I arrive at 230 pm for my appointment 2 weeks later &, after my blood draw, was handed a cup to do a urinalysis & was directed just around the corner to the scantily supplied restroom where there is a large sign on the wall instructing one on how to get a "clean" urine catch. Now, as a 53 yo woman I am no stranger in how to get a "clean" catch & was annoyed that there was not a single sanitary wipe - the "clean" in the clean catch - located within this restroom. Furthermore, there was no hand soap located within this restroom for me to wash up after using the toilet as the hand soap dispenser on the sink had been completely removed?!? WTF?! I left feeling like this lab was a complete joke & questioning if I had made the right decision entrusting a lab that doesn't align itself with basic hygiene practices. Where does my tech wash his hands?!?!? F$#%ing GROSS! I certainly will not be using Quest for labs ever again.

Multiple emails and calls, with promises of callbacks and nothing on their end. When you provide a 10-day doctor's note, you should be able to back it up by filling out FMLA paperwork. It has 15 days to be filed, and this is my wife's job on the line. She had to provide me ADL, IADL, and emotional support during the time of the doctor's note, therefore making the FMLA very simple to fill out so I'm not sure what the hesitancy and lack of communication is for. She is friendly yet professional, caring and considerate, patient, reassuring, responsive to my questions.
