I have outlined the key situations below: Thursday, May 22, 2025 - Cellulitis Diagnosis and Treatment: On this day, my brother Andrew and I noticed that our mother was not feeling like herself. Andrew observed that her legs felt like they were on fire. I immediately informed Dr. King, who briefly examined her and diagnosed cellulitis, but did not provide any further details or explanation. Later that day, another provider confirmed the same diagnosis and began antibiotics, but there was no improvement by the following day. I then requested more aggressive treatment. The provider suggested starting ceftriaxone, but it took over two hours for this to be administered. After that, we saw significant improvement by the next day. (pictures attached) Friday, May 23, 2025 - Bathroom Assistance: While visiting Ruth, she needed assistance to use the bathroom. Winston, who came to help, initially assisted Ruth onto the toilet, but then left to tend to another patient. He was gone for so long that Ruth became distressed and started to cry. Despite pulling the emergency cord, no one came to assist her. Fortunately, another nurse eventually arrived to help, but this delay caused unnecessary distress to my mother. Request for Air Mattress: On Thursday, May 22, 2025, I requested an air mattress for my mother. I followed up on Friday and again on Saturday. Unfortunately, the mattress was not brought to her until late Sunday afternoon, which was an unacceptable delay given her condition. (picture attached) Bathroom Assistance and Nurse Interaction: On another occasion, a nurse assisted me in bringing my mother to the bathroom. However, once we arrived, Ruth was too fatigued to use the toilet. The nurse then left to attend to another patient. I was left to manage the situation, but when I tried to assist my mother in getting back onto the toilet, a different nurse entered and raised her voice at me. (the nurse was white and on the hefty side) She insisted that Ruth be returned to her wheelchair and put back to bed, even though I had already nearly helped Ruth onto the toilet. Sunday, May 25, 2025 - Fall Incident: On this date, Ruth fell and sustained injuries. The images of her injuries are attached for your reference. This incident is of particular concern, and I believe it warrants further investigation into the circumstances surrounding her fall. I am reaching out with the hope that these situations will be thoroughly reviewed and addressed. The care my mother received did not meet the standards I expect from a healthcare facility, and I would appreciate any steps that can be taken to ensure better care in the future.

    Dr. Czarnecki gets 5 stars and so do the nurses. The care they give and the understanding is…read moregenuine and bedside manner are top-notch. They are totally knowledgeable and experienced professionals. So why only one star overall? The people that answer the phone both medical and secretarial are atrocious. They are the very opposite of caring. They are callous and rude and are ready to do battle with patients that are suffering and in pain and need medical help or just have questions about their medications or some other issue. I've been placed on hold for several minutes probably more like 10-15 minutes. Then when they do answer they talk down to you like you are a clueless imbecile and not worth the time like the gunk stuck to the bottom of their perfect shoes. Good luck getting past them to even get an appointment or get your questions answered. You will end up feeling sicker and worse than before you called. I would just steer clear of this place to send a message to the administration and front desk and phone support staff. I have excellent insurance that pays top dollar and I don't need to be treated like a petulant child by haughty know it all secretaries and med techs. It seems consistent with the other reviews that the issue clearly lies with management and administration.

He was sent to Trinity Hill Care Center for rehabilitation. Unfortunately, 2 days before Thanksgiving he was discharged. I say unfortunately because he should not have been discharged. I was told his discharge included a home health aid that would be visiting daily at his residence to assist him with his blood sugar, food etc. (He wasn't at all able to comprehend even how to check his own blood sugar). No home health professional came to my brothers home. Trinity apparently did NOT schedule this as part of his discharge, but it appears as sheduled on his discharge documents which I have. For 16 days he was alone at his residence, with no help. He could not even walk after 4-5 days. How does this happen? My sweet brother was found unconscious on December 12 and passed away on Christmas Eve. Low blood sugar caused irreparable brain damage. They have no idea how long he was lying unconscious before he was found. I spoke to the home health facility listed on the Trinity discharge docs and they have no record of a phone call, intake, referral, they have nothing from Trinity regarding my brother.They said to me they have no record at all of his name anywhere. This vital part of my brother's care apparently was never put in place. It could have saved him. Likely would have. What happened Trinity? Well I asked and: Trinity Admin named George promised to call me back 2 weeks ago,... nothing. Then again another promise to call me back 1 week ago. ....nothing. This is not care. This is apathy. And this is a dangerous facility.

My CNA's have NEVER made my bed, I have to practically beg for Ice Water, When I was sick, they ignored the call light for up to an hour. This place is the worst place I have ever seen and unfortunately had the misfortune of being placed here d/t Insurance.

My mother, a vulnerable patient, has suffered significant financial loss and emotional distress under her supervision. While Kendra was supposed to be assisting with housing applications and DSS redeterminations, she instead gained access to my mother's online accounts and changed the passwords, locking her out. During this time, over $400 in food stamp benefits were wiped out. When confronted with the missing funds and the fact that a promised housing application for West Hartford was never filed, Kendra became hostile and unprofessional. To matters worse, the local management (Darcy, Kendra's Supervisor and Joy the receptionist) has been dismissive of these serious fraud allegations, even attempting to "settle" the theft of $400 in essential benefits with a $50 gift card. A police report has been filed. If you have a loved one here, monitor their accounts closely and do not allow this individual access to sensitive passwords or personal identification documents. The way they change case managers and therapists here is like changing underwear. They are not stable for their clients and from what I have seen in other reviews, they don't take the mental health needs of the clients here seriously either. Better yet, find another facility, one who delivers proficient customer service, does not treat your mental health like a joke or just a paycheck for them, and one who won't continue to cancel your appointments last minute with a response time of up to 50 damn years.

When I started "seeing" Gina (She only did telehealth), I was highly considering leaving because I wasn't fond of her and it didn't seem like she listened to me. So then I was assigned a new lady, Tia. The first appointment left a very bad impression of her. Apparently I was scheduled at a bad time for her (early AM) and she made it known she was going to have a talk with the person who did it. In my opinion, she was pretty intimidating. I felt like she couldn't care less about anything I had to say. Shortly after meeting her, she went in maternity leave and I decided I was done with IC. I now go to CBHA and feel the med prescriber actually listens to me. It's sad when a once good experience turns bad after 10 years. I'd honestly never go back. Much better places to be seen/heard. And please, no one from that agency contact me. Thank you
