They house over a half a dozen diabetic patients and don't even offer dietary diets for them. This place is a danger to any person that steps foot in the filthy doors, patient or not. Management is nonexistent and the DON-Tamesha avoided any type of resolution or responsibility and refused to speak to me because it was her "day off". She stated she didn't want me to have her number but happily gave her coworkers number up to avoid all responsibilities. It took calling an ambulance to the nursing home to check my family member out for someone to even talk to us. The staff do not care about your family members being here. They believed a renal failure patient with a blood sugar of almost 600 was not a crisis and allowed him to sit there all day without intervention. The employees are completely unqualified to be running a unit with physically impaired patients that require assistance. Night shift is nothing short of a dangerous situation with the lack of intelligence assigned to caring for these helpless patients. Staff failed to provide continued IV antibiotics causing my family member to get a secondary infection in the lungs causing respiratory distress and requiring draining and a chest tube. PT consists of moving to a chair and being left alone immobile for 2-3 hours. Call lights ring constantly and no assistance ever arrives. My family member waited 2 hours for bathroom assistance and no one ever came, he urinated all over himself and the urine remained on the floors for over 24 hours without anyone cleaning up. Patients are not bathed or assisted with so much as teeth brushing. The residents are smoking in the bathrooms which are connected to other residents rooms and stink up the entire facility. The utter disregard for the safety of these patients alone should warrant it be shut down. Not to mention the staff lying about what their positions are they hold. They attempted to make me sign a AMA form knowing it would instantly drop my family members insurance for his rehab and he would have no where to go. If this wasn't a Medicare facility these employees would be jailed and the facility would be shut down. MALPRACTICE AT ITS FINEST. Don't send your loved ones here unless you want them to die.

During a…read morewinter ice storm, the heat broke, and we found my grandparents in their recliners with towels wrapped around their heads trying to stay warm. Staff regularly called us asking if we could come in and help because they were understaffed. We asked for shower traction strips for months for my grandfather (a fall risk) and were given nothing but excuses -- eventually told, falsely, that they "wouldn't stick." They also put wander guards on my grandparents' ankles without explaining them, which left visible bruises, and refused to remove them even after confirming my grandmother was not an elopement risk. Both grandparents went in on little to no medication and came out with several new prescriptions. They even moved them into memory care for "babysitting" without telling us. When my dad questioned the level of care, Chad, the regional administrator for Cardon, argued with him and bluntly revealed that both my grandparents had dementia diagnoses -- before any care provider had even explained that to us. It was rude, inappropriate, and confirmed how poorly this facility communicates. The administrator was dismissive and offered zero solutions. Billing has been a disaster, with duplicate statements and refunds issued incorrectly. After speaking with multiple other families who had similar horror stories, we decided to move my grandparents out of Solarbron and into our home. If you value safety, transparency, and compassionate care, look elsewhere.

the facilities, nursing, staff and activities are great. physical and occupational therapy are…read moretruly awesome. the beauty shop is wonderful. unfortunately, the dietary department is worse than awful. i rarely get what i ordered on first pass. fresh fruit is said to be "always available upon request"- sorry i do not consider canned fruit as fresh fruit. meals are distributed in pieces. not uncommon to hear "they did not send your tea - they have to make it & will bring it up later, no fresh fruit today, but here is a bowl of (what looked to possibly be) mandarin oranges (or something else). or, once you get ready to eat your salad, you notice there isn't any salad dressing that was ordered. they misplace your meal order form and just send up anything to your room for you to eat. this may seem petty to some; however, meals are an important part of my day & i would like to not have to be hassled at every meal course in the day. it is sad that this department provides such terrible service and there doesn't appear to be any effort to correct the short-coming(s) of the dietary department. i was told by a staff member "this is an on-going issue day in & day out with dietary" i hope this will cause someone to take a look at the lack of quality of service demonstrated by this department. you have too many employees who care about the services provided by river pointe to allow this department to operate in this manner.

My 89 year old mother was sent to St. Charles Health Campus for skilled care and rehab after a…read moresuccessful surgery for a broken left tibia and fibula. In less than two weeks she ended up with a severe 2 inch necrotic pressure wound on the back of the left heel as a result of St. Charles' staff redressing and re-applying her surgical splint with no attention to protecting the heel. Next were severe pressure wounds on the top of her foot from dressings applied incorrectly and an ortho boot strapped too tight. I started weekly visits to Memorial Wound Care to try to turn this around. St. Charles was negligent and inconsistent in following the dressing instructions from Memorial Wound Care. I had to visit daily just to ensure her wounds had been properly attended to. I arrived one day to find mom missing all the skin on the back of the toe on her right foot! I was the first to notice and no one could tell me why or when it happened. Now she has severe injuries to both feet! She is NOT diabetic and has verified good circulation in her legs and feet. Mom also had numerous falls there and lost nearly 10 pounds. The management team in the skilled services area is extremely lacking and present little accountability or leadership to the skilled staff that is trying to do a good job. There is a lack of a thorough patient information transfer process between shifts enabling mistakes and inconsistencies in care. However, I was impressed with the rehab group staff, the activities staff, the environmental services manager and several nurses and aids who really were trying to do a good job under bad management. I finally moved mom to another facility for her safety and for better care. I only wish I would have done it sooner. Not one person from the management team acknowledged mom leaving or offered an apology for all that happened to her there. An important Red Flag --- the day after I moved mom my husband requested a meeting with the Executive Director to discuss the challenges with mom's care and to give him more insight into mom's and our experience. My husband left 3 polite messages over multiple days asking for a call back. When the Executive Director did finally return our call he was curt and refused to meet with us. He told my husband "I have complete confidence in my staff and have no interest in what you have to say". That is a direct quote I kid you not. If that is the way an organization is run from the top-down there is no way it is patient-centered or even safe for your loved one. Please consider my mother's experience and choose carefully. Mom will likely never walk again after all this and could even lose her foot.
