My mother passed away recently after staying at the nursing home for eight months. The level of comfort and aid my mother received was very good. Whatever request we may have had was typically handling quickly and completely. The compassion everyone showed her was very comforting. While several incidents occurred while my Aunt was a resident at Scofield; lost glasses, no podiatrist visits, lost clothing...Today I received a bill for an eye exam they arranged for 2 years ago !! At the time she was non communicative with severe dementia. Her chart also had directives per her primary care dr. for no more doctor visits, blood work, eye or dental appointments as she was in end of life stage. My sister was a rehab resident at Schofield three years ago. My sister recently passed away; however, I passed the facility the other day and remembered how I disliked this facility. The room was absolutely small and dismal, the furniture old and shabby. I felt terrible leaving her there, vowing to bring her home ASAP. The discharge planner at KMH insisted she go there for rehab. However, the rehab provided no actual rehab of any substance. Think long and hard about this place. read more

I find myself here again every few days. This time for someone in the rehab part of the facility. It's well organized and run. It's clean. The rehab patients seem "happy enough", given they are al there for rehab of some sort of course. It's not too loud and evidently the food is edible. Form my view, the parking is easy and it's easy to check in and out as you go in to visit people. They seem to have good security and know who is in the facility. 20190309U read more

HORRIBLE - Wife has been in 'rehab' for 11 days. She has waited hours for help to the bathroom, laid in soiled linens as a result that were not change because her floor did not have any fresh linens - waited for hours for a pain pill and in 11 days has gotten only received 5 full therapeutic sessions largely due to a quarantine on her floor. (we believe it food poisoning - but in any event have not been able to get results back in 5 days). My advice is: STAY AWAY!! read more

3/2022 This facility NEEDS A-LOT OF ATTENTION!!! Most of the staff is hugely inattentive, unprofessional, and MOST (but not all, there are some that have to run around picking up the slack and cover for them while still doing their own jobs), just seem to not want to work and do their jobs. It appears that there isn't any supervision whatsoever and the aides do whatever they like, hiding out in empty rooms on their cell phones, hanging out at the front nurse's station desk chit chatting, or else they're nowhere to be found. When you walk down the hallway and look in the rooms, it's pathetic some of the patients are naked or half dressed with nothing on the bottom, some literally hanging out of their beds, some slumped over in their wheelchairs sleeping, God only knows for how long, they have to beg to go to the bathroom, beg to go to bed actually beg for any kind of attention they are in need of after they've had their "light" on for a good five to ten minutes or so.... I realize that the facility might be understaffed, therefore I've tried to be understanding and patient but the lack of care our loved ones receive is shameful! I could go on and on but I hope I've made myself clear so far and please DO NOT send your loved one's there!! read more

My mother was in Schofield many times for rehab. This time unfortunately she was in there for comfort care before she passed away. The entire staff was absolutely wonderful. They made sure my mom was always comfortable and also made sure the family had what they needed. I would highly recommend Schofield to anyone that needed to place someone they loved there. read more

This is a terrible place for grandma. I had to silence people making noise around nana. They lack staff. I was anxious to get nana out of here. They have trouble with diverse populations. The staff is rude at times and lacks empathy. I am glad nana is home. I here staff talking about patients information. I really would have never done this place at all. I am forced to say A-OK, because the next lower option is "I experienced better", and I can't, unfortunately say that. My mother was previously in rehab at Northgate in North Tonawanda. Northgate is criminally negligent. I had to haul my mom out in a wheelchair, myself. They almost killed her. Schofield, by comparison, has a much nicer aura, in that the facility is nicer, the people are nicer, and they even allow pets. Many of the staff have been exceptionally kind, and I rely on them to be my eyes and ears, for without them, I would be terrified every minute I'm gone. However, Schofield is constantly short staffed, which means that during our stay (one month so far), my mother's room partner died, after repeatedly calling out for help in the middle of the night, and ultimately, having a heart attack (or so they claim), when nobody came. This same room partner fell multiple times in the room and had to crawl to the door to scream for help because no one came to answer the bell (even when both my mother and her rang separately at the same time). The second room partner likewise fell multiple times, and also crawled to the door to scream for help when nobody came for the bell. Today I write this post after spending 3 hours waiting at Buffalo General for Aries Transport to return my mother and I from a doctors appt to Schofield. Despite repeated requests for assistance from Aries (beware) and Schofield, we were left on our own. My elderly mother, who is frail, has multiple pelvic fractures, and cannot sit comfortably in a wheelchair without a brace, was utterly exhausted by the time we got back. Schofield personnel (those in charge) were, as expected, totally unconcerned. In fact, they never even bothered to find out why an 11 am appt would result in a 7 hour delay in returning, and never even checked or realized that my mother was missing in action. Statements were also made that my mother was ready to go home, when x-rays at the hospital showed that my mother's pelvic fracture had somehow worsened while in the nursing home. (X-ray compared to X-ray). Odd. Meanwhile, despite repeated requests for medical review due to increasing rather than decreasing pain (from the worsening of said pelvic fracture), no doctor was called. And, there is no doctor on staff. And, nurses (several of which have the bedside manner of road rats) are making medical decisions (such as assuming that my mother's extreme pain was caused by acid reflux). No, I'm not kidding. Meanwhile, my mother's blood pressure has gone from 150/74 to 99/57 in a matter of hours, and not a single thing was done. So, today, I have filed complaints with Medicare, Medicaid, the BBB, the District Attorney, and the Attorney General, as well as contacting an attorney. So, while Schofield has high ratings compared to others, the moral of the story is never leave your loved one unattended. Check everything, no matter what. Don't think because someone tells you they are taking care of it that they have. Don't rely on word of mouth. Put everything in writing, cuz you will need it later, possibly in court, and make sure you are regularly seen by a real doctor (not at the facility). Insurance will pay for transport, so don't hesitate to leave to be sure nothing important has been missed. While at this facility, I have often heard people scream for help, and found one resident with his arm bleeding profusely, with his arm stuck between his back and the wheelchair (don't know how it happened), while on blood thinners, in a room at the end of the wing (don't ever take a room at the end of a wing), notified the nursing station, and it still took 45 minutes before somebody showed up. I found another woman delirious and in pain (turns out she was dying of kidney failure, but not on pain medication, again, no clue why left to scream for days), and found others fallen out of bed, or out of their chairs, where staff were at the nurses station, not paying attention. And, on many occasions, my mother, and her roommates, rang the bell to go to the bathroom, and no one came, so they stayed in diapers or slept in their own excrement, all night. I don't think this should ever happen, but can speak for my mother and her roommates, all of which were of sound mind, in saying, it is humiliating to enter a rehab facility for a broken hip or pelvis, and to then be forced to literally crap in a diaper like a child, and to then have to be cleaned with washcloths covered in diarrhea, etc. Profoundly humiliating. And, lest you think, why the hell didn't you report it. I did. It doesn't help. Medicare and Medicaid (your only recourse) do absolutely nothing; less than nothing. My mom's room mate died 2 days after the end of a Medicare audit. And, the nurses and aides are actually (for the most part) awesome. However, they cannot help understaffing and apathy at the top. All that being said, Schofield is better than many others. Really. read more

Not at all impressed with this nursing home...our situation with our mother is already tense between siblings and this place makes it worse. Not having POA of my mother we are not allowed to get any updates on her nor are we allowed to ask for medical attention. Her foot was infected and she fell out of bed several times. It seems that no one knows anything from shift to shift when we ask. Can't express how horrified my sisters and I are with the care that our mother is getting and there seems to be no empathy for her either. She is not able to feed herself and at times her dementia makes her unruly and mean, at these times I have observed that the staff just ignores her and puts her in the hallway against a wall!!!! Where is the concern and the compassion. Only person I trust there is Amber, an aide. If we could get her out we would!!!! Just horrible. read more

Aides are nice, I believe they take pretty good care of my mom, though she has been declining a lot since she arrived, though I do think it is her dementia and hopefully not due to lack of nursing. There has been a lot of stiff gone missing, I have been trying to get money back from the one night gown, this is now going on for over two months, Her glasses are missing and they said they can not reimburse for them, seriously, they are new glasses I purchased about 2 weeks ago and now I have to dish out another few hundred for another pair, I am getting very frustrated and feeling like moving her somewhere else though I like the nursing aides. read more

My wife has been staying at the Schofield Residence for the past eight and a half years. In all the years that she has been there I can not find any fault with this nursing home. The food is varied and is hot and edible. She eats in a clean bright dining room. The last few years she has been assisted in her eating habits. They have varied programs going on each day from Bingo to musical interludes, entertainers, movies and cooking sessions. There are free 27 inch TV's for each residents and private phones are available to each resident. All the aides and nurses are courteous and friendly. The rehab is ongoing from bringing patients back to their former selves to range of motion for long term residents. The beauty parlor is available to both the men and women 5 days a week. I can only say high praises for this nursing home. It is definite plus in my life not to be worring about my wife's care. read more

I would not recommend schofield to anyone. The care is poor, the majority of workers do bare minimum at best leaving the good hardworking individuals left to pick up there slack. Most falls and accidents could and should have been avoided by simply having enough staff members. Communication between management and staff is also very poor. When you visit this facility I hope you are able to see past the fake smiles and the lies they tell you and hopefully see this facility for what it really is. read more

Initially seemed like a decent place, however there has been a huge staff turn over rate. I see new faces every time I visit my family member. Only a handful of staff have appeared to stay at the facility. Most of the summer, there was no air conditioning, residents, visitors and staff suffered consequences of this, except for those who remained at the nursing station where the air conditioning apparently was working. Residents continuously express dissatisfaction of care, but have expressed that they are fearful to express themselves to staff for fear of retaliation and do not share the truth with their families because they don't want to upset them or burden them. I have witnessed the residents assistance lights on outside their room for as long as an hour and no response from staff. The residents in wheel chairs are slouched, twisted, head dangling, have one shoe on, maybe one leg hanging off and staff does not attend to them to assure that they are comfortable. I have found my family member's shoe left in the dining area as well as items missing from the room. I have also witnessed residents entering other's rooms without permission. Food is pretty regularly missing from meal trays and has been requested but sometimes is never delivered. For those on special diets, the food consistency differs tremendously from meal to meal. The rooms are disgusting, dirty with old beat up furniture. Clothes are thrown are the floor, the drawers are a mess, they wont allow residents who need assistance to have shirts or sweaters with buttons because it is too time consuming, they put hospital gowns on residents rather than the residents pajamas to keep them warm. I have found food and wrappers on the floor of my family members room. The staff pay a lot of attention to the residents when the families are present, those without guests or family are ignored. The residence who need assistance are woken up very early and put to bed when it is convenient for the staff, not the comfort of the resident. A few residents are very loud and disturbing to other residents and this is ignored by staff. Some residents have expressed feeling afraid of these residents while others report that they blast their television to drown out the noise, otherwise they will lose control. It is annoying during a short visit, I cannot imagine what it is like for the residents, and this occurs day and night. Schoefield was remodeled recently, so the entrance and common area appears welcoming, it is all show. I will note that there are a few nurses and aides who appear to care about the residents and their comfort. But they are few and far between and I have witnessed an aide put my family member to bed in the evening in her soiled clothes. If I had control I would remove my family member however unfortunately I do not have that power, I feel an overwhelming sense of guilt daily, that takes over my every thought day and night. I feel powerless that others do not appear to be affected at all by the poor care that the person who raised them is receiving. shame, shame on family, shame on caretakers. read more
