I urge you to carefully consider this before contracting with any hospice provider. Unfortunately, our experience with Faith & Hope made an already painful situation even harder. In April 2025, my mom suffered a massive stroke. She was placed under Continuous Care (CC), which meant she required critical, around-the-clock nursing support. We were told her case would be re-evaluated after three days. However, the care was stopped in less than 24 hours and we were given only 4 hours' notice that services would be discontinued. At that point, it was clear our needs no longer aligned with Faith & Hope's philosophy of care. We scrambled to hire private help on extremely short notice. That night, my mom was in significant pain & experienced agitation. She needed her medication adjusted and an increase of the dosage frequency to help manage her symptoms. The next day, we made final attempts to Faith & Hope to reassess mom's eligibility for CC, even though the on-call night nurse, when we asked, did acknowledge that we were free to seek another hospice provider. We waited all day for a response from Faith & Hope, but heard nothing. Faced with another evening without adequate care, we transferred my mom to a different hospice. They gave us the right care without hesitation. Sadly, our mom passed away only 4 days later. The only positive experience we had with Faith & Hope was with Nurse Theresa. She cared for my mom with compassion, dedication and a true commitment to patient advocacy. Theresa's thoughtfulness and support helped guide my brothers and me through an incredibly challenging time, and for that we are deeply grateful.

Medicare and Optum covered the hospice side. I paid separately for 24/7 caregivers through Sincere Care Services, which I chose because I wanted Cantonese and Mandarin speakers so my mother would be more comfortable. Five Star was recommended by the Optum physician on staff. Separately, a family member recommended them too, through a friend whose father had passed the week before. I checked their ratings on Yelp and Google before moving forward, and I am extremely happy with the service we got from start to finish. This is the point where you decide between more hospital intervention, which can be painful, and accepting the situation and moving to hospice. Hospice is the wedding coordinator for a peaceful death. Things can still go wrong even when you plan everything perfectly, but Five Star was there at every step to handle whatever came up. The rough sequence for anyone about to do this: 1. Decide on hospice. 2. Line up caregivers. They are separate from Five Star, but Five Star will point you to good ones. I wanted Chinese-speaking caregivers and Mark made it happen. 3. Coordinate with the hospital on transportation. 4. Be home to receive the supplies. 5. Be home when your loved one arrives. Have a bottle of distilled water ready for the oxygen machine. 6. Have the nurse and the caregiver both there so you can learn the comfort meds. 7. I used 24/7 caregivers on 12-hour shifts. I made a point of being there whenever a new caregiver came on, so I could walk them through the medications and get their phone number. Nurses came once a day to check on my mother since she was close. You do not have to be there the whole time, but I tried to be there for shift changes. One thing I wish I had watched more closely was bowel movements. My mother had been constipated since the hospital because she was not eating. Another tip: put together a playlist of beautiful music to play in the room. Otherwise it is silence, or your loved one is listening to caregivers talk and scroll on their phones. Even when your loved one is not responsive, they are still listening and drifting in and out. Tell them everything will be okay. Thank them for everything they did for you. I think it helps them go peacefully. Anytime I needed something, Mark was a text or a call away and he always answered. Again, they are the wedding coordinators for a loved one's passing, and they will do everything within their power to help you through it.

But I know of what I speak and feel compelled to do so due to the extraordinary hospice care that was recently provided to my mother by Comfort Choice. Her hospice care lasted for over 8 months. During all of that time, and especially towards the end of her life, Comfort Choice provided excellent care - in all aspects of the term and our needs - and always went the extra mile. The help, compassion and attention provided by Comfort Choice made her later life more complete, easier and more comfortable. In particular, our hospice nurse, Pamela, provided what can only be characterized as stellar care. She was not just a caregiver, she was a true friend, confidant and advisor whom my mother trusted, cared for deeply and loved. She was always there for my mother - and for myself and our family. She helped guide all of us through this difficult process and did so with candor, great advice and compassion. We are so lucky to have found her and to have had her as our nurse, friend, advisor and guide during this trying period. If I ever have to do this again, my first call will be to Comfort Choice, and hopefully with Pamela.

I found Comfort Choice because of recommendations from families and staff at the assisted living, after my father was hospitalized. It took a day, maybe two at the absolute most, for the paperwork to get in place for them to start seeing him. They move fast, which is incredibly appreciated. It's been about 2 months and Pamela is nothing short of amazing. Most importantly, she provides my father with the care and comfort he needs, answering his questions about death and dying, what's going on with his body and his diseases, and also advocating for him by knowing what will help him be comfortable and live a happy life, including nutrition, activities, etc. Comfort Choice provided him medical equipment literally the day after I signed the paperwork. Unlike every other medical provider, I have had to do nothing, no chasing after the DME company, no calling around for authorizations, nothing. They took care of everything and bill Medicare directly. Pamela has also helped me understand what is going on with my father's conditions and the road ahead, she is very responsive, knowing that she has other patients and families to support, too. I admit that what I thought about hospice going in was actually very different and very limited compared to what it actually is. I recommend calling them, asking them all of the awkward, scary questions, they've heard them all before, and they will happily answer all of your questions and follow up with you very quickly.

(note:I had loved ones in both home health and hospice care) In a nutshell the home health part conducts unnecessary or made up visits and get them paid by charging patients insurance. The hospice care is another disaster that promises one thing at the beginning and than disappears after they admit your loved one. I also learned that their hospice care is headed by a physician that is an internist and has no idea about hospice. The patient is admitted, Shepherd bills for hospice start of care. (Hospice apparently involves a large sum of money that is paid to the provider at the start of care). At this point you are on your own. There are limited nurses available 24hours a day and the ones that are available complain about being overworked and underpaid. You are lucky if you can get a nurse to administer pain medication in under an hour after calling Shepherds office especially if you call after 10 pm like me. After researching hospice it was explained to me is that a large dollar start of care payment stays with the provider regardless of how long the patient stays alive. In my case that was less than a day. After numerous calls to their office I was put in touch with the owner. This turned out to be an older Egyptian lady named Odette. She mentioned being Egyptian and that her exucational background involved some high school outside the country but no higher education. She struggles speaking English and is very loud on the phone. She was rude and unprofessional when confronted by my questions related to charges made by her company. She laughed and said something in a different language to me and then said she had no comment and hung up on me. Shepherd discharged my loved one that was still with their home health after this call. I have made numerous calls to Shepherds office since and even struck a nerve with a staff member who was sympathetic towards my problem. They told me that Odette was involved in real estate early on and decided to get into home health and hospice after seeing how easy it was to take funds allocated to pay for citizens health care. Obviously this didn't take care of my grievance with shepherd so I'm having my loved ones government insurance look into this matter. After all is said and done it would have been easier to get another home health / hospice provider than to have gotten mixed up in this nightmare with Shepherd Home Health Care / Hospice inc. People Beware...

There are other places out there - one that recently I couldn't wait to leave - Sacred Heart. Ugh! What a nightmare! They talk sweet (artificial, artificial and more artificial) and although their words "say" they care, but deep down my personal experience with me was that they really don't give a damn about anything. Maybe they are "new" (a really good thing to check out before) and heaven only knows a place like that won't survive with service like theirs. Sadly, I experienced hospice before and was fortunate to learn about Shepard. The place my Dad is in had a connection with Sacred Heart (notice to the wise) so for my dad, I tried them (geez am I ever sorry!!!) I couldn't wait to cancel with Sacred Heart and Move my daddy to Shepherd Home Health. My dads doctor is Dr. Hanna and she works with them and there is no more caring a doctor than Dr. Hanna. Hope you all are able to benefit from my experience. All I can say is that today I needed to take camomile tea to calm my nerves after dealing with Sacred Heart...and didn't feel better no matter how many tea bags I used to calm me down, until I spoke with Shepherd and transferred my daddy.... what more can I say...? I'm so sad that there is a need but oh my gosh, when there is this painful "need" I certainly hope my family uses this company when it's "my time".

As soon as we decided to switch to hospice care, we've been in touch with a few places but Maximum Care Hospice felt most transparent and comprehensive. Other places were a bit unclear about who is coming from where and what exactly is being provided with hospice care. Switching to hospice care was a difficult decision, but we are so happy to have chosen Maximum Care. We had a wonderful nurse Victoria. It was a short seven days but she provided all the support that we needed. There was no judgement whatsoever (my father was difficult) and she was very respectful and kind. We had a somewhat unusual circumstance, where the police had to be in touch with them (due to my father's passing as a result of a crime), but they handled it all very graciously. Equipment delivery and pickup was excellent. They are contracted with a place called St Joseph's and every staff from there was respectful and friendly. We also had a wonderful woman come to bathe my father before he passed away She was very thorough and efficient. I believe she isn't an employee but someone who is contracted to provide personal care to several hospices, but she had told me that she tries to take every job requested by Maximum Care because they treat her very well. When my father passed, Victoria came immediately and connected us with their recommended mortuary. My cousin, who is experienced with contacting mortuaries at her church community, was very impressed how seamless and "easy" Maximum Care made the process. Thank you again, and I will forever recommend Maximum Care Hospice to anyone who needs hospice care providers.
