, Alicia, Cindy, and anyone else who has taken care and who is currently taking care of my FIL. This is his FOURTH stay here at this location. He has vascular dementia, is blind, and has Alzheimer's as well as a few other medical issues. Each time he ends up in the hospital and then we request for him to come here to this facility until he is strong enough once again to come back home so my husband and myself can take care of him. My FIL has been gradually declining in health, mainly in his mind and getting more difficult to care for and a lot of times makes it hard to want to care for him but everyone here at the facility is very good with him and have so much patience with him. He is not always easy to take care of and regardless if Don is having a good moment or bad someone is always here to lend a hand. I know some people would say it's "their job and what they get paid to do" however we can see these people care about their patient's needs. The center has always been clean and nurses have always been available anytime we have questions to ask whether it be lab results, doctor appts and getting the transport assigned for them to take Don to these appointments outside the facility. We have recommended this facility to many of our family and friends and will continue to do so. My MIL was also here at one point while my FIL was here as well. We have been a few places, and this by far has been the one place where we can leave from visiting our dad and feel safe knowing when we aren't here, he is getting cared for and we need not worry. I myself have been in the medical field for years and see these cna's ,nurses, PT/OT, doctors concerned about everyone and treat them as people and not just numbers. Even down to admissions, kitchen staff, and janitorial workers, everyone has been caring and helpful! The food here has been pretty good too. Also they have daily/weekly activities for everyone here as well and celebrate holidays for those that are here and can't be home with their loved ones. There are patios to sit outside and get fresh air as well as a large room in front of the facility where if we have a few visitors at once we will take our friends and family into for more private visits. This is also the room that is used for celebrations and pot lucks. The rehabilitation room is also decent size and has everything to assist someone to build up strength etc. (See pictures included with this post) Don will be coming back home soon. However, we know he will need care like this again at some point (unfortunately these visits are getting closer together as his disease progresses) and both Jason and I find extreme comfort in knowing we can call Lodi Skilled Nursing & Rehab again for our father's needs when the time comes. read more

I was there for almost 2 weeks. The admission department is one that work really hard with the insurance company. Ashliegh did not give up when the insurance company said no and she knew it was a yes. The physical and occupational therapy was on point, they help me build my strength and taught me how to navigate stairs so I could go back home. The Director of Nursing, Hansen Khan, RN has a great team of RN's, LVN and CNA's that work at station 1. They took pride in the care they given to their patients. Mr. Haneff made sure he let the patients know they could come to him if they have any concerns with the nursing staff. I want to mention the womb care staff, the kitchen staff also was on point. I really liked how the patients can get their hair,and nails done, i love the daily activities they have. I thought it was cool how they included all the patients in celebration by walking through the halls and in patients rooms like Halloween for example they dressed up.My issues was the food was not great and certain times of the day it gets cold. I really didn't wanna be there for rehab. I knew I needed it, but I was not a very nice patient. Everyone there was so kind. They made me feel like I really didn't need to be there and that they would take me through every step of it. They did you actively see the administrator, the head of nursing social services and many more people every day watching out for all of us in there making sure that our rehab went well. The nurses CNA's everyone went above and beyond to help me get out of there quickly. PT was great. They accommodated your time frames and push to the little extra that I needed. The only thing that wasn't great was food, but it wasn't horrible. It just wasn't what I was used to. We always had nice ice water always had someone to help us when we needed it. Their staff is friendly, kind and compassionate, they take the time to listen to your concerns and immediately implements a source or sources to address concerns, they are open and always willing to listen, my moms care during this last episode was surpassed anything I have experienced in the past with other facilities. Unless you have no other options, I suggest that you do not go here. I am very efficient and write down everything so when their stories changed, I knew I was correct. When my husband was being discharged, I made it very clear that I was definitely not able to take care of him at home. I knew that it was not possible for me to take him to any aftercare or appointments. I was told that home health, physical therapy, and even a doctor would be coming to our home. They did not order anything. I did not hear from anyone and the insurance company said nothing was ordered. They made an appointment with a doctor even though I had made it very clear that I could not take him to a doctor. It actually took me longer than it should have to realize that they really just lie all the time. The updates I received made me wonder if they had him mixed up with another patient. I don't really know what kind of care he received but I strongly suspect that they kept him drugged all the time to deal with a difficult patient. My mom was here for quite some time, she came in not even being able to sit up on her own. Not able to roll over, to be able to do it on her own because of all the help and care she had got. The Pt was amazing. Their nursing and wound care was amazing. They made calls to send Mom back to the hospital when it was needed. My mom is using a walker as needed but she is doing amazing. My grandmother has been at Lodi Nursing Home & Rehabilitation for some time. Our family is pleased with the quality of care she is receiving. The staff there is friendly, attentive, and available when needed. Administrators consistently communicate with us about my grandmother's well-being and plan for care. They have addressed all her needs in a timely manner. Several of the staff members have gone above and beyond to make my grandmother feel at home and loved. We visit often and have always found the facility to be clean. My grandmother enjoys all of the holiday events and regular activities such as bingo. My father was admitted here 24 hours ago and this morning he called my Mom and in her native language he asked "Come and pick me up please.." My Mother was very concerned and I came here with her to help. They asked why we wanted to take him because they didn't have an order and my Mom said, "he asked to go to the emergency room". The lady that my mom was corresponding with my Mom at the door said, "Ok we will get him but we haven't changed him yet" My Mom said, "You haven't changed him at all since he's been here?!". The lady didn't answer her and just put her head down. He had a surgery that has him blind currently and he said that he was being forced fed and it killed his appetite because it was hard to trust the person feeding with the attitude they were having. It seems as though when they receive bad reviews they have a canned message they copy and paste asking to call with concerns. When dealing with people's loved ones and serious health issues the last thing anybody wants to see is a generic message. The ball has been dropped and when help was really needed there seems to be none and even worse no accountability. read more

It is 10:30 PM. I arrived about 6 PM. I write this from my exceptionally cold room, wrapped tightly in my very cushy robe. It was in the 90's today. I was greeted very friendly, and promptly served a grilled cheese that looked delicious, but was quite deceiving. Beautiful toasting, but the one pathetic piece of cheese wasn't even melted. The side with it was sliced cucumbers in sauce. Not their fault, but I hate cukes. Got a tiny juice and they served me milk. MILK. I am lactose intolerant. Then things went downhill. This joint is falling apart. Maybe they're beginning to redecorate, but I don't think so. They arrived with my medicine....er, someone's medicine, just not mine. 2 pills I do not take, and 1 my Dr discontinued a couple of years ago. Not the 2 I currently take. Did I mention I'm shivering? The bed is turned down, and there are holes in the sheet. The wall in front of me is filled with gouges and holes where pics may have hung. The nightstand is peeling. Peeling. The bathroom is clean but shabby. No shower...? Asked for a little snack and some juice. Got an applesauce and some graham crackers, which is fine, but when asked for juice or something to drink was told No. Bro is coming to spring me tomorrow. I hope to get a little breakfast. No milk. Shivering and eyeing the one thin blanket. SAM I don't usually change a review so drastically. In fact, I don't think I've EVER changed one. Last night was pretty miserable, I won't change a word. This morning flipped everything. My medicine is correct, it warmed up to a normal temp, all the juice I want. The physical is still the same, but today the people themselves have started my day beautifully, and the treatment alone brought this review up. 1 to 4. Good job, people of today! =) read more

Everyone I talked to there was professional, courteous, really stretching themselves to be helpful in my urgent and stressful situation. This is one of the best facilities in the surrounding area! The facility is always clean and the staff is extremely attentive to the residents and visitors' needs. We even had a meal there while visiting and I was surprised at how good and nutritious the food was. The nurses are always willing to help as no job is too small or big for them. They only reason I'm giving one star is because my grandpa actually made it out of there alive, despite all the neglect, unsafe practices and uncaring people in this facility! It's a joke to see that, they are 5 star rated! They left medication out that was needed by my grandpa(to stay healthy) and worse, what if my kids would have taken it! When we brought it to their attention, they acted like well, he can go somewhere else, instead of fixing the actual problem. They don't care, they just want you and your Insurance's money. Most people are there to collect a paycheck. When you have a legitimate question about your family member's health, you would think you could go to the nurses station. Well, here they are having causal, unrelated work conversation and will ignore you. When they finally answer, they give you a look, like you are bothering them. The place smells good when you walk in and I will tell you that it's clean, but don't let that fool you! The majority of the people that work there don't have any empathy for your family members! My grandpa was lucky that he had people to check in on him daily. What about all the poor people that don't have a voice and they don't have family to speak up for them? If you don't care about your family member and their health, than Lodi Rehabilitation & Nursing is the place!!!! I wish I would of listened to the family members of patients who have stayed at this discusting facility before picking this place for my fathers recovery after his surgery. This place gave me a BAD vibe the moment we walked in FYI I should have listened to my gut before leaving him in the hands of these incompetent people. They are ALL their just for a paycheck!! BEWARE!!! I'm giving this disgusting place O stars. This place needs to be shut down. My family chose this disgusting place, only because we saw it had better reviews than other rehabilitation centers but boy, were we wrong. We decided to put my Dad here to regain his strength after open heart surgery and he was neglected and not taken care of. I walked in to visit him one morning and first thing he said to me was "get me out of here" and started crying. When I asked what had happened he said he pressed the call button for help many times all night and the nurse on duty came in and asked him rudely what he wanted. After asking her for assistance to the bathroom her response was he couldn't get up. He then asked "why not, I can get up with help I did earlier today" she said no to him again and gave him NORCO for pain which was unnecessary because he never complained of any pain, he was only asking for help to the bathroom. He said after she gave him Norco he began feeling dizzy and nauseated. We feel like this nurse needs to be terminated and thrown in jail. How dare her give my Dad Norco for pain without asking him if he was in pain. She clearly did it on purpose to shut him up because she didn't want to deal with him. To all the people reading my review, please think twice before sending your loved ones to this disgusting rehabilitation center. The whole time my Dad was there he wasn't being treated the way we thought he would be treated. They neglected him and didn't treat him right. The staff isn't very friendly. This place has been updated in so many ways since I first saw it about 4 years ago. The staff is beyond amazing, and always impressed with the knowledge and professionalism of the Marketing/Admissions Director Jennifer DeGuzman. She is a breath of fresh air in the skilled nursing field. I have many friends who have stayed at LNR for physical therapy and nursing needs and were absolutely thrilled by their care and experience. Very excited to see what's next for this facility and their generosity to the Lodi community as well. read more

This facility is a beautiful, remodeled, and amazing environment. The staff there are so hard working. The nurses are so proactive, the CNA's are very attentive, and the doctors come in on a regular basis. The rehab team works very hard and makes sure to come in to evaluate their patients as early as they can after admissions. I think too that the activities department is incredible. They have choirs, therapy dogs, board games, paint nights, movie days, church, and so much more. Honestly I couldn't say enough great things about this place. I had the opportunity to be a patient at Lodi Rehab in September,October and November of 2022. I was there to recover from a severe infection from a wound in my leg. I received Physical and Occupational therapy. I wet home after meating many of my goals. I was more able to take care of many of my personal needs. I also was seen by a nurse at least once daily, sometimes twice daily for the needs of my wound care. Saw a physician at least once weekly to review and update any orders pertaining to my wound. I found the wound nurse, Lili Smith to be very knowlegeable about caring for my wound daily. She was able to make many suggestions that were very helpful in beginning to heal my wound. She demonstrated a lot of much needed experience, as situations arose during my stay at Lodi Rehab. As the time for me to be discharged came, my wound had healed 50 percent. I was discharged to go home , and taught how to proceed with my wound care. Home health nursing was recommended, and is now able to care for my wound. I was able to hav e a skin graft upon my return home. The wound was ready for a graft. I m looking for healing to continue. I m so thankful for Lili and the team at Lodi Rehab that cared for me!!! I would recommend Lili Smith if you have a wound that needs experienced nursing care. I have had my wound for 4-5 years. I believe I ll one day I ll be able to celebrate complete healing!! They also treated my sister so well, she stayed night and day with my mom and they accommodated as much as possible and treated her as if she was family. I WISH NEGATIVE STARS WERE REAL! My family member has been here several times. When Karen was the director of nursing, it was an amazing facility. Every time my relative had a fall or pneumonia or what have you, Karen made it feel like it was her family member too. The most recent stay my relative had there was a new nursing director Fabio who completely did not care at all about the concerns of my relative. IM THE RESPONSIBLE PARTY! Fabio mislead me, lied to me several times. When I spoke to doug the administrator, he seemed more flustered and didn't know how to resolve issues. The kitchen lady Marie?? (I cant remember her name) was so rude to my relative and gave her food she wasn't allowed to eat. There's a nurse in the afternoon with neck tattoos who is so nice and professional to the patients but you can hear make fun of all his patients. I wish Karen still worked there. I don't know if it would have gotten this bad. But mark my words, MY LOVED ONES WILL NEVER GO HERE AGAIN! Oh and also, the person who reviewed and mentioned the leaky roof, they just painted over the mold and mildew instead of fixing the issue. Hopefully the ceiling doesn't collapse. read more

My husband's grandmother stayed here and we were all impressed! The staff was friendly and quick to help and the rooms were clean. Grandma was comfortable and well taken care of- we've had to say our goodbyes twice, but she's still alive (strong woman)!!! Lodi Nursing and Rehab has come a long way since it was Delta Rehab. The compassionate and hard working staff have turned this building into one they should be proud of. Each and every staff member are quick to address every comment, concern and compliment and they handle each one with poise. I saw a patient go from possible hospice to thriving in her assisted living community. Lodi Nursing and Rehab did that. They provided excellent care, compassion and patience with her and she is now excelling beyond belief. This is somewhere you want to send your loved one. So I've been to many nursing facilities around the San Joaquin area as an LVN and I must say that Lodi Nursing and Rehabilitation has been the best. The staff has been amazing! They all take pride in what they do and go above and beyond to make sure that everything is taken care of or will be taken care of for their patients. The collaboration and communication with their patients and the different departments on the facility is the best I've seen thus far. They have so much to offer for their community from setting up fun and exciting activities for the residence and participating in community based events. Let me start with the staff, the most amazing people that I had work with me to get me to come home. They were all caring devoted to their jobs spoke well of everyone else treated patients around me with respect and dignity. If you ever need rehabilitation or long-term this is my first choice for a facility that I would want to be at. My therapy sessions with physical therapy and occupational therapy or beyond my expectations of getting well they got me to a point that I didn't think I could get to, also knowing if I don't feel as good as I should that I could go back no questions asked I love this place I love the people that work there and I fell in love with some of the patients. They claim to be a five star. Well that is a lie because they just changed names to cover up for how bad they are. Well reading the reviews I think some of them were written by employees. While it is obvious they have cleaned up the front it is just a face lift on a poorly run facility. While there are a few good staff many of them don't care or worse would ignore and mock the patients in need. Very rude and unprofessional. I saw a lady they had left on a bed pan for an hour ignoring her calls that she was done. My GF was kept there longer than her Dr wanted, they don't follow the Dr instructions, the on call Dr is only there 3 times a week and the nurses won't call him to clarify a mistake. They told My GF that her wounds were closed but upon inspection not only were they not closed but one of them had green puss discharge. When the Dr gave her discharge orders they actually tried to get her to sign a AMA discharge paperwork if we hadn't been there. This is where you take people to die that you don't love. It would be better to take them to a field and shoot them in the head then take them here. read more
