I would NEVER recommend this facility, it was very dirty (rooms were never cleaned), there was no parking, the food was not balanced or nutritious, and completely inedible (prison food is probably better). The staff is slow to respond when a patient pushes the button for assistance. If I could give this place a negative 2 out of 10, I definitely would! They are a disgrace. They have a nice gym, that is the only positive aspect of this facility. They refused to allow a senior citizen to use the bathroom. Including senior citizens who went in and were able to walk to a bathroom. They insisted on them soiling themselves and then changing diapers. Staff was nasty and obnoxious for the most part. As a current resident at this rehab center I couldn't agree more with the numerous 1 star reviews. Just some of the sickening problems are 1) horrible food which includes chicken 3-5 times a week; 2) aids who are vicious to patients, 3) unprofessional aids who who question me about my religion (I didn't go to a rehab center to be questioned as to what my religion is or if I believe in God; 4) having a 'social worker' FINALLY make an appointment to see one only after I started screaming in the hallway about her weight and unacceptable delay in making an appointment ànd being lied to about how long it would take to allow me to return to my room after a clean out. Oñ the other hand my physical therapists are utter gems. Ĵust remember, this is a FOR PROFIT home, please remember that if you or a loved one ñeeds one. read more

My mother has been in this facility for a year and a half now and apart for the couple of days I took her out, the facility has never once let the residents even sit outside. A couple of weeks ago I ordered food to be delivered to my mom as I live out of state. The delivery man confirmed drop off at the front desk but my mom never got it. I ordered her a gift basket for her birthday, the delivery guy confirmed drop off at the front desk and again my mom never got it. I left a message with the social worker that my mom wanted to have her hair done in the facility and nothing was done. I called and spoke to the Administrator about all these concerns. He said he would look into it. Neither myself nor my mom have heard anything. I intend to contact the local Assemblyman and have him investigate this place. UPDATE: I received a call this morning from "Mark" and all concerns have been addressed. I will follow up with progress. UPDATE #2: Never heard back from Mark but did receive a call from another gentleman who also said he would look into the matters I pointed out. read more

Not sure if it was the right place to put my mom but she needed dialysis and not many nursing homes had that. My mom was here for 5 months, in and out of the hospital. She had a bedsore and was in constant pain and some of the nurses didn't believe her. Her bedsore did not get better in fact she developed an infection and eventually passed away. Some staff are great but others were just rude. They would fight in front of the residents. I would recommend doing research before putting your loved one there. read more

We went to visit a friend on Wednesday. We had to put on protective gear. We were told that there had been a Covid outbreak. I think that visitation should be cancelled.,when Covid is present. Furthermore when we went to see our friend,the man in the room with him was soiled. This had happened to our friend too,when he called for someone to help him to bathroom repeatedly, until he just couldn't wait and soiled himself. I find the conditions deplorable. Nasty staff. This place takes new people coming in who do not have Covid and places them in a room with people that do. They gave my mom Covid. Do not let your loved ones into this facility. They will get Covid. And on top of that it took them 11 days to give my mom a shower, they refuse to give her her meds with food as directed on the medication so she throws up, they don't want to change her diaper, it took 4 hours for them to get her a Tylenol, they don't give her all the physical therapy she is supposed to have. No one answers the phone, it just rings and rings and rings. They tell me that my mom and I are liars. They are rude, disrespectful, and they do not follow healthcare guidelines and are giving everyone Covid. Run, run, run away from this place it is a deathtrap. Every time I call the receptionist will not wait for me to finish my sen and forwards me to a room where my aunt cannot reach the phone. I had a fall from the bathroom into my room and it took the nursing supervisor three hours before calling an ambulance to go to the hospital. The ambulance didn't want me to take anything with me. No walker or clothing or glasses to read with. Nothing. They thought I'd come back to them after the hospital. Who the hell want to come back to a place that tells me they are too busy wait for the rent shift to call for an ambulance. If you will need Medicaid assistance, avoid this place. My brother is a patient in the rehab section. I have had to deal with this facility by telephone only, since I live far away. My experience has been enormously frustrating. Medicare is no longer covering his treatment 100%, and I have been trying to arrange Medicaid for him to cover expenses that are mounting higher every day. Catherine, the person in charge of Medicaid enrollment, did not responded to multiple attempts to reach her over the past few days. When I had another staff member corral her in person and get her to call me, she gave me an incorrect email address for sending the Medicaid information, so it won't go through. I tried to send a message about this situation on the facility's website, but got an error message in reply, so I can't reach anyone there either. I am being saddled with charges I can't afford to pay, and there is no one to help. read more

My mom was a resident at this nursing facility for nine years! During the early part of her stay I met many wonderful staff members who were kind & courteous which made me realize, at the time, that I had chosen the right facility for my mom. Unfortunately, during recent years the professionalism "went out the window" and in its place were nasty attitudes, especially from the nursing staff. In addition, the physician assigned to my mom's floor was very difficult to reach by phone. The poor excuse given as to why I couldn't reach the physician was because the facility is a "nursing home and not a hospital so the doctors are not always available." They might not have been available but they also didn't return phone calls! With regard to the nursing staff, on two occasions I had to report two RNs to their supervisor because of the unprofessional way they spoke to me and my sister. If ones choice is to work in a nursing home, they should automatically know there will be days that are going to be challenging. Having an attitude is completely unacceptable!! My mom's dementia progressed to the point where this facility was no longer capable of providing the care she needed. So instead of calling a family meeting to discuss the matter they shipped my 85 year old mother off to a psychiatric ward at a hospital where she was forced to stay an entire month pending an evaluation which was determined she didn't need in the first place! When my mom was finally cleared to return to the home they reluctantly took her back. Thank goodness I was already planning to move her to a more suitable nursing home where she currently is today, with a dementia unit that has made all the difference for her and she is doing so well!!! My mom spends her days singing, laughing, smiling & giving "hi fives" to her friends. Best move I ever made on my mom's behalf getting her into the nursing home where she now resides...the care is she getting is just amazing!! My family and I were totally appalled, to say the least, with the way Sheepshead Nursing & Rehabilitation Center managed our mother's care leading up to her transfer after being a resident for nearly a decade. I was in complete shock with the way my mom arrived at the new nursing home. She was wheeled into the lobby with two plastic bags which contained her belongings tied to both ends of the wheelchair like she was a mule. One of the bags could not hold the weight so her personal belongings were sprawled out all over the lobby floor. The following day, when I began organizing her new room, putting away her clothing, I realized she barely had any clothing at all... Sheepshead Bay Nursing and Rehabilitation Center kept my mom's clothing. When I emailed the social worker involved in my mom's transfer regarding the missing clothing it was ignored. Again, after nearly a decade of being a resident at this nursing facility...this is how Sheepshead Bay Nursing and Rehabilitation Center said goodbye to my mother! AN ABSOLUTE DISGRACE AND BLATANT DISRESPECT....SHAME ON THEM!!! I would have preferred to give this nursing facility a respectable review as the care my mom received in the early part of her stay was quite good as I mentioned. However, being that my mom was literally tossed out because she became too difficult to manage...a one star review is being generous!! read more

I think that this is an excellent rehabilitation center. They try to accommodate the patient and their family as well as they can. The food could be improved but I understand they are working on this problem. Also the administrative staff could be more accessible. My mom was recently placed in this facility for rehabilitation. She was told not to get out of bed or her wheelchair without assistance. When she asked for assistance she had to wait over an hour for anyone to come to her aide. It is only after waiting for so long my mom will then attempt to get up to use the bathroom will a staff member come and assist her. At one point while I was there I asked a staff member to help assist my mom and was told by this staff member, "Tell your mom to urinte in her diaper." How degrading and condescending to tell an adult something like that! While there I overheard a staff member yelling at a patient,"You are lucky to be here!" This patient was clearly in distress. When I reported the incident to the head nurse, this staff member claimed they only told the patient, " You are lucky." While walking around the floor, I noticed a patent who needed assistance with toileting. A staff member was with him and the bathroom door was left wide open. Where is privacy and dignity. The only positive aspect of this facility is ther 6 th floor gym where patients receive occupational and physical therapy for about 2 hours each day. The gym upstairs is state of the art. The rest of the 22 hours patients spend on their floors is lacking in respect and dignity by staff. read more

My Mom was admitted late Saturday evening around 6:30 last week. The male and female security guards at the front desk were wonderful and patient with everyone That was the end of any semblance of humanity. After a day and a half, I had my Mom transferred out. Mom is 90 but lucid and pleasant. She was taken to the 3rd floor and we just went to her room. Not one word was spoken to either one of us. Mon had a fractured sacrum and was in pain. Finally, I asked if I can have the phone and T.V. turned on. I was told to see the Nurse at the desk, Jennifer. Jennifer informed me that they do not activate them on weekends and need to wait until Monday. She expressed shock that my 90 year old mother did not have a cell phone. I then asked Jennifer if anyone was going to come and take my information. She was very annoyed and told me three times that she saw my chart and it listed two daughters. I told her three times that my Mom only had one daughter. Annoyed she checked the chart. And so it began. Donna, my mothers aid only knew how to speak nasty, for instance, if she put my Mom in the chair and Mom asked where they were going, she told Mom she did not have to know. Mom needed to use the bathroom and she told her she was going to lunch to wait for someone because they were short handed. Donna did not want to put Mom in and out of bed. Their was a male nurse Mr. McKay who was very very nice and after every incident Donna spoke to him in a foreign language in front of my mother. I was there for quite a while and everyone spoke nasty to the patients on the floor. The atmosphere was terrible. Three nursing supervisors called me to keep Mom there. They were all very nice but I am sure that they are not able to change the terrible atmosphere there. The administrator called and thanked me for bringing the situation to his attention... Anyone can see that this is the culture of this rehab. I would not bring my dog here. read more

I was there for one day and I was so discussed with this place I would not let my worst enemy going there it is a horrible and they clean with there eye close duck tape on window and on A/C my bathroom was not clean shit on the floor paper all over and when the guy came in to clean he clean worst them my granddaughter at 4 yrs. old the food is terrible and cold they microwave food it like eating cardboard if anybody read this please don't let your family members go their unless you never want to see them or be the same person that when in there . The staff on the 3rd floor are cruel. They keep switching rooms when the patient is incapable to do so. They call Mark the Administrative assistant and lie about people they DON'T like. This causes havoc among staff and patients. The 1st floor is slightly better, not by much. The first floor is the ONLY floor that had construction done to it. So, when people come into the Building they have something nice to look at. This place has changed for the worse. The nurses put dressings on my knee that was so tight that it opened the scar from my surgery. Mark is always on the staffs side. NEVER ON THE PATIENT'S. The only thing that changed was they DON'T charge for the phone or TV. The television hardly works. The phone connection is so bad you literally have to yell at the person on the other side. The food is very good, I have to admit. The only reason I was there, for the physical therapy rehab. There physical therapy is amazing. Derk is magnificent. read more

I recently had an accident and was sent to Sheepshead Bay Nursing & Rehabilitation Facility. I was here for 8 weeks. On the 7-3 pm shift (first floor), I was lucky enough to have Julie Santiago and Cristal Voltaire as my CNAs and Gina Massenat as my nurse. They made me feel well cared for. For the 3-11 pm shift Ms. Fuller was my CNA and she also took good care of me. My physical therapist was Roland and my occupational therapist was Eugene. They were wonderful in pushing me to get the most out of my therapy. They were encouraging and helpful! James Barrows from housekeeping (7-3 pm shift) was there in case I needed anything done in the room. He was helpful and funny which is the best medicine. The transporters (Justin, Delva and Jonathan) were funny and wonderful. My stay was necessary but because of the great care - it was appreciated!!! read more

Absolutely disgusting.Mom was transferred from another facility about 2 months ago.Has been in a wheelchair ever since.She has lost weight and has not had any excercise.She is currently in a room with 3 other inmates as i like to call the residents there.The staff stay in the lunch room watching the shows of their choice on TV instead of doing what they are paid to do. The food is horrible and visitors are limited to a place where they can see their loved ones.Been trying to get mom to see a specialist but they are not complying for some reason.Im currently looking for a new home for my Mom.Help!!!! read more

My husband has been here now for three weeks. First, I must say that the physical and occupational therapy is good. That's about all that is good in this place. The nursing staff is indifferent, the aides are short-handed, especially on weekends. Some are pleasant but too many are not. The patients are made to sit for HOURS in their wheelchairs in the hallway looking at the wall. Of the 21 days we've been here, this is the second day Direct TV is out all day. Patients have NOTHING to do for 22 hours but sit in the hall or lie in bed. If a patient came here with his or her sanity I can see them becoming mentally unstable after an extended stay. The food is horrible. The facility is undergoing "remodeling" in dribs and drabs. Patients are kept waiting in the hall for 4+ hours while they do construction in the room. The heat in the room is suffocating and the windows are impossible to open. Finally, and most egregious, it took me a week to figure out why my husband was acting irrationally and hostile. I was the one to realize they were giving him TWO different opioids. Thank goodness I caught it and insisted they stop one of the medications. They had one opioid listed as being for COPD when it was actually oxycodone. Cannot wait until my husband is strong enough to leave this horrible place. If you love someone, PLEASE do not send that person here. read more

I must agree with the other reviewer. The nursing home is very cheap and has cut staffing and they do not take care of people properly. They cut activities and charge you $30 a week for a TV set from the 1980s. They put all the people into the day room to vegetate for hours sitting in wet and soiled diapers. Most people are depressed and want to leave, but may not be able to because they don't have family. My mother ended up dying of sepsis from a bowel obstruction because the nursing and medical staff provided poor care and failed to monitor her. They did not even realize that anything was wrong with her until I pointed it out to the evening nursing supervisor, who then ordered some tests. When I called the next day, the day nursing supervisor had not even known that tests were ordered, and did not even know that the results had been there since early in the morning. By the time she finally checked them after I called there, they sent her to the hospital in the late afternoon. Too late for my mother, she was already septic, and died a few hours later. Although the rehab may be ok, and some aides do try to do their jobs, the nursing home is not 5 star as they claim. The state of all of the NYC nursing homes is horrible. There is big money in short term rehab, getting reimbursed about $1000 per day from medicare so they have incentive to keep you there longer than necessary. NYS needs to implement minimum staffing rules for nursing homes, and get out of the pocket of the nursing home lobby. There's got to be a better and cheaper way to care for our elderly. read more
